NOISE STARTED ROUGHLY IN JULY OF 2025, AND WHILE GETTING OIL CHANGED AT DEALERSHIP I SPOKE TO SERVICE REP AND HE SAID THERE SHOULD BE NO ISSUES DRIVING IT AROUND 63000 MILES. NOW THAT THE LIGHT CAME ON--68000 MILES--I WANTED TO GET IT LOOKED AT. SERVICE PERSON ADVISED ME TO TRADE IT IN FOR ANOTHER VEHICLE. i DO NOT WANT TO TRADE IT IN. PLEASE ADVISE ON AVAILABILTY OF GETTING RELACEMENT PART OR WHERE TO BEGIN TO FIND ONE. i LIKE MY JEEP IT HAS BEEN A DEPENDABLE VEHICLE, BUT WITH THE COMMON OCCURANCE I SAW ON THE FORUMS QUESTIONS ARE STARTING TO RISE ON THE DEPENDABILITY. My 2023 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting the Uconnect 5. These issues are widespread in 2022–2024 Ram TRX models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses all functions controlled through the display, including HV
... AC/defrost, backup camera, Advanced Driver Assistance Systems and settings, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates • Unresponsive screen These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: system rebooting, unresponsive screen, loss of vital vehicle health data and performance gauges, backup camera not displaying] Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

On Dec 4 around 6pm driving in Chicago area. Weather has been very bad, with snow and extreme cold, leaving roadways and especially alleyways (as they do not receive regular city plow/salt) covered in snow and ice. While creeping over hardpacked snow and ice down an alleyway to access my apartment parking area, the dashboard screen suddenly came on in order to display an ad for SiriusXM. I keep th

When it came on suddenly, I experienced a crisis response, slamming on the brakes, resulting in slide forward and to the side. In this instance, I was fortunate not to hit anything or have anything further occur, but it could easily have been very different. Why can an ad server turn this screen on when closed by the owner? Why can it serve ads in private vehicles at all? I attempted to contact the dealer regarding this and they placed me on hold and eventually hung up on me. I have scheduled an appointment with legal counsel to discuss my further options.Read more

I have been trying to register my vehicle in California for several months, however, in order to do so, I need the car to pass SMOG. Initially I was told that the issue was related to an emissions sensor and that I had to drive the vehicle a few more miles to get the sensor to register properly. This turned out not to be the case. A short time later, the fuel gauge completely stopped working.
... I took it to an authorized Audi repair shop that told me the issue was related to a the "Fuel Tank Sending Unit". That part was replaced by the repair shop, however, the issue persisted. The authorized repair shop is now telling me that it the entire tank needs to be replaced in order to fix the issue.. I have searched and this seems to be a common issue among Audi vehicles; the sensor stops working and the gauge stops responding. Audi seems to know that this is an issue. Can this possibly be investigated.Read more

While car shopping, I found a Ford hybrid listed with ~40,760 miles. Its public history shows a severe, unexplained odometer discrepancy. In September 2022, mileage was ~38,353. By May 2023, it spiked to 392,484. The current title shows 40,760 miles marked with a † (dagger), indicating "mileage discrepancy" or "not actual mileage." The seller is marketing it as a low-mileage vehicle. This misre
... presentation poses a serious safety risk. A car with potentially 400,000+ miles of wear is being sold as having only ~40,000. Critical safety components like brakes, suspension, tires, and the hybrid battery system degrade with use. Concealing this extreme wear risks mechanical failure, endangering the buyer and others on the road. The problem is documented in the vehicle history report. I did not purchase the car. This appears to be odometer fraud and title washing, turning a high-mileage vehicle into a deceptively "low-mileage" sale, creating a significant safety hazard.Read more

This incident happened twice in one week first with 2023 Nissan Sentra then replaced with a 2024 Nissan Altima both rented from Enterprise. I was traveling down the street from my house, a two-lane, steep hillside road, which I’ve traveled up and down for 30 years. I stopped behind a parked car on the right side of the road to let another car come up the hill and pass by before I continued down.
... As I continued down the hill, the car just began to accelerate and I could not stop it, I tried to break, but no response and the car kept accelerating for about 50 feet before crashing into. the granite hillside, lifting the car so it landed perpendicular to the road, setting off all airbags and blocking all traffic completely. Thankfully there was no other damage to other cars, homes or people, which was a minor miracle. To reach such an impact speed in such short distance suggests some kind of mechanical failure. My insurance paid for a replacement rental, a 2024 Altima and on my first drive down the hill, in the exact same spot, the steepest of the road, the exact thing happened, rapid acceleration and no breaks.Read more

Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: small fonts, error popups, screen bouncing and being unresponsive. Backup cam shuttering. Unable to modify driving mode,.... Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

To the National Highway Traffic Safety Administration, I am filing a formal complaint regarding a dangerous safety defect in the 2021 Tesla Model Y. On December 2, 2025 at approximately 4:25 PM, my vehicle suffered a sudden, complete electrical shutdown while I was driving, with no warning and no opportunity to reach safety. Critical details: The vehicle displayed an electrical system message,
... then instantly powered down. I lost all ability to steer or move the vehicle, which remained immobilized in active traffic. Hazard lights did not function, leaving the vehicle unmarked and invisible to oncoming vehicles. Electronic systems failed, requiring manual release of the driver door. The incident occurred at dusk and blocked the roadway, placing myself and others at significant risk. This failure presented an immediate threat to life and safety. I have since discovered that other Tesla owners have reported similar abrupt electrical or drive-unit shutdowns, suggesting this may represent a broader systemic defect. I am formally requesting that NHTSA investigate whether this issue meets the threshold for a safety recall and whether Tesla’s systems contain a defect capable of causing sudden, uncommanded shutdown while driving. Thank you for your prompt attention to this matter. I am prepared to provide all supporting documentation, including incident details, screenshots, timestamps, and service correspondence.Read more

In addition, the vehicle failed to accelerate as intended while depressing the accelerator pedal
... . The contact's daughter stated that the engine had seized, and the vehicle had stalled. The contact's daughter was unable to coast to the side of the road. No warning lights were illuminated. The vehicle was towed to the residence. The following day, the vehicle was towed to the dealer, where it was diagnosed with engine failure. The contact was informed that the engine needed to be replaced. The contact was informed that since the first recall repair had not been completed, the manufacturer denied the recall repairs. The vehicle remained at the dealer unrepaired. The contact received a bill from the dealer. The manufacturer was not notified of the failure. The contact's daughter stated that before the engine failed, every time the vehicle was started, there was an abnormal knocking sound coming from the engine. However, after several minutes, the abnormal sound disappeared. The failure mileage was approximately 125,000.Read more

I own a new manual 2024 Ford Mustang GT now with 800 miles. When turning on the unit on 2 occasions, one on November 20 and another on December 3, documented with photographs and videos, the unit presents a black screen with no image. Either in reverse or drive without the possibility of viewing any image. The agency was notified and commented that these failures had never occurred.

“My 2022 Mazda 3 is experiencing an ‘in-network malfunction’ related to the vehicle’s internal communication system. Multiple technicians have told me this is an internal vehicle network/communication fault, not caused by wear or damage. The dealer refuses to repair it unless I pay out-of-pocket, even though this appears to be a manufacturer defect in the electrical network system, (TCU)Th
... is malfunction affects the reliability of the vehicle’s electronics and could impact safety systems while driving. Mazda has not taken responsibility for diagnosing or repairing the issue, and I believe this defect needs to be investigated, as other owners may be affected as well.”Read more

Upon exiting a shopping center parking lot, I turned right onto a street which led up to a 1-lane construction site. I was travelling approximately 15 miles per hour, and drove approximately 50 feet when the car suddenly stalled. No fuel response to gas pedal. It still had momentum, and I was able to steer into a safe side spot. I sat there stunned. The car was still in drive gear. I put it
... into park and turned the car off. After another 1 or 2 minutes, I turned the car on again, and was able to drive it home another 2 miles. I did not have to drive through the construction zone. Had I been in the one lane construction, I could have been rear-ended because a stopped car would not have been expected by drivers behind me. It could have caused severe injury or even death to myself and/or driver coming behind me. This happened on Monday, Nov. 24, 2025, at about 2:30 pm. BTW, this is a hybrid vehicle. That option was not given when starting this accountRead more

I am reporting a serious safety issue with my 2021 Ram 2500 (VIN: [XXX] ). The truck has experienced three separate transmission failures within a few months. Each time the transmission was replaced, the vehicle failed again shortly afterward. At one point, the dealership returned the truck to me without transmission fluid, causing additional internal damage and creating an extreme safety risk. I
... have also had steering issues since the truck was new, which continue today despite multiple service attempts. Shortly after purchase, the truck developed severe mold and water intrusion after the roof stop lamp was removed or stolen while the vehicle was at the dealership for service. This caused water to leak into the interior and contaminate the carpet. The dealership replaced the carpet but the issues continued. The repeated transmission failures, steering defects, and water intrusion have made this truck unsafe to drive. The vehicle has lost significant value, and the dealership and manufacturer have been unable to correct the defects after numerous attempts. I am requesting an NHTSA investigation into these issues, including: •Repeated transmission failures •Ongoing steering defects •Improper repair practices •Water intrusion and mold exposure •Dealer negligence These failures represent a serious safety hazard to both myself and others on the road. My vehicle stooped running. I scheduled maintenance at Metro Ford. According to the diagnostic test completed at Metro Ford, there were powertrain issues. This diagnosis also appears on my Ford Motor Pass app. According to the service department at Metro Ford, coolant was leaking into the engine and that a new water pump and battery needs to be installed to get the car back running. We compli
... ed with the cost ($2,400 this included new water pump and battery) and 30 days later the car stopped running again. The car was towed back to Metro Ford and we were told that the engine is dead and that a new diagnostic check will need to be done with an additional $410 on top of the $2,400 which was paid to get the water pump and other items fixed previously. We were never informed that the engine will need to be replaced after the first diagnostic check. That information should have been told first since you are running the same check that now tells you the engine is dead.Read more

Rear suspension has continued to have issues. First noticed after about 20,000 miles. A clunk and gurgling sound in rear. Took to local dealership and they didn’t do anything about it. I own a 2022 Ram 1500 Bighorn with approximately 84,000 miles. This vehicle has repeatedly experienced a severe safety defect that began around 20,000 miles. On four separate occasions, the truck has suddenly decelerated in traffic without warning, would not shift out of gear, and in some cases would not shut off even when pressing the start/stop button. During the most recent incident, I was unab
... le to pull over safely because the powertrain locked up, putting myself and surrounding drivers at extreme risk. My wife and I as well as a good Samaritan stranger almost got ran over while checking to see what's wrong. Police showed up and called for a tow truck, since the incident was happening in a state hwy. Each time I brought the truck to the dealership, I showed photos and videos of the issue, but the dealership claimed they could not reproduce it. Now the dealership is telling me I must pay $1,127 just for them to look inside the transmission, and potentially $2,800–$11,000 for repairs. This is a safety-critical defect that began well within the warranty period and has progressively worsened. A sudden loss of power and inability to control the vehicle is dangerous and could result in a crash, injury, or death. I believe this issue should be investigated for a recall or manufacturer responsibility due to multiple reports of similar failures in Ram 1500 models. I have videos of the incidents in 2025 at 80k miles, and a video in 2024 at 43k miles that show the live incidents.Read more
