Since we bought our car, it has had a rattle upon startup. This was originally a recall, which we got repaired, but this repair did not make the rattle go away. I then had a transmission problem and had to have the transmission overhauled due to it having violent, jerky, unpredictable shifting between gears. My daughter was driving her 2016 CX5 when the coolant temperature warning message displayed on the instrument panel, advising her to pull over immediately. She had just driven on to the expressway, but luckily was still in the right lane and was able to pull over safely. The service tech reported that they began by plugging their diagnostic tool into the car and they received two engine codes: P111A-00 and P011A, and that a software update must be performed first. I had the car towed to Turnersville Mazda. They said that there would be a $215 per hour diagnostic fee. They found that the thermostat needed to be replaced as it failed to open during engine operation. This service incurred a $397.55 cost. After replacing it they noticed that a milky substance was produced, indicating motor oil mixing with coolant. They recommended replacing the engine due to a crack in the cylinder head at a cost of $10,300. For some reason the service tech elected to replace the spark plugs and update the software AFTER the catastrophic engine issue was discovered, incurring an additional $330 in charges. Prior to this incident there were no issues with the vehicle, no warning lamps or symptoms of any kind.Read more

... Similar to a previous issue with a failing battery. Replaced both batteries. Cleared codes. Van drives as normal for 1 afternoon. Next day hard start and rough idle. CEL on. Cylinder 1 misfire. Van operates normally once warm. Schedule follow up service for diagnostic. Next day, CEL off. Van operates as normal in the morning. Hard start in the afternoon with rough idle and CEL back on. Van runs rough tries to stall. Service shifter message returns. Start/Stop unavailable message returns. Parked van at home until service appointment. Day of service battery completely flat. multiple attempts at jumpstart finally get the van started. White smoke coming from exhaust mixed with smell of burning coolant. Notice coolant level has dropped in resevoir. CEL light is on, cylinder 1 misfire. Service messages. Tested battery voltage and alternator appears to be charging. Dropped van off with service. 2 days later report from dealer service department indicates leaking head gasket, and scoring of walls in cylinder 1 seen through endoscope. Recommendation is new motor. Van again requires jump start to relocate to our house. Van idles and drives rough. Using OBDII device and Torque app able to see that van has had over 1k misfires on cylinder 1. occasional misfires in other cylinders. Additional service appointment schedule with independent shop for second opinion on engine replacement. Van is unsafe to drive due to impending engine failure. which may result in crash.Read more

Dealer acknowledged this is an ongoing manufacturing issue. I contacted Mazda corporate to see if I qualify for after warranty assistance. No engine lights are on. Car has been maintained properly.
