The contact owns a 2024 Chevrolet Malibu. The contact received notification of NHTSA Campaign Number: 26V212000 (Back Over Prevention); however, the part to do the recall repair was not yet available. The contact stated that while driving at an undisclosed speed, the infotainment system screen glitched, displayed a green image and turned blank intermittently. Additionally, the contact stated that
... while reversing, the rearview camera was inoperable, and the message "Service Visual System" was displayed. The vehicle was taken to the dealer, where the contact was informed that there would be a charge for a diagnostic test, and that the warranty company would be contacted for coverage. The contact declined to pay a fee because the failure was related to the recall. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the failure. The failure mileage was unknown.Read more

The brake actuator and/or brake booster pump assembly failed on my 2016 Lexus ES300h. This is a known issues for this vehicle's braking system and results in a sudden loss of power braking assistance. The failure requires increased pedal pressure and eventually brkaing failure, creating a significant safety hazard and increasing the risk of collision. The dashboard displays multiple warning lig
... hts, including Braking Power Low and Brake Override Malfunction. Lexus has acknowledged this defect through an extended warranty and consumer warranty program, but Lexus is alleging my vehicle (despite showing issues prior to 150,000 miles) is now over 150,000 miles and not eligible. I am reporting this because a critical safety component should not have a "timer" on it when it involves primary braking of the vehicle.Read more

For the second year in a row my windshield has cracked. Nothing hit it, or happened to make it crack. Both cracks were long and coming from the bottom of the windshield under the passenger wiper blade. I noticed it today, 4/28/26 and the same thing happened in the beginning of April last year with the same circumstances (or lack thereof).

I am filing a complaint regarding a 2021 Volkswagen ID.4 involving loss of propulsion and unresolved defects following manufacturer-directed repairs. On February 19, 2026, the vehicle lost propulsion twice, including in an intersection. The vehicle powered on and shifted into gear but would not move, requiring it to be pushed out of traffic. This is a serious safety hazard. The vehicle was repaire
... d at a Volkswagen dealer under corporate technical direction, including replacement of the rear drive motor, replacement of control modules (gateway), and multiple programming attempts, including non-standard programming. After repair, new faults appeared. The vehicle now shows inconsistent odometer behavior, including “-- mi” and conflicting mileage values. The system continues to calculate range but does not reliably display total mileage. The dealer confirmed in writing that this issue is related to the prior repair, meaning the vehicle was returned defective. Despite prior loss-of-propulsion events and current system faults, both the dealer and Volkswagen corporate representative have advised me to continue driving the vehicle and have refused to arrange transport, even though the defect resulted from their repair. I have serious concerns the vehicle may again lose propulsion and that control systems and mileage recording are unreliable. The vehicle has been out of service over 60 days with no confirmed resolution. I request NHTSA investigate these failures and Volkswagen’s handling of this safety issue.Read more

I am writing to formally report a serious safety concern regarding my vehicle’s airbag system following a recent accident. On April 22, 2026, I was involved in a significant collision while driving my Toyota Corolla 2022. The impact was severe enough that the other vehicle involved deployed its airbags. However, despite the force of the crash, the airbags in my vehicle did not deploy. I am ex

This vehicle has suffered from persistent, substantial, and uncorrected defects, including but not limited to failures in the 48V mild-hybrid electrical system, lithium-ion battery system, DC/DC converter and coolant system. These defects have substantially impaired the vehicle’s use, value and safety as well as my safety, and remain unresolved despite multiple repair attempts at two separate au
... thorized Mercedes-Benz dealerships in St. Louis, MO. SUMMARY OF DEFECTS •Multiple 48 V battery failures and replacements •Multiple replacements of DC/DC converters and failures •Multiple Coolant System failures •Sudden vehicle shutdowns while driving •Numerous non-start issues •Numerous red warning alerts related to the 48 V system •Multiple towing incidents (One involving the police) Despite extensive repair attempts, including replacement of major components, this vehicle continues to fail and is currently still at Mercedes-Benz of St. Louis unrepaired. These repeated repair failures demonstrate the vehicle cannot be repaired to conform with warranty standards. SAFETY INCIDENT My most recent safety incident: On March 10, 2026, this vehicle shut down in the middle of a busy multi-lane roadway, requiring police assistance and emergency towing due to the safety hazard presented. The vehicle has remained in possession of Mercedes-Benz St. Louis since that date (over 45 days) and remains inoperable and unrepaired. This vehicle had also been towed to Plaza Motors two additional times for the same reasons and was there for many days adding to this amount (See attachments). MULTIPLE DEALERSHIPS/FAILED REPAIRS The vehicle has been serviced by both: *Plaza Motor Compay, St. Louis *Mercedes-Benz of St. Louis Despite independent repair attempts by both authorized dealerships, the same defects persist. This confirms that the issue is inherent to the vehicle itself, not a service related issue.Read more

I have a 2026 Nissan Rogue Platinum with Pro Pilot assist. When engaging the ProPilot cruise control with adaptive cruise control, if you are traveling on a road with stop lights and the car in front of you is stopped at a light and you car is traveling at 45 mph , if the vehicle at the light is more than a block away and you are approaching with no car icon on the dash, your car will not slow dow
... n until there is a emergency panic stop. I don't know if the car would stop in time. I applied brakes and came to a panic sideways stop inches from the car in front of me. I took my car to Woodbury Nissan and was told that that is the way the system is designed to work by [XXX] a service writer. He didn't want to discuss it further. I called the manage and he had me come in and have one of the service managers drive my vehicle. When he drove it and the car did not slow down, he had to hit the brakes. You could tell he was scared by the experience. They agreed to bring it in and test it but I knew they would do nothing. They told me there were no adjustments to ProPilot assist. Then I brought it in and left it. They called me back a few hours later and told me that they drove another Rogue and it did the same thing. They couldn't do anything for mr. I had a 2025 Hyundai SanteFe with adaptive cruise control. It did not stop like the Rogue. It slowed down when approaching the stopped vehicle before coming to a stop. If nothing is done to warn ProPilot owners of this issue, someone is going to get killed. If a car is cruising down the [XXX] and come up to one of those lights on the southern end the will never stop in time. Please let people know before people lose their lives in these vehicles. It's wrong for a company to take so much money from owners and use an engine that GM knew had issues and was faulty to place it into my vehicle only to have it fail and now make my vehicle undriveable and I am told it's another $8,000 to repair it. GM should address this issue with my vehicle and replace this engine that I had put in in 07/2022. GM was aware of this engine problem with my model car Yukon. SUBJ: Vehicle Safety Complaint – 2023 Cadillac Escalade Sudden Loss of Power I am reporting a safety issue involving a 2023 Cadillac Escalade ESV (VIN: [XXX] ). On April 19, 2026 (late evening into early April 20), while traveling approximately 70 mph on [XXX] , the vehicle suddenly and completely lost engine power without warning. This created an immediate safety hazard, nearly resulting in
... a collision. I was able to exit the highway, but the vehicle lost all acceleration and became disabled in the middle of a dark off-ramp with oncoming traffic approaching from behind. OnStar was contacted and Virginia State Police responded due to the unsafe location. A trooper remained on scene with emergency lights to redirect traffic and prevent a collision. Due to the hazardous conditions, the trooper arranged an emergency tow. I incurred $389.61 in out-of-pocket towing costs. The vehicle was taken to a tow yard, and my AAA Premier coverage then towed it 189 miles to Miller Brothers Cadillac in Ellicott City, MD. The dealership diagnosed catastrophic engine failure requiring full engine replacement at approximately 53,000 miles. There were no meaningful warning indicators prior to the sudden loss of power. This vehicle was purchased from Sons Chevrolet / Cadillac in Columbus, GA in December 2025 as a Certified Pre-Owned Cadillac and had already required multiple repairs shortly after purchase. The vehicle is currently at Miller Brothers Cadillac awaiting complete engine replacement and is available for inspection. This sudden loss of power at highway speed presents a serious safety risk and could have resulted in a severe accident or loss of life. Given the absence of warning and catastrophic failure at relatively low mileage, I am concerned this may indicate a broader safety defect rather than an isolated incident. I am filing a safety complaint regarding my 2025 Chevrolet Tahoe Premier for repeated brake assist/braking failures. Most recent incident: While driving, the check engine light came on. I turned the vehicle off for approximately 10 minutes and restarted it, but the check engine light remained on. Approximately 20 minutes later, multiple warnings appeared on the dash including: “Brake Assist Fai
... lure Press Hard to Brake,” “Speed Limited,” and “Service ESC.” At that point, braking ability was severely reduced. I was close to my destination and approached a stop sign. I intended to roll through the stop sign because the brakes were not functioning normally, but another vehicle suddenly approached. I pressed the brake pedal with both feet. The brakes were stiff and hard to press. The vehicle still did not stop properly and came to a stop in the middle of the road past the stop sign. After arriving at my destination, the vehicle sat for approximately 1–2 hours. When restarted, the brakes appeared normal again, but the check engine light remained on. In the MyChevrolet diagnostics app, it then showed warnings for Engine and Transmission System, Transmission System, and Four Wheel Drive System. While driving to the dealership afterward, the vehicle also began shaking. This same issue previously occurred on April 16, 2026. I was driving on the interstate when the brakes lost assist and the dash displayed “Service ESC,” “Brake Assist Failure Press Hard to Brake,” and “Speed Limited.” After reaching my destination, I was told by someone at the dealership to turn the vehicle off for 10 minutes to see if that would fix things, so I turned the vehicle off for about 10 minutes. When restarted, all warning lights were gone and braking returned to normal. I still drove the vehicle to the dealership that same day for service. This is a repeated, intermittent brake failure / brake assist failure creating a serious safety hazard and near collision risk.Read more

While driving my 2017 Honda CR-V, I experienced sudden, unexpected steering behavior consistent with a defective electronic power steering (EPS) gearbox. The steering wheel momentarily became difficult to turn in either direction, with a "sticky" or binding feel followed by a sudden release of resistance, causing oversteer beyond my input. When the wheel was turned to or near full lock, the EPS ap
... peared to apply assist opposite my input, briefly pulling the vehicle unexpectedly to one side. The failure occurred early on most noticably at highway speeds going down windy roads downhill. It became progressively worse in other driving situations over time. This unintended steering behavior reduced vehicle maneuverability and created a serious risk of loss of control, including coming dangerously close to unavoidably crossing the double-yellow line into oncoming traffic. I took the vehicle to Tony Honda in Kona. The dealer diagnosed a defective EPS steering gearbox and replaced the gearbox assembly, performed a four-wheel alignment, and completed the steering angle neutral position learning procedure. The diagnosis and repair exactly match Honda Service Bulletin 18-103 / NHTSA Recall 18V-663 (Honda codes C2N / P2O), covering 2017–2018 Civic and CR-V vehicles for an improperly secured torque sensor magnet that shifts within the gearbox and causes unintended steering input. However, my VIN was not included in the recall, and I paid $2,000 out of pocket for a repair Honda performs free of charge on recalled vehicles with the identical defect. The recall covers only 54 vehicles total, despite multiple owner complaints reporting the same failure on 2017 CR-Vs outside that VIN range. I request that NHTSA investigate expanding Recall 18V-663 to include additional 2017 CR-V VINs with the same defect, and that affected owners who paid for the repair be eligible for reimbursement.Read more

I was not injured other than a cut on the hand due to the bedcover and a headache from the deployment. No hard hit or collision and truck did not make contact with tree trunk or any front end damage that would ordinarily come from someone going into the ditch. (NHTSA Tracking Number 11726105) I am updating my existing complaint regarding my 2024 Hyundai Tucson (NHTSA Tracking Number 11726105) for an ongoing safety concern involving an intermittent warning chime while driving. This creates uncertainty because I do not know what the vehicle is trying to warn me about, which is distracting and creates a safety concern while driving. The dealership previously replaced both front seatbelt buckle assemblies after identifying intermittent high resistance as a possible cause. However, after that repair was completed, the warning chime occurred again while I was driving to the dealership for a follow-up visit. This confirmed that the issue was not fully resolved. I immediately returned the vehicle to the dealership, and they are now performing further diagnosis. I was advised that if the issue reoccurred, the next possible areas for diagnosis could involve the instrument cluster or the control module, both of which are safety-related systems connected to warning alerts and seatbelt functions. In addition, during this same visit, the dealership confirmed there is an actual oil leak requiring repair. This is significant because when I first raised this concern after an outside inspection in December 2025, I was told it was likely leftover residue from a prior oil change. The dealership has now confirmed it is a real leak and kept the vehicle for repair while providing a loaner vehicle. Because this vehicle was sold as Certified Pre-Owned and these safety and mechanical concerns continue shortly after purchase, I remain concerned about the reliability and safety of the vehicle.Read more

4/10/26 - I left my neighborhood to turn right, from a stop, onto a 55 mph highway. Headed to the grocery store, with my 6-month old daughter. The trucks transmission (F150 10R80 10 speed) would not shift any higher than 2nd gear, of 10 gears. This caused the truck to not accelerate at a reasonable speed and only do about 15mph. Two cars behind me nearly collided with me as I could not achieve or
... maintain speed. I quickly adjusted the steering and pulled the truck over on the grass shoulder. This avoided any accidents and only scared myself, my daughter, and two other drivers behind me. My daughter and I could have easily been severely wounded or killed. I did not know or expect the transmission would suddenly fail. This failure to change gears or hard shifts is a classic symptom of the Ford 10R80 transmissions CDF drum failure. Ford knows about this issue and refuses to recall these vehicles or repair them unless they are under warranty. My truck has 85k miles, just outside the 60k mile warranty. My neighbor with two small children has the same truck with the same issue, also potentially endangering their lives. This should be a forced recall by Ford to protect everyone on the road.Read more

2022 Tesla Model 3 with approximately 19,373 miles. Third-party tire shop inspection on April 25, 2026 identified visible tearing of front lower control arm bushings on BOTH sides (bilateral failure). Rubber bushing material has separated from inner metal sleeve on both left and right front lower control arms in identical pattern. No impact event or curb strike. Symmetric bilateral failure indicat
... es manufacturing/material defect, not impact damage. Failure occurred at mileage well below the 40,000-70,000 mile range commonly cited for this known Tesla Model 3 lower control arm bushing wear pattern. Suggests accelerated failure mode that warrants investigation. Photos from third-party inspection attached. URGENT SAFETY DEFECT - Repeated front hub overheating/fire risk (2025 Western Star 47x) I am submitting a formal safety complaint regarding my 2025 Western Star 47x The front wheel hubs have over heated to the point of failure on 6 separate occasions, resulting in a fire on two of those occasions. The issues have been documented with Freightliner and accompanied by photo/video evidence. Each inc
... ident required the vehicle to be towed to an authorized dealership, and repairs were performed under warranty. Despite these repeated repairs, the issue has not yet been resolved. The hubs are currently overheating again, indicating a persistent and unresolved defect. This is no longer a routine maintenance or warranty issue. This is an ongoing safety defect that presents a serious fire hazard and risk of injury, property damage, or loss of life. Key Concerns - - Six separate over heating and/or fire incidents involving the front hubs - Multiple warranty repairs with no permanent resolution - Recurrence of the same failure condition - Within days or weeks of warranty repair, the vehicles becomes unsafe to operate under normal working conditions due to over heating hubs, which break down the lubricating gear oil, furthering the problem, causing more heat with thinner oil, which eventually leaks onto the brakes, causing a fire. During the times where I was fortunate enough to catch it before a fire started, the fluid was so hot that it melted the plastic hub cap to the point of blowing up like a balloon before eventually rupturing. These instances were video documentedRead more

Manufacturing defect in the rear window frame (Backlite) causing severe water intrusion. Water is pooling directly onto the RF Hub (Radio Frequency Hub) electrical module and wiring harness. On Monday, [XXX], I was traveling northbound on [XXX] headed towards Roanoke from Salem at an approximate speed of 65 MPH. At approximately mile marker [XXX], before the exit to [XXX] , the vehicle started to lose power rapidly, and the car began to go into what I perceived as "limp mode" and was shaking. In an attempt to not be stranded on one of busiest and most dangerous highways in VA, I wa
... s able to get the vehicle pulled onto the shoulder of the on ramp of [XXX] going towards Roanoke. By this time, the vehicle had completely lost power and was not able to move, so I shut it off. At this point, I was stranded on the side of [XXX] , and extremely busy highway. This was a great safety concern, as the shoulder was very narrow and cars were moving through 2 lanes of traffic with many drivers exceeding 75 mph. My own personal safety was greatly jeopardized with the failure occurring with absolutely no warning and shutting off the car while on [XXX]. The vehicles failure was catastrophic and was undriveable. The vehicle was then towed to a nearby shop. There were no warnings whatsoever before this failure occurred. No engine lights were on beforehand, and the car drove completely fine and has been comprehensively maintained its entire life. The problem of a rod-bearing failure has been confirmed with a local service center, One Stop Automotive in Vinton. The Service Department at Southern Hyundai based in Roanoke VA is refusing to look at the vehicle and submit a claim to Hyundai USA , even though the failure has already been confirmed by a licensed mechanic.
