When driving with the adaptive cruise control on, the cruise will shut off without notice at random times. The forward collision warning light then comes on. After driving for some time the light will go out and the adaptive cruise will work again. This repeats many times on a trip randomly. This also leaves the vehicle without forward collision protection and automatic emergency braking. This is
... very dangerous at highway speed when you lose power driving with traffic. When reported to the dealer, they said it was a known issue without a solution and to not use the adaptive cruise and drive carefully without the forward collision protection active. This has been an issue since the car was new in November of 2024.Read more

The front subframe on the drivers side of the vehicle broke while maneuvering a right turn at a safe speed. This incident caused additional damage to the vehicle, with noticeable damage to the quarter panel. Additional damage is expected and may include the tie rod, wheel bearing, struts, sway bar link, etc. The driver and passenger were put at risk due to the severity of the break and the inabili
... ty to move the vehicle. The speed of the vehicle may have prevented further risk to the driver and passenger at the time of the incident. The issue with the subframe was confirmed by an independent service center due to the lack of a Mitsubishi dealer is proximity. However, a Mitsubishi dealer was contacted but no response was received. It is believed that this issue is associated with the discovered Unrepaired Recalls associated with this VIN. A police officer arrived at the incident and a tow truck was ordered by the officer due to the danger of remaining in the location of the incident. The issue with the subframe first appeared on Sunday, May 31st 2026 with a noise produced by the vehicle while driving on unsmooth roads and through turns. The vehicle was inspected by an independent service center on Thursday, June 4th 2026 and significant erosion was discovered on the front subframe. A Mitsubishi dealer was contacted on Friday, June 5th 2026 and a voicemail was left. No contact was received. The incident occurred on Saturday, June 6th 2026. I've owned it year and half. The tire walls are bad but green light on tread..all 4 tires. The mitsubishi lot said that shouldn't happen and sane companies service dept should have done something. But no, they are wanting me to pay almost 1k for new tires, a 25k mile brand new car should not need tires. Service dept blames brand of tires. The rear backup camera on this vehicle is excessively dim and difficult to see under normal daylight and ambient lighting conditions. The camera image is so dark that it significantly impairs the ability to detect obstacles, pedestrians, and other hazards behind the vehicle when reversing. This is a safety defect because the backup camera is federally-mandated safety equipment under FMVSS 111. The

No aftermarket solutions are available to correct the factory camera output. The problem has been present since the vehicle was new and is consistent, not intermittent. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The suspected failed components involve the 1.5L engine system and head gasket on my 2020 Honda Accord. The vehicle experienced frequent overheating along with repeated check engine light activation. Multiple mechanics, including Heritage Honda Parkville, indicated that head gasket failure is a co
... mmon issue with this engine type. The vehicle remains available for inspection upon request. 2. How was your safety or the safety of others put at risk? My safety was put at risk due to the vehicle frequently overheating while driving. An overheating engine can lead to sudden vehicle breakdown, engine failure, loss of power, or being stranded unexpectedly in traffic or unsafe locations. The recurring check engine light and unresolved engine concerns created uncertainty regarding the reliability and safe operation of the vehicle during daily commuting and long-distance travel. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The concern was initially discussed by technician [XXX] at Heritage Honda Parkville during my service visit on March 19, 2026. The technician advised that the “most common issue” with these engines along with my vehicle is head gasket failure. Additionally, multiple independent reputable mechanics later echoed the same concern, stating they have seen numerous Honda Accord vehicles with the 1.5L turbo engine require head gasket replacement and, in many cases, full engine replacement. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by Heritage Honda Parkville service personnel and has also been evaluated by multiple independent mechanics. The issue has additionally been reported to American Honda Corporate under Case Number XXX. The vehicle has not been inspected by police or insurance representatives. Issue: Total failure of Frontview Camera ADAS sensor, disabling Automatic Emergency Braking (AEB), Front Pedestrian Braking, and Lane Keep Assist. Technical Evidence: GDS2 telemetry confirms a "No Communication" status for the camera hardware. Core safety modules including the Electronic Brake Control (EBCM) and the Inflatable Restraint (Airbag) Sensing Module report active communication failure
... s (DTC U026A/U216A). These safety-critical modules are currently unable to receive the sensor data required for crash avoidance and occupant protection. Forensic Timeline: Vehicle NVRAM reveals a manual "Clear Codes" event occurred at 46,838.3 miles—exactly 0.4 miles after the sale contract was signed. The fault reappeared with a 0 km offset (immediate return) and has been active for 162 ignition cycles, proving the defect existed throughout the dealership's possession. Dealer Response: The selling dealer delivered the vehicle with an unsigned 125-Point Inspection. Despite being presented with GDS2 telemetry proof of the braking and airbag communication blackout, management refused repair, mischaracterizing the disabled emergency braking system as a "luxury feature" and claiming the vehicle is "safe" to drive. Symptoms: "Service Front Camera" warning, intermittent loss of HVAC controls, and gear display (PRNDL) blackout due to systemic serial data bus interference caused by the hardware failure.Read more

My 2021 Tesla Model Y HW3 experienced complete failure of all cameras, GPS, Automatic Emergency Braking, and Adaptive Cruise Control following the installation of software update 2026.8.6.1 — the OTA remedy update issued by Tesla for NHTSA Recall 26V283000. The vehicle was fully functional prior to this update. The failure was confirmed and reproduced by Tesla's authorized service center. The s
... ervice advisor confirmed in writing: "During the update, the computer failed to reboot properly. We attempted to perform a 12V reset but unfortunately it is still stuck in the boot loop state." The Autopilot ECU (Primary/Turbo A) is stuck in a permanent boot loop and cannot be recovered by software means. Safety impact: All cameras including rearview camera are completely non-functional. Automatic Emergency Braking and all ADAS features are disabled. The vehicle cannot safely detect obstacles or pedestrians. GPS navigation is frozen. Tesla has confirmed the computer requires replacement but is classifying it as a customer-pay repair citing expired warranty, despite the failure occurring during Tesla's own recall remedy software deployment. This vehicle meets every technical criterion of Recall 26V283000 — HW3 hardware, 2021 Model Y, software version 2026.8.6.1, complete camera failure — but does not appear in the recall VIN database. I am requesting NHTSA review whether this VIN should be included in recall 26V283000 or a related investigation.Read more

The vehicle is subject to an active safety recall involving the cooling fan module and fuse protection system. The vehicle was held at the dealership for several months related to recall and engine repair concerns and has since been retur

The recall specifically states that the defect may lead to a vehicle fire, creating a serious safety risk for vehicle occupants and others nearby. Although the vehicle has been returned to me, the recall remains open with no completed remedy available, leaving ongoing concerns regarding safe operation of the vehicle. An authorized Dodge/Ram dealership confirmed that the vehicle is affected by the active recall involving the cooling fan module and fuse protection issue. The dealership acknowledged the recall and stated that the repair/remedy remains pending. The vehicle has been inspected by an authorized Dodge/Ram dealership and discussed with Stellantis/Dodge customer care regarding the unresolved recall and extended period the vehicle remained at the dealership. Prior to dealership service, there were no specific warning indicators identifying the cooling fan module defect. The issue was identified through the manufacturer’s recall notice and confirmed by the dealership as an active safety recall affecting the vehicle.Read more

On April 16, 2026, within minutes of being released from a certified dealership, my 2019 Mercedes-Benz GLC 300 suffered a sudden and total loss of propulsion while traveling at high speed on the Belt Parkway (NY). Despite depressing the accelerator, the vehicle failed to maintain speed and stalled in active traffic. This created an immediate life-safety hazard, forcing me to navigate across multip
... le lanes without engine power to avoid a high-speed rear-end collision. The vehicle was released to me by Mercedes-Benz of Brooklyn on April 16, 2026, following a repair for which I paid $2,134.94. The dealership diagnosed the issue as a wiring harness/sensor fault and formally certified the vehicle as "Found OK" and safe for operation. The catastrophic failure occurred only 22 minutes after I departed the dealership lot. The vehicle had to be towed back to the facility immediately. Upon the vehicle being towed back on April 16, the dealership admitted via recorded voicemail that the initial $2,134.94 repair was a misdiagnosis. They subsequently issued a new diagnosis for a "Valve Body" failure and a new quote of $6,424. The dealership's Service Director has since admitted the vehicle suffers from concurrent transmission and engine failures, rendering the car a liability. This represents a critical failure of the powertrain and electronic control systems. Releasing a vehicle that suffers a total power loss on a highway immediately after a "certified" repair is gross negligence. I am reporting this to NHTSA as a catastrophic failure of a safety-critical component (Transmission/Valve Body) that resulted in a "dead-stick" situation on a major New York thoroughfare, posing a severe risk to myself and other motorists. The vehicle remains at the dealership. Mercedes-Benz USA has offered only a $2,000 "goodwill credit," which does not address the safety defects or the initial misdiagnosis. No recall has been initiated for this batch.Read more

The failed components include the Tesla Vision system (cameras), sensors, Active Safety Control Module (AEB), and Gear Selection UI. Symptoms first appeared on April 30, 2026, including black screens and "Autopilot Unavailable" alerts. Internal vehicle logs from Apr 30 to May 02 confirm persistent "SNA" (Signal Not Available) and "UNAVAILABLE" states for these critical safety modules well before t

This total "blind" state put my safety at extreme risk as I had zero visibility of blind spots and received no audible warnings. The Automatic Emergency Braking (AEB) was also recorded as "UNAVAILABLE," failing to prevent the collision. The problem is scientifically confirmed by the vehicle's internal diagnostic logs, which I have retrieved and am ready to provide for inspection. The accident has been reported to the local police and Progressive Insurance. I am currently coordinating with Tesla Service for a formal inspection of these software and hardware defects.Read more

Hello - the steering wheel has become very difficult to turn including inability to turn to the right or left at times, and seems to increase over time but has decreased while remaining stiff/difficult at times.I thought it was related to low/missing power steering fluid or the weather because the steering improved on a warm/warmer day. When I tested the weather theory it was another warm/hot day
... and later in the day meaning the car didn't require as much or any warm up as it usually does on cold days/earlier in the day. The steering was so difficult I couldn't turn the wheel to the right at all and it was extremely hard to turn to the left but less than difficulty trying to turn to the right. I checked under the hood and didn't see anything for power steering, and later learned the 2014 Ford Taurus does not require power steering fluid. This is a safety concern because it means drivers can't steer at all or only with extreme difficulty increasing the potential for vehicle accidents. I don't know/can't say if this is negatively impacting other parts of the vehicle.Read more

... d power steering fluid randomly fail. The vehicle was taken to the dealer, where a pressure test was performed indicating issues with the Continuously Variable Transmission, engine, and brakes. The electrical/infotainment display failure causes the factory rearview camera image not to display when the vehicle is shifted into reverse. The center infotainment display on my 2022 Volvo XC90 intermittently goes completely black and remains inoperable for days at a time. When the screen is black, the factory rearview camera image does not display when the vehicle is shifted into reverse. All
... sound, including warning signals, is inoperable. Climate controls are also inaccessible through the screen, and inoperable. This has happened repeatedly: the screen goes dark for several days, returns temporarily, then goes dark again. The failure affects rear visibility during backing events and creates a safety risk because the driver cannot see the rear camera image. I brought the vehicle to a Volvo dealer. The dealer stated that the infotainment head unit must be replaced and quoted approximately $4,000 to replace it with the same non-Google system. Volvo corporate denied assistance. I believe this is a safety-related defect because the failed infotainment display disables the rearview camera function.Read more

In the last 30 days of obtaining my first brand new vehicle, there have been a couple of incidents where I have put my truck in Park on an incline (hilly street or driveway), and you can hear the brakes grinding once my foot has been removed from the brake pedal. It gives the impression that the brakes have not stopped. It’s unusual to hear and in my concern, I activate the emergency park for ex
... tra security. It’s a little scary to think that the brakes won’t do what they’re designed to do.Read more
