The vehicle was hit in the front end while my girlfriend was driving it. Someone ran a red light and smacked into the front end of the van at approximately 50 miles per hour. The airbags did not deploy despite significant force. The car that ran the red light was small but the force was significant enough that she hit her head and sustained medically verified extreme whiplash.

The power steering system in our 2024 Mazda CX-90 intermittently "sticks" while steering, requiring noticeably increased effort to turn the wheel before it suddenly releases. The symptom matches the defect described in NHTSA Recall 24V-022 (sticky steering due to worm gear friction in the power steering assembly) and the ongoing NHTSA Recall Query into vehicles where the 24V-022 remedy has failed
... to resolve the issue. Component: Power steering gear / worm gear assembly. The vehicle is available for inspection upon request. Safety risk: The intermittent increase in steering effort followed by sudden release creates a real risk of overcorrection, particularly during lane changes, highway driving, and turns from a stop. The driver cannot predict when the wheel will resist or release, which compromises the ability to maintain a consistent lane position and respond to road conditions. Dealer/service confirmation: UNKNOWN at this time — we are scheduling a dealer inspection. We will update if the dealer reproduces or documents the condition. Manufacturer/police/insurance inspection: None to date. No crash has occurred. Warning lamps or messages: None. No dashboard warnings, no error codes, and no messages have appeared before or during the sticking events. The condition is purely tactile through the steering wheel and is intermittent. The vehicle's recall status shows no active open recalls, suggesting any prior remedy under 24V-022 was already performed, yet the sticky steering symptom is still present — consistent with the complaints driving the current NHTSA investigation.Read more

My CX-5 has 86,120 miles. I noticed a burnt oil smell coming into the car and later found oil droplets on my garage floor. I took it in to my neighborhood garage and they found an oil leak coming from the cylinder head. They recommended I take to my Mazda dealer. I had it towed to Pearson Mazda, Richmond,VA. They inspected it and stated I had a cracked cylinder head. I have a 100K extended warrant
... y that will cover most of the cost to repair it. However, I found the Mazda CX-5 2018-2021 models have a cylinder head defect. I found a Mazda Service Alert, #SA-031/21 and on Reddit a Mazda CX-5 owners page that has many stories of the same problem, cracked cylinder head. Some have had Mazda cover the repair cost or given a major discount and provided an extended warranty after repair. I reviewed this with Pearson Mazda and they stated that my car does not fall in the vehicle numbers that they will cover. This is a known issue and they will not acknowledge it for my car. I have contacted Mazda Corporate and waiting for a callback. Litigation has been filed for the CX-5 Turbo customers and they have received restitution. This is also a major issue on non-Turbo customer’s SUVs. I have read that the 2018-2021 manufacturing (casting) of the cylinder head was defective. However, Mazda is not admitting it because a RECALL would cost them millions to fix the cylinder head issue on all the affected vehicles. But, they are at fault and should work with all customers that have been affected by this issue. Therefore, I am requesting NHTSA to make this a public safety matter and have Mazda be accountable for the issue. Thank you!Read more

Unexpected Automatic Emergency Braking Activation at Highway Speeds in Cold Temperatures. While driving at highway speeds, the system activates a forward collision warning (audible alert and dashboard indicator), followed almost immediately by auto
... matic braking, even when there are no vehicles, obstacles, or pedestrians ahead. Conditions When Issue Occurs: * Ambient temperature: below ~40°F (4°C) * Road conditions: cold, often icy or snowy * Speed: highway speeds 45-75 * Environment: clear roadway, no visible hazards * visibility : clear System Behavior: * Audible warning and visual alert activate * Automatic braking occurs approximately 0.5–1 second after the warning * No obstacle is present to justify intervention Safety Risk: This presents a serious safety hazard because: * Sudden braking at highway speeds creates a high risk of rear-end collision * The warning-to-brake interval is extremely short, leaving little time for driver override. * Occurs in low-traction conditions, increasing risk of loss of control. * The event is unexpected and not predictable by the driver. Frequency: * Occurred multiple times under similar cold-weather conditions. * Appears repeatable based on temperature range. Attempts to Resolve: * Sensors/camera appear clean and unobstructed. * No prior collision damage to the vehicle. * Issue persists despite normal operation otherwise. * Similar reports observed from other owners in online discussions. This is not a one time occurrence, in the section below, i noted the last event but this is a daily occurrence in temperatures below 42F.Read more

Vehicle: 2021 Ford Mustang Mach-E Component: Image Processing Module A (IPMA) / Front Camera / Forward Collision Avoidance System The IPMA and front camera system have failed, disabling Pre-Collision Assist, AEB, forward collision warning, and adaptive cruise control while driving. The vehicle is available for inspection upon request. Warning messages "Front Camera Fault Service Required" and "P
... re-Collision Assist Not Available" first appeared August 24, 2022 and have recurred consistently over nearly four years. No accident, physical damage, or obstruction was present at any occurrence. This failure puts safety at risk by silently disabling AEB and forward collision warning during normal driving — systems a driver may depend on in an emergency braking situation with no advance notice they are unavailable. The problem was confirmed by a Ford dealership in February 2026. Their repair order documents: "RAN IPMA SELF TEST AND FOUND DTC SET U3000-49... HARDWARE TEST FAILED... FOUND TSB RELATED REC REPLACE MODULE." The dealer contacted Ford, who authorized 50% off the module replacement only, characterizing it as a "loyal customer discount" rather than a warranty or goodwill repair. I declined to pay for the fix. NHTSA Campaign 11028164 (superseding TSB 25-2156) covers this exact failure. Ford's own TSB acknowledges the defect and the dealer confirmed it applies to my vehicle, yet owners are being quoted up to $8,000 for a known safety system failure. On 3/15/2026 I was on my way home from Massachusetts to Pennsylvania, and about 35 minutes into my drive I got an alert stating "High Engine Temperature Stop Safely", so I pulled over immediately and turned the car off. After allowing the engine to cool down, I opened the hood and started the car to see if I could identify any issues, the "Engine Fault Service Now" alert came up. This was the firs
... t time this ever happened, this put me, my two dogs who were in the car with me, and possibly others on the road at risk, had the conditions escalated. (i.e. under hood fire). The vehicle was towed to an Authorized Ford Dealership, Mathieu Ford Sales, Inc. who diagnosed the issue on 3/17/2026. The dealership removed the spark plugs to inspect cylinders and found coolant intrusion into cylinders significantly on cylinder 2. Dealership referenced TSB 22-2322, is recommending short block assembly and related parts to remedy concern. Dealership advised it was safe and okay to drive the vehicle slowly to a nearby residence (to store the vehicle/retain possession), noting to pull over if an alert pops up and to have it towed if I received another message like before. The dealership advised it was not safe to drive the vehicle back to Pennsylvania. There weren't any issues driving the vehicle back on 3/17/2026 following the dealership's guidance; the car hasn't been driven since. The car needed to be jumped to start the vehicle on 4/17/2026 for a tow truck to load the vehicle onto its bed. The vehicle is currently back in possession of the dealership who diagnosed the issue. Upon doing research, this appears to be a widespread manufacturing design defect that Ford has failed to properly address/recall. My vehicle met the criteria under their Customer Satisfaction Program in 2022 - the vehicle is an Escape, 2017-2019, assembled in Louisville, that was Built before April 8, 2019, and which exhibited a covered condition, having low coolant. The current 6.2L engine recall was completed by Janssen Chevrolet in Holdrege, NE and it was noted the vehicle was fine. On April 11th, while driving down Interstate 80 in Nebraska, the engine suddenly died and would not restart. No warning lights or indicators illuminated prior to the failure and there were no symptoms. By sheer luck, it occurred before the head to head construction area with no
... shoulder and there was a rest area within coasting distance that and we were able to reach. I had little children in the car and we were stranded on the highway until friends were able to pick up the family and transport them to Lincoln and I was able to travel with the tow truck to deposit the vehicle at Husker Auto Group in Lincoln, NE. I was forced to rent a vehicle from a 3rd party as the dealer was closed and it was Saturday evening. On Monday, the dealer confirmed metal in the oil and filter and that the engine was completely dead and unable to be repaired, requiring replacement under the current recall. A much smaller loaner vehicle, insufficient to transport my family has been provided. It has been 13 days and there is no update on how long the vehicle will be out of service awaiting an engine.Read more

My 2025 Ford Ranger Lariat FX4 has experienced sudden and complete loss of accelerator pedal response while driving at low speed on at least 4 to 6 separate occasions. The engine continues to run normally, but pressing the accelerator produces no power and no vehicle response. The condition has consistently occurred while pulling into my subdivision, which has an inclined entrance. The truck also
... feels noticeably sluggish nearly every time I make this turn, even when full dropout does not occur. No warning lights, check engine light, or messages in the instrument cluster illuminate during these events. No diagnostic trouble codes have been stored that I am aware of. The vehicle is available for inspection upon request. The symptoms closely match two active Ford recalls — 26S10 (EGR valve failure causing unexpected loss of drive power, stated by Ford to be most likely at speeds under 13 mph) and 25V-239 (PCM reset causing temporary loss of acceleration). My vehicle is not currently listed as included in either recall per Ford’s VIN lookup. Numerous owner reports on public Ford Ranger forums describe identical symptoms on 2024 and 2025 Ranger trucks, many of which are also not covered by the existing recalls and for which dealers have been unable to find stored codes. A sudden, unannounced loss of drive power at low speed on an incline or while turning into traffic represents a clear crash risk. I am requesting that NHTSA investigate whether the affected vehicle population should be expanded.Read more
