I own a 2022 Ram 1500 Bighorn with approximately 84,000 miles. This vehicle has repeatedly experienced a severe safety defect that began around 20,000 miles. On four separate occasions, the truck has suddenly decelerated in traffic without warning, would not shift out of gear, and in some cases would not shut off even when pressing the start/stop button. During the most recent incident, I was unab
... le to pull over safely because the powertrain locked up, putting myself and surrounding drivers at extreme risk. My wife and I as well as a good Samaritan stranger almost got ran over while checking to see what's wrong. Police showed up and called for a tow truck, since the incident was happening in a state hwy. Each time I brought the truck to the dealership, I showed photos and videos of the issue, but the dealership claimed they could not reproduce it. Now the dealership is telling me I must pay $1,127 just for them to look inside the transmission, and potentially $2,800–$11,000 for repairs. This is a safety-critical defect that began well within the warranty period and has progressively worsened. A sudden loss of power and inability to control the vehicle is dangerous and could result in a crash, injury, or death. I believe this issue should be investigated for a recall or manufacturer responsibility due to multiple reports of similar failures in Ram 1500 models. I have videos of the incidents in 2025 at 80k miles, and a video in 2024 at 43k miles that show the live incidents.Read more

I am submitting this safety complaint as the vehicle owner of a Tesla that was involved in a collision on November 15, 2025. Although I am the policyholder and registered owner, I was not the driver at the time of the incident. My husband, [XXX] , was operating the vehicle, and Tesla Autopilot/FSD was engaged when the loss of control occurred. At the time of the incident, it was raining heavily,
... though visibility and road conditions were still within what Autopilot typically handles during normal operation. While driving under these conditions with Autopilot activated, the vehicle suddenly exhibited unexpected and erratic steering behavior, causing it to lose stability and veer out of control. The deviation happened rapidly and without sufficient warning or opportunity for my husband to intervene manually. The resulting collision appeared to be the result of an Autopilot system malfunction, not driver error. Key concerns include: Autopilot’s failure to maintain proper lane position Sudden abnormal steering input originating from the system Possible sensor, perception, or weather-related detection failure Lack of adequate warning or disengagement alert prior to the loss of control Tesla Insurance later determined the driver to be “principally at fault,” but this conclusion did not consider the possibility of Autopilot system failure under heavy rain conditions and does not reflect the sequence of events as experienced by the driver. As the vehicle owner, I am requesting that NHTSA review this incident as a potential safety defect involving Tesla’s Autopilot/FSD system. I am concerned that this malfunction—particularly under adverse weather—may also affect other Tesla owners and could indicate a broader systemic safety issue. I am willing to provide additional information, driver statements, documentation, or vehicle data upon request. Vehicle has been having AIS issues for a couple of years, requiring new batteries, new starter/relays and countless internal computer updates (none of which have helped btw...). This morning she had the AIS engaged, went to turn left at a green light and when she released the brake the vehicle threw itself into Neutral and flashed a "Auto Idle Stop System Problem" message on the dash. She put it i
... nto park and tried to restart, which failed, she then had to turn the vehicle off completely and restart from there. As I mentioned, none of the computer updates Honda has done for this vehicle have helped and it's my understanding there is a block of vehicles in this age range (all Passports, Ridgelines and Pilots) that continue to have problems with the AIS as well. Honda needs to disable this "feature" on any vehicles they cannot fix via computer updates IMMEDIATELY before they get someone killed. Not to mention, we are sick and tired of having to replace the battery and/or starter every couple of years. Honda should not be putting tax breaks above their customers safety and satisfaction. You know it's bad when their 30+ year techs go out of their way to buy a used Honda just to avoid the AIS because they don't know what to do with it at this point either.Read more

The Brake Hydraulic Control Unit (HCU) covered under Safety Recall C4B has been unavailable for several years, leaving my 2018 Ram 2500 with ongoing ABS, traction control, and electronic stability control malfunctions. This defect has already caused an actual accident. In January 2024, during snow conditions, the ABS and traction control failed while I was braking in my driveway. The truck slid u
... ncontrollably and struck a tree, causing over $10,000 in damage that had to be repaired through my insurance. This incident occurred specifically because the brake and stability systems did not function properly. The issue continues to create serious safety risks, especially in winter. The warning lights for ABS and traction control are on constantly, accompanied by continuous dinging that is extremely distracting and stressful to drive with. The truck loses stability on snow or ice, braking distance is increased, and the vehicle behaves unpredictably. I am now driving again in snow and ice this winter, and I am genuinely afraid this defect will cause another accident. Despite this being an acknowledged safety recall, every dealership I contact says the replacement HCU part is still “not available” for my vehicle. This has been confirmed repeatedly by multiple dealerships and by the manufacturer’s recall hotline. I have attempted to get this recall repaired for several years with no success. Because the recall remedy is unavailable, the vehicle remains unsafe to operate, yet I have no alternative transportation and must drive it daily. I also cannot sell or trade the truck, which has caused significant financial harm. I am requesting NHTSA assistance because the recall remedy has not been provided within a reasonable timeframe, the vehicle remains unsafe, and the defect has already contributed to a crash.Read more

We are contacting you today because our 2023 Jeep Wrangler 4xe started to exhibit some concerning issues: On a fully charged battery, our Jeep started normally in hybrid mode. After 10-15 miles (down to 51% battery capacity), power switched to Internal Combustion Engine (Engine) After parking, upon restart and re-restart, various malfunctions appeared on dashboard: - Straight to engine power (e

We were told the software update was successful. We have made an appointment with Kendall Jeep to drop our Jeep off on December 8, 2025. We are concerned that our vehicle is unsafe to drive and that MOPAR has not provided a fix for this continuing issue. We included a pic of our dashboard and warnings.Read more

Please add this to complaint number: 11702732 This is a supplemental complaint regarding the same safety issue I previously reported involving Safety Recall C4B on my 2018 Ram 2500 - complaint # 11702732 In addition to the safety concerns and the prior accident I described in my original complaint, the unresolved recall has also caused significant financial harm. I still owe approximately $15,10

Because the recall remedy has been unavailable for years, I cannot legally sell or trade the vehicle, even though it should have substantial equity. The truck’s resale value has dropped due to the open safety recall with no remedy, leaving me financially stuck with an unsafe vehicle I cannot get rid of and a high payment that I currently cannot afford due to job loss. The unresolved recall continues to put me in danger during winter driving, and the lack of remedy has created both a safety crisis and a serious financial hardship. I am filing this complaint because my vehicle is subject to Recall 68C (Battery Fire Risk), and the manufacturer's remedy is inadequate, subjecting me to ongoing risk. Immediate Safety Compliance Failure: I cannot safely comply with the required "park away from structures" directive due to my current living situation. Inadequate Remedy Concern: The proposed fix for 68C is software-based. This i
... s insufficient because the previous software fix (Recall 95B for earlier models) failed, leading to continued fires. I demand that NHTSA investigate and mandate a permanent, physical solution (a new battery with a corrected cell design) for Recall 68C, not another potentially ineffective software monitor. Financial Harm: I am also incurring significant, unplanned financial damage (increased fuel costs, paid FPL charging fees) due to the manufacturer-imposed loss of the vehicle's electric functionality. This must be addressed as part of the recall solution.Read more

The safety component that is covered under the SBC warranty is the entire brake master cylinder specifically the part that is bad is the brake pedal position sensor The car is undriveable without it, they have a 25-year warranty on it yet they have no parts and no way to fix it which is obviously unacceptable they've also refused to provide a loaner car until they come up with a way to fix my car
... leaving me on the side of the road with nothing to drive it's unacceptable! My safety and everybody's safety on the road is affected by this because the car won't stop if you drive it and you have really strong legs you can get it to slow down and eventually stop it's completely unsafe The brake pedal cannot tell the computer where it is in its travel which means the system doesn't know that you're applying the brakes which don't work. Yes the problem has been confirmed by two shops one of which has a snap-on scanner which I can attach the scan to showing the issue and currently the car is sitting at a shop that uses Mercedes-Benz only star system the same system that they use in the dealership there is no question at any level what is wrong with the vehicle. The car is not safe to drive to the dealership and now in the winter this $132,000 convertible isn't going to be driven in the salt just to get to a dealership to find out what they're not going to do for me I've already confirmed and share the scans and images and everybody I've spoken with at the dealerships agrees that it's ridiculous that Mercedes has a 25-year warranty on a part that they do not make do not have and cannot fix Yes there are two different brake malfunction lights that are on the dash you cannot clear them as though in a code way like you can on some things and the car is not to be driven, the problem first arose a couple months ago and we started diagnosing it until on the Facebook forum somebody told me that it's covered by a 25-year SBC warranty which when I contacted the dealer and Mercedes-Read more

I am reporting a recurring safety defect involving hydraulic lifter failure in Ram trucks equipped with the Cummins diesel engine (model years 2019–2024). This issue is widely acknowledged by Stellantis and multiple dealers. It typically begins with a knocking sound and has been documented in many cases to progress to complete engine failure. My vehicle has fewer than 20,000 miles. The lifters
... were already replaced once, which required the cab to be removed. Within less than 400 miles of that repair, the knocking sound has returned. I brought the truck to a second dealer, who confirmed the problem and informed me that Stellantis has an active STAR case acknowledging the defect. They stated there is currently no known permanent fix. This defect presents a serious safety concern. Sudden engine failure while driving, especially under load or at highway speeds, could lead to loss of power, loss of steering/braking assist, and potential accidents. In my situation, I frequently travel with family members who have medical conditions, and being stranded in hazardous or remote conditions poses significant risk. At this point, my vehicle is not reliable or safe to operate. I am requesting NHTSA investigate the hydraulic lifter defect in 2019–2024 Ram Cummins trucks due to the safety implications and the lack of an available permanent repair.Read more

I have reported this before and I am reporting once again as it is a very serious safety hazard. Vehicle: 2014 Jeep Cherokee Limited Component/System: Power Train – Transfer Case / Drivetrain (4WD/AWD System) I am submitting this complaint to report a recurring and potentially hazardous malfunction involving the transfer case on my 2014 Jeep Cherokee Limited. The vehicle exhibits multiple sym
... ptoms consistent with transfer case failure, including intermittent grinding and clunking noises from the drivetrain, jerking and binding during acceleration or low-speed turns, and difficulty engaging or maintaining 4WD/AWD modes. The vehicle has also displayed dashboard warnings such as “Service 4WD” and “4WD Temporarily Unavailable.” At times, vibration can be felt through the floorboard at highway speeds, and the vehicle has intermittently lost power to either the front or rear wheels. These issues pose a significant safety concern. The inconsistent torque delivery causes unpredictable handling, including sudden hesitation, surging, or traction loss during normal driving. The vehicle has also exhibited momentary drivetrain binding that feels similar to braking or wheel lockup, creating a risk of loss of control—particularly when turning, merging, or driving in adverse weather. There is concern that the transfer case could suffer sudden internal failure, potentially resulting in immediate power loss, immobilization, or inability to maneuver out of traffic. Given the frequency of transfer case and power transfer unit failures reported by other Jeep Cherokee owners of this model year, I believe this issue may represent a broader safety defect. The malfunction directly affects vehicle stability, traction control function, and the ability to safely accelerate or maintain speed. I request that NHTSA investigate this matter for a safety recall as soon as possible due to the potential for loss of vehicle control and the risk of a crash or injury.Read more

My 2015 Hyundai Sonata has a defective engine. The motor failed unexpectedly even though the car has low usage and has always been maintained properly. I was informed that many Hyundai engines from these years came with manufacturing issues, and mine appears to be one of them.

Check Engine Light comes on and off on its own. OBD read that the engine is not getting up to normal temp. Had it diagnosed and was told that the 3.6 V6 has issues with the oil filter housing cracking/leaking. Somehow the housing cracking caused me to lose coolant. To have everything repaired would be $1400.
