My vehicle has an open safety recall 25V785 related to improper software programming of the Integrated Control Module CPU, which can cause loss of drive power while driving. I brought the vehicle to an authorized Tempe Honda dealer (8030 South Autoplex Loop, Tempe, AZ 85284) to have the recall completed. The dealer confirmed the recall is a software ICM update, but refused to perform the recall u

I was slowly driving into a parking spot, perhaps half way into the spot when the car suddenly accelerated forward, went over the cement wheel stops and hit a cement wall. It resulted in me suffering a fractured sternum. I am absolutely sure I did not press down hard on the accelerator to cause this. I recall being slightly jerked back into my seat and then hitting the wall. I have no idea wha

Thus, the car has not been inspected by anyone as yet, but I most certainly want it thoroughly checked. It should be noted that the police report was taken in the emergency room while I was in pain and in total shock at what happened. That report was only my guess as to what might have happened.Read more

Description of the Safety Problem: In the early morning hours of January 8, while driving my Hyundai vehicle, I ran off the road into a ditch. I was extremely disoriented and activated the vehicle’s built-in Blue Link SOS emergency assistance system for help. I successfully connected with a Blue Link operator and repeatedly told her that I did not know where I was and that I was scared and nee
... ded assistance. While I was actively requesting help, the SOS call was disconnected. After the disconnection, I attempted to reconnect using the SOS system but was unable to do so, despite the vehicle having power and an active, paid telematics subscription. The system did not reconnect or provide further assistance. I was forced to call 911 from my personal cell phone to obtain emergency help. This resulted in a delayed response and a $1,300 towing expense that I would not have incurred if the SOS system had functioned as marketed. This incident raises serious safety concerns. The vehicle’s emergency SOS system failed during an active emergency, disconnected while the driver was disoriented and requesting help, and provided no ability to reconnect. This defeats the purpose of an integrated emergency safety feature and could place drivers at risk of serious injury or death. I am reporting this incident so NHTSA can evaluate the reliability and safety of Hyundai’s Blue Link SOS emergency system during real-world emergencies.Read more

I am reporting repeated electronic safety system malfunctions in my 2023 Honda Civic Sport sedan that occurred while driving at highway speeds and present a serious safety concern. In June 2025, while driving at highway speed, multiple warning lights and messages suddenly appeared, including: Brake System Warning, Road Departure Mitigation System Warning, Parking Brake/Brake Hold Warning, and Adap
... tive Cruise Control Warning. These warnings appeared simultaneously without prior notice, raising concern about whether the braking and safety systems were functioning properly while the vehicle was in motion. I took the vehicle to a Honda dealership for diagnosis. The dealer did not identify a specific electronic or mechanical defect and instead claimed the issue was caused by liquid spilled near the electronic parking brake switch. I was told the repair was not covered under warranty and paid approximately $600 out of pocket. No evidence was provided explaining how this would cause failures across multiple safety systems. In January 2026, the same group of warning lights appeared again under similar conditions while driving at highway speed. This recurrence strongly suggests the issue was not caused by driver error but may be related to an intermittent electronic, sensor, or control system defect. I have a scheduled dealer appointment tomorrow to address this second occurrence of electronic malfunctioning. Because these warnings involve critical braking and collision mitigation systems and occurred while the vehicle was in motion, they present a potential safety risk. I am submitting this complaint so NHTSA can document and evaluate whether this represents a broader safety defect.Read more

Vehicle Information: Year: 2021 Make: Mercedes-Benz Model: GLC 300 Engine: M264 Mileage at time of failure: Approximately 65,000 miles Complaint Description: While driving, my 2021 Mercedes-Benz GLC 300 began experiencing repeated engine misfires and rough running, accompanied by a persistent check engine light. These symptoms began shortly after I purchased the vehicle and progressively worsene
... d over time. The vehicle exhibited reduced engine performance and hesitation while driving. The condition was intermittent at first but became more frequent, indicating a progressive mechanical issue rather than a sudden failure. At approximately 65,000 miles, the vehicle was evaluated by an authorized Mercedes-Benz dealership and diagnosed with a catastrophic cylinder head failure, requiring full cylinder head replacement. The estimated repair cost is approximately $8,000. The vehicle has never been modified and has not undergone prior engine repairs. The failure occurred well before what would be considered normal wear for an engine component of this type. Based on the progression of symptoms, the dealer diagnosis, and similar reports from other owners of the same model and engine, this appears to be a systemic defect involving the cylinder head and valve train, rather than an isolated incident or maintenance-related issue. This condition presents a potential safety concern due to loss of engine performance, drivability issues, and the risk of reduced power while operating the vehicle.Read more

My 2018 Tesla Model X experienced repeated over-the-air software update failures starting in late December 2025. Each update attempt would run for hours and then fail with a timeout error. ... 6, 2026 as a failed Autopilot ECU 3.0 requiring full replacement at a cost of $1,808. The failure has disabled Full Self-Driving, adaptive cruise control, lane keeping assistance, automatic emergency braking, and all driver-assistance safety features. I am concerned this represents a systemic defect where Tesla's software updates are damaging or exposing failures in Autopilot hardware. Multiple other Tesla owners have reported this same pattern in 2024-2025: update timeout errors followed by complete Autopilot computer failure requiring expensive replacement. Tesla is charging customers to fix problems created by their own update process. The loss of critical safety features occurred solely due to Tesla's software update attempts.Read more

Includes state-of-charge calibration. · 24V-160 / 2024-141 (Recall) Renogy Lithium Battery Firmware Update, May 2024, revised August 2024. Recall firmware to v137.4.4. Inspection identifies stuck contactors and requires battery replacement when found. Service bulletins 23-120, 23-123, and 24-015 show that the system ran with outdated inverter and alternator settings, an oversized alternator fuse, and battery firmware that interfered with proper charging. These factors accelerate cell imbalance and degradation. Only after Entegra-approved updates were applied did the full extent of battery degradation become evident. Replacing the battery earlier would have risked repeat failure.Read more

Component: Engine (Theta II 2.4L GDI) / connecting rod bearing-related failure risk with abnormal internal oil loss. The vehicle is available for inspection upon request (engine, oil level condition, diagnostic history, and dealer records). Safety risk: While driving on the highway (approx. 70 mph), the vehicle displayed a low oil warning, and I experienced concern for potential engine seizure/st
... alling and sudden loss of power, which could cause a crash. I reduced speed and sought service. This condition presents an ongoing safety risk because sudden engine failure can occur without adequate warning. Dealer confirmation: The problem was confirmed by a Hyundai dealer. After a routine oil change, the low oil warning appeared within ~1,700 miles. A Hyundai service center inspected the vehicle and documented the engine was approximately 3 quarts low with no external leaks, indicating internal loss/engine deterioration rather than a leak. Hyundai then required additional steps (combustion chamber cleaning and monitored mileage), and the abnormal internal oil loss condition persisted. Manufacturer inspection: The vehicle has been inspected by Hyundai dealerships, and Hyundai performed campaign-related actions, including KSDS/ECM update (T3G) and later knock sensor replacement. No police or insurance inspection. No crash or injury occurred. Warning lamps/symptoms prior to failure: Prior symptoms included a Check Engine Lamp with DTC P1326 (KSDS-related) documented by Hyundai service in 2025, followed by continued abnormal internal engine condition. The low oil warning occurred while the vehicle was in motion (highway speed). The issue remains unresolved, and I do not feel the vehicle is safe to operate due to seizure/stall risk. Additional: I reported this as a safety defect because similar Theta II engine issues are widely reported and can result in stalling, seizure, or engine fire. Hyundai denied engine replacement assistance despite the documented condition.Read more

While driving at approximately 65 mph on the I-405 freeway in Irvine, CA during rainy conditions, my 2019 Mercedes-Benz C300 suddenly and completely lost acceleration without warning. At the same moment, a Check Engine Light illuminated on the dash. I was forced to coast across all lanes and stop in the emergency shoulder in a very unsafe area with high-speed traffic passing close by in low visib
... ility rainy conditions. Multiple vehicles nearly struck us, including one that had to emergency brake directly behind my car in the shoulder. I contacted AAA and the vehicle had to be towed because it was not safe or driveable. My girlfriend was also in the car and both of us experienced significant fear and distress during and after the incident. My mechanic retrieved fault code P019185 and stated this is a known software problem with the engine control unit that requires a Mercedes software update and reset of mixture adaptations per a manufacturer LI/service document. This means the manufacturer appears aware of the defect and the required software fix, yet I received no recall notice or safety warning. A sudden total loss of engine power at highway speeds creates an extreme crash risk and life-threatening danger to the driver, passengers, and surrounding traffic. There have been other serious drivability issues with this vehicle in the past (misfires, coolant pipe failure, etc.), but this incident was the most dangerous. I am reporting this because this defect appears safety-related and could easily cause a fatality if it happens to someone else at freeway speeds. Diagnostic Note from official Mercedes-Benz Fletcher Jones of Newport Beach Mechanic: “FAULT CODE P019185, WILL START WITH PERFORMING THE SOFTWARE UPDATE OF ME CONTROL UNIT, RESET THE MIXTURE ADAPTIONS PER LI DOCUMENT AND RECHECK.” Tow Provider: AAA — tow confirmation available if needed.Read more

I own a 2022 Ford Explorer purchased in April 2025. Since purchasing the vehicle, I have experienced multiple defects affecting safety and normal use. The instrument cluster becomes foggy, making it difficult to clearly see my speed while driving. The infotainment screen intermittently malfunctions and affects the backup camera display, reducing rear visibility. I also experienced fuel contaminati

The vehicle has been taken to the dealership four separate times for different issues. Despite these repair attempts, the problems have continued or new issues have appeared. Due to the ongoing defects and safety concerns, I do not feel safe or confident operating this vehicle.Read more

We've had this car on lease for 23 months, and it has had an escalating pattern of electronic, mechanical and brake issues. ... fe. There was an alarm with a turtle warning me to move over. On January 4, We parked our car in a parking lot and went into a store, when we returned all of the windows were lowered and the moonroof's window was also retracted. The car was beeping and making loud noises and then had issues starting. Prior to both of these issues, the car has taken to Mercedes repeatedly for locking passengers into the vehicle and blaring alarms, not allowing passengers to leave and the seatbelts will not retract. The airbag system has also malfunctioned during these periods. Mercedes has repeatedly been informed of these issues and has claimed that there were bugs in the car's code that they had fixed; however, each time they have fixed the issue a new issue has arisen. In late November 2025, a representative from Mercedes corporate asked for us to take the vehicle back to dealer for further inspection after which the dealership said they had carried out major updates and returned the car in early December. After that the car had been hardly driven until this week. This was the first major drive of over 60 miles.Read more

On five separate occasions the airbag system on my car has become inoperable. Starting in July 2025 I noticed an airbag error on the display of the vehicle. I brought the car in for service and they replaced the steering wheel. The airbag error occurred again in August, this time requiring replacement of a different wiring harness. The error occurred again and again went in for service in November
