My family and I were traveling from Maryland to Florida, with a planned stop in Atlanta. We were driving our 2013 BMW X5 x Drive 35i and were traveling southbound on [XXX] through Greenville, South Carolina when the incident occurred. While driving at highway speed, my husband suddenly noticed multiple error messages appearing on the vehicle’s display screen - air bag malfunction, 4x4 malfuncti
... on, stabilization, etc. He immediately began pulling the vehicle over. Within moments, he observed smoke and visible flames coming from the rear driver’s-side area of the vehicle. He urgently shouted for me and our daughter to get out of the car. What followed happened extremely fast, within approximately 15 seconds. I yelled for my daughter to unbuckle and run. In a state of panic, I grabbed our dog, and as we tried to exit the vehicle, the fire spread rapidly and aggressively. Thick, black smoke engulfed the area and we fled on foot, coughing and struggling to breathe from the smoke. By the time we reached a safe distance, the vehicle was fully engulfed in flames. Thankfully, we were able to escape without physical injury, but the incident was terrifying and could very easily have resulted in serious injury or loss of life. The vehicle was completely destroyed, along with all of our personal belongings - cell phones, laptops, clothing, luggage, etc. We have since checked for recalls associated with the vehicle’s VIN and were told none were listed. However, a vehicle should not spontaneously catch fire while being operated normally on a highway. The sudden onset, rapid spread of the fire, and lack of warning raise serious concerns about a potential defect, whether electrical, fuel-related, or otherwise. We believe this incident was preventable and that there may have been a failure in design, manufacturing, or warning. We had the insurance company and BMW inspect the vehicle, but the fire destroyed the car entirely, and a cause "could not be determined." The dashboard has warped and lifted significantly from the underlying structure, particularly near the defroster vents and windshield area. I had no control over the vehicle. Brakes were mush. My Mercedes also has a safety feature where if I get too close to a car, it will brake on its own. Red & yellow lights were flashing, the car shook during the entire event until it came to a stop. My insurance carrier, State Farm, had the car towed to the Mercedes dealership. The service technician saw no codes, therefore, they say nothing is wrong with the car. I know that cars do not always throw codes. I googled what my car did and it is a thing. The article gives suggestions as to what can be checked, but Mercedes refuses to check anything since no codes were there. State Farm asked to me ask for the black box info (Event Data Recorder), however, Mercedes refuses. The car is unsafe to drive. If it happened once, it will absolutely happen again. State Farm is having the car towed to a different body shop today, as Mercedes body shop is price gouging. I do not want to drive the car until they find out what happened and they keep telling me there's nothing wrong with the car. It is a safety hazard. Next time it happens, it could be a person in front, rather than a car. Can you help me? Photos show skid marks where the brakes were useless.Read more

On 5/28/2019 Recall V06 was completed on my 2014 Dodge Ram 2500. As part of that repair the adjustment nuts on the steering drag link were welded to prevent loosening and potential separation. On 3/3/2026 I had new front suspension components installed by Les Schwab Tire Center. During that service the technician advised a wheel alignment could not be performed because the welded drag link nuts p
... revented the steering linkage from being adjusted. Alignment is normally required after front suspension work and necessary for proper steering geometry. Les Schwab advised that grinding the welds off the drag link to perform the alignment and then rewelding the component would be unsafe because it could introduce hidden stress or cracking in the steering component. They recommended replacing the drag link with a Moog DS801538A redesigned component. The drag link had to be replaced to restore normal steering adjustment and allow the vehicle to be properly aligned. On 3/3/2026 I requested reimbursement from Dodge/Chrysler for the drag link replacement required due to the recall repair. The request was denied. On 3/4/2026 Chrysler representatives acknowledged the welded drag link rendered the vehicle unserviceable for alignment yet still denied reimbursement. Chrysler’s Vehicle Alignment Clarification Letter states that after the recall repair, owners should still be able to obtain an alignment using an approved procedure. In my case, however, the welded drag link prevented any adjustment necessary to perform that procedure, making the vehicle unserviceable for alignment until the drag link was replaced. The recall repair rendered a steering component non-adjustable and prevented normal service, leaving the vehicle less serviceable than before the recall repair. Documentation and photos are attached.Read more

I am reporting a serious safety hazard involving my Mercedes‑Benz EQB 350. Drive only 1.5 year, Jan 6, 2026, the vehicle suffered an electronic failure at LaGuardia Airport and became completely inoperable. It could not shift into Drive, Reverse, or Neutral and had to be towed to Mercedes‑Benz of Huntington for warranty repair. During service, the dealer performed a wheel alignment on Jan 16,
... 2026 without my authorization. I had previously declined alignment recommendations because my factory alignment was excellent and my tires were wearing evenly. Within four weeks of this unauthorized alignment (under 1,000 miles), all four tires developed severe inner‑edge destruction. Both rear tires wore down to exposed steel cords, causing air leakage and creating an imminent blowout risk. The front tires also show abnormal inner‑edge wear. The remaining tread on all tires is otherwise good, which is consistent with incorrect alignment rather than normal wear. An automotive engineer confirmed this rapid structural failure is physically impossible through normal driving and is a direct result of incorrect or defective alignment parameters set during the Jan 16 service. This unauthorized and incorrect repair turned my vehicle into a safety hazard for my family. I contacted Mercedes‑Benz USA (MBUSA) immediately and provided photos, documentation, and Case # CMS 20671727. For more than 14 days, MBUSA no response. Representatives gave inconsistent and implausible explanations, including blaming the towing process, which cannot cause this pattern of wear. MBUSA repeatedly redirected me back to the same dealer that performed the unauthorized repair, a clear conflict of interest. Their handling of this case shows dishonesty and an attempt to avoid responsibility, despite clear evidence of a safety‑related defect. I request that NHTSA review this matter as a potential safety defect involving electronic failure, unauthorized repair, incorrect alignment, and rapid tire degradation.Read more

The front upper grille molding/deflector located above the front grille and directly below the hood on my 2022 GMC Sierra 1500 has become loose and is separating from the vehicle. This creates a safety concern because the component appears at risk of detaching completely from the vehicle and becoming roadway debris, potentially striking other vehicles or causing drivers to take evasive action. The vehicle has not been involved in any collision or modification that would cause this condition. A GMC dealer inspected the concern and confirmed the vehicle is not currently included in an active recall despite an existing GMC recall addressing grille deflector detachment on similar 2022 Sierra 1500 vehicles. The defective component is currently still installed on the vehicle and available for inspection upon request. Due to the risk of detachment while driving, vehicle operation at highway speeds now presents a potential hazard to surrounding traffic.Read more

During interstate travel in rainy conditions, the vehicle appeared to lose traction and hydroplane. The tires appeared to have adequate remaining tread; however, they did not provide expected traction and the vehicle felt as though it was driving on ice. This was inconsistent with my prior experience operating the vehicle for over a year in various weather conditions without encountering similar i
... ssues. The Electronic Stability Program (ESP), traction control system, and 4MATIC all-wheel drive system did not appear to intervene to stabilize the vehicle. The vehicle spun and was involved in a crash. Despite the impact, no airbags deployed. The vehicle was rendered undrivable and towed from the scene. It is currently located at a collision repair facility and is available for inspection upon request. The suspected systems involved include the Electronic Stability Program (ESP), traction control system (TCS), 4MATIC all-wheel drive system, and Supplemental Restraint System (SRS/airbags). The vehicle has not yet undergone a full manufacturer engineering inspection, but such a review has been requested. The loss of traction at interstate speed in wet conditions created a serious safety risk to the occupants of the vehicle and surrounding motorists. The apparent lack of stability control intervention and the absence of airbag deployment increased the potential risk of injury. The vehicle was inspected by law enforcement at the scene, towed, and has been reviewed by insurance representatives as part of a claim. A manufacturer inspection is pending based on a defective claim that was filed. There were no warning lights, alerts, or messages related to traction control, stability control, or airbags prior to the incident.Read more

ICCU Failure causes the car to not work.
Incident date: Feb 21, 2026
I am reporting a safety concern involving BMW and Toyota Supra vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. These vehicles utilize a variable-displacement engine oil pump containing an internal thermoplastic adjusting component responsible for regulating oil pressure. Field failures are increasingly being reported in which this interna
... l component fractures, resulting in sudden loss or instability of engine oil pressure. The failure frequently occurs during cold-start operation in low ambient temperatures. When the component fails, oil pressure regulation becomes erratic or is lost entirely. In many cases, drivers receive little or no actionable warning prior to engine damage or vehicle disablement. Reported precursor symptoms include inability to electronically measure engine oil level, abnormal oil pressure behavior, or sudden drivetrain malfunction warnings. Failures may rapidly progress to engine shutdown or mechanical damage caused by oil starvation. Loss of lubrication can cause the vehicle to become disabled without warning while driving or shortly after startup. This creates a safety hazard, particularly when vehicles become immobilized in active traffic or during winter conditions the cold is when they fail the most, where occupants may be exposed to freezing temperatures while awaiting assistance. Replacement of the oil pump requires extensive disassembly of the vehicle and repairs commonly exceed $6,000–$8,000 for just the pump, assuming the motor is unharmed. Owners are currently unable to determine whether their vehicles contain the original thermoplastic oil pump design or a later revised version utilizing metal internal components, as VIN-based parts information now identifies updated components in the car even when no replacement has occurred and the old components are actually in the car. Multiple BMW and Supra owners are experiencing this costly issue. Please investigate this issue.Read more

The vehicle's Autopilot/Full Self-Driving (HW3) computer has failed at approximately 70,000 miles. Tesla Service has diagnosed the vehicle as requiring replacement of the HW3 computer. As a result of this failure, multiple safety related systems are inoperative, including collision avoidance, lane assist, lane departure warning, automatic wipers, and other driver assistance features including ad
... aptive cruise control. The vehicle is also unable to receive over-the-air software updates, including potential safety updates. In addition, the vehicle experiences system lag, including delay when shifting into Drive or Reverse. The center display is slow to respond, and certain media and software features are no longer functional due to inability to update. This condition increases safety risk because active safety systems designed to assist with collision prevention are disabled. The failure appears to be hardware-related and not due to normal wear. Tesla has confirmed the need for computer replacement but has declined coverage as the vehicle is outside warranty. The failed component should be available for inspection upon request.Read more

I purchased my vehicle 2021 Mercedes-Benz GLE 350 from Mercedes Dealership in Ontario, and had it for only about 4 months before it catching on fire while parked at the doctors office. I purchased it [XXX], and it caught fire on [XXX] while at the doctors. I didn't drive much since I work from home. ... tor appointments. Mileage at purchase was 59982. Last mileage 60,292. Insurance currently has the vehicle at Copart in Colton while doing their investigation for the claim I filed. I have the police report and fire report as well. At the time of purchase, I asked if there was anything wrong or if any know issues with vehicle, I was reassured no issues. Reviewing the insurance CCC report, I come to find out the vehicle has 3 recalls with possibility of fire incidents. The carfax provided doesn't say that either recall issue was performed as another recall issue with the fuel pump was performed and disclosed. Beyond devastated that I have no vehicle now and that I expected this one to last much much longer. There was no indication of anything wrong with the vehicle, no warning lights nor check engine. Fire was indicated to start in engine compartment on the passengers side by the fire investigator. This incident affected 3 families. As a teacher, I find everyone's life precious and its sickening to know that my life and other lives were in danger. Had I waited 5 more minutes in the vehicle, I fear that this message may not have existed at all. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While traveling at highway speed on the interstate in my 2021 Chevrolet Silverado 2500 HD (approx. 120,000 miles), the vehicle experienced a sudden hard jolt followed by a momentary rear wheel lockup. This occurred without warning. Immediately after, the transmission defaulted into a fail-safe condition and became locked in 4th gear. The vehicle would move but could not shift properly, resulting i
... n abnormally high RPM and engine load at highway speeds. I was forced to continue driving several miles in this condition to reach a safe exit. The unexpected drivetrain behavior and temporary wheel lockup created a serious safety risk, including potential loss of control and risk of collision. The dealership identified transmission diagnostic trouble codes P2715 and P2733 (pressure control solenoid performance). Transmission fluid was found to be burnt and contaminated with metal debris. The vehicle is subject to GM Special Coverage N242454441 for a known control valve body defect that can cause harsh shifting and reduced engine power. Although code P0747 (referenced in the bulletin) was not present, the failure involved transmission hydraulic pressure control. The dealership is replacing the valve body under the special coverage but is not addressing the internal transmission damage, stating they cannot determine whether the damage resulted from the covered defect. The sudden rear wheel lockup and transmission fail-safe condition at highway speed present a significant safety concern involving unintended drivetrain behavior.Read more

RECALL NO: 19V-868 | VIN: [XXX] | CASE #: [XXX] VEHICLE: 2013 Mercedes-Benz GLK350 (X204) DESCRIPTION OF SAFETY PROBLEM: This complaint concerns a failure to adequately remedy NHTSA Recall 19V-868. Since [XXX], my vehicle has been grounded at Mercedes-Benz of Westmont. The dealer performed the recall out of sequence, ignored documented circuit faults, and refused mandatory Xentry commissioning and
... SCN Normalization, leaving the SRS system inoperative and uncertified. NHTSA records (Ex B-27) confirm the recall remains "Incomplete." MANUFACTURER ABANDONMENT OF SAFETY OVERSIGHT: On Feb 26, 2026, an MBUSA Executive Referral Manager (Case #[XXX]) stated by phone he would not read my 61-page safety complaint and classified the failure to verify the recall as a "dealer issue" due to franchise laws. Only 28 minutes after admitting he would not read the file, he sent an email falsely claiming he had "reviewed the documentation" and closed the case. By refusing to audit a documented failure to certify a safety repair, MBUSA is in active violation of its duty to ensure recall completion under 49 U.S.C. § 30120. COERCIVE SAFETY THREAT (49 U.S.C. § 30122): Service Manager Blake Farmer and Advisor Harry Arroyo explicitly threatened to remove the new airbag and reinstall the original defective recalled inflator (Ex B-6) during a billing dispute. This is a direct violation of 49 U.S.C. § 30122, which prohibits a dealer from knowingly making a safety system inoperative. DEALER NEGLIGENCE & ADDITIONAL RISKS: Dealer negligence regarding Part #204-440-68-41 (Ex B-17, B-26) caused a 5-day delay, resulting in a nationwide backorder. Furthermore, technician video (Ex B-10) shows an active oil leak (Code P2610-29), a fire/stalling hazard per MBUSA TSB LI05.20-P-067123 that the dealer refused to diagnose. ATTACHED: 61-page filing with 28 exhibits and 3 notarized affidavits. The 60-day federal deadline is April 4, 2026. 1. COMPONENT FAILURE & TECHNICAL IMPAIRMENT: The vehicle is experiencing a total failure of its hybrid and internal combustion systems. On Jan 30, 2026, the vehicle received the "68C" battery software update. By Feb 6, the system failed, rendering Electric Mode Unavailable and triggering "Electric Mode unavailable" warnings. This is not a software glitch; it is a failure of the manufacturer's offi
... cial remedy to conform the vehicle to warranty. Additionally, documented external oil pooling on the engine cover indicates internal pressure/seal failure directly related to the sand-contamination defect in Recall 78C. 2. HIGH SAFETY RISK: My safety is at extreme risk every time I operate this vehicle. • Recall 78C (Engine Fire/Failure): The manufacturer admits a 100% defect rate for my engine block (NHTSA 25V766). The defect involves abrasive sand circulating in the engine, which leads to "catastrophic engine failure" or fire. Stellantis has issued an advisory in multiple jurisdictions because this failure can happen without warning at highway speeds. • Recall 68C (Battery Fire): The high-voltage battery has an internal short-circuit risk. The fact that my EV system failed on Feb 6 proves the internal battery cells are unstable. Driving a vehicle with a known high-voltage fire risk and an unfixable engine defect creates an intolerable safety hazard for me and other motorists. 3. SUBSTANTIAL IMPAIRMENT OF VALUE AND USE: The vehicle’s value has been destroyed. I am paying for a high-performance hybrid that cannot use electricity and an engine that is self-destructing from the inside. Because the manufacturer has no remedy for 78C after 3+ months, the vehicle is fundamentally unrepairable. The safety directives (Park Outside/No Charging) and the physical mechanical failures constitute a total impairment of the vehicle I purchased.Read more
