This defect presents the following safety concerns: 1. The persistent, non-dismissible warning light desensitizes the driver to critical dashboard warnings, undermining the purpose of the vehicle's warning system. When drivers are conditioned to ignore a permanent warning, they are more likely to overlook new, actionable safety alerts. 2. The emergency call (eCall) system itself is rendered non-functional, meaning the vehicle will not automatically contact emergency services in the event of a serious collision where the driver is incapacitated. 3. The warning creates a visual distraction on the instrument cluster during driving. BMW's only offered remedy is full replacement of the TCB module at a cost of $1,500–$2,500 to the owner, despite this being a manufacturing defect (faulty antenna seal adhesive) and not a wear item. The defective component is located under the headliner and is not accessible for owner maintenance. I am submitting this complaint to report a serious vehicle safety incident involving my 2017 Lincoln MKZ Select that resulted in a catastrophic engine fire while the vehicle was in operation. Prior to the incident, the vehicle underwent a long block engine replacement performed by Murgado Ford Lincoln of Chicago pursuant to Ford Technical Service Bulletin (TSB) 22-2229. After the vehicle was returned to me following this repair, the engine later experienced a sudden catastrophic mechanical failure while I was driving. The vehicle rapidly began emitting smoke from the engine compartment and subsequently caught fire. The fire escalated quickly and rendered the vehicle a total loss. At the time of the incident, the vehicle was being operated under normal driving conditions. There was no collision or external cause that would have triggered the fire. The event appears to have originated within the engine compartment following the recent engine replacement. The vehicle was subsequently towed and is currently stored at City of Chicago Auto Pound No. 6 following the fire incident. The vehicle has not been repaired or altered since the event and remains available for inspection. Given that this vehicle had recently undergone a major engine replacement under a manufacturer technical service bulletin, I believe the circumstances warrant review to determine whether the incident may be related to: a defect in the replacement engine assembly improper installation of the engine or related components fuel, oil, or electrical system issues associated with the repair. This incident presented a serious safety risk to the driver and could have resulted in severe injury or loss of life. I was fortunate to escape the vehicle safely before the fire fully developed. These symptoms match the known failure mode of defective 6R80 transmission lead frames, which NHTSA has previously identified as a serious safety hazard due to sudden loss of power, erratic shifting, unintended acceleration, and dashboard failure. The vehicle became undriveable and could not be safely transported to a Ford dealership. It had to be repaired at a nearby independent shop (Auto Stop Elite of Purcellville, VA). The conductor plate was replaced on February 12, 2026, at a total cost of $1,689.59. Despite this being a well‑documented safety defect affecting many Ford F‑150 owners, no recall or service campaign was ever issued for my VIN. No warnings appeared on Ford’s website, Carfax, or through any Ford communication. Other owners have received reimbursement from Ford for the same failure, yet Ford provided no notice or remedy in my case. This defect placed my son’s life in danger and represents a clear safety issue that should be addressed. I am requesting that NHTSA review this incident as part of the ongoing pattern of conductor plate/lead frame failures in Ford’s 6R80 transmissions. We are informing you because we have contacted Ford and they have done NOTHING to resolve this. We had to have it repaired and were never aware that there was a safety recall. We expect to be reimbursed in full for this repair by Ford. To date - we are being ignored and will now pursue with the Virginia Attorney General's office as well. No coolant on the ground (there was never any evidence of a coolant leak on the ground). Diagnosed (1/07 and 1/14) as internal coolant leak by two different technicians (an independent service center and a Ford dealer). Need to replace engine block, etc. Cost for repair over $10,000. Unable to drive car for at least two weeks. Cost of repairs were over $1500, before final diagnosis made. Car cannot be driven safely without repairs. Overheating occurred in May 2025, December 2025, and February 2026. During these events the engine temperature rose abnormally and coolant had to be added to the reservoir. Drivetrain problems including transmission jerking, vibration, and driveshaft repair also created drivability concerns while operating the vehicle at highway speeds. These issues have been inspected and confirmed by an authorized Lincoln dealership service department. Numerous repairs and diagnostics have been performed including APIM replacement, cam phaser repair, heater hose replacement, wheel bearing/IWE replacement, driveshaft repair, and engine diagnostic work approved through warranty processes. Symptoms prior to failures included overheating, vibration, excessive fan noise, whining while driving, and transmission jerking. The vehicle and components are available for inspection upon request. Root Cause: Manufacturer-Installed Plastic Shield Design Defect (B8/B8.5 and B9 Platforms) The corrosion is caused by a design defect present on the Audi B8/B8.5 and B9 platforms: plastic aerodynamic shields are installed by the manufacturer on both sides of the rear subframe. These plastic shields trap road salt, water, and debris directly against the bare steel subframe, preventing it from draining or drying. Even vehicle owners who regularly wash the undercarriage—as I did—cannot prevent salt accumulation behind these shields. This is not general road salt exposure. The plastic covers are an Audi-designed component that actively concentrates corrosive materials against the subframe. The corrosion therefore originates from a manufacturer design decision, not from owner neglect or normal environmental exposure. III. SAFETY RISK The rear subframe is a primary structural and suspension component. It mounts the rear suspension, differential, and drivetrain to the vehicle. Loud clunking noise from front end when turning right or left and accelerating from a stop or slow speed. I’ve been told by dealer that this is from defective front CV axles. I’ve also been told that this could be a safety issue if the component fails completely. Both General Motors and Honda are aware of the issue.

I purchased the vehicle back in June of 2023, brand new, fresh off the lot and from then the issue began. I had a leak in the fuel line and the engine blew. That was all fixed. But the main issue was the loud vibrating noise that the service department tried to correct multiple times and couldn’t tell where it was coming from. From both service centers and other sales rep, the 2024 Chevrolet Trax 1RS has this issue and no matter the updates and corrections that were attempted it couldn’t be corrected. I submitted a complaint to corporate where they decided to send a “technician” from their office. Only the tech could not duplicate the issue but everyone else could. Because of that the buy back option was off the table and I was given a 2,000 certificate for another car, I still owed 21k for the trax. While driving in grocery store parking lot. The brake booster completely failed with no notice almost causing me to hit someone. I had to throw the truck in park while moving to avoid hitting pedestrians. The brake pedal is completely compressed to the floor as I was stomping the brakes in an attempt to stop. Had I not thought quick enough I would of hit at least 2 people.

My vehicle is experiencing the exact symptoms described in that program, but the support period has expired even though the mileage is well within normal expectations for a hybrid of this age. The sudden loss of braking assist is a serious safety hazard. There was no warning before the failure, and the vehicle became difficult to stop immediately. This appears to be part of a widespread pattern of brake booster failures on this generation of Prius, and the defect poses a risk of crash or injury.
