My 2023 Honda Odyssey Elite has recurring, intermittent safety-related malfunctions across multiple systems. The most serious issues are: (1) door lock/unlock and rear door opening failures preventing emergency egress (children trapped), (2) intermittent “no-start/no-response” where push-button start does nothing (no click/no crank and sometimes no lights/electrical response), and (3) intermit
... tent dashboard/instrument/electrical abnormalities while driving. I also previously experienced dangerous driver-assist behavior including unintended automatic braking and lane/steering behavior in construction zones. Door/egress: Rear doors sometimes will not open from inside or outside even when the vehicle audibly indicates it unlocked. I have video of my children unable to exit the back seat. This is an emergency egress hazard. No-start/no-response: Multiple events where the vehicle appears electrically dead and will not start, then after waiting ~10–30 minutes it starts without a jump. Stranding hazard. Recent examples: 12/31/2025 – doors would not open initially; after entry, no lights/electrical and start button produced zero response; after ~10–20 minutes it started. 01/05/2026 – while driving, dashboard/instrument display went off; buzzing noise believed HVAC related occurred. 01/05/2026 – after parking at Costco, vehicle again would not start (zero response); after ~30 minutes it started. 01/06/2026 – locking/unlocking malfunction again; rear doors would not open despite audible unlock (video). Repair history: Authorized Honda dealer (Bell Road Honda) previously stated “cannot reproduce,” replaced DVD/recall items and performed a “reset,” but did not diagnose root cause; problems returned worse. Honda Customer Service case opened. Injuries: Prior unintended automatic braking event (reported around 08/29/2023) caused injury (my legs hit dash; child had marks from car seat) and near rear-end collisions. This submission is a supplemental update to my existing NHTSA complaint regarding my BMW X3. On [XXX], at approximately [XXX], while the vehicle was traveling approximately 40 MPH with the doors locked, the rear passenger door opened while the vehicle was in motion. A [XXX] child was secured in a rear car seat at the time. A [XXX] child witnessed the event while I was dropping him off at schoo
... l. No audible or visual warning alerts activated before or during the incident. This event follows prior electronic access and system reliability concerns reported in November and December 2025, including operation without key presence and unauthorized access behavior. I believe these events may be related and indicative of a broader electronic or body control system failure. Due to the seriousness of this incident involving minor occupants, I no longer consider the vehicle safe to operate. The vehicle is pending inspection and has not been driven further. I am submitting this update to document a potential child-safety defect and to ensure this information is considered as part of NHTSA’s review. While driving, most often at dawn/dusk and commonly with cruise control engaged, the vehicle will falsely detect a vehicle ahead when none is present. It then triggers sudden Automatic Emergency Braking along with a crash/forward-collision indicator, causing an abrupt speed drop of 15–20 mph. This is a major safety concern for my family and me while commuting in our family vehicle. If a vehicle
... behind us follows too closely and does not brake as rapidly as this faulty feature activates, it could cause a severe rear-end collision, or the driver may swerve into the other lane and collide with other vehicles to avoid hitting my vehicle, which has suddenly decreased speed. Other safety concerns related to this faulty braking feature include passenger whiplash and strains when the vehicle suddenly decelerates, instability on wet/snowy/icy roads, and the loss of an important safety feature. This vehicle safety feature cannot be used due to faulty braking.Read more

I am writing to report a serious and ongoing safety issue with my vehicle. While I was actively driving in traffic, my car suddenly shut off completely without warning. This occurred while the vehicle was in motion, creating an immediate and dangerous situation. During this incident, the steering wheel locked up multiple times while I was driving, making it extremely difficult to control the vehi
... cle and safely maneuver through traffic. This put not only my safety at risk, but also the safety of other drivers on the road. In addition to this, there are multiple mechanical and safety-related issues with the vehicle: •The driver’s seat is stuck and cannot be adjusted. •The front doors will not lock. •The back doors will not unlock. These issues significantly increase the risk of injury or harm in the event of an emergency, accident, or even while the vehicle is parked. Given that the vehicle shut off in traffic and experienced steering lock while driving, I do not feel the car is safe to operate. I am requesting that this matter be reviewed urgently, as this vehicle presents a serious safety hazard to myself and others. Please advise on the next steps for inspection, coverage, or resolution as soon as possible. Thank you for your prompt attention to this matterRead more

OEM Tires: 225/55/R18 - Bridgestone Turanza All Season I am writing to raise a formal concern regarding the premature wear of OEM tires installed on my leased 2025 BMW X1. At approximately 15,000 miles, I was advised to replace the tires at 4/32" tread depth, which is significantly below the longevity historically expected of OEM tires. This raises serious concerns around vehicle safety, environ
... mental impact due to early disposal, and the burden placed on consumers. Past OEM tires demonstrated substantially better durability, and I am seeking clarity on what has changed in design, materials, or standards that now allows for reduced tire life. I also request clarification on how this aligns with the safety commitments and environmental responsibility, and whether consumer interests are being adequately considered. I look forward to your written response. Regards, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

In 2023 & 2025, my vehicle had been brought into the Genesis dealership to complete NHTSA 24V868000 safety recall by receiving a new software (Service Campaigns 907G & 025G). However, despite that, on [XXX], the vehicle's ICCU failed while I was traveling at highway speeds. The ICCU is the primary component that the safety recall was supposed to address, yet it failed anyways. The car suddenly and
... dangerously reduced propulsion on the freeway, and I had to slowly maneuver the car onto the shoulder where the car then completely lost all propulsion entirely. My family and I were in the car, and our safety was at extreme risk since the car lost propulsion without warning on the freeway and other vehicles had to swerve around us. There was the very real possibility that had I not reacted quickly, that we would have been fully immobilized on the freeway, blocking traffic and creating a real safety hazard for other vehicles. There were no warning lights or any indications that the part was going to fail -- it failed immediately, and only after that did multiple warning messages appeared due to systems downstream of the ICCU failing as a result of the ICCU's failure. The dealership (Genesis of Dublin) confirmed that the failure was specific to the ICCU and the vehicle had its ICCU replaced under the 10 year powertrain warranty. I have documentation proving that the problem was caused by the ICCU's failure. In addition, the ICCU's replacement with an identical ICCU simply means this extremely dangerous hazard can occur again without warning in 1 to 2 years' time, since the replaced ICCU is the same unit that failed before - no updates to the ICCU have been announced by Genesis or its dealerships. I reiterate: the problem persists despite 24V868000 being preventative recalls completed. Recall 907G Completed 10/26/23 Recall 025G Completed 01/13/25 The above mitigations are not enough to fix the problem and avoid a dangerous situation. Incident date: Dec 23, 2025
I am reporting a safety concern involving a 2024 Lexus RZ300e related to battery range performance and charging requirements in cold weather. In real-world winter conditions in Massachusetts, the vehicle experiences severe range loss, requiring excessive and repeated public charging to remain operable. On a representative day, I was required to use multiple public charging networks (ChargePoint,
... FLO, and Tesla Superchargers), spending over 4 hours physically charging, consuming approximately 105 kWh of electricity, and paying nearly $50 in public charging costs, with additional home charging still required for the next day. Many charging stations are outdoors and poorly sheltered, requiring prolonged exposure to freezing temperatures, wind, and precipitation. This creates a significant safety risk due to driver fatigue. To earn a modest daily income, I must drive for extended hours. When combined with approximately 4 hours per day spent charging, this results in up to 16 hours in a single day spent driving or waiting in the vehicle. Charging time is not rest, as I must remain alert, manage charging sessions, and relocate the vehicle. This extended time causes physical exhaustion and reduced alertness, increasing risk on public highways. These conditions are particularly concerning because my wife is pregnant, and the vehicle was leased as a family vehicle. Prolonged winter charging exposure and fatigue pose risks not only to the driver but to passengers and other road users. The vehicle has been inspected by a Lexus dealer. Dealer documentation states that battery degradation cannot be directly measured, usable battery capacity data is unavailable, and cold-weather range loss is considered “normal.” No corrective repair or adjustment has been offered. The issue is reproducible in winter conditions and remains unresolved. I believe the vehicle’s real-world charging burden and resulting fatigue create an unreasonable safety risk and warrant review.Read more

Dash cluster will go blank upon starting. This has happened every few weeks since I bought it in Nov 2019. Reported to local dealership in ealry 2020, but when you turn off Jeep, the next time it restarts, everything looks fine and they could not reproduce the problem at the dealership and had no answers as they had not seen it before, as no codes were being given. Now the most recent issue, the security system light comes on randomly upon starting. Also have to push start button more than once to get it to actually start, so this could be different issues. A recall for the cluster finally came out last year i think, but my Jeep was not on the recall list even though it DOES have the problem and has had it for years. Would love some help or guidance!Read more

Vehicle: 2024 Jeep Wrangler 4xe Manufacturer: Stellantis / Jeep Recalls Involved: •High-Voltage Battery Fire Risk Recall (park-outside / do-not-charge) •Engine Casting / Sand Contamination Recall (Recall 68C) — risk of sudden loss of propulsion and fire NHTSA Case: If referencing dealer/Jeep case → Case #94839085 Mileage: [insert mileage] VIN: [insert VIN] Complaint Description: My 2024 J
... eep Wrangler 4xe is currently subject to two serious safety recalls: (1) a high-voltage battery recall requiring the vehicle to be parked outside and not charged due to fire risk, and (2) Recall 68C involving engine casting/sand contamination that can lead to unrecoverable loss of propulsion, engine failure, and potential fire. Jeep has notified me that no repair is currently available for either recall and has provided no estimated timeline for a remedy. As a result, I have been advised not to drive or charge the vehicle. This has left me with a vehicle that is unsafe to operate, unsafe to store, and unusable for daily transportation. I contacted Jeep Customer Care and the dealership for assistance. Jeep has only offered a limited 30-day reimbursement-based rental that requires me to pay out–of–pocket and does not cover the full period during which my vehicle is unsafe and unrepairable. Jeep has not provided a loaner or replacement vehicle despite the indefinite loss of use. This situation raises serious concerns because: •The vehicle cannot be safely driven, •There is a real risk of engine failure or fire while driving, •There is a risk of battery fire even while parked, •A remedy is unknown and unavailable, •Jeep has not provided a safe, usable transportation solution. I am filing this complaint because I believe the severity of these defects, the lack of an available remedy, and the manufacturer’s inadequate interim support pose a significant safety risk to me, my family, and the public. I am requesting NHTSA to review this matter.Read more

On October 8, 2025, my wife and I purchased a 2025 Ford F-150 Lariat, 3.5L Full Hybrid, with Ford's BlueCruise hands-free driving feature. As first-time users, I reviewed the owner's manual and Ford’s videos to understand BlueCruise. After several uses, I noted unpredictable behavior and now report a serious safety issue warranting federal investigation. On October 15, 2025, while driving north
... on I-95 near Boston with BlueCruise engaged in hands-free mode on an approved route, I pressed the Memory 1 recall button to adjust my seat. A chime sounded, and the instrument panel stated that Memory Recall is not active while the vehicle is moving. Unexpectedly, pressing the button deactivated BlueCruise, causing immediate deceleration and returning steering control to me. During a shallow left turn, the vehicle proceeded straight, almost deviating from the lane. I quickly regained control, preventing a lane departure. The owner’s manual and Ford’s resources do not mention that Memory Recall can deactivate BlueCruise. This undocumented behavior is highly concerning. On October 20, I contacted Ford (Reference Number [XXX] ), but their response was inadequate, offering only a generic process without technical team access or follow-up commitment. BlueCruise’s lane-centering struggles with unclear or conflicting lane markings. On I-95 sections under resurfacing, temporary markings are placed 1-2 feet from fainter original lines. BlueCruise inconsistently tracks either set, causing unpredictable lateral movements, including sudden lane-edge excursions or rapid shifts over bridges where markings misalign. When another vehicle overtakes closely, crossing into our lane, BlueCruise momentarily loses lane-keeping ability due to obstructed markings, resulting in rapid steering movements. While such overtaking reflects poor driving by others, an automated system must handle these common scenarios reliably. Incident # 1. June 2025 Vehicle has approximately 3400 miles on it. I was driving southbound on I 35 freeway in the Lakeville Minnesota area. I was in the center lane of the freeway traveling at about 65 mph. Traffic was light. There was no one in front of me. Suddenly, the car broke hard. I was thrust forward against the seatbelt with my hands, pushing against the steering wheel as the car slo
... wed rapidly and came to an abrupt stop from 65 miles an hour. I had no ability to get the car to go. The gas pedal had no effect as this car came to a rapid stop. I found myself stopped in the center lane of the freeway frantically looking around to see what cars were coming, can I find the four way flashers. A car passed me in the right lane and a car passed me in the left lane. Then I was able to limp over to the shoulder out of the traffic lanes. I shut the car off immediately and tried to compose myself. After a moment or two, I turned the engine back on and the car operated normally. At the dealership they fixed some things but they said they didn’t find anything that was directly related to the unexpected rapid braking. Incident #2. July 2025 Vehicle has approximately 4300 miles I was driving northbound on 35E taking the entrance ramp to eastbound Highway 36. I was driving on the entrance ramp beginning to accelerate to highway speed. there were no vehicles in front of me, not on the ramp, or on the highway. As I neared the end of the ramp the vehicle suddenly broke hard thrusting me forward again. It came to a complete stop. Fortunately, for the second time, there was no one behind me. This was the second time the vehicle engaged the brakes in an emergency stop for no reason. The Dealership said they couldn’t find anything cause of the incident. The vehicle remains at the dealership. I have requested reimbursement of my purchase price from Honda. We have an arbitration hearing schedlued on Oct 27, 2025Read more

My 2016 T & C was parked on an incline when about 5 minutes after I exited the vehicle, it began to roll backwards. Van was in park and off. It rolled back and hit a church sign which stopped it from going over a steep bank and into traffic on Route 29 near Gretna, Va. Extensive damage to sign as well as probably total loss of my van. Since purchasing the vehicle in January of 2024 , I have had the same repeated safety issues that the dealership and Chrysler were made aware of . I was told that no other vehicles of my make and model had the same issues so I was not listened to . While driving vehicle has slammed on the breaking system for no apparent reason at different speeds. Last time this happened I was on the thruway doin
... g 65 mph. This has happened over 10 times and has been extremely scary and upsetting. The Hybrid system alert has flashed over 30 times and the vehicle has stalled with loss of power. The transmission either jerks or there is a large lag of power -sometimes gas pedal needs to be pinned in order for vehicle to go forward. I have complained to the dealership and Chrysler about the safety issues and how the battery has not work as expected upon purchasing and all the safety issues. I have been mislead by Chrysler on the performance and use of the vehicle. I have asked for a resolution for over a year- lemon law, buy back , but I have to continue to drive an unsafe vehicle. First issue, the entire screen went black while driving. Couldn't see navigation, speed other critical information. Was at the dealership a few weeks got it back and the hazard lights no longer worked but they did before it had gone to the dealership. Screen went out again and took it back. Dealership had it a few weeks fixed hazards and started a ticket with Kia engineering. Screen still goe
... s out randomly. Electrical is messed up and car won't start randomly while screen blinks on and off. Been in the shop multiple times for weeks at a time for this issue and they say there is nothing wrong because they can't duplicate it while they have it. They have had to pick it up from my home because it wouldn't start and still say there is nothing wrong with the vehicle.Read more
