These codes pertain to a manufacturers defect with Lexus, wherein Lexus generated a customer convenience program that said any LS460 that is less than 10 yrs old OR less than 150k miles can be brought to any dealer and have this brake system replaced at Lexus expense. This is a SAFETY issue due to a manufacturers defect from Lexus; therefore I should not have to pay for something simply because I have a low mileage car. Tires are Douglas Touring, and Sceptor. Those options were not available. I bought a set of tires that belongs to two different brands. I even filed a set of NHTSA reports in this regard earlier this year in February 2025. Now, two tires have gone down to less than 7/32, and as stated above has reached 2/32. I even shared a picture of peeling tread that I noticed at the time of purchase. This repeating issue suggests high likelihood with quality concern with these tires. For that matter, suddenly, I was asked to change brake rotor and pads, while previous inspections did not suggest any such change. Upon restarting the car, power was restored and the check engine light didn't illuminate again. There was slight shuddering in low gears. We made an appointment at the same First Texas Honda mentioned above to fix the problem, but after inspection they insisted that the problem was caused by loose battery cable unrelated to the fuel pump recall. The dealer inspected the vehicle and confirmed excessive valve leakage into the exhaust system, requiring a complete cylinder-head replacement. They warned the failure will worsen and could result in sudden power loss again. This defect poses a serious safety risk. If the power loss occurs while merging or at highway speeds, it could easily lead to a rear-end collision. The vehicle has relatively low mileage for such a severe internal engine failure, and I have learned that other owners of this same engine have reported similar issues. Sudden stalling or loss of acceleration places occupants and other drivers in danger. There were no warning messages prior to the failure, and the problem has recurred. The vehicle is available for inspection, and the defect has been confirmed by a Mercedes-Benz service center. This appears to be a mechanical defect in the engine design, potentially involving valve seat failure. ... looking around to see where the crackling noise was still coming from. I looked up towards my Sunroof and discovered that a large hole in my Sunroof. Fortunately, my sunshade was closed and shield the debris/glass from spraying my face and body. I took the car to the V.W. dealership and explained the infraction to them. They diagnosed the problem and contacted me later that day to let me know that they were sending the request to V.W. corporate to get authorization to repair under warranty. The dealership did not detect any signs of rocks or debris causing the explosion. Corporate denied the dealerships request to repair under warranty. I filed the damaged Sunroof under my auto insurance. Im currently awaiting for the adjuster to inspect the damaged Sunroof that is at the dealership. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. Following this incident, I took the vehicle to a local auto shop, which identified diagnostic code P030002, indicating a misfire in Cylinder 2. I then promptly brought the vehicle to an authorized Lexus dealership. Their diagnostic confirmed the same code and attributed the failure to a “Cylinder 2 direct fuel injector leaking.” Lexus has already acknowledged this defect in TSB L-SB-0007-21, demonstrating that the manufacturer is aware of this recurring issue. My vehicle is five years old with fewer than 100,000 miles, yet this defect resulted in a dangerous loss of engine power under highway conditions. The dealership quoted over $3,000 to replace a single injector, and the remaining injectors may fail at any time, further compounding the safety risk. Despite its knowledge of this defect, Lexus has not issued a recall, implemented preventive measures, or offered assistance, citing that the vehicle is slightly outside the 70,000-mile warranty. I have therefore been left with a vehicle that is unsafe for continued highway operation. Given the severity of the incident, the known recurrence of this defect, and the manufacturer’s lack of corrective action, I respectfully request that NHTSA investigate this matter, as it presents a substantial and ongoing hazard to consumers and the driving public.Read more

I was told that the previous recall for cracked dashboards ended in 2017 and that it was cosmetic only, not safety related, but that the part was available for $964.50, not including labor, tax, etc. I feel that a safety related recall should be issued, the previously known dash cracking problem didn't end when their recall did, the fact that my vehicle is primarily garaged preserved the interior integrity, I don't believe I should be penalized for my dash not cracking 8 years earlier.
