While braking on the interstate, this car threw error code C1391: Abnormal Leak of ACC PRESS. This is the brake system accumulator pressure - meaning lost all assistance to braking - it took a long time to slow down and come to a stop. Easily could have caused a serious crash had I been on a hill or coming up on stopped traffic. I did some research and found this is a KNOWN ISSUE with Toyota an
... d SHOULD HAVE BEEN A RECALL! Toyota skirted the issue by offering customers that encountered the problem a CUSTOMER SERVICE PROGRAM where they would pay for a new brake pump and master cylinder. HOWEVER, Toyota put TIME and MILEAGE CONSTRAINTS ON THE PROGRAM, and it just so happened our car had exceeded those limits - so we had to pay full price for the repair of the 2 new brake parts - over $3,000! The 2014 Toyota PriusV that we bought new and had always serviced it properly was hardly worth that, but it was our only car, so we paid it. BUT will never buy another Toyota now - and NHTSA needs to fine Toyota for this poor brake systemRead more

In April 2025, my 2021 Mercedes-Benz GLB 250 (81,000 miles) suffered repeated engine issues caused by a cylinder head failure. The dealer attempted a cylinder head replacement, but during their own test drive after repairs, the engine completely seized and failed again on their property. This indicates a severe defect in the engine assembly that risks sudden power loss while driving, which could c
... ause an accident. Prior to the failure, there were warning messages about misfires and rough idle. The problem has been confirmed by an authorized Mercedes-Benz dealer and the car has been kept at their shop since April, with no resolution. I am concerned this is a recurring safety defect affecting other GLB 250 vehicles. The vehicle is available for inspection and remains undriveable due to the failed engine. I am submitting a formal safety complaint regarding my 2024 Kawasaki Ninja ZX‑6R. Today, June 20, 2025, I received three official “Do Not Ride” letters from Kawasaki. These letters state that, due to a possible internal engine defect, the motorcycle is unsafe to operate. My authorized Kawasaki dealer has confirmed that my unit is affected by a manufacturing issue that can cause engine seizu
... re without warning, leading to serious risk of crash, injury, or death. It’s also important to note that Kawasaki notified dealers of this issue on June 4, 2025—over two weeks ago. Despite this, I only received my first formal customer notice today, June 20. In the meantime, I was unknowingly operating a potentially dangerous motorcycle. Kawasaki has not provided a fix, recall repair, or buyback option. The bike is currently parked and unusable, while I continue to incur insurance costs, loss of use, and storage burden. The issue appears to stem from an over-torqued crankshaft bolt or related internal engine component — a known problem on other 2024 ZX‑6R units already recalled. I am requesting that NHTSA investigate this case further and ensure all affected motorcycles — including those not yet officially listed by VIN — are promptly included in the recall campaign and made eligible for full safety repair, buyback or additional compensation.Read more

From this information, it appears that we meet the criteria and are suffering the same issues as are referenced in the ZJC materials, but our VIN is not included in the program. On three (3) separate occasions, my 2019 VW Tiguan has produced an error stating "Check Starter System" on the console and shut down electrical systems such as instrument panel, brake lights, headlights, taillights, back up camera, and other electrical systems while driving. The right rear axle shaft assembly wheel bearing seal failed. The right rear ABS Sensor, aka (rotational Speed Sensor, wheel speed sensor) failed. These parts are available for inspection. The vehicle unexpectedly accelerated without me pressing the accelerator as I approached a red light at an intersection, putting my safety and others at risk. I had to slam on the brakes with all my might to get
... the vehicle to stop from going through the intersection. The road was wet, and the front wheels locked while the rear wheels continued to be driven by the engine without any input from the accelerator pedal. Eventually, the vehicle stopped, and the engine acceleration went back to normal operation. The problem has not been reproduced or confirmed by a dealer or independent service center. The vehicle and parts have been inspected by me, as the vehicle owner. The warning lamps suddenly illuminated on the instrument panel just prior to the incident. The warning lamps came on as I was approaching the intersection. The warning lamps that came on were the engine diagnostic indicator lamp aka Malfunction indicator lamp, the Anti-lock brake system warning light, the brake assist system warning light, the electronic stability program warning light. I also hear what sounded like the ABS Control Unit rapidly cycling just prior to the uncontrolled acceleration while nearing the intersection. I used diagnostic equipment and performed a visual inspection of the components, concluding that they needed replacement. Grease from the leaking right rear wheel bearing seal fouled the right rear wheel speed sensor. The wheel speed sensor cable develops cracks near the wheel hub. I believe these parts are defective and can cause erratic, uncontrollable, unsafe driving risks that could lead to accidents and injuries. The symptoms and warning indications went away after the parts were replaced. I still have these parts as I feel they should be inspected by NHTSA personnel to make a safety recall.Read more

The HVAC controller failure causes risk of burn to passengers. When operating the HVAC system on Air Conditioning, the system will direct heat from the engine bay without any temperature management resulting in hot engine air being sent through the HVAC system and out to the vents. In some circumstances the hot air is only sent through the rear vents and other times the system sends hot air to al
... l vents with a high variability of when this occurs. In one experience, the hot air was only sent to the rear vents causing the rear vent’s metal knob to become very hot and resulted in my child burning their hand on the knob. The dealer has confirmed the issue on my vehicle and has ordered parts under warranty (currently on backorder for 30+ days) to resolve. The issue of the HVAC unit blowing hot air is common to many Ineos Grenadier vehicles according to the dealer and is also commonly raised on Grenadier vehicle forums. The immediate safety risk occurs when the heat is not apparent to the driver because the controller is only directing heat to the rear and the vent knob becomes a burn-point for passengers. This issue can occur regardless of the temperature setting on the driver controls whether being on auto or manual and the temperature setting of the HVAC set as cold air. My vehicle is available for inspection if requested.Read more
