At approximately 11:30 AM on Saturday, August 5, 2023 I was traveling northbound on I-17 just before exit number 278 for State Route 179. I felt the vehicle shake violently and my wife saw a portion of a tire rolling along the side of the road into the desert. I was able to safely pull off to the shoulder, it was then that I noticed the driver side rear tire had completely separated from the rim.

At the time of the incident there was approximately 5355 miles on the vehicle which was purchased new in January 2023. I was luckily able to bring the vehicle to a complete stop without incident and notified Roadside Assistance.Read more

The trim surrounding the windshield on the passenger side detached and flew off at high way speeds. While no vehicles were close enough to be impacted by this part of the vehicle detaching, it could create a hazard had there been a vehicle close by. Since inception of purchasing the vehicle in January 2019, I have experienced a sudden jolt, banging sound/feeling. In the beginning the issue was subtle. In addition, I have felt a sudden loss of speed. It should be noted I did not seek any service at this time. This prompted me to call mopar to see if my current vin was possibly on that first recall of jeeps however i was advised it was not even though i had experienced the same issues described. Fast forward to as of recently august 2023, I received a recall letter with this current vin and proceeded to the nearest dealership for repair. I was advised the repair was a software update. As I was leaving the dealership after the update, I felt the loss of power, jolt and grinding when turning to backup with the service 4wd illuminated. I reparked and walked back inside the dealership when I was advised the repair was only a software update. In addition, I was advised a diagnostic test could be done for $175. I was advised to wait a couple of key cycles to see if the issues corrects itself. The software update has since not corrected itself and I feel as though I am at risk of an accident. This issue is getting worse with my vehicle and is a safety concern. I feel as though I am being penalized for faulty PTU which is on the manufacturer for costs to assure correction/repairs made. In early July, 2023 various warning and fault lights began to show on the dashboard: engine start system, brake lights, start/stop system, and others - indicating malfunctions with systems important to safety. Also, the car alarm system sounded without cause and key pods would not lock/unlock the doors electronically. These warning lights proliferated over several days. We took our car into our
... local Audi dealership. They diagnosed the problem as follows: "Performed GFF and found code for 3393 comfort system control module replacement. Control module faulty due to control unit being submerged in water and fried the control unit. Water leak due to rear drains clogged causing the water to overflow. Performed/fixed the water leak, cleaned out the pools of water that have accumulated, replaced the control unit 3393 comfort system control module, performed control unit replacement test plan and program keys." The Audi Q5 is well known to have design/parts problems in the sunroof drainage system. As has occurred in thousands of other Q5s, those issues caused the rear cargo area to flood, frying an important electronic component and activating warning/fault lights for a range of other systems. Malfunctions in several of those systems could have caused critical safety issues. We are fortunate because we decided to take our Audi directly to the dealership service center. Repairs cost several thousand dollars.Read more

My 2007 Ford Edge / 2.0 Ecoboost engine model (with only 69,000+ miles) had a distinct unsettling shake upon starting, and would occur occasionally during regular driving - causing Check Engine light to appear. Ford dealership inspected and verified that "coolant leaked into the 3rd cylinder - causing a crack in the engine - and only remedy is a total engine replacement." Obviously now an unsafe v
... ehicle to drive. Upon my research / lengthy investigation, I found hundreds of similar failures with this model (to include 2015-2018 Ford Edges with 2.0 Ecoboost engine). Models with this engine version will eventually fail - usually around the 70,000 mile average landmark (which is ironically slightly out of from Ford's 60,000 warranty coverage - rendering this "not covered" by Ford) - and Ford has apparently known this for years. Ford's choices / options offered to me (and upon reading, hundreds of others) center on my being responsible for paying for the replacement - making me responsible for the cost (quoted price from the dealer was around $8200.00) on a vehicle with only 69,000+ miles on it - with a flawed engine design that Ford has been aware of. Was told (in similar if not exact terms) that they aren't responsible for the replacement cost because "There is no Recall on this", and "you are out of warranty" (but by less than 10,000 miles). Or another option is "you can trade it in" (for substantially below what it used to worth). In addition to my being responsible for the cost, the Edge is now worth less than what it was, as if I resell it, I'll need to disclose the problem - and engine replacement. In summary, this is not only unacceptable on the surface, but morally reprehensible. A recall on this (and other similar models), making Ford responsible to costs regarding this well known engine flaw is the right course of action not on for myself, but for the (at least) hundreds with the same problem - and thousands that will eventually have the problem. Thank you.Read more

In the fall of 2022, the driver door on my 2021 Ford Escape would not close properly and I took to the Ford Dealership as I told them it felt unsafe as I felt the door wanted to fall off when driving as the door was physically moving in and out. They laughed at me and said that would never happen. After two visits, they realized the weld had broken on the door and said they would order a new one.
... They said they had never seen that on a passenger vehicle. It took 12 weeks for the door to arrive and they first said they would cover the rental vehicle and then came back the next day and said they would not cover a rental vehicle even when I said I felt unsafe driving it. So I kept driving it because it was my only vehicle. Ford Corporate tried to charge me $571.00 to pay for my part of the repair. They also dismissed that it was a larger problem. I have documentation regarding these interactions. The dealer stepped in and covered my portion of the repair. After they replaced the door, I reported the door appeared to be falling of the hinges and they investigated and said because they do not replace doors at the dealership often, they improperly installed the door as one of the hinge bolts was already missing. In April, 2023, I traded my 2021 Escape in for a 2022 Escape as I did not feel safe because of the door and the dealer agreed. It cost an additional $5,000 to get out of the lease, but it was worth my safety.Read more

Subject: Concerns Regarding TSB T-SB-0017-23 for Toyota Camry HV Vehicles Dear NHTSA, I am deeply concerned about the recently released Technical Service Bulletin (TSB) T-SB-0017-23 by Toyota Motor Sales, USA, Inc. This TSB addresses a potential issue with the active grille air shutter actuator in specific 2018-2020 Camry HV vehicles equipped with the A25A-FXS engine. Releasing the TSB in April
... 2023, after the warranty periods for many affected vehicles have expired, is unfair to owners. A 2018 Camry HV's warranty period of 36 months/36,000 miles ended in 2021. This situation raises significant concerns as it burdens owners with the financial responsibility of repairs for an issue that was not their fault, potentially compromising safety if repairs are not promptly carried out. Toyota's inadequate efforts to ensure all affected owners have access to necessary repairs are disheartening. I urgently appeal for a thorough investigation and decisive action to ensure that regardless of warranty status, all owners of affected Camry HV vehicles can obtain essential repairs. I would like to bring attention to a similar TSB, T-SB-0319-17, issued by Toyota in November 2017 for Prius Prime 2017 and 2016-2017 models. This TSB addressed a comparable issue with the active grille air shutter actuator. Toyota extended the warranty for affected vehicles in California by 180 months or 150,000 miles, aligning with the manufacturing year and ensuring all vehicles were covered. The repair was covered under the California Emission Warranty for specific states. The discrepancy in approach between the two TSBs raises concerns about the fairness and consistency of Toyota's actions. It is crucial for Toyota to extend similar warranty coverage for affected Camry HV vehicles, ensuring all owners have access to necessary repairs without financial hardship. Thank you for your attention to this matter, and I urge prompt action in addressing these concerns.Read more

My vehicle recently depicted warnings on my screen showing " Front Left Safety Restraint System Fault" " Service is Required". I also sometimes get a visual warning showing a depiction of an airbag. Now I don't know if anything is wrong, but Tesla is going to charge me $400 to fix this problem. My cars warranty expired in April of this year. My concern is, if this is an airbag problem shouldn't
... it be coved by a recall? Or does your agency need to receive a large number of complaints to issue a recall? To answer your general questions, the warning messages started one month ago, late May. Beside the warning message, the airbag warning lamp would sometimes appear. I have a service appointment with the Tesla Service Center on July 1, so I should know more then.Read more

On every trip in my 2023 Model Y there is phantom braking for no reason with no cars in front of or around me. It happens on the highway on perfectly clear days, with no hills, no signs, no shadows or bridges, with no cars in front of me or behind me on completely straight roads in addition to happening when there are other vehicles around. I will be traveling at highway speeds and the car slows e
... nough to be terrifying and throw objects around the car. I have stopped using cruise control and auto pilot because I’m scared of the car. I reported the problem to Tesla on the phone and through their app the first time it happened multiple times on one trip and they said that they looked at the logs and that it didn’t happen. Since my family was with me in the vehicle, I have witnesses. I was told to self report by pressing the right flywheel and log every time it happens. I did that but since it happens a lot I refuse to use the features. I think Tesla should allow us to return these vehicles for refunds.Read more

Rear ended the vehicle in front. Vehicle in front had emergency braking activate, our pilot did not activate emergency braking. It does often go off on 2 lane highways when a large vehicle is coming the other way or when trying to take a freeway exit, but apparently not when you're about to hit someone.

In late summer of 2022 I had a visual warning "BRAKE BRAKE BRAKE" while I was driving on a highway. There were no cars in front of me for at least several hundred yards. This was distracting to me as the driver, but fortunately there was nothing that required my attention at the time. Over the ensuing weeks this happened several more times, always on a highway at highway speed. At first I thought
... it might be reacting to a sharp shadow from an over pass, but it happened on overcast days as well as sunny days. It did not happen at night. The frequency of these visual alerts gradually increased until October 8, 2022, when the visual warning flashed and the brakes were applied by the automatic braking system. Again, there were no cars ahead of mine. If a car had been tailgating me, it would probably have caused an accident. I took it to the dealer the next day, and after interrogating the computer, they determined that no fault was recorded. Privately, I was told that this was a known issue, but it could take up to a year for the software patch to be released, and in the meantime there was no recourse. I sold the car back to the dealer that day, purchasing another car that they had on the lot, because I felt that the vehicle was unsafe and I was unwilling to drive it any longer.Read more

WHILE I WAS DRIVING MY FORWARD COLLISION WARNING INDICATOR STARTED FLASHING ON MY DASHBOARD WHILE A DINGING SOUND WENT OFF. THE INDICATORS WOULD SWITCH BACK AND FORTH BETWEEN BOTH WARNINGS, THEN MY MIRROR SENSORS WERE INDICATING THAT SOMETHING WAS NEXT TO BOTH OF MY MIRRORS WHILE NOTHING WAS TH
... ERE. THEN ALL OF MY WARNING INDICATORS BEGAN FLASHING AND CONTINUED TO FLASH. ONCE THESE ISSUES STARTED MY BACK UP CAMER NO LONGER WORKED WHILE I WAS IN REVERSE. I BROUGHT MY VEHICLE DAYS LATER WHEN THE ISSUE PERSISTED TO THE PLACE I BOUGHT IT AND THEY ARE ORDERING A NEW BACK UP CAMERA WITH NO REAL EXPLANATION. CONSIDERING THIS RELATES TO A COLLISION WARNING SOMETHING IN THE VEHICLE MEANT TO PREVENT A COLLISION, NOT WORKING PROPERLY MY OPINION IS THIS IS A SAFETY ISSUE.Read more

Engine oil consumption is outrageous. I am having to add oil every 200-300 miles. Just had an oil change in march and have had to add oil twice. There is a recall on the 2013-2015 models for the Santa Fe and 2011-2019 for the Santa Fe sport but the 2016 model that I have is not included in this recall. The recall is specifically for engine oil consumption issues which is exactly what I am having b

The automatic windshield wipers came on and would not shut off. The car was clean, the day was sunny, and the camera lenses were free of dust or anything else that could cover them. While driving along, the wipers started and would not shut off. I went into the main monitor/screen that has the wiper settings but I was not able to activate the wiper/off button, so the wipers kept working. Pressing
... on that button would not activate it. I was in autopilot at the time, i.e., cruise control + lane keep assistance. I have FSD/full self driving, but have never used it since it is still in beta testing. I’m using Tesla software # 2023.7.10 I checked the various Tesla websites, and you can turn off the wipers only if you disable the ADAS features. After I turned off the ADAS, I was able to activate the wiper/off button. But this is a terrible situation, the ADAS needs to be activated, but if I activate, it wipers will begin working whenever they feel like it, regardless of the weather outside. This is clearly a software problem with the Tesla ADASN needs to be corrected. I don’t want to be driving along with auto pilot on, and then the wipers will simply turn on. Any help you could provide would be greatly appreciated to correct this dangerous situation. There are countless hundreds of complaints like this on the various Tesla websites about spontaneously functioning wipers when the weather is bright and sunny.Read more
