On Sep 25 2025 my wife parked car in parking lot to check phone messages. The car was in park. When she finished she placed the car in drive. It began to accelerated rapidly even after the brakes were applied and struck a tree. Her foot was not on the accelerator. The car then rolled over onto the driver side door. A police officer in a nearby building saw the crash and said the car was probably a
... t 30mph when it hit the tree. He and two other officers got my wife out of the car. After the car was towed away we contacted VW customer care and asked that the car be examined. The said when the car was returned to drivable condition they would examine it. Hartford insurance totaled the car and said they were not responsible to examine the care and referred us back to VW. Again VW will not go the a salvage yard to examine the car. The car had intermittent electronic signal issues and a previous recall on doors needed to be redone because originally the wrong part was installed. I did experience a previous incident months before where I felt the car accelerated quicker than I expected from a stop with slight pressure on the pedal but it was only momentary.Read more

October 30, at approximately 8:00 AM MDT. While I was heading to work, I was approaching an intersection with a vehicle in front of me making a left turn in the opposite direction was a vehicle also preparing to make a left turn and as I approached the intersection the vehicle turned in front of me. I fully applied the brakes, and the pedal went to the floor, my truck did not stop or slow down. I
... ended up striking the vehicle turning in front of me, fortunately at a low speed of approximately 11-12 miles per hour. My vehicle coasted to a stop after which I was able to carefully pull off to the side of the road. My truck was deemed drivable at the time after the accident but I noticed that the pedal is spongy and it took pumping the brakes to get them to apply. I almost had another accident around 10:30 AM, when coming to a stop behind several other vehicles. Again I applied the brakes and they went to the floor. The truck very slowly came to a stop, finally stopping 5 feet from the rear of the car in front of me. This was extremely nerve racking because I thought I was about to have another accident. After talking with my insurance company, they deemed the truck unsafe to drive and had it towed to the repair shop. Additionally, back during a trip to California from September 28 to October 15. We were towing our 2024 East to West Alta Travel Trailer. During the trip I noticed very sporadically the trailer brakes would apply to much or more often appear to not apply the trailer brakes at all. Multiple time I had to use the manual brake booter switch in order to stop the truck. I believe that this issue with the brakes was actually beginning during that trip. I also have done some research and there appears to be several other complaints with the same model year 2024 Ram 2500. When I get my truck back from being repaired I am scarred of it not braking and getting involved in another accident. Something needs to be done before any serious injury or death occurs.Read more

I am submitting this formal complaint regarding a potential recall failure involving a 2005 Mazda Tribute driven by my fiancé, [XXX], who died following a crash on [XXX]. Mazda has opened Case #[XXX] in response to my inquiry, and I am requesting federal oversight of this investigation. Joey’s vehicle may have been affected by NHTSA Recall 12V357000, which addresses throttle body malfunction an
... d unintended acceleration. Service records show that a dealership performed a recall service, but the parts used may have been ineffective or incomplete. The crash sequence—rear-ending a stopped vehicle followed by veering into oncoming traffic—matches known symptoms of this defect. I am requesting: •A formal review of Mazda’s handling of Recall 12V357000 •Verification of Joey’s VIN-specific recall compliance •Investigation into the dealership’s repair actions and part quality •Oversight of Mazda’s internal case (#[XXX]) and its findings This issue raises serious concerns about manufacturer accountability and public safety. I am submitting supporting documentation and service records for review. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Incident date: Dec 31, 1969
Since the recall was announced, my truck has experienced constant ABS and traction control alarms, including an unending beeping warning chime that makes the vehicle unpleasant and unsafe to drive. My local Ram dealer confirms that the repair parts (HCU/ABS module) are not available and that there is no estimated time frame for replacement. This situation has now persisted for months, leaving me
... with: •A continuously beeping alarm while driving or idling, •A loss of confidence in braking and safety systems, •And a significant loss in resale value, since I cannot sell or trade the vehicle while this open safety recall remains unresolved. Impact This recall defect has made my vehicle effectively undriveable and unsellable. I have made multiple attempts to have it repaired, but the dealer and Stellantis representatives continue to tell me that parts remain on national backorder. As a result, I am deprived of the safe and normal use of my property. Under 49 U.S.C. § 30120, a manufacturer must provide a free and timely remedy for a safety defect. The ongoing delay and the lack of any interim solution (such as a loaner or disabling the alarm) clearly violate the spirit of this requirement and constitute a constructive deprivation of use. Requested Action I respectfully request that Stellantis: 1.Provide a functional remedy immediately (replacement HCU or equivalent). 2.If parts are still unavailable, provide a loaner vehicle, temporary compensation, or buyback option until the repair can be made. 3.Confirm in writing the expected date when replacement parts will be available for my VIN. I am filing a concurrent complaint with NHTSA to document this safety-related hardship and ongoing loss of vehicle use and value. Documentation Available upon request: •Copy of dealer service report indicating unavailable parts. •Photographs/videos showing active ABS and traction control alarms. •Records of my prior attempts to schedule service.Read more

I was slowly entering a handicapped parking place right in front of a business. The distance from the front of the parking space to the building wall was about 15 feet, one car length. I approached the place to stop in the parking spot when in a split second the car violently surged forward and would have crashed into the cinderblock wall had there not been a supporting post on the building and a
... metal frame with equipment which stopped the forward movement. The surge was explosive, almost like the acceleration of an airplane taking off. The whole incident took a second or two. There was no opportunity for any driver response to the surge. Had we not been using seatbelts, the front seat passenger & myself would have gone into the windshield and perhaps been killed or very seriously injured. Had I not been driving toward the building when the violent episode occurred, I likely would have hit someone in the parking area and killed or seriously injured people or crashed into park cars. Our injuries were relatively minor due to the seatbelts. The passenger will need therapy on her neck and back. A week later we are still emotionally stressed. There were no indications there was anything wrong with the car prior to this incident. It had only 27,000 miles with new tires and no apparent service issues. This hybrid car, which goes about 22 miles on a charge, was about at the point when it would have transitioned from electric to use of the gasoline engine. The badly damaged car was towed to a lot and later picked up and taken to the insurance facility for evaluation. I have not yet heard back. A NYS trooper investigated. He told me twice to have the Insurance Company look at possible vehicle defects when they examined the vehicle for insurance purposes. I passed on that information, but it appears the insurance company only evaluates the car for damaged. I have not yet gotten a report back yet, but it appears the car is totaled.Read more

The Antilock Brake Module (part # 68228991AD) and Antilock Brake Hydraulic Pump (part # 68196033AA) have failed for no apparent reason. The dealer states there's a national backorder for the module with no delivery date noted. The module controls the anti lock braking system, traction control, and cruise control, which are all safety concerns. The module cannot work without the associated pump.
... This problem has been confirmed by two dealerships and an independent service center. There were no warnings prior to the failure, which occurred on 10/18/25. The cost of the pump is $1290 from the dealership, which is outrageous. There are remanufactured modules and pumps out there, but they are outrageously priced. RE: Continental Pro Contact GX SSR MOE 245/40 R19 98 H M+S Original factory installed Mercedes E450 tires. I had 3 blowouts from hitting small potholes on 3 different tires in 1.5 years. These are complete immediate blowout going completely flat on impact. It could have very dangerous if I were driving a little faster and fortunately there was no other damage to the car or other persons.
... In each case, I hit a small pothole, and the tire blew out, flat to the ground. Because these are run flat tires, on the first 2 incidents I was able to drive home and to the dealer for tire repair/replacement. The third one was so bad that I could not drive all the way home and had to get towed. But all 3 incidents were scary and dangerous because of the sudden blowouts. The tires were not repairable and had to be replaced. I believe that the Mercedes Tire inflation instructions is unsafe and they should put more air in the tires to protect from the rim cutting into the tire when hitting a pothole. The driver's door "B" pilar placard says the "cold tire pressure: Front 41 psi Rear 47 psi". Mercedes repair shop inflates the tires according to the Fuel Filler Door decal that says " Front 36 psi, Rear 36 psi." That is a big difference in pressure -- as much as possible 11 psi under pressure. I believe these run flat tires failed because of too low of tire pressure when hitting the pothole. If the tires were inflated with the higher pressures as on the door pilar, the wheel rim would not have been able to cut into and through the tire wall because the higher tire pressure would prevent the impact of the rim cutting through the tire sidewall. I believe this a serious safety issue and Mercedes should be inflating this type run flat MOE tires to a much higher pressure and should not use the low-pressure instructions that are on the fuel door label. There should be a recall and repair instructions to increase run flat tire pressure on Mercedes E class vehicles.Read more

Faulty CANBUS connector/CANBUS failure. This issue tends to be a random and inconvenient occurrence which can be difficult to initially diagnose. One dealer said our issue was due to the ABS module; replaced it and it did not fix the issue. Replaced the CANBUS connector and the issue has resolved. This is a known issue among jeeps both new and old and can pose a serious safety risk. To answer the question of how fast the vehicle was going when this issue occurred; the failure of this part happens at both highway speeds and residential speeds. There are currently 52,XXX miles on our jeep and the failure started, roughly, April of 2019. Jeep was purchased, brand new, April of 2018. It started with the jeep failing to start then grew to the jeep entirely stalling out while driving.Read more

This is a very common issue reported by many owners Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. The vehicle has not yet been inspected by Tesla, law enforcement, insurance, or third parties. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps or messages appeared.
