I am writing to formally report a serious and ongoing issue with my 2022 Audi A5 involving multiple component failures. Most recently, both front windows suddenly stopped functioning and would not roll up, leaving my vehicle fully exposed. As a result, my car was subjected to rain, causing water intrusion and damage to the interior of the vehicle. This issue is especially concerning given my prior
... history with this vehicle. Since purchasing the car, the sunroof has already required repair twice within the past two years. Now, I am experiencing a complete failure of both front windows. This pattern of repeated issues raises serious concerns about the overall reliability and quality of the vehicle. I rely on my vehicle daily to transport my children and manage my responsibilities. Due to this failure, my vehicle is currently not secure or usable in normal conditions, which has placed me in a very difficult position. Additionally, I have been informed by an Audi technician that this is a known issue with Audi A5 models. Given that this defect has been acknowledged, it is extremely concerning that no preventative solution or recall has been implemented to protect consumers from this exact situation. This failure is unacceptable in a vehicle at this price point and presents a risk for further damage, including potential electrical issues due to water exposure. I am requesting that Audi of America take full responsibility for this matter and provide coverage for: Repair of the window system Any resulting interior or electrical damage caused by water intrusion A thorough inspection of the vehicle to ensure no additional damage has occurred I would also like this issue formally documented as part of any ongoing investigation into known defects affecting the Audi A5. Please provide a prompt response with next steps toward resolution. I can be reached at your earliest convenience.Read more

Over about seven years and roughly 70,000 miles of ownership, this 2012 BMW X3 generated repeated “Drivetrain malfunction” warnings and loss of drivability despite extensive repairs by multiple authorized BMW dealers. I paid approximately $15,000–$20,000 out of pocket for dealer repairs, and my third‑party service contract (CarShield) paid an additional roughly $15,000–$20,000 directly t
... o these BMW dealers during this period. In December 2025, an authorized BMW dealer diagnosed a drivetrain malfunction as failure of the fuel pump control module, allegedly caused by a small water spill on the rear seat, charged me $1,690.71 to replace the module, and returned the vehicle as repaired. Shortly afterward, while driving normally, the same “Drivetrain malfunction” warning reappeared, and drivability issues returned, indicating the defect was not corrected and raising serious concerns about the safety and reliability of the drivetrain and related electronic controls. The dealer’s attribution of the failure to minor interior water exposure also caused the service‑contract administrator to void coverage for the fuel system, leaving me with this large repair bill for an unresolved safety‑related condition that ultimately forced me to dispose of the vehicle at a significant loss.Read more

The internal core of the Exhaust Heat Recovery System (EHRS), part of the center muffler/front pipe assembly (#28600-L5210), suffered a catastrophic failure. This allowed approximately 9 gallons of engine coolant to leak directly into the hot exhaust stream. Safety Risk: This failure creates a severe visibility hazard. During acceleration from stop lights and stop signs, the vehicle emits massive,
... thick clouds of white glycol smoke. The smoke is so dense that it completely obscures the driver's view of trailing vehicles and prevents following drivers from seeing the roadway or my vehicle's brake lights, creating a high risk of rear-end collisions. Additionally, the rapid loss of coolant caused the engine to overheat and lose power while in motion, nearly causing a stall in active traffic. Confirmation: The failure was confirmed by the service department at Washington Hyundai (Invoice #174281). This is the second failure of this specific component (previously failed in Jan 2025). The manufacturer issued TSB 23-EM-009H for this defect, acknowledging it affects 2020-2023 Sonata Hybrids, but no safety recall has been issued. Symptoms: The issue was preceded by a "sweet" burning smell and a sudden "Engine Overheating" warning on the dashboard. The failed component has been replaced by the dealer, but the manufacturer refused to replace the coolant-poisoned Oxygen (O2) sensors, leaving the vehicle in a technically compromised state. The failed unit is likely held by the dealer for warranty parts return.Read more

1. On multiple occasions, the adaptive cruise control has slammed the brakes at freeway speeds with no vehicles in front or next to me. The dashboard display showing vehicles in proximity indicated no new vehicles were sensed during these incidents (meaning the automatic braking was randomly triggered with no visual or audible warning). On one occasion at night, I was in the left lane of the freew
... ay at going 75mph when this system slammed the brakes, slowing me to almost 40mph, causing the car behind me to have to swerve into the left shoulder to avoid rear-ending me. 2. The lane-keeping assistance will randomly start steering the car towards the edge of the lane. Sometimes it will actually steer the car over the lane markers, triggering the lane-departure warning, which proceeds to fight the lane-keep assist’s steering to keep the car in the lane. This will even occur after having steered the car correctly in the middle of the lane for some time. This poses a risk when the current lane is adjacent to a highway median, when there is a car in the lane next to me, or when there is no shoulder on a highway. I have had the ADAS systems on my 2023 Acura Integra recalibrated twice in the last 4 months and neither issue has improved. This is a major safety issue with Acura’s ADAS system which I have not experienced while driving multiple cars of other brands with the same feature set.Read more

I purchased this vehicle in June 2024 as a demo vehicle with approximately 7,000 miles on it. Within one month of ownership, the vehicle began experiencing serious problems that have continued to worsen. The vehicle currently has approximately 34,000 miles, and the issues have been ongoing from the time of purchase through the present. Since purchasing the vehicle, it has been in the dealership mu
... ltiple times for the same recurring problems, including but not limited to: * Powertrain issues and loss of power * Reduced acceleration while driving * Complete electrical failure while driving * Engine and electrical warning lights repeatedly turning on * Vehicle running excessively hot * Water pump replacement * Replacement of sensors * Thermostat replacement * Ongoing engine and electrical system malfunctions Despite these repairs, the same problems continue to reoccur. Each time I return the vehicle for service, I am informed that it is a “different issue,” even though the symptoms remain unchanged. This strongly indicates a defective powertrain and/or electrical system that has not been properly repaired. The most serious concern occurred while I was driving on public streets, when the vehicle lost acceleration, electrical power shut off, and the vehicle completely shut down, nearly causing me to be hit by other vehicles. This incident created an extreme safety hazard for myself, my passengers, and other motorists. I have made several formal complaints directly with GMC, and more than four case numbers have already been created regarding this vehicle. Despite these complaints and repeated repair attempts, the problems persist. Given the repeated failures, numerous repair attempts, documented safety risks, and the fact that these issues began shortly after purchase, I believe this vehicle qualifies for relief under applicable lemon laws and consumer protection laws. When driving on the highway going 65mph the car slipped in neutral and error showed on dashboard "transmission malfunction stop." I could steer and break, but not accelerate. Pulling over on side of highway, I was able to put in park and then drive. I pulled off the exit and the car did the same thing within 10 mins. Car was towed to Mercedes dealership in Warwick, RI. I was made aware after
... inspection that there was nothing wrong with the vehicle nor the transmission, but rather there was a needed software update that was causing the issue that had been identified previously. Regular service was due, but no indication whatsoever of the error I saw nor any other error codes. No error showed up for the technicians in inspecting the car. I was further informed, after research, that the software update was being drafted and that it would have to pass NHTSA governance. So I'd like to know if this is accurate? If NHTSA is aware? If the software update is sitting with you for approval? What is Mercedes obligation to share this with drivers noting they were aware of issue? If my [XXX] was driving she could have been killed. UNKNOWN exact component, but it is a critical failure within the electrical/charging system or the 48V eTorque system. The dealership previously suspected the eTorque system and replaced the belt tensioner, but the failure continues. The vehicle is currently available for inspection and has been sitting at an authorized Ram dealership for the last three weeks. The vehicle suffers a complete and s
... udden loss of motive power while driving. When it stalls, the truck automatically engages "auto park" and applies the parking brake. This has happened repeatedly while pulling into active traffic and while passing through busy intersections (most recently in downtown Chicago). This sudden, uncontrollable stop in the middle of an intersection creates an extreme and immediate risk of a severe rear-end or T-bone collision for myself, my passengers, and surrounding motorists. Yes, the dealership was able to successfully replicate the stall while their technicians were driving it. But I don't think they had any type of computer hooked up to it. However, the attempt at repair during the second service visit involved replacing the eTorque belt tensioner based on my description of the failure, acknowledging that a slip could cause the issue. They have not been able to replicate during this last visit. Despite putting over 400 test miles on the truck during the current (third) visit, they cannot get the intermittent failure to occur while hooked up to their diagnostic equipment. Yes, the vehicle has been inspected multiple times by an authorized Ram dealership. It has been in their possession for over six weeks in total across three separate service visits.Read more

2025 Chevrolet Blazer EV AWD purchased 5/23/25 (under 3,100 miles) The vehicle's Telematics Control Module (TCM) has failed, disabling the built-in navigation system, OnStar services, remote start, and My Chevrolet app connectivity. The dealership confirmed the TCM needs replacement and the part is on backorder with no ETA. This is a known issue affecting this model. I am reporting this as a saf
... ety defect for the following reasons: The 2025 Blazer EV does not support Apple CarPlay or Android Auto. With the built-in navigation disabled, the only way to navigate is by using a phone, which requires visual attention to a separate small screen while driving. Under Pennsylvania's hands-free law (75 Pa.C.S.A. § 3316.1), use of handheld devices while driving is a primary offense, making phone-based navigation both unsafe and legally problematic. On at least one occasion, the navigation system appeared to be functioning normally at the start of a trip but was providing materially inaccurate directions mid-route (e.g., instructing me to turn onto a road I had already been traveling on for miles). A system that displays as operational while delivering incorrect routing data is dangerous because the driver has no way to assess its reliability before departing. GM's EV Concierge characterized the loss of navigation as a non-safety feature issue and suggested using a phone holder. This response is inadequate given the vehicle's lack of CarPlay integration, which makes the built-in navigation the only safe, integrated means of route guidance. This vehicle has also required three prior unrelated warranty repairs in 10 months: a recall for a faulty rear parking brake harness (9/25), a 12V battery failure requiring flatbed towing (11/25), and a faulty tire pressure monitor (12/25). The pattern of recurring failures across unrelated systems at minimal mileage indicates a systemic quality issue with this vehicle.Read more

My 2025 Honda Odyssey has had trouble turning on since it has been about 9 months old. It doesn't always turn on from being off. I took it in for service and the dealership replaced the battery. It continued to not start. T
... he new battery did not improve the symptoms. I scheduled another appointment for service and the dealership kept lying and gaslighting me into believing that they did something to improve the symptoms. They claimed to have updated the computer system software/firmware versions. I asked which ones were updated and they said that none were, it was all up to date. They claimed to have "reset" the auto-start-stop system. I asked how that would improve performance when the vehicle doesn't start from being off as well. The vehicle is unsafe to drive. It turns off at stop lights, stop signs, in medians when I am trying to make turns. The engine not restarting in parking lots from a complete stop is also very concerning. I hate and fear my Honda and I am scared to drive it. I have taken it to multiple dealerships to get it fixed and none will do anything. I have a video recorded after this occurred at a red light. The vehicle has now been out of service for approximately 71 days. Most recently, the authorized dealership confirmed that the required parts to complete the repair are unavailable, there is no estimated timeline for parts availability, and the repair cannot be completed at this time. As a result, there is currently no repair plan in place. This situation raises safety concerns due to the loss of a critical safety feature (rear visibility via the backup camera) and the presence of unresolved electrical system damage. Damaged or compromised wiring may create the potential for electrical malfunction, system instability, or other hazards if the vehicle were to be operated. The inability to repair the vehicle, combined with the lack of available parts and absence of any repair timeline, raises concerns about the overall safety and reliability of the vehicle. The vehicle remains at the dealership and is available for inspection.Read more

The vehicle developed a leak in the AC condensate drain. The leak is directly above the safety restraint control module. The module got wet deactivating the vehicle and causing havoc with the airbag system. Many electrical systems turned off such as the power to the steering wheel buttons( the left blinker, the right blinker, and the horn). Luckily this happened while my vehicle was parked but i
... f this happened while someone was accelerating or driving on the highway they would lose control of the vehicle. I spoke to a Tesla Service Tech about the concern and he informed me that this AC leak happens very often in the 2021 and newer Tesla Model S vehicles and that Tesla is aware that the AC drain leaks onto the Restraint Contol Module but has not created a technical service bulletin or a recall for the safety issue.Read more

I own a 2019 Honda Accord 1.5L with 112,985 miles. The vehicle developed a blown head gasket with coolant leaking into Cylinder 3. The failure was first confirmed via a combustion leak (CTR) test performed by an independent Honda specialist (Hond-Auto Specialist) on March 2, 2026, and later confirmed by an authorized Honda dealership (Vandergriff Honda, [XXX] ) via borescope inspection on March 23
... , 2026. Repair estimate is $4,799.81. This is a known defect. American Honda Motor Co. issued two internal Priority/Action Required memos to all Honda Service Managers and Advisors — April 2023 and August 2024 — confirming they were actively investigating this identical head gasket failure on 2018-2022 Honda Accords equipped with the 1.5L engine. Despite this internal investigation spanning multiple years, Honda has not issued a recall. A federal class action lawsuit [XXX] v. American Honda Motor Co., Inc.) has been filed alleging Honda knew of this defect before these vehicles were sold. I am requesting NHTSA investigate and compel Honda to issue a formal recall for all affected 2018-2022 Honda Accord 1.5L vehicles. The vehicle has experienced repeated failures of the integrated telematics system, including loss of GPS location accuracy, loss of data connectivity, navigation malfunction, and intermittent failure of voice command and steering wheel controls. On one occasion, the vehicle experienced a complete display blackout while driving, during which both the infotainment screen and digital instrument clus
... ter went entirely black. The condition has occurred repeatedly and persisted despite multiple repair attempts. The vehicle has been out of service for approximately 82 cumulative days for diagnosis and repair of these issues. An authorized Chevrolet dealership ultimately diagnosed a failure of the telematics control module, which was replaced under warranty. Following this repair, the same data connectivity failure recurred and was documented with photo and video evidence. The vehicle was returned to the dealership for further diagnosis. The failure is intermittent and may not be present during every inspection. The telematics system supports navigation, driver information displays, and emergency communication features, including transmission of vehicle location and system data. Loss of GPS and connectivity may impair the vehicle’s ability to provide accurate location information during an emergency. The prior display blackout resulted in loss of driver information while the vehicle was in motion. The condition has been reported multiple times to authorized Chevrolet dealerships. Diagnostic procedures and programming were performed, and the telematics module replacement confirms a system-level failure. Recurrence after component replacement indicates the condition has not been permanently resolved. Prior symptoms included loss of GPS signal, incorrect location, loss of connectivity, navigation failure, and intermittent driver assistance alerts.Read more

I have had numerous safety warning messages display upon startup and also while driving. The messages are usually combined with a cold ambient temperature and do not display during warm temperatures. The FCA dealership has attempted to fix this issue on at least six different occasions so far without any change. To date, FCA has replaced the original battery, they have updated, flashed the ECU mod
... ule, performed NHTSA Recall B4C for flashing the cluster, and most recently they replaced the ORC module. FCA indicated that NHTSA-25V882 was not applicable to my VIN despite my VIN production date in April '25; within your suspect period. Original issue remains, FCA STAR case generated however cannot replicate error condition for FCA engineers. The next scheduled visit to FCA is to replace a telematics module for possible link to issue with the Uconnect5 system. This safety issue was first noted before 800 miles and continues through 2768 miles thus far. As this issue pertains directly to a vital Air Bag sensor issue and the inability to call for a SOS assistance, I expect the NHTSA to raise this concern with FCA USA and to determine the cause of this safety defect and issue a new Recall Notification for repair and/or replacement of affected parts.Read more
