I am reporting ongoing safety concerns with my 2018 RAM 2500 (6.7L Cummins) related to repeated emissions/aftertreatment failures and the lack of timely repair options. These failures directly affect drivability and can create hazardous situations if the vehicle enters derate mode while towing or driving at highway speeds. Over the past year, my truck has suffered multiple emissions-related compo
... nent failures, including DEF dosing issues, SCR/DPF system faults, repeated warning messages, and the threat of speed-limiting derates. The vehicle is used for normal personal use, is well-maintained, and has not been modified. Despite proper maintenance, the same types of failures continue to recur. A major concern is that local RAM dealers have extremely limited ability to service these issues in a timely manner. Appointments are often weeks out, and when the truck is already in a failure state it is unsafe to continue driving. I have previously reported safety-related defects to NHTSA (ABS/HCU), and those issues later resulted in recalls—so I know that reporting matters. The combination of emissions failures, derate risks, and lack of dealer support creates a situation where the vehicle may suddenly lose power or become unsafe to operate, especially when towing or traveling in rural areas with limited service availability. I am requesting that NHTSA review whether there is a broader pattern of emissions-system failures on RAM heavy-duty trucks with the 6.7 Cummins and whether these failures pose a safety risk due to the forced-derate logic and inability to obtain timely repairs.Read more

In October I brought my car in due to my car not correctly going into park. It almost fully gave out the time the recall started for dodge darts gear shifter, though there were times my car wouldn’t go into park and release the key months before. The shop ran codes that mentioned the ignition switch. They replaced the ignition switch hoping that’d fix the problem, but it didn’t. They brough
... t it to a dodge dealership to have it reprogrammed hoping it’d allow the switch to work. After no success they looked further into the problem and dodge confirmed it was the gear shifting cable. When I called dodge to have them look into it more, they let me know there’s nothing they can do and refused the attempt to fix it, saying there’s no remedy. No other codes were mentioned, other than the ignition switch that was replaced / reconfigured by dodge. Dodge confirmed it was the gear shifter by looking at the part. Due to the shop not knowing what to do, and not being able to fix the problem I haven’t had the best experience with being able to obtain documentation. Their receptionist let me know they deleted the file on my car. Since then I’ve reached out asking for proof of documentation from dodge, and called dodge as well, but haven’t been given the documentation I’ve requested many times.Read more

My 2025 Chevrolet Colorado continues to experience roof denting caused by the vehicle’s structural weakness in the roof panel when exposed to normal car-wash air dryers. This problem began in 2023 with my previous model year Colorado, and it has carried into the 2025 model despite GM having issued an internal service advisory in 2023 describing the need to install additional roof braces to preve
... nt this exact failure. GM acknowledged the issue two years ago but did not incorporate the recommended reinforcement into manufacturing. As a result, even routine exposure to the forced air from automated car-wash dryers causes the roof to bend inward. I have had this inspected multiple times, and the root cause has been consistently identified as inadequate roof support. The problem has not been corrected at the design or production level, and dealerships are not taking further action. The roof denting raises concerns about structural rigidity, potential weakening over time, and whether the roof would perform correctly in rollover or impact scenarios. This is an ongoing defect affecting normal use of the vehicle, with no resolution provided by GM despite documented awareness since 2023.Read more

... for service I had them do an oil change I hadn't drove 300 to 400 miles and all of my oil was gone from my car when I called Kia they had me bring it back in couldn't find anything wrong with it and said they want to do a oil consumption after I had already documented oil that I had to put into the car three times in the week up until my appointment because every time I check the car it was empty of oil. Now Kia is telling me to drive a thousand miles on it so they can do an oil consumption however my check engine lights on my oil lights on and it is clearly burning extremely high amount of oil. So in caution of not blowing my engine I added oil to my car yesterday one quart and now my oil light is back on after maybe 20 miles of driving. Subject: Engine Stalls Upon Acceleration from Stop - Near Collision Description: I am experiencing intermittent engine stalling on my 2012 Honda CR-V. When I press the gas pedal to proceed, the engine unexpectedly shuts off and loses all motive power. On several occasions,
... the vehicle has stalled multiple times in succession before it would stay running. In one severe incident, I was attempting a right turn onto a busy road from a stop sign. The vehicle stalled mid-turn, leaving me stranded in the path of traffic. A vehicle approaching from behind nearly rear-ended me because I could not clear the intersection. There are no warning lights prior to the stall. This loss of power at intersections poses an immediate safety risk of collision. I suspect this may be related to the known VTC actuator/timing chain defect affecting this model year.Read more

RAM does over the air software updates. An update they did early summer 2025 was likely for the RAM RHO and not the TRX. A lot of features changed to RHO graphics and the U Connect 5 system became unstable and would not load properly. While in this state, the ability to select vehicle functions was not possible. This makes the system work for a while until it decides to be unstable and not load properly again. It does this at inopportune times and is very annoying, and I feel makes the vehicle unsafe. The sad thing is the vehicle was working nicely before this OTA update and RAM seems extremely slow to acknowledge the issue. Almost all of the graphics now show RHO instead of TRX and one wonders what this has done to the performance of the vehicle since they have different power trains.Read more

The contact owns a 2019 Audi A3. The contact stated that while driving 65 MPH for a 50-mile trip, the message "Transmission Failure" displayed, and the check engine warning light illuminated. The transmission then inadvertently switched from automatic to manual transmission mode and became stuck in second gear. Additionally, the vehicle jerked and decelerated to 35 MPH while driving. The contact a
... ctivated the hazard lights before pulling over to the right shoulder to turn off the vehicle. The vehicle was restarted, but the failure persisted. The contact stated that the vehicle was consuming an excessive amount of fuel very quickly. The contact was able to drive to the destination. The vehicle was taken to an independent mechanic, who retrieved DTC: P173500 (Electrical Malfunction with the Position Sensor for Clutch 1 in the Transmission). The vehicle was then taken to the dealer, but no cause for the failure was found. The contact then drove the vehicle for a long distance at high speeds and was able to duplicate the failure. The vehicle was taken back to the dealer, who determined that the mechatronic hardware, DSG transmission cover, gasket, and fluid needed to be replaced. The vehicle was not repaired. The contact related the failure to Technical Service Bulletin: 2058504/1 (Transmission malfunction, DTC P173500 and/or P173600 stored in the TCM). The manufacturer was made aware of the failure, but informed the contact that the VIN was not included in the Technical Service Bulletin. The failure mileage was approximately 34,500.Read more

📝 Complaint: 2014 Cadillac SRX Dim Headlights Subject: URGENT SAFETY HAZARD AND DEFECT: Severely Dim Headlights on 2014 Cadillac SRX (Affecting 2010-2016 Model Years) – Failure to Act Despite Class Action and NHTSA Complaints Vehicle Information: * Year: 2014 * Make: Cadillac * Model: SRX * VIN [XXX] 4 * Owner/Complainant [XXX] r * Contact [XXX] [XXX] m 1. The Defect and Safety Hazar
... d The headlights on my 2014 Cadillac SRX have become severely and progressively dim, constituting a major safety hazard for night driving. * Hazard: The lack of adequate light output dramatically reduces visibility, compromises the driver's ability to see obstacles, and increases the risk of a severe accident during operation at night, dusk, or in poor weather. This is a critical failure of a core safety component. * Cause: The root cause is a well-documented design flaw in the headlight assembly (affecting 2010-2016 models) that allows moisture to enter the sealed unit. This moisture degrades the internal reflective coating, permanently destroying the light's effective output. 2. Failure to Act Despite Known Issues General Motors (GM) and the appropriate safety regulators have been aware of this critical defect for years, yet no mandatory safety recall has been issued: * NHTSA Complaint History: According to public records, complaints regarding this exact issue have been "pouring in" for the last five years or more. The high volume of complaints clearly demonstrates a systemic, widespread, and dangerous failure across this vehicle platform. * Class Action Lawsuits: This defect has been the subject of class action litigation against General Motors, further establishing that the company is fully aware of the design flaw and the resulting safety hazard. 3. The Unacceptable Repair Burden I am being forced to bear the cost of replacing the defective assembly with a new part that utilizes the same fundamentally flawed design. I had just left my job in Albany and was headed to the Latham area, while merging into traffic on I-90 West my 2020 Honda Accord EX-L started to rattle and slow down, numerous lights illuminated simultaneously on my dashboard. I immediately pulled off the north shoulder and restarted my vehicle to no avail, the dashboard lights were still lit and my vehicles speed and steering were compromised. M
... y safety was put at risk due to the immediate issues as previously described while in heavy traffic, I could have easily been involved in an accident due this issue. The system Keeler Honda advised me that had failed was allegedly the head gasket leaking coolant into the engine by means of cylinder four. I was later informed by Keeler that this is a common issue with the accord 1.5 L turbo engines. The problem has been verified by Keeler Honda. There was no prior warning light on my dash that would have indicated there was an issue with my vehicle, the lights only all appeared once the incident had occurred.Read more

I am submitting a safety complaint regarding my Subaru vehicle’s infotainment system. The system displays unsolicited, full-screen SiriusXM promotional pop-ups while the vehicle is in motion. These ads override the current display without warning. Most recently, a SiriusXM trial offer appeared and took over my entire screen while I was driving over 60 mph on the highway at night. The sudden cha
... nge in brightness and the complete obstruction of the display were extremely distracting. It required my attention to dismiss, creating a dangerous situation at high speed. This is not a one-time occurrence. These ads have appeared multiple times, and I have no option to permanently disable them. I did not consent to advertising in my vehicle, much less advertising that interrupts the display while driving. A vehicle’s infotainment system should never display promotional content that interferes with safe operation, especially while in motion. This behavior poses a clear distraction risk and compromises driver safety. I am requesting that the NHTSA investigate this issue, as manufacturers should not be pushing advertisements that override critical vehicle interfaces during active driving. Thank you for your attention to this matter.Read more

My 2020 Camry has been nothing but a problem and Toyota is noncompliant with helping me. The service recall for the fuel pump was fixed at Vaughn Toyota in Bastrop La .. a week later I still had the same issues. They refused to put another fuel pump on my car even thought the fuel pump had a one year warranty. I then took it to Landers in Little Rock and since I had “codes” I had to pay $180 d
... iagnostic fee. Fast forward to now, still have the same crappy fuel pump. I took it to Ryburn Toyota and was told to replace the charcoal canister and vent valve . That was $1000. Problem not fixed. All my lights stay on, I often get a charging system error notification. It goes into reduced power mode. And has left me stranded numerous times on the side of the road. I want whatever is wrong with my car fixed. I have never owned a vehicle with this many problems. I thought Toyota’s were good vehicles but I promise if I ever get rid of this one, I will not purchase another Toyota vehicle again.Read more
