The low voltage light came on. Voltage was at 11.5V. I continued driving. Voltage dropped below 9V. I pulled into a parking lot. At low speed, the power steering shut down, braking assist, parking sensors, and other functions shut down and the vehicle stalled. Vehicle automatically shifted to Park and engaged the electronic parking brake. Car would no re-start and was towed to the dealer. Car is
... currently at the dealer with a diagnosis to replace the 12V battery as a first step. I am concerned to drive the vehicle based on similar consumer complaints related to PE24018. If root cause is not addressed (eg, only replacing 12V battery), I feel my safety is at risk if the vehicle shuts down again in a situation where I was unable to pull off the road before the vehicle shut itself down due to low voltage.Read more

Vehicle: 2022 Volkswagen Atlas 2.0T (VIN: [XXX] ), current mileage: . Failure: The engine water pump / integrated thermostat housing failed due to a coolant leak. I first noticed coolant odor and dried pink/white residue around the pump area on //. Coolant level dropped between services and I observed active seepage. Dealer diagnosis on // confirmed failed water pump/thermostat module and recommen
... ded replacement. Frequency/Progression: Began as a slow weep that progressed to an active leak. Coolant requires top-offs; leak is visible after parking. Safety risk: Sudden coolant loss can cause engine overheating and potential stalling while driving, creating a crash hazard and risk of engine damage. Context: This appears to be the same EA888 2.0T water-pump/thermostat leak pattern documented across VW/Audi vehicles; VW previously extended coverage to 8 yrs/80k miles for many 2014–2021 models in a settlement, but 2022 Atlas is not listed—leaving similar failures unaddressed. (Request ODI review for potential broader coverage.) Attachments/Proof: Photos of residue and leak, dealer repair order/estimate for pump module, service history, and receipts (upload available). The engine of my 2023 Toyota Tundra failed at approximately 34,500 miles. The failure involved sudden loss of oil pressure and severe internal damage, with metal shavings found in the oil and oil filter. The oil filter itself detached during the incident and was later recovered with metallic debris inside. The vehicle and the recovered filter are available for inspection upon request. While accele
... rating from a stop light on a busy morning public road, the engine suddenly lost power, illuminated multiple warning lamps, and entered limp mode with “low oil pressure” and “engine malfunction” messages. The truck rapidly leaked oil, creating a roadway hazard and putting me, my wife & son, and other drivers at risk of accident. I was able to limp the vehicle home .5 miles from the incident at reduced speed, but complete engine seizure was a possibility. Toyota of Fort Walton Beach inspected the vehicle, confirmed metal shavings in the oil pan, documented codes P05202 and P052049 (low oil pressure and oil pressure sensor stuck in range), and noted “oil was very metallic.” The vehicle was inspected by Toyota dealership technicians and corporate representatives. I also have possession of the oil filter and photos/videos from the inspection. At the moment of failure, the dash displayed low oil pressure, check engine light, and a message regarding engine/electronic malfunction. Prior to this event, there were no warning signs — the truck had been serviced two weeks earlier at 33,360 miles, and I drove approximately 1,200 miles without incident until the catastrophic failure. This failure matches the description in Toyota Recall 24V-381. My vehicle’s build date (February 27, 2023) is less than two weeks outside the published recall production window, yet the symptoms and inspection results are identical. I respectfully request NHTSA investigate whether additional 2023 Toyota Tundra vehicles outside the current recall window are affected, as my case demonstrates the same hazards.Read more

Ford has issued a TSB 22-2372 regarding the rear camera image going blank when placing the vehicle in reverse. This issue clears by restarting the vehicle. The cause is a software error in the Image Processing Module and SYNC 4 system. The danger is backing up without being able to see anything besides what is in your mirrors without the software update to fix the issue. Numerous children have alr
... eady died because of vehicles without the safety of a back-up camera. I took my vehicle to the dealership to have it fixed and was quoted almost $1000 dollars to update the software. I complained and the dealership forwarded my complaint to Ford who gave "assistance" bringing the bill to $361 dollars. My question is why a mandatory recall hasn't been ordered for this obvious safety issue? Luckily, I can easily pay this but what about those who cannot? I asked Ford (case #CXH-xxx) why they are not providing the fix for free since it is a safety issue and they used the excuse of the warranty being expired. Hopefully NHTSA will make Ford fix this safety issue for fellow consumers. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Component/System Failed: The rear suspension strut assembly on the passenger side fully dislodged while driving at approximately 30 mph. Inspection: The vehicle has been inspected by the manufacturer’s service department and corporate case team. Warning Signs: No warning lights, error messages, or abnormal noises were present prior to the failure. The dealership classified the failure as impact damage, claiming an external force caused the rear strut to detach, which excludes it from warranty coverage. However, my insurance inspection found no evidence of significant impact or collision, noting only minimal scuffing and no structural deformation. This indicates the issue was more consistent with a mechanical or structural defect rather than road damage.Read more

Also, the adaptive cruise control is set on the furthest distance, but it follows very closely to the vehicle in front of me. Like a 1.5 second length at the most. When I
... change the setting to follow closer, it will follow a vehicle within inches. I believe this causes a safety issue with a potential crash.Read more

While driving, the vehicle just stopped. Tried to navigate to get to side of the road, but ended up stuck in the middle of the lane, in the middle of the road. My pilot also has some sort of lock, so I could not put it in neutral to be pushed out of oncoming traffic. Tried to jump start but the battery was fine. Road side assistance took 3 hours to come and tow us. Once at a dealership, diagnostic
... s was run, starter was checked. But turns out the engine had ceased. The vehicle had randomly not started back up when the eco shut the car off when the brakes were pushed down, but this was extreme! Very dangerous! I regularly change my oil, only use synthetic oil and have my car serviced, fluids topped off. I have been on the wait list for the fuel recall, but Honda on Gilbert, in Gilbert, AZ does not answer calls, call back, or offer rentals for recalls. But this situation was scary and unacceptable. There is other recalls for the 2018 Honda Pilot for engines ceasing up. This is poor workmanship for Honda. Yes, this has been confirmed. My vehicle is currently had Honda Superstition Springs in Mesa, AZ, because I did not trust to go back to Honda Gilbert to timely correct the issues or communicate. Yes, You can inspect the vehicle. Engine ceasing while driving put my self and others at risk. No warnings.Read more

Honda issued Service Bulletin 20-117 regarding a Warranty Extension for the 2016–2017 Pilot and 2017 Ridgeline addressing a Keyless Start System Error with DTC B12C5. Following an NHTSA investigation, Honda extended warranty coverage to 9 years or 125,000 miles under this bulletin. My 2016 Honda Pilot currently has 107,000 miles and is only six months past the nine-year time limit. Earli
... er this year, the Keyless Start System warning indicator began appearing intermittently—illuminating for a few seconds before disappearing for days or weeks. Recently, the warning began to stay on continuously. Per Honda’s initial guidance, I replaced the key fob battery and even swapped fobs with my wife, but the issue persisted. On October 16th, I brought the vehicle to Honda of Chantilly, where technicians diagnosed a faulty start/stop button requiring replacement. I then opened case #15920037 with Honda, but they declined to cover or reimburse the repair, despite it being a known defect covered under the extended warranty for others. I respectfully request that the NHTSA assist in having Honda honor the warranty extension for this documented issue, given the close proximity to the coverage limit and the long-standing nature of the defect.Read more
