Vehicle Safety Complaint – Automatic Sliding Door Injury to Child To the National Highway Traffic Safety Administration, I am submitting this complaint to report what I believe may be a serious safety defect involving the automatic sliding door on my Honda vehicle. On June 9 at approximately 5:00 p.m., while the vehicle was parked in a residential driveway, the automatic sliding door severely
... injured my [XXX] son. He had unbuckled himself from his car seat and slid out of the vehicle as the automatic sliding door was closing. I witnessed the entire incident. As the door closed, his entire hand, as well as the skin on his leg, became trapped in the door. The automatic sliding door did not stop or reverse after encountering his hand and leg. Because the door is automatic, I was unable to immediately release him and had to wait for it to reopen before I could free him. Those moments felt incredibly long while my son was trapped and screaming in pain. When I was finally able to remove him from the door, his hand was severely injured, visibly deformed, and had significant swelling almost immediately after the incident. To my knowledge, nothing obstructed the normal operation of the door, and the automatic sliding door was being used in its normal operating mode. The fact that the door did not stop or reverse after trapping my son's hand is the primary reason I am submitting this complaint. Immediately after the accident, I took my son to an urgent care facility. The providers there instructed us to seek immediate evaluation by a pediatric orthopedic specialist, and we went directly to a pediatric orthopedist. He was diagnosed with a broken hand. This injury has resulted in medical expenses, significant emotional trauma for our family, and a complete loss of confidence in the safety of this vehicle. I was driving my 2021 Nissan Rogue on the highway, returning from the airport after picking up a friend, when the check engine light illuminated and the vehicle began jerking on acceleration. The vehicle lost power and would only move very slowly, leaving me traveling well below highway speed in active traffic. This created an extremely dangerous situation, as surrounding vehicles were approaching

I did not feel safe continuing to drive the vehicle, so I stopped driving it and had it taken to Crest Nissan of Frisco (Frisco, TX) for diagnosis. The dealership retrieved diagnostic trouble codes P1217 (engine overheat) and P0300 (random/multiple cylinder misfire). Upon inspection, they found low coolant, pressure-tested the cooling system, and confirmed that coolant is entering multiple cylinders, causing the misfire and overheating. The dealership states the engine cannot be repaired and the long block engine assembly must be replaced, at a quoted cost of $6,955.49. The vehicle is barely outside the manufacturer's warranty by time and mileage. There was no warning prior to this failure, and the vehicle has been properly maintained. This is an internal engine failure (coolant intrusion into the cylinders), not a maintenance-related issue. I am aware of other 2021 Nissan Rogue owners reporting the same failure — coolant entering the cylinders/mixing with oil, misfires, overheating, and dealers requiring full engine replacement shortly after warranty expiration. Since fall of 2025, my 2023 KIA Sorento has repeatedly been in the shop for multiple issues. Most notably, a completely wrecked steering column. All of these problems have been confirmed by their technicians. I continue to pay my $600 a month vehicle payment, as well as insurance as KIA consumer refuses to assist in any way and does not have any answer as to when my vehicle will be fixed. I do not have a vehicle that will fit my family due to this issue. I have asked them to buy back my vehicle, as it is clearly a lemon, but they state I have waited too long to request the buy back under lemon law in CT. Under normal operation, the air reaching the back row is severely restricted, leaving the rear cabin isolated from proper climate regulation. My safety and the safety of others have been severely put at risk during travel. Because of the near-zero airflow, the rear passenger compartment quickly traps extreme ambient heat or freezing cold. This environment is highly unsafe for vulnerable occupants riding in the back seat—specifically young children, elderly family members, and animals—who cannot properly regulate their body temperatures and face an immediate risk of heat exhaustion, heatstroke, or hypothermia. In the most recent incident, I was turning left across the roadway from a stop with an oncoming vehicle approaching. I applied full accelerator and the vehicle did not respond at all for approximately 2 seconds — no engine response, no movement — leaving me stationary in the oncoming vehicle’s path. Power then engaged violently and without warning, causing the vehicle to lurch across the road. I was nearly struck. Similar events have occurred multiple times throughout my ownership; this was the closest to a collision. DEALER CONFIRMATION: Not yet inspected by a dealer. A dealer service appointment is being scheduled. OTHER INSPECTIONS: The vehicle has not been inspected by the manufacturer, police, or insurance representatives. WARNING LAMPS/PRIOR SYMPTOMS: No warning lamps or messages have appeared before, during, or after any of the events. The failure is intermittent and occurs without warning. The condition has been present since I purchased the vehicle in October 2022 and has occurred regularly throughout ownership — numerous occurrences over more than three years. No warning lamps have ever accompanied it. ADDITIONAL: Condition persists with Idle Stop & Go disabled, ruling out engine-restart delay. The vehicle caught fire earlier morning of 7/9/2026. It was sitting in front of house unoccupied. The alarm went off twice which pressing the key fob shut it off until the third time when the alarm would continue until it died down. I went to my door to check and noticed the car was on fire.

All 3 bolts that connect my tailpipe underneath my car failed. This caused the tailpipe to disconnect from the car, meaning I was pushing the pipe as I drove. The main entry door lock assembly failed mechanically, physically trapping occupants inside the living coach with no mechanical release and forcing an emergency window escape. This same door mechanism unlatched and swung completely open while transit on public highways, creating an extreme crash/ejection hazard. Additionally, the total electrical failure to the LP propane detector left occupants with zero protection or warning against toxic or explosive gas leaks. Has the problem been reproduced/confirmed by a dealer? Yes. All ongoing mechanical and structural failures have been thoroughly documented and confirmed across multiple repair visits by Camping World RV Sales. Furthermore, the electrical propane detector failure and structural roof compromises were explicitly reproduced and confirmed via an independent 45-point safety inspection. Has the vehicle or component been inspected…? Authorized dealer technicians have extensively inspected and documented these safety defects during multiple detentions totaling over 220 cumulative days out of service. The manufacturer (REV Group) has been formally served a legal demand letter regarding these structural and life-safety failures. Were there any warning lamps…? A chronic Chassis Check Engine Light first appeared in July 2023. WHAT FAILED: A panoramic moonroof shattered before delivery and was replaced by the dealer before sale. Despite the replacement, loose tempered-glass fragments remained inside the occupant cabin, including the front passenger compartment and the second-row floor where my two minor children ride. ... eft in occupant areas create a foreseeable laceration hazard during normal vehicle use, particularly for small children riding in the second row. The presence of enough glass to tear the passenger-seat leather demonstrates that hazardous fragments remained in the vehicle after the moonroof replacement. CONFIRMED BY SERVICE CENTER: Yes. The selling dealer Hyundai of Kirkland was aware that the moonroof had shattered and was being replaced before delivery. The salesperson disclosed the moonroof damage and showed us the vehicle in the service area while repairs were being performed before sale. INSPECTED / REPAIRS: Loose glass fragments were discovered the day after delivery, prompting an immediate call to the dealer and a return visit for cleaning and inspection. Glass fragments continued to be found afterward, requiring additional follow-up visits, including a second unsuccessful repair request approximately one month later. The dealership was aware that loose glass had damaged the seat; on 09/08/2025, the salesperson directed me to repair the tear myself with clear nail polish rather than initiating a warranty repair. See file attached with evidence. No repair orders or service documentation were provided for these interior repairs. The dealer ultimately performed cleaning, third-party leather restoration, and replacement of the passenger-seat cover on 11/04/2025. Because loose glass continued to be discovered after the second repair attempt, I purchased a Prosidium protection plan for future glass-related damage. WARNING SYMPTOMS: None. Since buying the vehicle I’ve had issues with the truck going into limp mode with codes p0700 as well as p0747. I have taken it to the dealership twice now, when it has arrived the codes were no longer there. My vehicle started beeping aggressively, a red indicator started flashing over the frunk area of the vehicle representation in the dash and my vehicle entered red turtle mode. This red turtle mode severely reduced the vehicle's speed and ability to maneuver on the freeway. I was almost struck by numerous vehicles while attempting to g

Once I got to the side of the road, I power cycled the vehicle. when that didn't revolve the red turtle issue. I pressed the frunk open button. I heard the opening mechanism actuate. I then reseated the frunk latch and closed the frunk. Once the vehicle was turned back on, all issues were resolved and the vehicle behaved as normal.Read more
