This is concerning the brake system (disk rotor & pads) on a vehicle with 2600 miles when detected. Vehicle manufacture date was March 2021, owner possession July 27, 2021. There has not been any one's safety compromised. Front Rotors are experiencing scoring which I can only conclude began during delivery of 2300 miles from Nappanee, IN. to Bend, OR. by manufacture. Mileage at inspection was 7700 miles My concern is that this abnormality of scoring taking place with such low mileage and that the condition more than likely began on the delivery drive. Discovered an approximately 4" long crack in the hub plate of a rear wheel. The crack goes through the serial number stamp on the wheel (possible stress riser caused by the serial number stamp?). The wheel has been taken out of service and is available for inspection. No other agency has inspected the wheel. There were no symptoms of an issue (no abnormal tire wear was noticed and no vibrati

I was driving home this weekend in my motorhome and noticed smoke coming out of rear of vehicle. I thought we were on fire. Fortunately I was able to get off the road and no injuries occurred. However, there will be many injuries if this problem is not recalled. The vehicle has a Cummins ISL Diesel motor. Vehicle has 33K miles. The #1 fuel injector line fails. ... many of times. Cummins also has a TSB for failure. Also, the "revised" part comes with a new bracket to prevent future failure. So Cummins is aware of problem. The fuel is spraying all over the engine compartment and straight on the exhaust. This is dangerous and people will die, or at the very least cause extensive damage. This is a sporadic occurrence which is a very safety hazard and an accident is going to happen causing death to others. When slowing down or stopping completely and ready to accelerate, the Brake Applied - Power Reduced comes on locking al
... l wheels and you are only able to go a maximum of 5 mph. You need to hit the brakes hard to release the locked wheels and continue. This does not help the person behind you when you are suddenly hitting the brakes to release this Brake Applied - Power Reduced". This may happen numerous times regardless if you are slowing down, stopping, turning right or left. There was a recall NHTSA #21V202/Manufacturers Recall #21S17 which was corrected due to not enough grease in rear end. Shortly after that the Brake Applied - Power Reduced light occurred of which an entire new rear end was installed into the unit. At Wallwork Ford Truck Center they hooked up the IDS and scanned all modules for codes; all modules passed. They also removed, reset and reinstalled a brake lamp switch after contacting the Ford hotline contact ID #118372635, but the test drive showed the Brake Applied - Power Reduced light came back on. They hooked the IDS computer back on the unit which came back as 'all modules passed'. We are stumped and frustrated as this unit in brand new and cannot be driven anywhere without this "Brake Applied-Power Reduced" light coming on. There is only 2616 miles on the unit.Read more

The SCR/DPF/DEF emissions control system is faulty and results in the vehicle becoming undrivable at unpredictable instances when no actual fault or deficiency exists. This may result in the driver becoming stranded in area with no cell phone signal and during times of harsh weather. It may also leave motorists stranded in unsafe locations with insufficient room to park safely and no capability to
... move to a safe location. This is a result of the system “de-rating” due to faulty sensors or DEF delivery systems or a variety of other causes. Because the system will derate the engine when the sensors indicate a low DEF condition when the DEF tank is sufficiently full, it is impossible for the motorists to predict these failures and subsequent strandings. It is also worthy of note that in the case of diesel motorhomes, these are not professional drivers, they are often older, and their vehicles serve as a conveyance of both their personal possessions and valuables.Further the operative word is in many cases “homes” and these motorists often have no other place to go to seek shelter. It is therefore both unsafe and unethical to force such severe restrictions on the owners of diesel motorhomes. This segment of the diesel vehicle driving population is entirely different from the light truck, SUV, bus, and big rig driving populations for the reasons stated above and as such the derating function of the emission control system must be disabled in motorhomes until the engineering and programming of these systems evolves to the point of reasonable reliability and predictability -conditions which currently clearly do not exist. In my case I have had four instances in less than two months disabling the vehicle when no actual fault exists, after the DEF header has been replaced twice and the vehicle is still unreliable. ... are back ordered until late August - mid September. After discussion with a Cummins service center in Chattanooga, TN it is deemed unsafe to drive the 90 miles to there location. We are told that the engine will derate down to only 5 miles per hour which will lead to a very unsafe driving condition for ourselves and anyone on the road with us. My understanding is that it's just a sensor messing up - the regent was performed as normal while the engine light was lit - indicating all was working with that system.Read more

I have recently returned from a cross country trip and had intermittent AC issues and now realize that my rear lights were also out during those incidents. There was no indication of all rear signal lights being defective in the cockpit other than the AC not working. Vehicle is parked until I get some resolution from Freightliner. I spoke with them today and they said I should take the vehicle to the service center and I explained that I did not feel it was safe to drive the Motorhome in that condition.Read more

While staying at a campground in Creede, Colorado my motorhome displayed several fault codes relating to the emissions system. I immediately called Spartan (the chassis manufacturer) to report the faults and they explained my Def Head sensor had failed and that I should try to get to an authorized service center as ASAP. So the following morning I left Creede ( a remote location that is far away
... from any authorized service provider) in route to Fredrick, Colorado, the closest service provider. I made it to Littleton, Colorado, 25 miles south of Fredrick when the engine derated to around 5 miles per hour. I then had to arrange to have my 45’ motorhome towed the remaining 25 miles. Fortunately for me, the derating happened at a truck stop so I was able to safely maneuver the vehicle to a spot to await the towing company. However, this could have been tremendously hazardous had the derating occurred on the highway, for me, my fellow drivers and the towing company personnel. Now Spartan, the chassis manufacturer that is responsible for the Def Head senor is telling me it may be 3 months until they can provide a replacement part and I should just be patient. Fortunately we have friends in Denver that we are able to stay with for a few weeks while we wait, but we may be forced to rent hotel rooms to wait out the part turn around. We explained to Spartan that we live in Land O Lakes, Florida and are for all intents and purposes we are stranded and are forced to bear great financial burden until Spartan decides to send the service provider the parts. It is also our understanding through communications with fellow RV’rs this same cycle is happening with great frequency putting lives at great risk and financial hardships. I certainly hope you can take action to cause Spartan to action to provide either the parts required to solve this very well known problem with the def head sensor components or at least a temporary fix to enable the vehicles to be driven.Read more

Since purchasing this RV new, I have had the DEF Head fail twice. 1st time was Sep 2019 with a mileage of 14066 and the 2nd time was April 2021 with a mileage of 16018. Both times DEF Header Assemblies were replaced. When these fail the motor can de-rate at any time. Very dangerous when traveling down the highway. Both times the low DEF warning and check engine lights came on.

We got the check engine light on after starting up our coach on July 6. We got several fault codes: SPN 3364 FMI 9, SPN 1761 FMI 9, SPN 3031 FMI 9. The codes indicated that we had a faulty DEF Header and it needed to be replaced. And if we drove the coach at some point it would "derate" to 5 miles an hour and we would not be able to drive it. We were told by the chassis maker (Spartan) that
... we would likely not be able to get a replacement part until September or October due to the global microchip shortage. And even though there was nothing actually wrong with engine or the DEF (we had a full tank) there was no work around. Because we were in West Yellowstone where there was no diesel engine service nor a place we could leave the coach for several months. We decided to drive the coach to our next destination, Lander Wyoming which had two Cummins engine shops. We made it there, but then got the "stop engine" code - SPN 5246 FMI 16. The coach derated and we could no longer drive it. The Cummins service shop was able to clear the "stop engine" code, so we could drive it. But the check engine codes would remain until the DEF header was replaced. And countdown would restart to "stop engine". On the irv2.com RV blog forum, Spartan chassis owners, with Shaw DEF headers have reported the DEF header failure. A number have had multiple failures. Since it seems as though it may be months before a supply of DEF headers will be readily available and there is nothing fundamentally wrong with the engine, it would be tremendously helpful for Cummins or Spartan to design a workaround while the parts supply is rebuilt and it seems likely that Shaw needs to redesign their DEF Header to reduce the failure rate or Spartan needs to find a new supplier.Read more

The component required for replacement is not available till late Aug or Sept. As we live full time in the RV we are now stranded unable to travel until the part is available. Although our example appears to be an inconvenience we have dozens of other RVers experiencing this same problem and unaware of being stranded until the failure occurs. Whatever is causing the failure of these DEF sensors, owners should be notified and a general recall for replacement should be issued.Read more

The DEF sensors regulation was passed by EPA in 2016, stating the DEF concentration be measured in the DEF tanks of vehicles. I believe that Shaw Development is currently at the sixth generation of the DEF sensor which are still failing regularly on brand new 2021 motorhomes. That is six attempts to make a part that continues to leave people on long trips stranded. As a female driving my coach, this is a major problem. First, because I cannot even drive my brand-new coach anywhere but also because the prospect of getting stranded on the side of the road, as a female, is too frightening to risk. These vehicles, built on a Spartan K3 chassis and powered by a Cummins ISX-12 engine are experiencing very high failure rates in the DEF system control head. When the control head fails, it sends error codes that result in derating engine power dramatically and/or immediate engine shut-down. This creates a very dangerous situation for anyone driving a motorhome that experiences this problem. In

Spartan Chassis says replacement DEF control heads are in extremely short supply and they will not provide one to a repair facility unless the affected vehicle has experienced a full derating; an initial warning message of a failing control head is deemed insufficient cause for them to provide a replacement part. Their official response is that “you must drive it until it fails before we will provide a replacement unit.” This means drivers of these motorhomes will either be subject to sudden (and, potentially very dangerous) engine shut-down, or they must cease using their expensive motorhome until such time as Spartan Chassis feels they again have a sufficient stock of replacement units. Many motorhome owners use their vehicles full time; they are their home. The inability to safely use them places a huge burden on those owners. I believe NHTSA needs to issue a recall on affected vehicles and direct Spartan Chassis to implement a software emergency bypass of the engine derating caused by a faulty DEF control head. It is my understanding that this software bypass solution already exists and is currently implemented on emergency vehicles (fire trucks) produced by Spartan Chassis.Read more

The mounting brackets for the rear axel sway bar was missing a bolt in each bracket. Passenger side bracket was hanging below sway bar, driver's side had top bolt missing and was loose. Talked to Ford tech and was told to install a grade 8 bolt with a lock washer and nut. These bolts are M16s, grade 10.9, can I replace them with 5/8 inch grade 8 bolts?

Shortly after departing from J&J RV Park in Hot Springs, Arkansas, in my vehicle, a motorhome, two yellow check-engine lights illuminated on the dashboard. I contacted the vehicle's chassis manufacturer, Spartan Chassis in Charlotte, Michigan, and was told the codes indicated the vehicle's Diesel Exhaust Fluid (DEF) head was defective and needed to be replaced. The gentleman told me not to drive the motorhome and to remain in a safe haven off the road, which in my case is the RV park. He said the limited number of DEF heads in Spartan's inventory are being held for customers whose engines have displayed red stop-engine lights, gone into limp mode and stranded the customers on the side of a road. Those customers receive a new DEF head by overnight mail, are back on the road within a couple days and are able to continue their normal travels. The limited inventory does not allow for customers like me who are not broken down somewhere to receive the DEF head quickly. Spartan's inventory is not anticipated to be replenished until sometime in August 2021. Therefore, if I choose to follow Spartan's recommendation, which I plan to do, I have to cancel campground reservations and summer travel plans and stay in place for at least two months. I'm on a waiting list to receive the part. This is a well-known, long-term problem on motorhomes manufactured by Spartan and other companies, and is supposedly affecting the trucking industry. It has been much discussed on online forums. Spartan's DEF head is produced by Shaw Development LLC in Bonita Springs, Florida. The problem has not been confirmed by a local dealer or independent service center. The vehicle is available for inspection.Read more

Diesel Exhaust Fluid Control unit failed 04/28/21 @ 21,151 miles. Unfortunately this can cause the motor to de-rate with speed restricted to 5mph. Very dangerous on the freeway. Problem was confirmed by the Cummins codes generated and verified by both Spartan motors as well as the repair facility that performed the repair. No warning lamps, symptoms, or messages prior to the failure.

On 5/23 I started getting a code "SPN3031 FMI9" which says the DEF temperature is rising too fast, no other codes were present in the fault list (the engine was running at the time I grabbed the fault list). Only the engine outline icon was lit. I had only driven 3 miles from my house to fill up the RV with fuel. I turned the engine off and restarted, getting the same code. After driving back
... home I pulled up the list again and it was the same. On 5/25 I contacted the local repair shop and they took my information. They called back on 5/26 saying that Spartan (chassis manufacturer) thought I should have more codes. Early on 5/27 I pulled the fault list from the RV with the key in accessory position and I got 2 fault codes, the SPN3031 and also a "SPN1761 FMI9" but it also showed different icons than the prior query, also showing the "check engine" and "Regen" lights. Since I had seen 2 different fault sets I decided to remove the key and try again. I only got the single engine outline icon and the SPN3031 code. I didn't receive the SPN1761. I provided this information to the repair shop shortly after they opened and about an hour later I got a call back from Spartan. They indicated that I would need a new DEF head, the one I had was failing and it was a very common problem this year such that they had none in stock. They also said they weren't sure when they could get me a new one or how many miles the RV could be driven before it would start to derate it's power. I fully support the DEF improvement in air quality, but it is not safe to have a vehicle that is going to start derating power potentially leaving it useless and no ability to get the warranty replacement parts quickly. Manufacturers should have to supply parts for warranty claims before they can sell new items when there is a shortage. The consumer shouldn't be left with an inoperable vehicle while they sell the limited supply to new consumers for a profit.Read more
