After loading our two children ages 2 and 5 in to vehicle and securing in car seats, the car doors were closed and would not reopen. The children were unable to open the doors themselves. The trunk and other entries did not open. There was no obvious way to open the doors or the issue the car was experiencing. The battery cover would not open. Eventually after I searched the Internet I determined
... that the 12V battery died (no warning whatsoever, doors worked before loading children) and this caused all occupants to be detained in vehicle as the emergency release is the mechanical means of opening the door and this emergency release is not intuitive or obvious. Within 20 minutes I was able to use an arcane hot wiring method to unlatch a door with a spare 12 V source and a hidden 12 V port not immediately obvious either. Thankfully not in setting of an emergency, not a sunny or excessively warm day and had internet access to research this event. If any of the above were true, life limb and property loss would easily have occurred.Read more

Subject: Safety Complaint: Engine Failure and Stall Risk – 2017 Ford Edge 2.0L EcoBoost Description of Incident: I am reporting a dangerous engine defect in my 2017 Ford Edge (VIN: [XXX] ) involving the 2.0L EcoBoost engine. My vehicle has experienced total engine failure due to coolant leaking into the engine cylinders. This is a known design flaw documented by Ford in TSB 19-2346, yet no saf
... ety recall has been issued for the Edge model. This defect poses a significant safety risk as it can lead to sudden engine stalling, loss of power while driving, and potential engine fires due to overheating. Impact and Financial Burden: My vehicle is currently inoperable and at a repair shop. I am facing a repair estimate of $7,000 – $10,000. Furthermore, my third-party warranty provider (CarShield) has denied the claim, specifically citing that because Ford acknowledged this as a known manufacturing defect via a TSB, they are not liable for the repair. Owners are being left stranded with dangerous, failing vehicles because Ford has not transitioned this TSB into a formal recall or Customer Satisfaction Program (CSP), despite the clear risk of engine failure at highway speeds. At approximately 6:00 PM on December 29, 2025, I was parking my vehicle in the Montague Foods parking lot at 8761 Ferry St in Montague, MI 49437. As I was pulling into the parking space, I had firm pressure on the brake pedal and was not pressing the accelerator. Despite braking, the vehicle suddenly lurched forward unexpectedly. The vehicle struck the parked car in front of me with such force th
... at it moved the large SUV, bounced off, and continued to lurch forward repeatedly. While I was actively trying to stop the vehicle with the brake pedal, it continued moving forward, ran over a curb, and behaved in an uncontrolled, bucking manner. I would describe the experience as similar to a bucking bronco. During the incident, I was yelling, “What is happening?!?” due to the unexpected and frightening behavior of the vehicle. At no point during this incident was the accelerator pedal pressed. No warning lights, alerts, or messages appeared on the dashboard before or during the event that I can recall. Both my vehicle and the parked vehicle sustained damage. Fortunately, there were no injuries. This incident raises serious concerns about the braking, acceleration, or vehicle control systems in this model. I believe this represents a significant and serious safety risk not only to the driver of the vehicle but also pedestrians and other vehicles. I am afraid to drive the car again. I respectfully request that the National Highway Traffic Safety Administration investigate this issue to determine whether it represents a vehicle defect affecting safety, and require Honda to issue any necessary recall or remedy neededRead more

On December 31 at approximately 300 p.m., our 2015 Honda Odyssey experienced a sudden and complete loss of motive power while traveling eastbound on the Sam Houston Parkway South just after crossing the Buffalo Bayou Toll Bridge at highway speed. While driving normally, the Check ABS warning light illuminated. Immediately afterward, multiple dashboard warning lights began flashing and the engine a
... bruptly shut down with no warning. The vehicle lost all ability to accelerate and became disabled in active traffic lanes. Steering and braking were severely affected due to the sudden loss of power. Hazard lights were activated while the driver attempted to move the vehicle from the middle lane to the shoulder. We narrowly avoided a collision and were able to stop near the Pine Avenue on-ramp. Once stopped, the vehicle completely shut off and would not restart. All vehicle systems appeared inoperative except for the hazard lights, indicating a possible electrical system failure. Emergency services were required due to the dangerous location. The vehicle was occupied by five people, including three children. The sudden loss of motive power at highway speed created an extreme safety hazard and could have resulted in serious injury or fatality.Read more

According to the mechanic to whom we took the vehicle, there was a massive short in the mechatronics component of the transmission. The mechanic also informed us that there was no proactive action we could have taken prior to this incident to prevent its occurrence. The car and its components are still available for inspection. The short caused the vehicle to immediately lose power while being
... driven between 60 and 65 mph on an Interstate Highway, and the vehicle had to be pulled over immediately. No warnings were provided before the short occurred but the failure resulted in the generation of a warning message after the fact. The vehicle had been serviced recently, including a basic inspection of the vehicle's electronics and transmission. I am unaware of the problem having been reproduced by a dealer or independent service center, but having researched the issue I discovered that other owners of similar make, model, and year numbers have experienced similar issues. Neither the vehicle nor its components have been inspected by the manufacturer, police, or insurance representatives, though, as noted previously, we did have a mechanic inspect the vehicle and faulty transmission to determine the cost of fixing/replacing the transmission. I am a software engineer and owner of two 2020 Tesla Model X (Raven) vehicles. Both exhibit a serious safety defect after installing firmware 2025.44.25.4 with FSD v12.6.4. Vehicle configuration: Model S/X with MCU2 (Intel Atom) and Hardware 3 (HW3). Defect description: When Full Self-Driving (FSD) is engaged, the vehicle intermittently enters a critical failure state while in motion: • The i
... nstrument cluster (driver display) suddenly goes completely black • The center screen issues a red “Take Control Immediately” alert with alarms • The infotainment computer performs a watchdog reset and reboots (Tesla logo appears) while the vehicle is traveling at highway speeds This has occurred multiple times on both vehicles. Suspected cause: The 2025.44 software update appears to introduce additional dashcam telemetry writes (speed, steering, FSD state) to the USB device while recording video. On MCU2 (Intel Atom), this creates an I/O and CPU bottleneck that starves display and safety-critical threads, triggering an operating-system watchdog reboot. Disabling Dashcam immediately stops the crashes, indicating a resource-exhaustion condition rather than driver error. Safety risk: During the blackout and reboot, the driver loses all visual feedback from the instrument cluster while being instructed to take control. This creates a high risk of collision, especially at highway speeds or in complex traffic environments. Severity: Loss of driver display and system reboot while driving constitutes a safety-critical defect. This condition could lead to serious injury or death. I request NHTSA investigation into instrument-cluster blackouts and system reboots on Tesla HW3/MCU2 vehicles when FSD and Dashcam are enabled.Read more

On Thursday, December 18, 2025, while driving approximately 70 mph on US-30 in Indiana, my vehicle suddenly displayed a warning indicating speed would be limited and that I needed to place the vehicle in Park and restart it. At the time, I was traveling in the left lane with semi-trucks beside and behind me. The abrupt loss of speed and ability to accelerate created an immediately dangerous situat

After placing the vehicle in Park and attempting a restart, the vehicle did not recover as expected and could not be shifted out of Park. I was stranded on the shoulder of a high-speed highway with heavy truck traffic. I have an open case with Jeep Customer Care and have made numerous calls. I have been routed between Customer Care, the Recall team, and Roadside Assistance. The vehicle currently has multiple open recalls, including those for potential power loss due to software defects, engine contamination with debris that can cause failure, fire risk, or loss of drive power, and fire risks from the hybrid battery system. No representative can confirm whether these recalls are related to my failure, despite the symptoms (sudden speed limitation, failure to restart, and inability to shift out of Park) closely matching descriptions in the power loss and engine failure recalls. On December 18th I was promised a case manager callback within one business day. That did not occur. I called again 12/22 and was routed to Roadside Assistance again, which cannot help me, and then to Recall and I just received a supervisor callback from the Recall department, and he provided no additional resolution. I have repeatedly requested escalation in all of my calls and have been continuously told there is no escalation. The dealership refuses to perform the 68c recall for my 4xe wrangler, [XXX] They said they are worried that there will be a glitch with the recall and they will have to hold on to my jeep for 3-4 weeks until a battery replacement is available. They don’t want to do this because there are already 3 cars in the dealership awaiting the same battery replacement. Given the seriousness of the recall

I am reporting a serious safety concern with the rear backup camera on my 2019 Audi e tron. The camera provides extremely poor visibility at night and in low light conditions, to the point that it is effectively unusable. The image is very dark and grainy, making it difficult or impossible to see pedestrians, vehicles, curbs, or other obstacles while reversing. This creates a significant safety h
... azard, especially in residential areas, parking lots, and garages where pedestrians may be present. The issue occurs under normal operating conditions and is not related to dirt or obstruction on the camera lens. The problem has been consistent since ownership and does not appear to be an isolated incident. Given that backup cameras are federally mandated safety equipment, this level of nighttime performance undermines their intended purpose and increases the risk of accidents, injuries, or property damage. I believe this may represent a design or hardware defect that warrants investigation.Read more

I completed my 70,000 maintenance on my vehicle on Monday. Friday when I got in my car after shopping, the dash light for parking brake kept coming on. When I put it in drive, the engine light, park assist light, collision assist light, and multiple warning lights kept cycling on my car’s screen. I drove back to the dealership and they kept the car from Friday morning until Monday afternoon befo
... re a diagnosis was handed to me. The write up states that the vehicle is “is slipping in every gear” due to “internal mechanical failure.” I am at 70,000 miles and I purchased that car on Jan 4, 2021. Therefore I am out of warranty. I have only had the dealer work on my car except for purchasing tires from an outside source. This is a catastrophic failure of a part that should not be failing. The tech is not even guaranteeing that this will fix the problem.Read more
