The passenger-side (right) headlight assembly suffered an internal mechanical failure and is stuck aimed too high. It cannot be calibrated or lowered to factory specification. This poses a severe safety risk by blinding oncoming traffic at night. The vehicle was inspected by a manufacturer service center on July 13, 2026. The factory technician confirmed in the official service notes that the righ

COMPONENT: The 10-speed automatic transmission failed on 6/29/26, locking the vehicle completely in neutral. The neutral override could not be disengaged even by dealership technicians. Vehicle is currently at [XXX] Chevrolet, [XXX] and is available for inspection. SAFETY RISK: I have 3 young children who are regular passengers. The failure occurred without warning. Had it happened at highway spee
... d in oncoming traffic, I would have had no ability to move the car — creating a direct risk of a serious or fatal collision. CONFIRMED BY DEALER: Yes. Don Hewlett Chevrolet received the vehicle on 6/30/26 and confirmed the failure is directly related to two active federal recalls: NHTSA #24V-797 (10-speed transmission valve body failure/wheel lockup) and NHTSA #22V-036 (driveshaft assembly/loss of propulsion). Vehicle has been unrepaired for 13 days with no timeline provided. OTHER INSPECTIONS: Vehicle has been in possession of Don Hewlett Chevrolet since 6/30/26. No manufacturer representative, police, or insurance inspection has occurred. WARNING SIGNS: None. No warning lamps, messages, or symptoms appeared before the failure. The power train locked suddenly and completely without any prior indication on 6/29/26. Despite the dealer confirming this is recall-related, they have indicated I may owe partial repair costs. Under 49 U.S.C. § 30120, GM must remedy recalls at no charge regardless of mileage or warranty. I am requesting NHTSA investigate both the failure and GM's attempt to charge the owner for federally mandated recall repairs. Attached are pics of my failed 7 pole connector. The part number is (Hopkins #XXX). I took the RV to 24/7 Motorcoach and Victoria, Parts Manager, [XXX] Ext #5) filed claim #XXX on XXX. The claim was "Under Review" for almost a month. Now shows Approved Processing. I have a trip planned for tomorrow with my enclosed car trailer and did not have time to wait for the part. I purchased the replacemen
... t part myself from NAPA Auto Parts (Part #755-2621) and installed it myself. The same failure occurred. One of the pins pulled out and the lights/turn signals/brake/hazard/running lights/brake controller did not function properly or at all. I took the RV back to 24/7 Motorcoach and they removed the entire old back part of the connector which was connected directly to the 7 wires with spades, heat shrink tubing and silicone. They replaced the part with a different part (Hopkins #48474) which does not require a wiring harness. My Dynamax DX3 32KD did not come with a wiring harness. The original part #XXXrequires a harness to be installed properly and Hopkins sells an optional harness (part #40985) which MUST BE USED with part #40950. That is why the pin kept pulling out and the other pins were disconnected from the connector. In addition, the 20amp fuse in the engine compartment that provides power to the connector was blown. I have the blown fuse. I also have all of the part from the failed connector and all of the heat shrink and silicone pieces along with the failed connector. Clearly, Dynamax installed the connector incorrectly and this caused the failure. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am submitting this complaint regarding a leaking evaporator core in my 2021 Kia K5. The evaporator core failed at approximately 59,600 miles, causing the air conditioning system to stop cooling properly. The vehicle was diagnosed by a repair facility, which determined that the evaporator core was leaking refrigerant and required replacement. An evaporator core is a major HVAC component that sho
... uld not fail at such low mileage under normal operating conditions. This failure affects the vehicle’s ability to properly dehumidify and defrost the windshield, which can reduce visibility and create a potential safety hazard during rain, high humidity, or cold weather conditions. I have maintained the vehicle as recommended and believe this component failed prematurely. Due to the extensive labor required to replace the evaporator core, the repair cost is significant. I am requesting that NHTSA review this issue to determine whether there is a pattern of premature evaporator core failures in 2021 Kia K5 vehicles and whether an investigation is warranted. Vehicle Information: * Year: 2021 * Make: Kia * Model: K5 * Mileage at Failure: 59,600 miles * Failure: Leaking evaporator core causing refrigerant loss and loss of air conditioning performanceRead more

On my 2015 MINI Cooper Hardtop 4 Door (VIN: [XXX]), the driver's side halogen headlight suddenly stopped working. When I attempted to replace the bulb, I discovered that the headlight connector/pig tail had melted on one side, one of the electrical wires had burned completely through, and there was a strong odor coming from the rear of the headlight assembly. This was not simply a burned-out bulb
... . The connector and wiring sustained heat damage that appears to have resulted from an electrical failure. Because of the melted connector, burned wiring, and strong burning small, I believe this created a potential fire hazard. It must be noted that there have not been any changes to the lighting or electrical system since purchasing my car and I am the first and only owner. I contacted my mini dealership and was informed that the connector is not available separately for my year, make, and model for my car and the only option is the purchase the entire headlight assembly. The cost for the assembly is $615 and while the exact charge for labor is not known at this time, from my experience, it will be between $200-$500. After researching his issue. I found reports from other MINI owners describing similar failures involved melted headlight connector and burned wiring on vehicles with halogen headlights. Based on these reports and my own experience, I am cornered this may represent a recurring safety-related defect rather than normal wear and tear. I am submitting this complaint so NHTSA can evaluate whether this issue warrants investigation. I have photographs documenting the melted connector, burned wiring, and resulting damage. I purchased my vehicle brand new on June 6, 2026, with only 13 miles on the odometer. After driving home (about 44 miles) and another 10 miles the following day, the vehicle sat for just two days. When I tried to start it, the 12-volt battery was completely dead. The dashboard displayed a Parking Brake Malfunction message, multiple warning lights flashed, and then everything went black. I contact
... ed the dealership, and Subaru Roadside Assistance responded. The driver recommended jump-starting the vehicle instead of towing it because it couldn't be shifted into neutral with a dead 12-volt battery and would need to be dragged. After the jump, he noted the high-voltage battery still had about 70% charge and suggested driving the vehicle to recharge the 12-volt battery. The dealership tested the battery and told me everything checked out fine. About a week later, after moving the vehicle the night before, it was completely dead again the next morning. My brother-in-law jump-started it so I could make my scheduled Subaru Love appointment. Once again, the dealership tested the battery and said everything was normal. Just a few days later, after sitting for only one day, the vehicle was dead for a third time. I've since found numerous reports from other owners experiencing the exact same issue. The recommendation to use a trickle charger is not an acceptable solution for a brand-new vehicle. I've never had to keep a daily-driven vehicle on a battery maintainer just so it would start after sitting for one or two days. I also can't carry a trickle charger with me or plug it in if I'm traveling. I'm asking that the root cause be identified and repaired rather than masking the problem with a battery maintainer. A brand-new vehicle should not repeatedly leave its owner stranded after sitting for only one or two days.Read more

On five separate occasions, while turning onto a high-speed roadway and attempting to accelerate to the flow of traffic, the vehicle suddenly lost the ability to accelerate. Despite pressing the accelerator, the vehicle would not exceed approximately 35 mph and then began to decelerate. During both incidents, a "Check Hybrid System" warning appeared, and I was forced to quickly pull over to the si

This loss of power while merging into traffic created an extremely dangerous situation. Having three young children in the vehicle made the experience especially frightening, and I no longer have confidence that the vehicle can be relied upon to operate safely. Because this defect affects the vehicle's ability to accelerate safely in traffic, I am deeply concerned that it could result in a serious collision if it happens again. I took it into the dealership and the service center experienced the same issue. They thought they had resolved the issue and that it was a "one off" occurrence but it happened to me again just 3 weeks after the attempted repair. I notified by Ford or my dealer that the HL3Z parts used in my 2019 repair were known to have the same defect as the original phasers, despite Ford having since transitioned to an updated part (ML3Z) referenced in its own Customer Satisfaction Program documentation (CSP 21N03 and Supplements #1–4). I contacted Ford Customer Service in July 2026 requesting assistance with the recurrence and was told no help would be provided. I am concerned this failure can progress to loss of engine power or a more serious failure while driving, based on the known failure pattern of this component.
