Beginning in July–August 2025, my 2019 BMW X3 (VIN [XXX] ) began experiencing multiple safety‑critical system failures affecting federally regulated components. The rearview camera intermittently failed to activate and later stopped functioning entirely, resulting in loss of rear visibility required under FMVSS 111. The Emergency Call (eCall) system displayed repeated malfunction warnings, ind
... icating loss of automatic crash notification capability. The vehicle also displayed “Driver Assistance Restricted,” “Collision Warning Malfunction,” and pedestrian detection failure messages, showing that forward collision warning and related ADAS functions were not operating properly. During the same period, the telematics control unit (TCU) lost functionality. This caused loss of ConnectedDrive services, backend communication, remote diagnostics, remote status reporting, and safety‑related over‑the‑air updates. These failures match known BMW defect patterns involving water intrusion into the antenna/telematics module and backend provisioning issues. I submitted a detailed written escalation to BMW of North America. BMW issued two brief responses on February 11–12, 2026 that did not address any of the safety issues, did not provide the engineering basis for excluding my VIN from the applicable recall, and redirected me to a dealer, which has no authority over recall determinations. BMW closed the case without providing any investigation results, defect determination, or technical explanation, contrary to obligations under 49 CFR § 573. These failures materially impair federally regulated safety functions, including rear visibility, collision avoidance, emergency crash notification, and telematics‑based safety communication. The pattern suggests a potential broader defect affecting additional vehicles. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2026 GMC AT4 3.0L Duramax had a catastrophic engine failure on Sunday, February 8, 2026. It went into reduced acceleration and also fault code PO6DD. Thankfully I was not on a highway. This obviously resulted in the truck being towed to a dealershi

This could have been major safety hazard if the engine had failed in a major freeway/highway.Read more

My backup camera began malfunctioning in November 2025. As a result, I contacted Ford directly and opened a case with the Automotive Safety Office in November 2025. Today, I followed up with Ford to check on the status of my case and was told there has been no update. I was also informed that because there is no CSP number associated with the 15-year extended warranty for Edge models that are not actively recalled, Ford cannot take any action. I was essentially told that the warranty “does not exist,” despite the fact that it has been widely reported by major news outlets and automotive publications. I was further told that if my vehicle were currently at a dealership, they might be able to help, but since it is not, they cannot do anything. Ford also stated that while the issue could potentially be recalled in the future, there is no action they are willing to take at this time. This is a known Ford defect, and despite extensive documentation and public acknowledgment of the issue, Ford is refusing to repair it.Read more

While driving, my 2017 Ford F‑150 experienced a sudden loss of the Output Shaft Speed (OSS) sensor signal, causing the transmission to stay stuck in a higher gear and triggering the wrench light. The truck lost power and would not shift properly. A diagnostic scan showed two transmission codes: one for the OSS sensor and a second code indicating an intermittent electrical signal drop. I contac
... ted a Ford dealer before the repair, described the symptoms, and asked for an estimate. The service advisor did not disclose any known issues and never called me back after saying he would check parts availability. These symptoms match the known failure pattern of the molded lead frame in the 6R80 transmission. My truck exhibited intermittent behavior, harsh shifting, and unsafe drivability. I had to pay $1,250 out of pocket to replace the lead frame to restore normal operation. I later learned that NHTSA has opened an Engineering Analysis into 2015–2017 F‑150 trucks for this exact issue (OSS signal loss due to internal electrical degradation of the lead frame). My vehicle’s symptoms, diagnostic codes, and required repair are consistent with the defect under investigation. This failure creates a safety hazard because the truck can suddenly lose the ability to accelerate or shift correctly, especially in traffic or at highway speeds. I am requesting that NHTSA include my vehicle in the investigation and that Ford be required to address this defect.Read more

The manufacturer has denied our claim and we are tasked with getting home to permanently park the RV. The manufacturer communicated with the shop regarding their finds and the denial, but has not contacted me in any way. All recalls/TSBs have been performed. The rear TSB has not fixed the rear frame sagging and the front TSB was not 100% completed by a dealer, in that several lag bolts were not replaced and none of them had adhesive applied. We have been fortunate that we have had no doors open while traveling. I composed a list of the items we found and submitted it with photos to the manufacturer. ... the vehicle was unexpected and alarming. Children, pets, elderly, and others could have been inside of the vehicle. Thankfully, they were not. Tesla inspected the vehicle and stated the failure was related to the battery coolant heater. The failure resulted in a complete vehicle immobilization and access failure. I had previously experienced restart warnings on the dashboard, but I did not expect a total lockout and loss of access. The inability to access the vehicle was unexpected and alarming. It may be a known safety issue. It should be recalled. This condition presents a serious safety risk because occupants could be trapped inside the vehicle and the vehicle cannot be accessed or moved without external power. In extreme heat or cold, or in a medical or emergency situation involving a child or pet, this failure mode could create a life-threatening situation. Importantly, even after multiple requests, I have not received a copy of the diagnostic report, or anything other than informal emails or calls. On Feb 3, 2026, I purchased a used 2023 Ram 2500 with ~40,107 miles from a large national used car dealer for ~$46,000. I drove the vehicle with my son as a passenger. On Feb 4, I had it inspected by a third-party shop. Both upper ball joints were worn beyond spec — the wheel moved in and out by hand on the lift, indicating imminent wheel separation risk. All 4 shocks and both front struts were
... leaking oil. All 4 tires were cupped from prolonged suspension failure. Front brakes near end of life. Estimated repairs: ~$7,000. These components do not fail at 40K miles under normal use, indicating prior abuse/neglect that existed before sale. The dealer's pre-sale inspection failed to catch an obvious, critical safety defect. I wanted to keep the vehicle and have it repaired. The dealer refused to authorize outside repairs, insisted on doing it themselves, but could not even look at it for over a week. They did not request a copy of the inspection report documenting the defects. I was forced to return the vehicle the same day. The ball joints posed a wheel separation hazard. My son and I were exposed to this risk on public roads. This raises serious concerns about the adequacy of this dealer's vehicle inspection process.Read more

I am writing to formally file a complaint and request resolution regarding a serious safety issue involving my 2012 Ford Fusion SEL (VIN: [XXX] ). I have been a loyal Ford customer for over 20 years and have owned only Ford vehicles. Unfortunately, my recent experience with Ford Customer Care and my local dealership has been extremely disappointing and concerning. A few weeks ago, my 2012 Ford F
... usion suddenly lost power steering while my young son was driving home from school. This created a terrifying and dangerous situation that could have resulted in a serious accident or injury. Thankfully, he was able to safely pull over, and no one was hurt. However, this was clearly a major safety failure. After this incident, I contacted Ford Customer Care and my local Ford dealership to seek assistance. I was informed that the Customer Satisfaction Program related to the EPAS steering system had expired for my vehicle and that Ford would not provide further support. However, this directly contradicts the information shown in my Ford app. The Ford app clearly lists Customer Satisfaction Program Campaign 15N01 – “EPAS Steering Effort” and instructs me to schedule a free service with a dealer. I later learned that an update was performed in 2015, but the EPAS steering system itself was never replaced—only updated—and that the program was considered a one-time offer. It is extremely upsetting to now face costly repairs for a known safety-related issue that was clearly not resolved correctly the first time.
