On April 16, 2022, around 6:45PM, I drove my new Mazda CX5 (@ 3500 miles) into our church parking lot and turned right down the first row of cars. It was light out and the weather was pleasant. I turned right into a parking space with my foot on the brake. The car was facing and perpendicular to a sidewalk that was about 10 feet away. It was separated by a grassy median. I turned the wheel to the right at a 90 degree angle to avoid hitting her. I kept pressing the brake but the car kept accelerating until it crashed into a large tree. The airbag deployed. Both the pedestrian and I were transported to the hospital. The pedestrian had bruises on her leg and hip, but fortunately was not more seriously injured and was released from the hospital. I was admitted to the hospital for contusions to the heart. The car hood was crushed and the car was totaled. The incident was terrifying. The police estimated the car went from nearly a standstill to about 40mph in about 15-20 feet. There were no warning lamps or messages prior to the accident. I checked the NHTSA website after the accident and downloaded 16 similar complaints where low mileage Mazda CX5s accelerated out of control when drivers were attempting to park or stop.Read more

... uel, about 1/4 tank, the low pressure fuel pump is unable to supply enough fuel to the high pressure fuel pump. This results in sudden loss of power, and also hard starting. I believe the fuel pump is the problem because it was manufactured by Denso, who recalled 2,000,000 similar fuel pumps two years ago. Those pumps were recalled because they created the exact problem we experienced. Toyota recalled vehicles using these pumps as far back as 2013. However Mazda and other OEM's were allowed to only recall vehicles going back to 2018. EVEN THOUGH IT IS THE SAME PUMP. When the manufacturing defect that causes a safety problem is well known, why are some manufacturers allowed to limit their recall? Why aren't all cars using the pump forced to be recalled? If a recall is excessive, why aren't manufacturers required to provide an extended warranty? NHTSA, you dropped the ball on this, and you need to fix it.Read more

This is a clear safety hazard concern that should have been addressed before I was allowed to drive the vehicle away from the dealership. THE dealership informed us that no one has this part and do not know if the will ever have another shipment. we contacted salvage yards received the same answer. I have sent 8 emails stating problems to the manufacture (Mitsubishi electric company) no response thus far. This is my daughter first year in college she saved her summer money 6600.00$ and purchased this car in March 16, 2022 from Knox auto sales 107 NC-55E, Dunn NC 28334. First day we had all fluids changed. The third day she went to visit her dad 20 minute drive ATF light came on went to auto zone code warning electrical communication, purchased new battery drove car for 5 days good took car to college in Charlotte came home birthday Friday check engine light and TCM code went to church Easter Sunday almost didn't make it home. On March 7, 2022 while driving the car to the dealer to take care of a Check Engine Warning Light, the car started to shake, all the dashboard lights turned on and the car stopped. My car was immediately towed to the dealer, and hours later I was told that my car needed a new engine. I felt paralyzed with awed when I heard that my 2015 Mazda CX-9, which was just 6 years old with no more than 90,000 miles, needed a new engine. I have been taking this vehicle to the Mazda dealer for all its repairs and maintenance needs, so I can be confident that my family were safe in this car. Unfortunately, this has not been the case. My vehicle only has 3,500 miles on it. ITs a huge safety hazard. I transport my mother who is 93 years of age. I don't feel confident driving it. especially on the highways. I had engine failure with a complete loss of power and a shuttering of the steering wheel while pulling into a parking lot and going less than 30 miles per hour. The temperature gauze went from cold to hot suddenly. There were no leaks of coolant or oil and no signs that the engine was out of water or that the water pump had "melted". On the final car repair trip we found out that the engine has a

The engine light NEVER came on, I had complained about the tire pressure light blinking on and off in November 2021 and the deceleration issue I was having. The dealer did a coolant flush 11/2021. In early March 2022 the engine shut off as I was backing out of the driveway. I took it to a repair shop nearby they could not find anything wrong except that the car went into limp mode. They also did a coolant flush and radiator cap exchange since I forgot I had just done one on Nov. Less that a week later the engine failed again. We towed the car to the dealer for the third trip in less than four months. I am really disappointed that Mazda failed to recall the engine. I could have been seriously hurt if I had been traveling at high speeds. The CX9 had less than 135,000 miles at 12 years old.Read more

I am also a part of a Mazda 3 community and am active on the forums, and in the last month I have seen at least 30-40 other 2021 Mazda 3 owners also report the same exact error code, with the same short circuit issue being cited as the problem by the dealership. Parts are on backorder for weeks for most of these owners, and some of them were told by the dealership to keep their car while they waited for the part, whereas my own dealership told me it was unsafe to drive the car and to have it towed to the dealership instead.Read more

I cannot be certain that the new OEM transmission part was re-engineered to correct for the original engineering defect and I am greatly concerned that the issues will repeat themselves soon, putting my family and me at risk. In 2020 I was sent a recall notice for my headlights. I took my car into Foothills Lincoln Mazda in Spokane, WA for the repair. They stated that my headlights were the wrong production dates so they did nothing for the recall yet marked the repair as "completed" according to Mazda. Now both my headlights are having issues with the daylight running lights with one light flickering and the other not

I have checked the documentation that I could find for the recall and found that in the repair procedures for Mazda Recall Campaign 4320A NHTSA Recall #: 20V-063 , it states the VIN number range and production dates effected and my vehicle was in the range for both. I contacted the dealer about this and they told me to contact Mazda customer service. I contacted Mazda customer service and they stated that the repair was marked completed in their system. I sent them documentation showing that the dealer didn't do any repairs. They said that the production dates didn't fit. I showed them my VIN and production dates along with the repair procedure showing that my VIN and dates fit the recall. They then stated that the lights themselves didn't fit. I sent them more pictures illustrating that it did. This process has gone on for two months now and they are saying that it doesn't fit the recall but can provide no documentation to me as to what the effected VIN numbers are or the production dates are that my vehicle doesn't meet. Can someone please assist me with this issue as I have a one year old daughter and I am concerned with the safety of my vehicle?Read more

The Mazda SCBS (Smart City Brake Support) system malfunctioned in my vehicle. This system can inadvertently apply the brake if the system malfunction. It was told to me by the dealership that it was a known issue with the CX5s around this model year and I really think it should be a recall. Not having this safety-critical system working properly because of a manufacturer defect isn't a great thing
... to put on the traveling public. Not having this system could cause people to become complacent expecting the system to work properly and stop them from rear-ending another vehicle. The warning light randomly came on one day, and would also come on from time to time which the manufacturer states as "normal". I had the system inspected by Mazda and they told me "This is a common issue that we have seen frequently." I had to pay out of pocket $800 to get this safety-critical system repaired that really should be covered by the manufacturer due to a malfunction in their system.Read more

This is relating to NHTSA Investigation PE 14-005. Although this car is a 2012 Mazda CX9, my wife driving the car had the same issue where she heard a hissing noise and the brakes were very difficult for her to depress or to stop. A brake issue is a major defect where the NHTSA should have initiated a recall in accordance with 49 U.S.C. 30118-30120, which required notification of all owners.
... Why the NHTSA allowed Mazda to only issue a 'Special Extended Warranty' is beyond me, and smells of a back door deal. Nonetheless, I was unaware of this issue with the Mazda CX9's as I was never notified. My wife drove the vehicle home and I purchased the replacement parts at my own expense.
