I am submitting this complaint regarding a safety-related incident involving my 2023 Hyundai Santa Fe on February 14, 2026. While driving, the vehicle suddenly began to overheat, creating an immediate safety hazard. I was close to my home and was able to return there safely, but once parked, I observed coolant leaking from the front of the vehicle. Due to the risk of further overheating or engine
... failure, the vehicle was not drivable and had to be towed to a local Hyundai dealership. At the time of the incident, the vehicle had fewer than 38,000 miles and was purchased brand new. Despite this, the dealership informed me that the repair would not be covered under warranty, claiming the radiator damage was caused by “road debris.” However, they did not provide any evidence of an impact, puncture, or external damage. The only documentation offered was a video showing coolant spewing from the radiator after a technician inserted an object into the grille. This video does not demonstrate any pre‑existing road damage or confirm that debris caused the failure. I requested proof of actual damage multiple times, including photographs or inspection notes showing an impact point, but none was provided. Without verifiable evidence of external damage, I am concerned that this may represent a premature component failure or a potential safety defect in the cooling system. The sudden overheating while driving, combined with the lack of substantiated cause, raises significant safety concerns for me and potentially for other owners of this model. Thank you for your attention to this matter.Read more

Vehicle: 2014 Toyota Prius v Hybrid Mileage: 79,000 miles The vehicle experienced sudden failure of the brake booster / brake accumulator system. Multiple warning lights appeared including ABS, traction control, and the red brake warning light. Diagnostic code C1391 was retrieved. The brake pedal became soft/spongy and the electric brake pump began cycling frequently. Braking performance felt re
... duced and inconsistent, creating a potential safety risk. This vehicle has very low mileage (79,000 miles) and has been well maintained. The failure appears to be related to a known defect in the Prius braking system. Toyota previously issued Customer Support Program 20TE07 addressing brake booster and pump failures on 2012–2014 Prius v vehicles. Despite this known issue, the repair is not being covered and the cost quoted is several thousand dollars. A failure of the brake booster/accumulator system significantly affects braking performance and should not occur at this mileage. This appears to be a widespread issue affecting Prius vehicles and represents a potential safety defect involving the braking system.Read more

Subject: Safety Concern – Backup Camera Failure Causing Complete Safety System Malfunction (2022 Kia Telluride) I am reporting a safety concern regarding a 2022 Kia Telluride related to the rear backup camera assembly. The backup camera failed due to water infiltration through the camera seal, which entered the camera housing and caused an electrical short. When this occurred, the failure did
... not only disable the backup camera but caused the entire vehicle safety system to malfunction, disabling multiple safety features until the issue was repaired. When we brought the vehicle to the dealership, they stated that this was not the first Kia Telluride they had seen with the same issue. The quoted repair cost from the dealership was approximately $1,200. I ultimately replaced the camera myself, which restored the safety systems to normal operation. I have retained the failed camera assembly, which clearly shows the location where the seal failed and water entered the unit. Because the failure of this component disables multiple vehicle safety systems, I believe this may represent a broader safety concern if it is occurring in other vehicles. I am submitting this report so the issue can be reviewed for potential defects affecting other vehicles. Vehicle information and additional details can be provided if needed. Thank you. I purchased this vehicle just over a year ago as a CPO model with only 8,000 miles on it. It now has just over 23,000 miles. In this short time of ownership, it has been back to the dealership 3 separate times for serious EV system issues. The first visit involved a software update intended to correct a 12-volt charging issue that could prevent the vehicle from charging and potentially cause ICCU
... failure. Shortly after that update, the 12-volt battery failed, despite being <18 months into service, completely bricking the vehicle and leaving me stranded waiting for a tow. The battery was replaced under warranty. Less than 2 months later, while >30 miles from home, the vehicle displayed a “Check Electrical System Failure” warning. I was able to limp-mode the car back to the dealership. I am now being told that the main EV battery has failed and must be replaced, possibly also the ICCU. The dealership cannot provide a firm timeline for repairs; parts availability could mean anywhere from 2 weeks to 4 months without the vehicle. This pattern of failures raises serious concerns about the reliability and safety of the EV6. Repeated electrical system issues create a very real risk of being stranded unexpectedly, potentially in unsafe situations such as on the side of a highway. To date, I have already lost four days of use due to service repairs, and I now face being without the vehicle for an extended and uncertain period. While a loaner vehicle is being provided, I will incur fuel expenses at a time of rising gas prices, undermining the financial rationale for purchasing an EV. Meanwhile, I continue making payments on a vehicle I cannot use, which is depreciating in value. I understand that new technology carries some risk. However, despite prior updates and even a model refresh, the ICCU issue appears unresolved, and Kia does not seem to have a clear root cause identified. For a vehicle marketed on innovation and dependability, this experience has indicated otherwise.Read more

Approximately one to two months ago, the driver-side front door latch on my 2017 Tesla Model X malfunctioned and prevented me from exiting the vehicle through the driver door. I first attempted to open the door using the manual interior door handle (manual mechanical release), but the door did not unlatch. I then attempted to open the door using the electronic door release, but it also failed to
... open the door. When attempting the electronic release, the door actuator clicked multiple times but the door did not open. Even while pushing outward on the door while activating the release, the latch would not disengage. Because the driver door would not open using either the manual or electronic release mechanisms, I exited the vehicle through the passenger side door. Later, after applying significant pressure to the driver door and repeatedly attempting to open it, the door eventually opened. Tesla service has indicated that earlier front door latch assemblies require a retrofit and that both front doors must be retrofitted. I was given an estimated cost of approximately $1,500 or more for the latch retrofit plus an estimated diagnostic fee of approximately $200 or more. I am concerned this represents a safety defect involving failure of both the electronic and manual door release mechanisms, which prevented normal occupant egress through the driver door.Read more

Incident Description: I am reporting three recurring safety hazards with my 2019 Ram 1500. 1. Intermittent ABS/ESC Error: The vehicle recently displayed an ABS error message on the dashboard. The error cleared itself after the vehicle was power-cycled (shut off and restarted). This intermittent failure of the Anti-lock Braking System and Electronic Stability Control is a severe safety risk, as
... the system could fail during an emergency braking event. My VIN is currently excluded from Recall 24V-653, despite my vehicle exhibiting the exact symptoms described. 2. Unintended Forward Lunging: Before reaching operating temperature (cold start), the truck abruptly lunges forward while idling or under light braking. This occurs without driver input and has been reported by numerous other Ram owners as a cause for rear-end collisions. 3. "Shift into P than Desire Gear" Error: While coming to a stop, the dashboard displays "Shift into P than Desire Gear," and the transmission slips into Neutral. This causes a sudden loss of motive power in traffic. I am aware of reports where this has caused vehicles to "lock up" or stall at higher speeds. Safety Risk: The combination of an unpredictable transmission (lunging/slipping to neutral) and an intermittent ABS failure makes this vehicle unsafe to operate. The manufacturer's current recall list for the 2019 model year is clearly incomplete, as my vehicle is suffering from these documented defects but remains ineligible for a recall repair.Read more

Water intrusion has been observed on the rear seating surfaces and interior trim during rain and snow events. The condition has been reproduced multiple times during precipitation and car washes. A dealership inspected the vehicle and performed a sealant
... repair following manufacturer guidance; however, the water leak persists and the crack remains visible. Water entering the passenger compartment presents potential safety risks, including moisture exposure near electrical components, mold growth, and occupant health concerns. Water has entered the rear seating area where child passengers ride. No warning lights or electronic alerts appeared prior to the condition. The issue appears related to a structural failure of the rear backlite frame seal. The condition remains unresolved and may worsen over time if not properly repaired. The component is available for inspection upon request.Read more
