I am submitting a formal complaint regarding a persistent and dangerous issue with the MMI (Multi Media Interface) system in my 2020 Audi S4. Since February 2025, the system has been spontaneously rebooting both while the vehicle is idling and in motion. When this occurs, the screen goes black and critical safety systems such as the rearview camera, parking sensors, and navigation become unavaila
... ble. The system requires driver interaction to restore functionality once it restarts, creating a major distraction. These reboots now occur every few minutes, severely affecting both usability and safe operation of the vehicle. Initially, this issue happened infrequently (about once every two weeks), but over time it has become constant. Based on extensive online reports from other owners—including threads such as this one on AudiWorld: [XXX] appears to be a widespread defect affecting other Audi A4/S4 vehicles of the same year. This malfunction compromises several driver assistance and safety features, and I believe it presents a serious hazard, especially if it occurs during parking, reversing, or navigating unfamiliar areas. I urge NHTSA to investigate this matter and determine whether a recall or safety bulletin is appropriate. Thank you for your attention. Description of Problem: While driving under normal highway conditions on July 2, 2025, my Tesla suddenly and violently phantom braked — meaning it rapidly and without cause applied the brakes forcefully. There were no obstacles, vehicles, or hazards present. The sudden deceleration caused a severe flare-up of a recent cervical disc replacement surgery I had undergone. I experienced extreme nec

I contacted Tesla requesting the logs from the incident and have received no response. I am in the process of retaining legal counsel and intend to pursue this further as a product liability and injury case.Read more

However, no solutions or alternative options were provided to address this issue. At the beginning of April, 2025, Toyota began sending unrequested solicitations to subscribe to its “connected services” to the multimedia display in the center of the dashboard of the RAV4. Those solicitations take over the entire screen and prevent any other use of the display until the driver takes affirmative steps to dismiss the solicitation. They also interrupt the connection between an
... iPhone and the display, which requires the driver to completely reset the connection between the iPhone and the display. It can take up to five or six separate steps to click through additional screens, in different locations on each screen, before the display returns to its original state. This is particularly dangerous when the prior use of the display was for navigation, as the map cannot be returned to without going through those screens and presses. The steps required to remove these unwanted solicitations create a materially dangerous likelihood of distracted driving. I have never at any time given Toyota permission to send me these unwanted and intrusive electronic message solicitations, and I have explicitly revoked any and all such permissions which Toyota might claim were created by my purchase and.or operation of the vehicle. I request that an inquiry be opened and that the inquiry determine that Toyota's forced interruption of the operation of a vehicle such as my RAV4 is a safety defect and that Toyota be ordered to discontinue its dangerous practice.Read more

At only 44,318 miles, my 2019 Acura MDX began showing clear transmission problems in the lower gears — including delayed acceleration, gear slipping, and jerky movement when starting or turning at low speed. These symptoms posed an immediate safety risk, especially when merging into traffic or making turns across oncoming vehicles. I first brought the car to Oakland Acura, which incorrectly diag
... nosed it as a transfer case issue. Their misdiagnosis delayed the repair process and added unnecessary cost and time. I then had the vehicle evaluated at Acura Concord, where it was correctly diagnosed as transmission failure in the lower gears. An independent, certified mechanic also confirmed the same diagnosis. Despite multiple confirmations of premature transmission failure — and a full maintenance history — American Honda offered only a $900 discount on a $9,600 repair, citing the vehicle being just outside the time warranty (but with low mileage well under 50K). I believe this is a manufacturer defect, not a wear-and-tear issue. Safety Concern: This transmission failure is not just inconvenient — it is dangerous. The vehicle hesitates when accelerating from stops or low speeds, which can lead to accidents at intersections, highway merges, or left turns. The misdiagnosis further delayed identifying and addressing this issue. I am filing this complaint to urge NHTSA to investigate whether this is part of a broader pattern with Acura MDX transmissions, and to help prevent similar risks for other drivers and families.Read more

Vehicle Information: * Make: Toyota * Model: Corolla Hybrid AWD * Model Year: 2024 * Current Mileage: [14,000] * Mileage When Problem Noticed: ~8,000-9,000 miles Incident Description: My 2024 Toyota Corolla Hybrid AWD has a severe steering defect. After a right turn, the steering wheel consistently fails to return to center, remaining 5-15 degrees (up to 20 for sharp turns) off-center to the
... right. It does not self-correct. Occasionally, this also occurs after left turns, but less frequently. The vehicle requires constant, active manual correction to stay straight. If I release the wheel, the car can drift right, posing a significant collision risk. The steering feels "floaty" and lacks the expected resistance to return to center. This issue began abruptly around 8,000-9,000 miles. Multiple dealership visits and alignment checks have not resolved it. No prior accidents or modifications. The Lane Tracing Assist (LTA) system functions smoothly, keeping the car centered. However, immediately after disengaging LTA, the manual steering defect reappears, indicating an underlying mechanical problem, not an electronic one. Safety Concern: This defect creates a serious safety hazard. It demands constant driver attention and effort, leading to distraction and fatigue. This compromises directional stability and significantly increases accident risk, as the car will veer off course if not continuously corrected. I urge NHTSA to investigate this defect. Similar "sticky steering" or "poor return to center" issues are reported in other modern vehicles, and Toyota itself issued CSP 22TE02 for a related "slower to return to center" issue in 2018 Camry models, proving this is a known type of defect, not normal operation.Read more

While towing my fifth wheel trailer, the truck intermittently disconnects the trailer brakes while in motion. This results in a sudden loss of roughly 75% of my braking capability, creating an extremely dangerous situation. I have a professional driving background, which has helped me avoid an accident so far, but this issue could easily result in a collision, especially under emergency braking or
... downhill conditions Service Attempt: On June 17, 2025, I brought the vehicle to Madras Chevrolet in Oregon. The dealership acknowledged the issue but stated the truck is “within GM parameters,” so no repairs were made. They claimed the problem might be with the trailer, but I’ve confirmed the trailer works properly with other tow vehicles. This appears to be a systemic electrical or control issue with the truck. Safety Concern: No vehicle should be allowed to disable trailer brakes while moving. This is a clear design or control system fault. I am concerned that other GMC or GM truck owners may be at similar risk and urge the NHTSA to investigate this problem before it results in serious injury or death. PS. “Front passenger Safety restraint system fault, service required” RCM2_a636 message keeps appearing on the screen. It happened right after I bought the car, so I took it to Tesla to fix. This could involve seat belts, impact sensors, occupant sensors, or related safety components. • If the airbag warning indicator stays on, the vehicle should not be driven until inspected. • Recurring faults require immediate Tesla service attention. • Pre-tensioner or sensor damage can compromise proper seat belt functionality.” This is a serious safety issue that should be addressed in a recall as it could jeopardize occupant safety of the seat belts or air bags do not work. It also appears to be somewhat widespread and persistent. Tesla also does not appear to have a permanent fix as the first time I took it in they made a comment about one repair already having been done on this before I got the car as a certified used one from Tesla.Read more

I am the original owner of a 2014 GMC Sierra 1500 (VIN: [XXX] ) and began experiencing serious mechanical and electrical issues while the vehicle was still under warranty around 40,000 miles. These included engine RPM fluctuations at idle, vibrations under acceleration above 65 MPH, and intermittent misfiring — all symptoms that are now believed to be early indicators of defective lifters or val
... ve train components. I also experienced repeated electrical failures including a rear turn signal malfunction due to melting/corroded taillight sockets, a thumping rear-end noise, differential knocking, and body panel issues (hood lifting at highway speeds). These were reported multiple times to authorized GMC dealers (Mike Haggerty GMC and Rizza GMC), but I was told there was no issue or no repairs were made. GM refused to assist when I escalated the issue in 2017 via a detailed email, despite all problems occurring within the warranty period. I’m now dealing with a persistent ticking sound from the engine and loss of power, which matches widespread reports of lifter failure in GM’s 5.3L/6.2L V8 engines. These failures pose both a mechanical and safety risk due to stalling, misfiring, and potential electrical fires (in my case, from a failed taillight socket). A class action lawsuit ([XXX], et al. v. General Motors LLC) has been filed regarding these known defects, yet GM has not issued a recall. I’m filing this complaint to add my case to the record and urge NHTSA to investigate these issues.
