The steering hasn’t completely failed but it is not operating correctly. The steering wheel feels like it’s stuck in a detent or notch when going straight and takes a little bit of force to turn it left or right to make minor corrections. Then the change of direction is sudden not gradual, possibly impacting me and the traffic around me. It feels like I’m fighting against the Lane Departur
... e system but there are no warning lights coming on . When it moves out of the “notch” the steering feels somewhat heavy. This, so far, has been an intermittent issue, but a very disturbing one. I’ve been to the dealership with it but everything seems normal when I go in. This all started at just over 42,000 miles. Everything points to the electronic rack and pinion beginning to fail, as hundreds of 2017- 2019 CR-V owners have indicated. There seems to be a system failure on these cars. A very important system but no recall from Honda???Read more

Had an issue with driver assist radar when in complete control. Until you’re not! Brakes lock up skidding tires and causing seat belts to lock up. It happens so suddenly it scares you to the point of heart attack. This has happened: merely backing out of driveway, when accelerating in your lane with car in turn lane beside you turning right. And most recently on FL Turnpike while in stopp
... ed/slowed traffic due to emergency vehicles trying to get to scene of Naples wildfire. When I accelerated as traffic resumed at slow speed the car locks up brakes, squalling tires and slewing sideways. It startled me so much my heart was racing and I didn’t know if the car was still running or not. There was an 18 wheeler coming up behind and I was frozen not knowing what to do next. I don’t know if sensitivity can be adjusted or what it needs but my heart can’t take many more of these incidents. I will report to dealer as well. I know there is a Reddit thread about these same issues that my daughter found on researching this issue.Read more

What component or system failed or malfunctioned, and is it available for inspection? The engine experienced catastrophic internal failure. A connecting rod broke and punctured the engine block, rendering the vehicle completely inoperable. This failure is consistent with overheating and internal engine damage. The vehicle is currently available for inspection upon request. How was your safety or
... the safety of others put at risk? The failure occurred suddenly while driving, without warning. The vehicle lost power immediately, which could have resulted in loss of control, inability to accelerate, and exposure to traffic hazards. This created a serious safety risk to myself and others on the road. Has the problem been reproduced or confirmed by a dealer or independent service center? An independent mobile mechanic inspected the vehicle and confirmed catastrophic engine failure. Based on the condition of the engine and damage observed, the failure is consistent with severe overheating and internal mechanical breakdown. Full written diagnosis may be pending. On December 24, 2025, I was traveling from Arizona to California when WITHOUT WARNING I lost all power from my Ford EcoSport while on the 10 freeway. At that moment I was able to navigate to the side of the road. This placed myself and others in an unsafe situation on the freeway. I have lost confidence that this vehicle will ever be safe and that my safety is at risk and that it is unrepairable as it is still sitting at the dealership it was towed to. By way of background, on May 19, 2025 I brought my Ford EcoSport for a pre-scheduled appointment to a dealership in Phoenix, AZ to have recall manufacturer number 23S64 and 25S12 completed. I was told by the service tech that they could not assess the NEED for 23S64 due to a recent oil change in the vehicle. I later learned that 23S64 is a 100% replacement rate and that a recent oil change should not matter. I won't go into the negligence of this dealer in Phoenix, AZ., but for sure this vehicle is not safe for me or anyone on the roadways.Read more

I am reporting a manufacturing defect involving General Motors’ Active Fuel Management (AFM) system. In October 2024, my 2016 Chevrolet Silverado 1500 LT, at 96,000 miles, experienced a lifter collapse which caused sufficient wear to the camshaft that the engine needed to be replaced, which was performed. The malfunction indicator lamp turned on and the truck was driven an additional 2 miles to
... a safe location and subsequently towed to the repair shop. Before then, there had been no indication that a lifter had collapsed or that there was damage to the engine sufficient to warrant a replacement. In November 2025, I experienced another lifter collapse (on the replaced engine). While the mechanic recommended an engine replacement, the warranty company only authorized a replacement of the affected side cylinder head and lifter, which was performed. In February 2026, I experienced another lifter collapse, and the truck is currently being evaluated by a repair shop. These failures have occurred without warning and aligns with thousands of consumer complaints regarding AFM-related oil consumption, lifter collapse, and engine damage in GM vehicles. This is a known design defect that GM has tried to address through Technical Service Bulletins and engine redesigns, yet many consumers continue to experience premature failures at low mileage. This defect presents a safety and reliability concern, including loss of power while driving and significant financial burden to vehicle owners. I respectfully request that NHTSA investigate this ongoing defect.Read more

Description of Incident: On February 9, 2026, at approximately 3:00 PM, I was driving my 2021 Kia Soul on Calvine Road and Winterberry in Elk Grove, CA. A vehicle suddenly merged into my path. I immediately attempted to apply the brakes, but the pedal failed to depress or engage normally. Despite the imminent collision, the Forward Collision-Avoidance Assist (FCA) failed to provide any alert, and
... the Automatic Emergency Braking (AEB) system did not activate. To avoid a high-speed, rear-end collision, I was forced to perform an emergency steering maneuver toward the curb and into roadside bushes to stop the vehicle. This maneuver resulted in severe mechanical failure, including a broken driver-side axle, damaged rims, and broken brake pads. The dealership (Elk Grove Kia) has provided an estimate of $9,461.67 for these repairs but has failed to explain why the primary safety systems and service brakes failed to function as designed during this emergency.Read more
