My vehicle had no warning lamps, messages or any other symptoms before driving that day. Vehicle was well maintained. Driving on a rural straight paved road that is in excellent condition, at approximately 55 miles per hour, smoke suddenly came through the dash top (defroster along windshield) and bottom (underneath the dash - heavier volume of smoke); almost immediately, the horn started soundin
... g independently, a message appeared that read engine failure, brakes failed simultaneously and could not stop the vehicle. Only possible way to get the vehicle to stop was to remove the ignition key, put lever in park, pulled park break up. Flames now under the dash & smoke was pouring out from under the hood. My safety was at risk and had to exit the vehicle as fast as possible from drivers side with cars approaching. After exit, saw flames in the tire well also; witness saw flames under the car therefore he was yelling "get away from the car, it's on fire". Got away as fast as possible. The witness called the fire department who arrived extremely timely (and police even sooner). FD attempted to put out fire with water first, then needed foam as it ultimately turned into a gas tank fuel fire. The vehicle was not inspected by a dealer since there was nothing to speak of left. The Police, Fire Department, Tow Company, and also the Insurance Claim Inspector did view and/or inspect the remnants of the vehicle. The insurance inspector took possession of the remnants of the vehicle from the tow company. My safety was extremely put at risk as I could have died from many factors including brake failure, smoke inhalation, burns from the fire, getting hit from another vehicle upon exit from the burning vehicle, or heart attack.Read more

My name is [XXX] , and I am filing this safety complaint regarding my 2025 Toyota Sienna, VIN [XXX] , subject to Recall 25TA12. The vehicle has been under recall for a safety issue related to second-row seat rails, and I have been informed by both the dealership and Toyota representatives that they cannot guarantee the vehicle is safe for transporting children. I continue to pay full monthly fina

They financed my car) for this vehicle, which I cannot safely use as intended (for transporting my four children), and the dealerships in my area do not have suitable loaner vehicles. I am filing this complaint to document the ongoing safety risk and the financial and logistical burden this situation has created. I request that this recall and its handling be reviewed, and that appropriate action be taken to protect consumers. Thank you. So just recently I was on my way home from my parents house, and I take a highway to get there from my home. It’s a direct shot, fastest way to get to and from. I was merging right to get in the turn lane to the highway I take, and I heard a click, and my steering wheel felt like it locked up. I look at the dash, and see it says “power steering” and then “ESC OFF”. The turn to merge onto
... the highway is a pretty sharp turn over a very steep hill. I had to force the steering wheel to the right, and crush on the brakes to get on the side of the road, where I could stop and inspect. I inevitably ended up giving up, because it’s freezing outside and I am not a mechanic. But it didn’t happen the rest of the way to my home. Since then though, it happens every time I drive the car. Turning the car off and on, while moving the steering wheel from left/right seems to temporarily fix the issue. Until the cars is idling again, and then it clicks off. I have a video of my steering wheel shaking and moving on its own, but this website only lets you upload pics or files. This started the next time I drove the car. Which was the day after the incident. I have not had any service done to it, as I can’t afford it. It is available for servicing upon request. I have had an check engine light and ABS light on for sometime. But I think that’s because the warning lights weren’t reset after my dad changed my oil, and my brake pads.Read more

I am the single owner of this 2017 Ford Escape with an Eco-boost 2.0L engine. This vehicle has only been serviced by Ford dealerships. at approximately 60K miles I noticed a lag in acceleration between 1200-2200 RPM. At 88K this lag became more violent, and I took the car to a Ford dealership for diagnosis. No warning lights had illuminated. The ford dealership advised me that there was cool
... ant intrusion in the cylinders, and an entire new long block engine was required. After researching I learned that the 2017 had a design flaw that allows coolant to leak into the combustion chambers. in 2020 Ford redesigned the engine from an open deck to a closed deck. Due to Fords design flaw, I required a completely new long block engine and was able to confirm it was the new closed deck design at a cost of $14K. This problem boils down to the engine's open deck cooling design, which causes premature gasket failure and sometimes cracks between the cylinders. These problems don't affect the earlier versions of these engines due to their closed deck system, which was used prior to the introduction of the 2017 Escape. Ford has acknowledged this problem, but the fix isn't a simple one - it requires a long block replacement, which is obviously a labor intensive and expensive job. How to we get Ford to be responsible for the costs associated with changing the engine out from their flawed design? This could have been an on highway incident since there were no warning lights or other visible issues.Read more

I am reporting a potential safety-related defect involving the high-voltage battery system and associated diagnostic software in a 2023 Volkswagen ID.4 Pro S AWD. The vehicle has experienced abnormal battery behavior, including accelerated degradation and confirmed system faults affecting battery management, charging, and vehicle communication. An independent EV diagnostic test shows the high-vol
... tage battery at approximately 91% state of health, representing about 9% degradation in under two years of normal use. The testing facility stated this rate of degradation is abnormal for the vehicle’s age, where typical expectations would be significantly lower. In addition, vehicle diagnostics show confirmed and recurring fault codes involving critical systems, including the CAN Gateway (inter-module communication), the high-voltage battery charger/battery management interface, and the emergency call and communication module. These faults are repeatedly logged as “confirmed” and “test failed since last DTC clear,” indicating persistent system-level issues rather than intermittent events. These systems are responsible for battery condition reporting, charging control, communication between vehicle control modules, and safety/telemetry functions. Limitations or failures in these systems raise concerns that battery condition and integrity may not be accurately detected or reported to the driver. Volkswagen dealerships have declined to perform the definitive manufacturer-certified battery capacity test, stating it will not be authorized unless battery capacity is already below warranty thresholds. As a result, more accurate and intrusive testing is unavailable due to manufacturer policy rather than lack of concern. I am also aware of an independent investigation released in January 2026 documenting production-related battery cell defects in Volkswagen Group MEB vehicles from model years 2021–2023, involving increased battery self-discharge and diagnostic measures that mayRead more

Incident date: Jan 11, 2026
While driving 75MPH, the vehicle shut down with a message to pull over. All acceleration was lost. We had to navigate, without warning, across 3 lanes of traffic to pull over on the right side of the highway. This was EXTREMELY unsafe as we were slowing down and essentially cutting off other vehicles who were honking at us as this was a traffic hazard. All 6 members of my family were in the ve
... hicle. We had this happen another 3 times before arriving home. I took it to my local dealer, who could not replicate it. They performed a software update, which I have since learned would NEVER fix this problem. We had it happen to us 5 more times, so I took it to another dealership, who initially could not replicate it. In all cases, we had the following check engine light codes be triggered. I have screen shots of them from my personal OBDII reader: P0A43 "Drive Motor 'A' Position Sensor Circuit Intermittent", C2225 "Status: MIL - PCM Disabled EDU: No Sub Type Information". After doing my own reading, it seemed many others have experienced the exact same symptoms and it has required a transmission repair/replace. I also learned most people had a battery charge on the high voltage battery (as this is a plug-in hybrid) when the vehicle shuts down. I charged my van, took it for a ride at 75MPH and it occurred within the first mile. I took it back to the dealership, and at this point, I refuse to drive it until the hardware fix happens. It seems like this should be a recall for all vehicles that can have this problem. It is EXTREMELY unsafe as it occurs at 70MPH+ speeds.Read more

For several years I contacted Ram dealerships and the manufacturer regarding this issue and was repeatedly told that repair parts were unavailable. During this time, widespread reports of identical failures were being discussed on owner forums and among dealerships. A federal safety recall was eventually issued for the ABS control module, and I received my official recall notification letter approximately one year ago. Despite the recall being active, the manufacturer and local dealerships continue to state that no replacement parts are available and that there is no estimated timeline for when a remedy will be provided. As of today, my vehicle remains unrepaired and continues to operate with disabled braking safety systems and constant warning alarms. This is a heavy-duty truck that is regularly operated in Iowa, where inclement weather conditions including snow, ice, and heavy rain are common. 10R80 ten speed automatic transmission first exhibited indications of failure 9/16/2020, Ford's response being reprogram PCM. Transmission has progressively declined in reliability complete with harsh shifts, jerking, erratic up and downshifts, suddenly reverting into neutral at highway speeds. Safety of self, wife and other family members imperiled due to the vehicle suddenly lurching, jerking,
... and slamming forward in an unsafe manner at highway speeds. Ford dealerships have reprogrammed the transmission PCM four times over several years with progressively worsening transmission functionality. Dealership service specialist has confirmed that the transmission failure is due to internal components with burnt fluid as a component. But, Ford will not allow them to open the transmission up, denied repeatedly by the manufacturer as the issue with this 10R80 transmission is widespread with thousands of complaints filed, sone with NHTSA and others online and with Ford Customer Service to no avail. Ford is denying this manufacturer's defective transmission to avoid financial liability likely in the multi million dollar range over a wide array of Ford products with this faulty transmission installed in them. During the four times that Ford dealerships have reprogrammed the transmission's computer at the behest and mandate of Ford Motor Company, this vehicle's transmission repair was covered by both the manufacturers warranty as well as a privately purchased Ford Extended Service Plan (Extra Care). Ford chose deny coverage under either warranty though it is warrantied under both plans explicitly. This failure can result in vehicular accidents because of transmission's erratic and flawed functionality. Stranded on an interstate highway due to this failure especially is dangerous and may leave the vehicle exposed to collision.Read more

... fe place to pull off the road. It was a traumatic experience as a large truck swerved into the other lane of traffic to avoid hitting me, almost causing an accident. Luckily I made it home safely. I had a mechanic look at it the next day and he said the engine needed to be replaced. Not convinced of the diagnosis, I had the car towed to another mechanic who confirmed the engine would have to be replaced. I had no issues with the car or the mechanics of the car prior to this. I could not for the life of me understand how my 2017 Acura MDX with only 74,000 miles on it, could possibly need a new engine unless it was defective. So I then towed the car to the Acura dealership where they confirmed, the engine needed to be replaced. However, when I asked about the engine recall, I was told my VIN is not included in the recall although it is the same exact issue. Acura did offer a one time goodwill assistance for 25% of the repair costs if performed at Acura. Admission of guilt knowing my car has a defective engine is what that says to me about Acura's offer of 25%! Since the car was useless without a engine, I have since had it repaired by an independent mechanic. I'm very disappointed in Acura.Read more
