While driving the vehicle warnings alerted me that the battery was low and to start the vehicle ( it was running as I was driving) then the alert changed to pull over when possible. At this time the power steering and power brakes started to fail by the time I pulled into a parking lot the vehicle completely died and I had to push it into a parking spot. I called Land Rover they towed it to a deal
... ership. The dealership in Orlando Florida said that the DC to DC converter failed and that it would need a new one (but the part is on a back order because it’s a common issue and they have no idea how long it will take to get the part) it will take at least 30 days or more to get a new part. Before I managed to pull over in a safe location the steering became very heavy and the brake pedal got very hard and barely stopped the vehicle ( almost had a collision) and steering into the parking lot was extremely difficult.Read more

During a recent incident where a tire issue caused damage to the wiring harness for the parking brake, resulting in a Parking Brake Failure notification on the screen, the vehicle would not stop or turn off. The vehicle displayed N for neutral on the dash, but the vehicle would drive forward unless the brake was depressed. In addition, we were unable to place the vehicle in Park, which meant tha
... t we were unable to shut it off. The only way to stop the vehicle from driving on its own and crashing into something was to place concrete blocks under the tires! Unplugging the 12v battery also had no effect! The vehicle kept driving as if it was in drive, and there as no way to stop it without placing the foot on the brake pedal the entire time or placing the blocks I referenced. The only way I was able to shut it off, was to pull what's known as the Fireman's Loop, which I'm guessing firemen pull in the case of an accident. I wasn't aware of this mechanism, and only learned of it after posting on numerous social media apps regarding this issue. This is a Major issue, as we are talking about a moving and unstoppable car! Tesla needs to build some mechanism that allows the car to be shut off by the driver or to override the screen and place the car in park. Pulling this firemans loop and unplugging the 12v battery, required me being in front of the vehicle, risking my life, as the vehicle could've easily moved if the concrete barriers didn't stop it, and also required me taking 30+ minutes to remove components from the Frunk, just to access these components!!Read more

The vehicle’s infotainment touchscreen intermittently malfunctions due to apparent LCD ghosting or phantom touch inputs. The system registers commands without driver interaction, resulting in unpredictable and uncontrollable behavior. The failure presents as: •Ghost touches and phantom screen inputs •Uncommanded radio, media, and menu changes •Audible warning or confirmation beeps without
... driver input •Sudden and unexplained changes to the instrument cluster display •Periods of complete touchscreen failure •Loss of reliable control of audio, navigation, and vehicle settings These events occur while the vehicle is in motion and without warning. The driver is unable to stop or override the inputs, forcing attention away from the road in an attempt to correct or understand the malfunction. No warning lights are present prior to or during the failure. This condition creates a significant driver distraction and represents a safety hazard. The infotainment system is integrated into core vehicle functions, and its failure directly interferes with safe operation. In some instances, the system becomes unusable altogether. The issue is intermittent, difficult to reproduce on demand, and not resolved by restarting the vehicle. Based on the symptoms, this appears to be a hardware-related touchscreen or digitizer failure rather than user error. Given the prevalence of many similar reports involving GM infotainment systems exhibiting ghost touch behavior, this condition may represent a broader defect affecting this platform and model year range. Re: Touch Screen has a mind of its own - Page 4 - 2014 - 2019 Silverado & Sierra - GM-Trucks.com. Summary of the Safety Defect The passenger-side window of my 2024 Winnebago Era 70X fell out of the vehicle. The window detached from the frame and separated from the RV while the vehicle was stationary. This failure represents a serious safety hazard, as the window could have fallen out while driving, potentially causing injury to occupants, other motorists, or pedestrians. Winnebago has acknowl
... edged the issue but has stated that the replacement window is on backorder with no estimated timeline. The vehicle cannot be safely operated without this window. Details of the Incident The passenger-side window unexpectedly detached from the RV. The window assembly appears to have failed at the adhesive or mounting points. This failure could have resulted in the window falling out at highway speeds. The defect compromises structural integrity, weather sealing, and occupant safety. The RV has been unusable since the incident. Manufacturer Response Winnebago Customer Care has stated: The window is “still on backorder.” No estimated date for replacement is available. They have not provided a temporary fix or safe-operability guidance. This leaves the vehicle in an unsafe, non‑operational condition. Why This Is a Safety Concern A window detaching from a motor vehicle is a critical safety defect because: It can detach while driving and strike other vehicles or people. It exposes occupants to road debris, weather, and potential ejection hazards. It compromises the structural integrity of the vehicle’s sidewall. It prevents safe operation of the RV. Requested Action I am requesting that NHTSA investigate this defect to determine whether: The window detachment is part of a broader pattern among Winnebago Era models The adhesive/mounting system is defective A recall or corrective action is warranted I am also requesting that NHTSA document this incident as part of its safety defect database.Read more

The air conditioning system in my 2020 Honda Civic EX has a refrigerant leak at the evaporator assembly, causing very weak or almost no cooling even when set to the lowest temperature settings. This happens consistently, including in moderate to hot outside temperatures, making the cabin uncomfortably warm during drives as it could get pretty hot in inland southern California. The A/C evaporator a
... ssembly is the failed component, and the part remains in the vehicle available for inspection if requested. This issue creates a safety risk because reduced visibility from window fogging in humid or rainy conditions (due to lack of proper dehumidification and cooling) can impair safe driving. In extreme heat, it also increases driver fatigue and distraction from discomfort. The problem was confirmed by a Honda dealership through diagnostic testing, which identified the evaporator leaking refrigerant. They previously repaired a related condenser leak under warranty, but the cooling issue persisted and was later traced to the evaporator. The vehicle has been inspected by the dealership (manufacturer-authorized) on multiple occasions for this A/C concern. No specific warning lamps or messages appeared on the dashboard related to the A/C system. This appears to be part of a broader pattern reported by owners of similar 2016-2021 Honda Civics, where condenser failures (sometimes covered by extended warranty) are followed by evaporator leaks, possibly related to the vehicle's use of R-1234yf refrigerant and system pressures. I recently received partial goodwill assistance from the manufacturer toward repair costs, but the issue highlights a recurring defect in the A/C system that affects cooling reliability. This car is driven by my 20 years old daughter as a commuter car to college and she is really suffering. I feel I am putting her at risk and need to get it fixed ASAP at whatever cost.Read more

I would like to formally file a safety complaint regarding my Chevrolet vehicle and to express concern over Chevrolet’s handling of a known defect. On January 22, while driving to work in cold weather conditions, my vehicle experienced a sudden and dangerous malfunction. The engine unexpectedly revved to approximately 4,500 RPM, the check engine light illuminated, and the dashboard displayed an
... “Engine Power Reduced” message. Despite removing my foot from the accelerator, the vehicle did not slow down as expected. I was able to pull over to the side of the road and bring the vehicle to a stop. I immediately contacted my local Chevrolet service department and was advised to bring the vehicle in six days later, the following Friday. Due to the safety risk, we did not use the vehicle during this time out of concern for a potential accident. When the vehicle was inspected by the Chevrolet dealership, we were informed that Chevrolet had identified this issue as being related to cold weather operation and that a custom shield had been designed to address the problem. We were also told that Chevrolet vehicles were not adequately designed for cold weather conditions, which led to this failure. Despite this acknowledgment and the fact that Chevrolet has engineered a corrective part, we were informed that the repair—costing approximately $1,600—is not covered under our extended warranty, as the shield was not installed on the vehicle at the time of purchase. The vehicle remains unsafe to drive. When accelerating, engine RPMs surge from approximately 2,500 to 4,000 RPM, and the vehicle can suddenly decelerate, creating a serious risk of rear-end collisions if a following driver is forced to brake abruptly. This issue represents a significant safety hazard. Chevrolet has acknowledged the defect by creating a corrective part, yet is declining to cover the repair cost, leaving the responsibility with the customer for a known design issue. Please let me know my next stepRead more

My 2025 Toyota Sienna Limited is subject to NHTSA Safety Recall25V668 (Toyota Recall No. 25TA12 (Interim Notice 25TB12)) regarding improperly welded second-row seat rails. The recall notice, which was mailed on 11/21/2025 and received by me the first week of December, 2025, states that these welds may fail in a high-speed collision, preventing the seats from restraining occupants and increasing th
... e risk of serious injury. Toyota interim safety instructions advise that second-row seats should not be occupied until repaired. Prior to receiving this notice in the mail from Toyota I was unaware of the defect. There were no service lights or notifications in the car's electronic panel. I have since learned that the notice was issued on 10/7/2025. My vehicle has been grounded at Conicelli Toyota Service Department in Conshohocken, PA since 12/22/2025 - It has been inspected for this defect and to date the service department manager does not have the parts to repair it and cannot give me an expected date for the part to be available and repairs performed. I purchased this vehicle in full on 3/6/2025. Since the recall was issued on 10/7/2025, I have contacted my Toyota dealer several times to inquire on an estimated date for repair of this safety issue. I have also filed a complaint /report with Toyota Brand Engagement Center. As of 1/29/2026, Toyota has failed to provide a definitive date for the repair or a timeline for the availability of replacement parts. I purchased this vehicle to safely transport my four grandchildren -ages, [XXX] , [XXX] , [XXX] and [XXX] to and from school/daycare every day. Because I cannot safely use the second-row seats, the vehicle is unusable for its primary purpose of transporting my family. I believe something is wrong with the electrical system tied into the infotainment system of the car, which displays the radio, heating, cooling, shows the rear camera controllers, as well as some fo the lighting in the sport L is not fully lit, for example where the shift gears is there a light that goes across the front, mines if red, only 1 side is actively turninig on, but the other side is of

I have videos of this for my records, and I did report this directly to Honda, but they indicated there is no recall for the infotainment system, I previously had a 2021 Odysessy and there was a recall for soemthing very similar, not sure why they would continue to use the same system in the newer vechiles, clearly something is wrong here, so I am documenting this for my records, but I believe Honda needs to step in and correct this issue before other electrical problems occur and someone gets injuredRead more

I am reporting a safety-related defect involving the Automatic Emergency Braking (AEB) system in my vehicle. The AEB system has repeatedly activated without any apparent reason, causing the vehicle to brake suddenly when there are no obstacles, vehicles, pedestrians, or road hazards present. These events have occurred under normal driving conditions. I have taken the vehicle to the dealership at

Despite these visits, the unintended braking continues to occur. There have been passengers in the vehicle during these incidents who witnessed the sudden, unnecessary braking and can confirm that no hazard was present at the time. This defect creates a serious safety risk, as unexpected braking could easily result in a rear-end collision or loss of control. I am increasingly fearful that this issue will cause an accident. Vehicle: 2006 Jeep Liberty VIN: [XXX] Summary: The front lower ball joint on my 2006 Jeep Liberty failed while the vehicle was in normal use. This failure matches the defect described in NHTSA Recall 06V288000 (Chrysler F23), which involves corrosion and separation of the front lower ball joints. My VIN does not currently show this recall as open, but the failure occurred exactly as described in
... the recall documentation. Details: In November 2025, the front end of the vehicle collapsed due to a lower ball joint separation. The joint failed without warning, causing loss of steering control. The vehicle had to be towed home by AAA. The failure is consistent with the recall condition involving water intrusion, corrosion, and separation of the lower ball joint from the steering knuckle. The vehicle still has other open recalls (fuel tank and rear lower control arms), but the front lower ball joint recall does not appear as open in the NHTSA VIN lookup. I believe the recall may have been incorrectly marked as completed in the past, or the original repair was not performed or not performed correctly. The defect has clearly recurred. Safety Concern: This is a serious safety hazard. The ball joint separation caused sudden loss of steering and could have resulted in a crash. The defect is identical to the condition described in Recall 06V288000. Requested Action: I am requesting that NHTSA review this case and require Chrysler/Stellantis to perform the recall repair for the front lower ball joints, as the defect has reoccurred and the recall never expires. On January 26, 2026, at approximately 6:50 AM, I experienced a serious safety-related malfunction in my 2023 Toyota Mirai while entering traffic on Inglewood Avenue in Lawndale, California. I was pulling out of a parking lot into active traffic at low speed. At the moment I was entering the roadway, the vehicle suddenly lost propulsion/power without warning. Simultaneously, the vehicle’s infota
... inment/center display went completely black, consistent with a system reboot or electrical interruption. After a brief pause, the display rebooted and turned back on, and immediately afterward the vehicle unexpectedly surged forward. At the time of the power loss, my foot was lightly on the accelerator pedal as I was merging. When the vehicle’s systems reinitialized, the car applied power consistent with the pedal position, resulting in a sudden forward movement. The accelerator was not fully depressed, and this was not a full-throttle event, but the surge occurred without warning while the vehicle was in active traffic. The sequence of events — loss of propulsion, apparent electronic system reset, followed by unintended propulsion upon re-engagement — created a hazardous situation while entering traffic and raises serious concerns about the vehicle’s electronic control and power management systems. The vehicle’s behavior was unpredictable at a critical moment when controlled acceleration was required, which could have resulted in a collision or injury. There were no warning lights or messages prior to the event. I am reporting this incident because it appears to involve a safety defect related to loss of propulsion and unintended surge while in motion. I am reporting a serious nighttime visibility and driver-assistance safety issue with my 2025 Honda Passport. During evening driving, approximately 60% or more of the upper portion of the windshield appears blacked out or severely dimmed, significantly impairing forward visibility. This condition worsens on downhill grades, where an even greater portion of the windshield darkens. Visibility improv
... es when high beams are manually activated; however, driving continuously with high beams is unsafe and inappropriate. This creates an unreasonable risk of collision, particularly on unlit roads and downhill grades, as pedestrians, road curvature, and hazards are difficult or impossible to see without improper use of high beams. In addition, multiple ADAS systems operate erratically. Adaptive cruise control delays braking on downhill grades until the vehicle accelerates approximately 9–10 mph above the set speed, delays acceleration after braking and resuming, and disengages with minor steering inputs. Automatic high beams operate inconsistently. These issues persist despite recalibration. The condition has existed since early ownership and continues at approximately 20,000 miles despite multiple service visits. A Honda field technician confirmed all systems were calibrated but did not perform a nighttime drive. A dealership shop foreman conducted a nighttime drive but did not share findings, and I was told he was unavailable and had already spent “enough time” on the vehicle. My initial service record included the notation “do not diagnose.” I was initially told the dealership had never encountered this issue; later I was told they had seen it on other vehicles but did not know how to fix it. A dealership-recommended nighttime comparison drive with a 2026 Honda Passport did not exhibit these issues. I also regularly drive a 2024 Honda CR-V that shows none of these nighttime visibility or ADAS problems.Read more

On at least 3 occasions the multimedia touch-screen display console has gone blank without warning lamps/messages. This causes a hazardous condition for 2 main reasons: 1. When this occurs, the reverse camera is no longer visible, thus increasing
... the chance to crashing into rear crossing traffic or pedestrians. On [XXX], (odometer 16339 mi) I reported it to SW Toyota in Lawton OK. They said that other owners had reported this problem but that it would take months to receive the replacement display, so I should take it in to the Toyota dealer in my home dealership in MS. I did so on 1/16/25, (odometer 17637mi). This dealership, J. Allen Toyota, Gulfport, MS, inspected the vehicle and said that they could not reproduce the problem.
