Following a day trip involving sub-freezing temperatures and full depletion of the traction battery, the vehicle was plugged in at home and appeared to be charging. The following morning, I enable the preheat via the remote before leaving the house for work, and notice the CEL/MIL before starting the car. Car was cold, charging indicator was flashing green as if it was accepting charge. I went to
... investigate and was met with the following list of codes: P1E00, U2603, U2604-U2606, U2617-U2624, P1FD2, P1FD3, P3042, C326C, P0096. Car turns on but seems to only be accessories; no power steering. Seems like the state of charge indication kept transitioning from empty to full. I unplugged the car to see if it could still drive, but no luck. getting "Shift to Park" when I put it in drive. Switching to HOLD does not engage the engine. After plugging it in again, the car no longer receives charge and simply holds the orange charge indicator light. In an attempt to get the car into limp mode, I tried clearing the codes, but no luck. After reading the codes again in the afternoon, the following codes posted: P1E00, U2622, U2623, P1FD4, P0AFA. These codes and behavior are consistent with the following NTSB service documents, which call for the replacement of the BECM: Special Coverage N232432680 Technical Service Bulletin 18-NA-261 However, the vehicle in question does not appear to be one of the 19,000 vehicles covered under the special coverage, per GM’s warranty look-up. The vehicle has not yet been serviced for the above events, but it is assumed the BECM is failing, which is available for inspection. My safety was not put at risk this time, but it is possible for the BECM to fail in an unsafe manner. Additionally, the associated repairs are costly since the BECM is not easily accessible, requiring lifting the car and removing the entire traction battery.Read more

On Monday, December 15, 2025, I was "creeping" into my garage when the brakes failed and I ran into my garage wall damaging my wall and the front of the vehicle. On Tuesday, December 16 ,2025, while slowly pulling into a parking spot at Sam's Club in OKC, the brakes again failed, and the forward crash system prevented running into another vehicle. On that same date while stuck in bumper-to-bumper
... traffic, the brakes again failed, and I was able to pull into the median rather than hitting the car in front of me. That same evening, while backing out of a parking spot at a restaurant, the brakes failed in reverse and again the vehicle reverse crash avoidance system stopped without hitting another vehicle. Later, as I was leaving the parking lot, the brakes failed at the STOP sign, and I was able to turn right in the Merge Lane avoiding hitting another oncoming car. At the very next STOP sign, the brakes failed again, and I was able to get the car into PARK avoiding another head on collision. All these brake failures happened at speeds under 5 mph or barely "creeping" forward or reverse. I drove the car to Cummins Chevy Dealership in Weatherford, OK, the next day and related the issues to them. They gave me a "Loaner" for use until they fixed my car. The Service Manager did ask me if I had "Tagged" my car yet and I said I had. So far, GM and Cummins have been very helpful and understanding. I had no issues "braking" the car at speeds above 5 mph. I have only had this vehicle for 3 weeks when the issues started. I love the vehicle and hope this is only a minor glitch or some bad part that can be replaced. I file this complaint proactively in case it is a bigger problem with this line of vehicles. Thank you.Read more

I am leasing a 2024 Tesla Model 3. The vehicle has a steering column defect that was previously acknowledged and temporarily repaired by Tesla Service. Within days, the issue returned. Tesla has now informed me that the steering column has a known design flaw, that replacement parts are still in development, and that any repair attempted now would fail. Despite this, I am being asked to continue
... driving the vehicle until at least January. This defect affects the steering system, a critical safety component, and raises serious concerns regarding vehicle control and driver safety. I am reporting a repeated safety-related defect involving exhaust intrusion into the cabin of my 2022 Volkswagen Taos. The vehicle experienced loss of power, coolant loss, and engine warning indicators, and was serviced twice by Volkswagen Southtowne in Sandy, Utah. First Incident – Vehicle Returned as “Safe to Drive” During the first repair visit, the dealer diagnosed issues involving the
... manifold/exhaust area, head gasket, coolant system, and catalytic converter. After keeping the vehicle for an extended period, the dealership informed me the vehicle was “safe to drive,” although the repair was not fully completed. I relied on this representation. After the vehicle was returned to me, I began experiencing significant physical symptoms only while driving the vehicle, including nausea, dizziness, and vomiting. These symptoms occurred repeatedly during operation and would subside after leaving the vehicle. At the time, I did not understand that these symptoms were caused by exhaust entering the cabin. Second Incident – Defect Confirmed and Additional Repairs Performed Weeks later, the symptoms persisted and I returned the vehicle to the same dealership. At this second visit, the dealer reported coolant leakage and additional gasket failures. They removed the cylinder head, replaced gaskets, and performed additional exhaust-related repairs. I was provided a loaner vehicle during this extended repair. After this second repair, the symptoms while driving stopped. Safety Concern Based on the timing of symptoms, the repair records, and the resolution following the second repair, I believe that exhaust gases were entering the cabin of the vehicle during normal operation for an extended period. This created a significant safety hazard. I was repeatedly assured the vehicle was “safe to drive” when it was not. I unknowingly operated the vehicle while being exposed to exhaust fumes for several months. The condition caused physical illness, impaired my ability.Read more

My Tesla Model 3 is experiencing premature suspension failures involving the lower lateral links and stabilizer links. These components were already replaced once, and they are failing again in less than 30,000 miles. This is not normal wear for suspension parts on a modern vehicle. These failures cause clunking, instability, and looseness in the front end—conditions that affect the vehicle’s
... ability to track straight and maintain control, especially over uneven pavement or during emergency maneuvers. This isn’t an isolated case. There are extensive reports from other Tesla owners across multiple models (Model 3 and Model Y in particular) showing the same premature wear on these suspension links. Many owners have parts failing even earlier. Once the car is out of warranty, Tesla charges thousands of dollars to replace these components, despite clear indications that the suspension was under-engineered for the weight of the vehicle. Tesla knows this is a recurring issue but refuses to issue a recall or goodwill repairs. A repeated failure of critical suspension links in such low mileage suggests a defect. These parts should not be wearing out this fast, and the resulting instability poses a real safety risk. I’m requesting NHTSA investigate these premature failures.Read more

My 2022 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: Radio reboots without notice(sometimes volume is very high), black screen while driving, loss of HVAC, camera not available, system rebooting, etc. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

I am reporting a series of safety-related defects involving the electrical, cooling, and fuel systems of my 2021 Porsche Cayenne E-Hybrid. The vehicle was initially brought to the authorized Porsche dealership due to an electrical system error, and the dealer kept the vehicle for an extended period. After approximately five months, the vehicle was returned to me without a clear explanation of the
... root cause or details of the repair. The very next day after receiving the vehicle, a coolant temperature warning appeared while driving, indicating abnormal engine overheating. This created an immediate safety hazard, as overheating can lead to sudden engine failure, loss of power, or a potential fire risk. I returned the vehicle to the dealership again for further repairs. Afterward, another serious issue occurred: because this is a hybrid vehicle, I went to refuel, but the fuel door would not open, and a fuel system error warning appeared again. This made the vehicle unsafe to operate, especially while already on the road. Due to these repeated failures, I was required to tow the vehicle multiple times. These problems — electrical malfunction, coolant overheating during driving, fuel system errors, and a non-functional fuel door — occurred repeatedly even after prolonged service by the dealership. The combination of electrical faults, overheating risk, hybrid system malfunction, and fuel system failure presents a significant safety concern. I will provide additional documentation to support this report. I request that NHTSA review this case to determine whether an investigation is warranted, as these issues may indicate a broader defect affecting this model.Read more

My truck thinks my parking brake is engaged so it constantly flashes the “brake” light, constantly chimes while driving and says parking break malfunction, talk to dealer. I brought it to Webber family Ford in Detroit lakes MN 56501 and they checked it over including the safety recall location for recall 23S35. They said it was not the recall location and I would have to bring it in Monday for
... another appointment. When I got it home that day I crawled under to check that location. I see visible green which usually means corrosion and the protection around the harness seems warm. I also noticed the "recall" was not completed how it is shown on fords recall paperwork using the zip tie as a wear location so it isn't the harness itself. That is not how mine is. I have photos to prove that. I spoke with corporate ford today and they said they can't re open the recall because it has been closed and it is past its 12 month warranty being it was done in October of 2023 before my ownership and at a different dealer. I don't believe it is fair to me the current owner to pay for the cost of a rerepair of a recall that was done incorrectly.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The brake hydraulic system failed due to a rusted and leaking brake line running from the front to the rear of the vehicle. The damaged brake line is still on the vehicle and is available for inspection upon request. 2. How was your safety or the safety of others put at risk? The brake line lea
... k caused the brake pedal to go to the floor, significantly reducing or eliminating braking ability. This created a high risk of being unable to stop the vehicle, which could lead to a collision and put myself, passengers, pedestrians, and other drivers in danger. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue was confirmed by an independent service center (Busse Automotive) during a brake system evaluation. They traced the brake fluid leak to a rusted brake line section that needs replacement. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? No, the vehicle has not been inspected by the manufacturer, police, or insurance representatives. The only inspection so far has been by the independent repair shop. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Not that I know of.Read more

My vehicle only has 36,000 miles on it and I just had to spend $789 to have the rear shocks replaced!! I also purchased the extended warranty plan and this repair wasn't covered. Very disappointing especially with such low mileage.

I am reporting a serious coolant system defect on my 2023 Hyundai Sonata Hybrid with about 13,500 miles. The vehicle develops an internal coolant leak while driving. Coolant burns off and white steam/smoke comes out of the exhaust, especially during acceleration. When this happens, the coolant level drops quickly, the interior heater stops working, and the car soon displays an engine overheating w
... arning, forcing me to stop driving. This has happened multiple times and makes the vehicle unsafe. When I checked under the hood, coolant was at a low/empty level after overheating events. The car becomes undriveable once the alarm appears. I took the vehicle to a Hyundai dealer, and even though the vehicle is only 2 years old, they told me it was “out of warranty.” I had to pay about $1,500 for repairs, and since the dealer refused a loaner, I paid $250 for a rental car. I called Hyundai USA on 9/13/2025 and opened Case Number XXX. Hyundai acknowledged they are aware of coolant issues on the Sonata Hybrid but said no recall exists. This issue appears widespread. Many other owners report the same symptoms: white exhaust steam, coolant smell, coolant loss, no interior heat, and engine overheating. Hyundai released TSB 23-EM-009H, describing a failure where coolant leaks internally in the Exhaust Heat Recovery System (EHRS), causing white smoke and overheating—matching my symptoms exactly. This defect is a safety hazard because overheating can occur suddenly while driving, risking engine damage or breakdown in traffic. I request NHTSA investigate this issue as a potential recall-level defect on the 2023 Sonata Hybrid. The instrument cluster failed completely. All gauges were offline, and the vehicle became stuck in the ACC/IGN “on” state. The push-to-start button would not disengage, the exterior lights stayed on, and the truck acted as if the ignition was on even when the engine was off. While driving, no speedometer, RPM, warning lights, temperature readings, DEF/emissions information, airbag indicators
... , or other instrument cluster functions were available. The vehicle also displayed a false door-open message. The issue was confirmed by the dealership, and the truck was declared unsafe to drive. The vehicle has remained out of service for months while waiting for the replacement instrument cluster, which has been unavailable due to manufacturer part shortages. The problem can be reproduced and is fully documented by the service center.Read more

I am reporting a safety issue involving my 2025 Tesla Model 3 Performance. My steering wheel began shaking while driving, and the vehicle felt unstable, especially at speed. I brought the vehicle to Tesla Service in Las Vegas. Their inspection found that the outer tie rod nut on the passenger front side was loose, which created steering play and caused the steering wheel shake (documented in Tesla
... invoice #XXX). A loose tie rod nut is a serious steering safety defect that can result in loss of steering control. Tesla performed a four-wheel alignment and tightened the components but stated the issue is “maintenance” and not covered under warranty, despite the vehicle having only 12,890 miles. Tesla also stated the steering rack had “no issues,” but they did not perform any repair on the steering system beyond tightening the loose tie rod nut and charging me for alignment. They also did not explain how a nearly new vehicle could develop a loose steering component. A loose outer tie rod nut should never occur on a new car unless there is a defect or improper assembly from the factory. This poses a significant safety hazard, especially at highway speeds. Tesla did not treat this as a steering safety defect and only attributed it to alignment. They declined warranty coverage and did not fully address the root cause. I am filing this complaint because steering components should not loosen on a 2025 model vehicle, and the situation could have resulted in loss of control or an accident. I want NHTSA to be aware in case this issue appears in other vehicles of the same model year.Read more

The vehicle uses hydrogen fuel which is stored in three high pressure cylinders. Any fires or damage to the vehicle that affects the fuel system can result in an explosion. During a recent recall, customers were recommended not to park near structures because the car can explode and cause property damage. This is applicable to all hydrogen fuel cell vehicles available in the US.

I own a 2014 Porsche Panamera S E-Hybrid (VIN XXX). I experienced repeated hybrid system warning lights and high-voltage battery fault codes. I brought the vehicle to Porsche Atlanta Perimeter for a safety inspection. The dealership did NOT perform the required PIWIS Hybrid Battery Test Plan, did NOT check individual cell voltages, and did NOT assess the hybrid battery control unit. Instead, they
... provided a basic code scan and immediately recommended a $37,000 battery replacement without verifying the root cause. This is a high-voltage system handling 300+ volts, and an improper or incomplete diagnosis is a safety concern. Porsche headquarters was contacted by the dealership, but the dealership stated they received no technical support or guidance. Porsche appears unable or unwilling to diagnose early-generation hybrid vehicles properly, leaving customers without accurate safety assessment of the high-voltage system. I am filing this complaint because improper hybrid battery diagnosis can lead to thermal events, sudden loss of power, or misinterpretation of critical battery faults. I am requesting that NHTSA review Porsche’s handling of hybrid diagnostics for this model. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
