The engine began knocking on us, towed to dealership. Safety concern was dying on the highway with other cars around. The dealer said it needs a new engine. Not sure there were any indications of failure or not. Some records indicate in order for Hyundai to warranty, this car needed to have had campaign 953 KSDS installed prior. Some records such as the TG3 TXXI warranty state that in April 2022,
... the KSDS campaign update was no longer required for the Hyundai Theta II engine warranty. owner did not perform KSDS 953, based on not receiving the information. The reverse braking feature registers cross traffic or movement that is not near the back of the vehicle. This cause the braking system to engage. I was doing a U turn on 4 lane main street with a center divider. I didnt fully clear the turn leaving me to reverse to complete to turn. When i put the car in reverse, the sensors picked up movement across the street, engaging the brakes and leaving me
... stuck in the middle of the road with oncoming traffic coming straight at me. It would not disengage when I tried accelerating and I had to quickly throw my car into drive and pull into a driveway, that was luckily there, just to avoid being T-boned by oncoming traffic. This is not the first occurance as it happens often in parking lots locking my brakes due to any movement from behind. There is a manual feature in the dashboard to turn it off but it automatically resets every time the car is turned off, requiring you to have to manually disengage the feature everytime and you cannot disengage while the brakes are locked.i took it to the dealers but they said since it is a "feature" it is not considered a safety issue since there is an option to disengage even though it has to be reset during every use. They could not help or remedy the issue and instructed me to file a complaint with Kia manufactures since they are only the Kia dealer.Read more

The issue involves ongoing water intrusion through the panoramic sunroof, front power windows, and windshield seals. The vehicle is available for inspection upon request. This recurring leak has caused extensive interior water damage, including visible mold growth in the carpet on multiple occasions. This mold presents serious health risks to both me and my passengers, especially my sister, who is
... undergoing cancer treatment and has a severely compromised immune system. Water pooling beneath the carpet also threatens critical electrical systems located in the floor area, including the Vehicle Electrical System Control Module (J519), CAN/LIN bus wiring, door control harnesses, and footwell circuits. Corrosion or shorting in these systems could lead to unpredictable failures in safety-critical functions, even if not immediately apparent. Although Volkswagen dealerships have cleaned the sunroof drains as part of routine service packages, no formal diagnosis or confirmation of the leak’s source has been made. Independent mechanics have confirmed clogged drains and flushed debris to restore water flow. In 2019, I replaced the windshield at my mechanic’s recommendation because the interior glass was frequently soaked with morning condensation. Despite these measures, the sunroof continues to leak whenever it rains. No warning lamps or dashboard messages have ever appeared related to this issue. The first visible symptom occurred several years ago—possibly as early as 2019—when water began leaking from the passenger-side sun visor and headliner immediately after a car wash. Given the persistent nature of the leak, the associated mold hazard, and the risk to vehicle electronics and occupant safety, I believe this defect warrants manufacturer accountability and recall consideration rather than a temporary repair.Read more

Took my 2022 VW Taos in for its 20,000 mile service. Upon picking up the vehicle I was told everything looks great and I'm going to want to watch my rear brakes, the pads are worn 50%. I inquired as to wear on the front, near new at 11. I had also asked if they could check on the sluggish or jack rabbit take offs. They said that had been corrected thru a computer update at a previous visit. I said
... it was minimally improved? and had hoped VW had figured out the cause. I dwellled on the rear brake issue overnight, discussed the situation with my BIL, and called my service advisor in the am to say we believe the sluggish/rapid acceleration isn't brake related, but rather the brakes aren't completely releasing during stop to acceleration. He said he suspected that also and discussed it with his service manager who agreed. He said they submitted an inquiry and are asking VW to look in to this and as my rearbrakes are out if warranty by my "in service date", to see if VW will waive the pad replacement for low odometer. I've complained about and filed a NTHSA report for the acceleration problem a couple years ago. This is still a serious safety issue and abuse of the paying public.VW needs to step up before someone has to die. Brakes are nothing you wanting failing doing 70 mphs on I-90 thru mountain passes. Shortly after my warranty expired last september ([XXX]) my 2020 Audi A4 Allroad started to experience random episodes of the car's main entertainment touch screen rebooting in the middle of the car being operated. This usually would only happen once or twice during maybe an hour long car ride. Then this spring, starting in mid-March the car started to experience the MMI reboots more often, say 6-
... 7 times over an hour long car ride. This started to happen more often, and then it would be around 4-5 times in a 20 minute car ride. I noticed that when this would happen, it would cause issues with the backup camera (the camera would stop function during the 60 second reboot. Also, if a parking sensor (or cross-traffic warning sensor, incorporated with the backup camera) was detecting something near the car during the reboot and the camera turning off, it would throw a safety system communication error on the Audi Virtual Dashboard. It was clearly a safety issue and it made maneuvering to park on a busy street very dangerous for folks outside the car and inside. I went to the dealership twice to get the MMI reboots fixed, and received two software updates and a 5F module replacement (quoted at $5k, Audi offered the part 90% off) - all three attempts resulted in the car with a MMI rebooting even faster. It now reboots and has the same issues as before with the car MMI rebooting between every 2-5 minutes during operation. You can find reports from numerous other 2020 Audi A4 owners with this same rebooting MMI issue here, many of whom have already submitted their own NHTSA safety problem reports in this long thread: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Broken Accelerator Pedal – 2022 Toyota RAV4 Hybrid Vehicle Information: •Year/Make/Model: 2022 Toyota RAV4 Hybrid •Mileage at Incident: [53000] Incident Date: [August 10, 2025] Description of the Problem: While driving my 2022 Toyota RAV4 Hybrid, the physical accelerator pedal broke under normal driving conditions. The pedal construction is extremely flimsy and failed suddenly, rendering
... the vehicle unsafe to operate. If this failure had occurred during a high-speed maneuver, while merging into traffic, or while avoiding an obstacle, it could have caused a serious collision. I was fortunate that traffic conditions allowed me to safely pull over. There are no recalls or technical service bulletins publicly listed for this issue, yet this is a critical safety defect. A broken gas pedal can result in complete loss of acceleration control, which could cause injury or death. Steps Taken: •Stopped driving the vehicle immediately. •Documented the defect with photos. •Contacted Toyota and my local dealership for inspection. •Reporting this defect to NHTSA so it can be investigated and tracked. Safety Risk: Loss of accelerator function can cause sudden loss of power in traffic, preventing safe maneuvering and significantly increasing the risk of an accident.Read more

I pulled up to a red light in a protected left turn lane. My car is set to use regenerative braking and hold/not roll once stopped. I felt my car roll forward a bit, which shouldn't happen when these settings are on. I quickly checked my rearview mirror, assuming someone had hit me from behind to cause me to roll forward. No one was behind me. I hit my brake and the car suddenly accelerated at ful
... l throttle speed, I would guess, into the intersection, despite the light being red for me. I quickly glanced down at my foot to confirm that it was on the brake pedal and it definitely was. I continued to repeatedly push the brake pedal down to no avail. I was steering and tried to navigate my way through 4 lanes of moving traffic because they had a green light. I hit the front end of a car, which didn't slow my car at all. I went on to hit another car's front end, which didn't slow my car at all either. My car then charged out of control to hit the curb on the opposite side of the street facing traffic. Hitting the curb stopped my car finally. I noticed that the airbags on the right side of my car were deployed. Components that failed/malfunctioned: brakes, fuel/propulsion system, speed control, automatic emergency braking, collision warning/avoidance, traffic signal detection Tesla Insurance has deemed the car a total loss. I also reported the malfunction to their Vehicle Support but haven't gotten the report or the data report from the car that I requested yet. I was told that the report would be complete by 8/7/25. The car was placed on a hold for the investigation, so it should be available for inspection. I do have the dashcam videos and photos of my car, if needed. To Whom It May Concern: I am writing to formally request the National Highway Traffic Safety Administration’s assistance regarding a critical safety defect in my 2015 Mercedes‑Benz GL 450 (M278 engine). I purchased this vehicle in October 2024 with 76,000 miles, and it now has about 87,000 miles. Alarmingly, I recently received a quote for $13,000 to repair a camshaft failure and extensive
... electrical damage. I was unaware of these issues when I purchased this car. I bought it under the illusion that it would be a safe vehicle for me and my children. As a single parent and teacher I invested in this vehicle what I could afford and felt I had purchased a safe, reliable vehicle. I am devasted to have a proposal for repairs in excess of $13,000 from a Mercedes dealership who diagnosed the car. This caused me to do some research and I found the GL450 has a known and widespread defect. I also found, that this causes the cam shaft to need to be repaired and that this is not normal wear-and-tear—it is a systemic design vulnerability common across GL450 models. This can result in the engine seizing from what I was told and that directly poses a substantial risk when driving. From what I found this is a common issue with this car and I did not know that when I purchased it. This is a design flaw, not a misuse of the vehicle. Finally, a $13,000 repair to make this car safe and road ready is astronomical and is not aligned with a normal maintenance cost- manufacturer accountability is warranted here. I respectfully request that the NHTSA: Investigate whether a safety recall or extended warranty program is appropriate for this known defect in the GL450. Require Mercedes‑Benz to provide a comprehensive repair solution for me and repair this defect in my car's engine under the manufacturer's expense. And finally, for Mercedes to make customers affected by this aware of this engine defect and long term risks associated with this issue. Respectfully, Alyssa BeginRead more

Coolant is leaking into the head gasket of the 2014 Ford Fusion Hybrid similar to the other 2014 Ford Fusion cars (they have a recall on them for this issue). Upon calling the nearest Ford dealership, I was told that the recall did not apply to my vehicle, even though the same recall has been made for other 2014 Ford Fusions (just not the hybrid specifically). The safety of the driver and passenge
... rs in the car is at risk due to the coolant fluid mixing into the engine oil, eventually causing a blown head gasket and potentially engine failure. This problem was identified within minutes of getting a routine oil change, as coolant was found in the cap of the head gasket. The vehicle has not been inspected by the manufacturer, police, or insurance representatives. There were no warning signs. The car notified me when I needed to change my oil, but otherwise, no dash lights came on and no notifications were sent to the dash.Read more

In November 2024, my 2019 RAM Rebel displayed “SERVICE SHIFTER,” “PARKING ASSIST,” and “CHECK ENGINE” lights. Dealer performed software updates, but one week later the lights returned. Dealer then replaced the parking assist module, but two weeks later the same issue happened again. I have brought the truck in multiple times since. Problem continues, and now the truck has begun putting itself into park while driving or when stopped. This is an ongoing and serious safety hazard. I also added photos of others affected by the same issue.Read more

The vehicle looses power and hesitates to accelerate from a stop start. This happens when The HV Battery State of Charge (SOC) is below 30%. •The vehicle is in catalyst warm-up mode (cold start). The Motor Generator (MG) is in charge mode. As a result of this scenario I have almost been T boned 3 times while attempting a left turn from my home across a dual carriage lane. And again nearly rear

The Lucid drive assist feature gets confused by lane splitting motorcycles in California. This incident has occurred to me multiple times, including most recently today. I drive in the high occupancy vehicle Lane, and during rush hour in the evening commute the left lane drives at somewhere between 20 to 30 mph. Lane splitting motorcycles are able to go slightly faster than that, at around 30 to 4
... 0 mph. When Elaine's splitting motorcycle passes between my car and the next one over, the drive assist feature gets confused as the motorcycle moves further away from the front of the car. The drive assist feature thinks that traffic has cleared, and the car begins to lunch forward at a high speed. I have to intervene to prevent my car from crashing into the car immediately in front of it, which is still moving at a much slower speed.Read more
