Yesterday, January 30, 2026 as I was driving (approximately 30mph) my car seized, came to a stop, dashboard lit up with multitude of words/symbols. After approximately 15 seconds I was able to use the gas pedal and the car drove. My safety was at risk, this sudden stop jolted me hard. If there was another vehicle behind me I would have been hit. The warning messages were so quick and cleared
... away with seconds that I cannot recall. I drove to my local dealer and the battery cables were checked and ok. I scheduled an appt for Monday Feb. 2nd as this was Friday. On January 21st this local dealer replaced my battery, after months of "computer glitches" that were never resolved with two dealers and 4 dead batteries.Read more

Summary of Incident: On August 16, 2025, while my daughter was operating the vehicle at approximately 35 MPH, the front driver-side wheel detached entirely from the vehicle, resulting in a catastrophic loss of control and the vehicle being declared a Total Loss. Technical Details & Safety Hazard: The vehicle had undergone a comprehensive wheel and brake service at a licensed dealership, Ron Tonki
... n Hyundai, in Gresham, Oregon, 39 days (approx. 600 miles) prior. Post-accident inspection by an independent tow operator and mechanical review revealed: *A sheared wheel stud on the failed hub, which dealership service management later admitted was "likely over-torqued" during service, leading to latent metal fatigue. * A systemic failure of wheel mounting: Lug nuts on the other three remaining wheels were found to be loose or missing. Safety Risk: This incident represents a critical safety failure. The detachment of a wheel at speed poses a life-threatening risk to the occupants and other motorists. The systemic nature of the loose hardware suggests either a failure in service procedures, a defect in the lug/stud hardware compatibility, or improper torque specifications being applied to this model. I am reporting this to ensure a record of this structural failure is maintained should it be part of a larger pattern of hardware fatigue or service negligence.Read more

A handheld scanner was used to retrieve DTC: P1326 (KSDS – Connecting rod bearings failure). The vehicle was towed to the residence, and then towed to the dealer. The dealer diagnosed the vehicle and informed the contact that the rod bearings had failed, causing engine failure. The dealer notified the manufacturer of the failure; however, the manufacturer declined to repair the vehicle. The manufacturer informed the contact that the prior owner had submitted a complaint regarding the failure in January of 2024. The vehicle remained at the dealer unrepaired. The vehicle experienced a loss of propulsion 2 times when idol at traffic lights due to an uncommanded shift into neutral while stopped at a traffic light, requiring the vehicle to be restarted to regain drive. These events occurred after a longstanding history of front end vibration and grinding noises that have been repeatedly reported to a GM authorized dealer since October 2024. The most recen
... t GM authorized dealer repair order documents the loss-of-propulsion event, metallic oil and states that the issue could not be duplicated, that no root cause was identified, and that engineering could not rule out a connection to known recall-related engine issues after 32 days at the dealer with 198 miles driven. At this time, no corrective repair has been identified, and the condition is intermittent and unpredictable, creating a safety concern when operating the vehicle in traffic. I uploaded some repair orders including one from the first incident in October 2024 and then I uploaded the most recent repair order from January 2026 where they are still unable to replicate the issue or say the car is safe to drive as this is potentially related to the L87 recall per service manager notes . There are several repair attempts in between the original and the extended out of service for loss of propulsion in December and January.Read more

On January 7, 2026, shortly after delivery of a used 2015 Hyundai Tucson (VIN [XXX] ), the vehicle experienced immediate mechanical failure while being driven, resulting in sudden loss of power and drivability. Multiple warning lights illuminated on the dashboard at the time of failure. The vehicle became disabled and left the occupants stranded roadside in Fairhope, Alabama. The exact component
... that failed is UNKNOWN at this time. The failure appears related to the engine, powertrain, fuel/propulsion, and/or electrical systems. The vehicle was not operable after the incident. On January 8, 2026, the selling dealership (Sandy Sansing Nissan, Pensacola, Florida) towed the vehicle back to their dealership. The vehicle has remained in the dealership’s possession since that date and is available for inspection upon request. This sudden failure created a significant safety risk due to loss of propulsion while in traffic and exposure to roadside hazards. The problem has not been independently repaired or evaluated by the consumer. No manufacturer inspection has occurred to the consumer’s knowledge. The dealer has not provided a diagnostic report or repair findings. No warning lights or symptoms were disclosed prior to delivery. The failure occurred almost immediately after delivery. The vehicle was sold and delivered by a licensed dealership. The sale is currently in dispute because the vehicle became disabled almost immediately after delivery due to this safety-related failure. My 2025 Toyota Sienna Limited is subject to NHTSA Safety Recall25V668 (Toyota Recall No. 25TA12 (Interim Notice 25TB12)) regarding improperly welded second-row seat rails. The recall notice, which was mailed on 11/21/2025 and received by me the first week of December, 2025, states that these welds may fail in a high-speed collision, preventing the seats from restraining occupants and increasing th
... e risk of serious injury. Toyota interim safety instructions advise that second-row seats should not be occupied until repaired. Prior to receiving this notice in the mail from Toyota I was unaware of the defect. There were no service lights or notifications in the car's electronic panel. I have since learned that the notice was issued on 10/7/2025. My vehicle has been grounded at Conicelli Toyota Service Department in Conshohocken, PA since 12/22/2025 - It has been inspected for this defect and to date the service department manager does not have the parts to repair it and cannot give me an expected date for the part to be available and repairs performed. I purchased this vehicle in full on 3/6/2025. Since the recall was issued on 10/7/2025, I have contacted my Toyota dealer several times to inquire on an estimated date for repair of this safety issue. I have also filed a complaint /report with Toyota Brand Engagement Center. As of 1/29/2026, Toyota has failed to provide a definitive date for the repair or a timeline for the availability of replacement parts. I purchased this vehicle to safely transport my four grandchildren -ages, [XXX] , [XXX] , [XXX] and [XXX] to and from school/daycare every day. Because I cannot safely use the second-row seats, the vehicle is unusable for its primary purpose of transporting my family. The vehicle has been effectively "out of service" for safety reasons for more than 90 cumulative days. I am submitting a vehicle defect complaint pursuant to 49 U.S.C. Chapter 301 regarding a potential safety related defect involving engine, electrical, and powertrain systems. The vehicle experienced early engine and electrical malfunction at relatively low mileage, requiring replacement of the engine wiring harness and removal and reinstallation of the engine and transaxle assembly. These are majo
... r powertrain and electrical repairs not reasonably expected on a nearly new vehicle and raise concerns regarding vehicle reliability and safe operation. The vehicle displayed a Malfunction Indicator Light warning indicating an engine and emissions system malfunction, with warnings that continued operation could cause additional damage. Such warnings and failures create a risk of reduced power, drivability issues, or further system failure while operating the vehicle. Insurance repair documentation reflects the selection of a recycled LKQ drivetrain related component, including a recycled engine, transaxle, or CVT related component, which raises additional concerns about durability and safety when installed in a nearly new vehicle. Early powertrain and electrical failures involving engine management and emissions systems may implicate compliance with federal motor vehicle safety standards and warrant review under NHTSA defect monitoring authority. I am requesting that NHTSA log, track, and evaluate this complaint for defect trend analysis, potential noncompliance, or further investigation under 49 U.S.C. Chapter 301 to determine whether similar failures are occurring in other vehicles of the same make and model.Read more

All four cylinders misfired in my car while driving causing the catalytic converter and all four fuel injectors to need to be replaced. My car suddenly stopped driving in a rural area with only a few minutes of warning when the engine light came on. I was not even able to drive to a nearby shoulder. I was stranded on the side of the road in 5 degree weather until a tow truck could come.

Vehicle Safety Defect Report I am submitting this vehicle safety defect report regarding a serious and dangerous failure in a 2021 Nissan Rogue. The vehicle experienced complete electronic shutdowns while driving on the highway. During the most severe incidents, on December 19, 2025 and again on January 14, 2026, the vehicle activated the automatic emergency braking system without external cause,
... shifted into PARK, and shut down completely, leaving the vehicle immobilized in live interstate traffic during heavy rain. The dashboard displayed multiple simultaneous warnings (engine, battery, collision sensors). A repair facility later diagnosed corrosion in the Transmission Control Module (TCM). The module was not replaced, only cleaned, and the failure recurred. This defect presents an extreme risk of collision, serious injury, or death. A vehicle that shuts down and electronically locks while in motion is unsafe to operate. I respectfully request that NHTSA investigate this electronic defect affecting critical safety and control systems. Videos, dashboard images, repair records, and documented evidence are attached. While driving on an interstate highway during heavy rain, my 2021 Nissan Rogue experienced a complete electronic shutdown. Multiple warning lights appeared simultaneously, the automatic emergency braking system activated without cause, and the vehicle suddenly shifted into PARK and shut off, leaving me immobilized in live traffic. This occurred on December 19, 2025, and the same failure happened again on January 14, 2026. The vehicle could not be moved and required towing. A repair facility diagnosed corrosion in the Transmission Control Module (TCM). The module was only cleaned, not replaced, and the failure recurred. The automatic transmission shift cable, has malfunctioned. The center piece that connects to the gear shift has melted, and prevents the car from starting and shifting gears. The damage was created due to high temperatures from the motor, being that it is very close to it. This should be replaced by the manufacturer.

Two weeks ago I heard a faint ticking sound. I had a professional look at it and they said that I had a cylinder sticking and to add an additive the next oil change. I still had 1,500 miles until then and so I did not do that. That noise stopped a day later. Last Friday, it started to tick again, and then it got very loud. I live rurally and was 45 minutes from home. The engine went from ticking t
... o full clunking and very low power in just a moment. I luckily found a small engine shop to check the codes, but as I was parked there, the car would not go in reverse! He had to clear the codes to get it to go into reverse. I had to then slowly drive 45 minutes back to our small town. I drove to and immediately parked it at our local repair shop. It can not be driven now. The lights that came on were the Rear axle system off, forward collision system, service traction control, engine light flashing, and service ESC light. Not to forget that it would not go into reverse. Also the suspension system seemed to malfunction and not lift or lower the car.It seemed to clunk and try, but nothing happened. The whole car basically went down. I see there is a recall for this year or car, but my VIN is not included. I am hoping that it will be included as the repair shop is telling me it will be close to $10,000.Read more
