The contact owned a 2025 Honda Civic. The contact stated that while driving at an undisclosed speed into a parking spot, the brake pedal was depressed and went to the floorboard. The vehicle failed to come to a stop. The vehicle drove over a parking median in front and struck a tree. The contact then attempted to steer the vehicle to the left, but crashed into another vehicle on the passenger si
... de. The vehicle flipped onto its driver's side, where it came to a stop. The message "Brake" was then displayed. The air bags deployed, shattering the front driver's side window. OnStar came on the speakers and informed the contact that help was on the way. The EMT arrived, and the contact was able to exit the vehicle through the sunroof. The contact sustained injuries to her left shoulder and arm, as well as a sore neck and stomach. Medical attention was received. A police report was filed. The vehicle was towed to a tow yard, but had not yet been deemed totaled by the insurance company. The manufacturer was not made aware of the failure. I am writing to formally report Hyundai’s failure to remedy Safety Recall 25V607 involving defective first and second-row seatbelts in our 2025 Hyundai Palisade. The recall was issued in September 2025 and stated no remedy was yet available but the vehicle remained safe if belts were checked after fastening. In December 2025, the front passenger seatbelt in our vehicle began unlatching while dri
... ving at highway speeds, even after confirming proper engagement as instructed by Hyundai. We have repeatedly contacted Hyundai of 110 (Farmingdale, NY) regarding remedy availability. A remedy became available in early December, and we were put on the dealership's list to order parts from Hyundai directly on December 30, but the parts still continue to be on backorder. We have minor children who must now rely on seatbelt-based car seat installation due to weight limits on LATCH. This is our only vehicle and it is virtually unusable at this point because I cannot in good conscience put my child in a car seat secured by a defective seatbelt. Given the documented malfunction of the seatbelt system, we are extremely concerned about the continued safety of this vehicle. Hyundai has not offered: - A loaner vehicle - A repurchase - A timeline for remedy We respectfully request investigation into Hyundai’s delay in remedying this federally mandated safety recall.Read more

At approximately 5:50 PM on Feb 12, 2026, a collision occurred while the vehicle was under the control of "Full Self-Driving (Supervised)." While parked in a commercial lot, I engaged FSD to initiate a route home. A stationary semi-truck was positioned behind my vehicle. Upon engagement, the FSD system failed to detect the presence of the stationary truck. The vehicle initiated a rearward/lateral
... maneuver at a speed of 1 mph, directly striking the corner of the semi-truck. Safety Failure Details: The collision resulted in significant damage to the right rear quarter panel and associated sensors of my Tesla Model Y. Dashcam footage and system telemetry confirm that FSD was active (Supervised mode) during the entire duration of the maneuver. The system's occupancy network and vision suite failed to identify a large, stationary object within its immediate path of travel at a crawl speed. Request for Investigation: I am reporting a critical failure of the FSD software’s object detection and path planning capabilities. The system commanded a movement into a clear obstruction without issuing any takeover alerts or applying emergency braking, despite the low speed and high visibility of the obstacle.Read more

This complaint involves a 2023 Jeep Grand Wagoneer L Series III, a full-size luxury SUV. The vehicle has experienced repeated electrical and powertrain failures culminating in complete loss of propulsion. On 01/22/2026, while traveling approximately 45–50 mph, the vehicle abruptly shut down without warning. The engine lost power, dashboard displays went black, and multiple warning lights illumi
... nated simultaneously. I was forced to steer to the shoulder while a trailing vehicle had to swerve off the roadway to avoid a collision. This was a near-miss event and represents a serious safety hazard involving sudden loss of motive power at highway speed. Prior to that event, the vehicle had already stalled at a traffic light and had exhibited repeated check engine warnings, low-voltage faults, and module communication errors. The servicing dealer documented multiple diagnostic trouble codes affecting the Body Control Module (BCM), Ignition System Control Module (ISCM), and Powertrain Control Module (PCM). Manufacturer technical service bulletin software updates were applied in attempts to correct the issue. Despite these interventions, the electrical instability persisted. On 02/12/2026, only days after an auxiliary battery replacement intended to address voltage concerns, the vehicle would not start and required towing to the dealership at my expense. Additional fault codes are currently under review. The vehicle is also subject to an open recall with no remedy available, and the power running board motor has failed with replacement parts unavailable. This pattern of electrical failure, sudden shutdown in motion, and inability to reliably start presents an ongoing and unacceptable safety risk to occupants and surrounding motorists. The vehicle is available for inspection and has been repeatedly evaluated by an authorized dealer.Read more

Description of Incident: This is a recurring safety defect that has occurred multiple times from August 2023 to February 2026. The following description refers to the most severe incidents: 1. NATURE OF DEFECT: The vehicle experienced a sudden "Limp Mode" event where all dashboard warning lights illuminated simultaneously, followed by an immediate loss of propulsion while in motion. Subsequently,
... the vehicle suffered from a dangerous 3-second acceleration lag/delay. This 2023 Lexus LX 600 creates a life-threatening risk on public highways, as the driver cannot reliably control the vehicle's speed or power to avoid collisions. 2. MANUFACTURER’S ADMISSION: The vehicle has been presented to authorized Lexus dealerships. While the dealership could not provide a permanent fix, the manufacturer’s own internal tracking system now explicitly confirms: "REMEDY: NOT AVAILABLE." This is a formal admission by Lexus/Toyota that the vehicle has a catastrophic system failure with no known fix. 3. REQUESTED ACTION: Lexus/Toyota has refused to repurchase the vehicle, forcing me to drive a dangerous asset they admit they cannot repair. I request that NHTSA immediately initiate a safety investigation into this loss of propulsion and acceleration lag issue for the 2023 Lexus LX 600, especially given the "No Remedy" status. A mandatory recall is warranted.Read more

I have a 2007 Hyundai Sonata SE v6 3.3L and on January 9th, 2026 I had an incident where on the road my charging system light on signifying alternator failure. I made it home, but the battery was dead which was recently purchased. After recharging the battery I conducted tests with a multimeter and have determined the alternator has failed. The alternator has accrued a coating of oil overtime that
... was unknown until my research into the matter of root cause. Hyundai manufactures a V6 engine family called Lamba in which a version called Lambda MPi G6DB was introduced in 2007–2009 Hyundai Santa Fe (CM) and 2005–2009 Hyundai Sonata (NF) models. In 2023 Hyundai issued a recall for the 2007-2009 Santa Fe for a Cylinder Head Cover Oil Leak Repair where oil drips from the valve cover directly onto the alternator causing it to fail. I have a 2007 Hyundai Sonata 3.3L V6 engine where the very same issue has occurred with photo evidence that has caused my alternator to fail and have tested the issue. This vehicle model was not included in the recall despite the engine being exactly the same as the recalled model, the year being the exact same as the Santa Fe, and the alternator in the exact placement where the oil drips into the alternator causing failure just like the Santa Fe recall. The Santa Fe recall with the same issue is documented with National Highway Traffic Safety Administration below: [XXX] Hyundai has yet to issue the same recall for the Hyundai Sonata brand of vehicles, despite the same issue occurring and a known issue with the Lambda MPi G6DB engine design effecting multiple models in the same way documented across multiple owners of these vehicles with various fixes suggested on Youtube, Reddit, and enthusiast forums for this engineering failure that causes considerable risk if the alternator fails in operation on roadways. While driving, my vehicle has intermittently lost power without warning, creating a serious safety hazard. The loss of power occurs unexpectedly while the vehicle is in motion and is not associated with any driver input, weather, or road conditions. The issue has occurred on more than one occasion. The first occasion in March 2025. The issue appeared to be temporarily resolved but later recurred, i
... ndicating the underlying problem was not corrected. When the loss of power occurs, the vehicle does not respond to acceleration input and behaves as though propulsion is cut off completely. This happens suddenly and without warning while driving. This condition creates a dangerous situation, particularly when driving in traffic or at speed as was the case on 12/4/25 when driving 70 mph on the freeway, the second time the issue occurred for us. The car was immediately brought to the dealership on 12/4/2025 and remains there today. On the first occurrence the dealership attributed it to low battery charge and replaced both batteries. The dealership has performed inspections and diagnostics including:Battery testing and replacement,Wiring and harness inspections,Software updates to vehicle control modules,Extended road testing.Despite these efforts:The issue has not been permanently resolved,The dealership has stated the concern could not be duplicated,The dealership has indicated that fault codes were not always present. A STAR case has been opened, Jeep Customer Care has been contacted regarding this issue, and a field technician has been requested to come to the dealership to assess the vehicle. Despite repeated service visits I am submitting this complaint to report a recurring, intermittent loss-of-power condition that may not consistently trigger diagnostic fault codes but poses a serious safety risk for the car occupants and other drivers on the road surrounding this vehicle.Read more

On 1/19/2026 I had my 2023 Jeep Wrangler 4xe serviced at a Jeep dealership to have recall 68C corrected. I received a call from the dealership later that day stating a new battery was not needed, only a software update was needed. They completed the software update. On 1/31/2026
... when I started my Jeep in the morning, the check engine light was on. After stopping at a red light, my Jeep felt like it was losing power. To get it to move, I had to push the pedal down to the floor, the Jeep jerked some and did drive but felt like it was going to shut down. I pulled in the parking spot. Turned the car off and tried to turn it back on. It would not start. The red battery light appeared, a symbol of a charger (accel.), and an error message stating, "service charging system". None the hybrid options were available and electronic mode showed not available. I had to have the Jeep towed to the dealership that recently completed the recall. The Jeep did start when the tow truck driver arrived, but I was worried that it would stall and leave me stranded on the beltway. On 2/2/2026 I received a call from the dealership stating the issue was with the battery and that it needed to be replace but it will take "some time" to receive. On 2/5/2026 I received a call from the dealership stating the battery should be received around 2/26/2026. I was reviewing online comments from other Jeep owners impacted by recall 68C and the issue that I experienced appears to be a common theme. On February 3, Tuesday drove home from work. The battery light came on. I Drove to AutoZone and they placed it on Diagnostic reader. The diagnostic reader said that the battery had no charge. They told me I need a new battery. I replaced the battery. The next morning, I placed the jeep into auto start. So the Jeep can warm up. When I went outside, there was smoke coming from underneath the hood of
... the Jeep. I quickly turn off the Jeep and pop the hood. My husband open the hood and can see smoke coming from the alternator. He turned around to turn off his vehicle that was running. I started to video recording the incident and stopped it right away when I started to see flames coming out of the alternator. My husband and I quick action was to grab snow that was next to the Jeep to throw it onto the alternator to put the fire out. It took multiple piles of snow that the husband went and grabbed the fire extinguisher and sprayed the alternator to extinguish the fire. Once the fire was extinguished. We kept an eye out to make sure everything was good. We left the hood up to cool things down. We called morgan Towing to have the Jeep towed to their location to look at the situation. Morgan Towing picked the jeep up on February 4 Wednesday. They replaced the serpentine belt and the alternator. They called us to let us know the Jeep was done. Picked up the Jeep today, February 5 Thursday. I Drove it literally down the street and I can smell strong odor of smoke along with the battery light came back on. I turned back around and brought it back to Morgan Towing. I called my auto insurance and waiting to hear back from them. I am going to call the Dodge Jeep dealership and see what they have to say if if there’s was any recall on the Jeep. Upon my research so far, I didn’t see no recall but again I’m gonna call the dealership.Read more

I am reporting two safety-related problems in my 2022 Lexus GX 460, both present since shortly after purchase and the vehicle is available for inspection upon request. 1. Power Train / Speed Control (delayed acceleration / no throttle response): When I transition from the brake pedal to the accelerator pedal quickly (normal driving), the vehicle intermittently displays a warning message indicatin
... g the accelerator is being pressed while the brake is also pressed (or similar wording). When this happens, the vehicle does not respond to the accelerator for several seconds even if I press the accelerator to the floor. This has put my safety and the safety of others at risk because the vehicle fails to accelerate when needed (e.g., when merging, turning, or avoiding traffic), creating multiple dangerous near-miss situations. The warning message appears at the time of the event; I did not notice a warning lamp or message consistently prior to the event. This issue has not yet been reproduced or confirmed by a dealer or independent service center. A dealer service visit is scheduled for next week. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives for this problem. 2. Seats (driver seat causes leg numbness/spasms impairing pedal control): The driver seat bottom is unusually firm, and after approximately 30 minutes of driving my legs become numb from the lower thighs down to my feet and I experience spasms to the point that I cannot safely operate the brake and accelerator pedals. This is a safety risk because it can prevent proper pedal control while driving. This condition has been consistent since shortly after purchase. It has not yet been confirmed by a dealer or independent service center, and the vehicle/seat has not yet been inspected by the manufacturer, police, or insurance representatives for this issue.Read more

During each time the code P0720, output speed sensor, will appear and I will have to clear the code and it will wo

The truck is available for inspection. My safety and others were put at risk since I lost the ability to control my throttle making my acceleration and deceleration erratic. Also, when I was stalled at the green light it was on a road that had a speed limit of 70 which I could have been hit by another vehicle predicting me to drive away from the stop light when I could not. It hasn't been confirmed by a dealer or independent service center but matches the description of the NHTSA investigation into the issue. The vehicle component has not be inspected yet. Prior to the issue, no warning lights have popped up. I purchased the 2022 Lexus RX 350 used a couple of months ago. While driving down the highway the instrument panel went blank. I was terrified. I continued to drive not knowing what speed I was driving. The instrument panel went panel came back minutes later. Another incident occurred when the car was parked in my driveway. I turned on the engine and the instrument panel was blank. No numbers
... and no information on the instrument panel. As I drove down the street slowly the instrument panel returned to normal. A third incident occurred when occurred as we were driving on the highway. The light on the instrument panel began to darken. My husband was driving and I was in the passenger seat I was able to access the light app on the dash and restore the light to the original setting. This car is not on the recall list. It should be because this is a matter of life or death. The car had 59000 miles on it when we bought it so it is not under warranty. I contacted Lexus and they refused any help until I take it to a Lexus dealer and have it diagnosed. I don’t drive it daily because I have other vehicles but I would like to be able to drive it and have full use of the instrument panel. The problem repeats itself.Read more

The front passenger seat sensor failed. Without the sensor properly detecting weight, the airbag will deploy no matter the weight of the front seat passenger. Tesla has fixed the sensor placement in newer models as the service tech told us this is a common issue in later models where the only fix is to pay over $5k to replace the entire seat otherwise the airbag will go off no matter what.

When coming from a stop, the car will not respond to the gas pedal being engaged OR it will begin to engage and then no response leaving the car rolling for a few seconds until a response is recieved and the car engages again. This has caused us to be put in harms way MANY times with oncoming traffic when making turns or in front of oncoming traffic in intersections. The dealer states this is not
... a known issue however it is all over the internet that hundreds of reports of this issue have been made. The dealer states no fix since they don't "have a problem". MAJOR safety issue. We have reported this many times to Lexus now with no resolve or help from them.Read more

This vehicle has a recurring, safety-critical defect involving intermittent engine misfires, flashing MILs, reduced engine power, sudden loss of acceleration, and noticeable engine shaking. Despite multiple repair attempts and extended diagnostics by a dealership, with manufacturer technical assistance, the defect continues. The defect has repeatedly caused the vehicle to stall and lose propulsion
... while driving, creating a serious hazard. Documented events include: •FIRST INCIDENT: The MIL flashed and engine power was reduced while driving. The vehicle was taken to the dealership for evaluation and repair. A diagnostic code confirmed a cylinder misfire. •SECOND INCIDENT: Several days after being returned to the owner, the MIL flashed again and engine power was reduced, requiring careful maneuvering. The vehicle was returned to the dealership for further repair and remained in their custody for about a month. •THIRD AND MOST SEVERE INCIDENT: A few days after being returned, the MIL flashed again. The vehicle abruptly slowed, shook, and temporarily stalled in active traffic, requiring immediate intervention to avoid a collision. The engine restarted after pulling over safely. The vehicle has undergone compression testing, coil and spark plug replacement, battery replacement, and extended diagnostics with manufacturer technical support. Despite these efforts, the defect persists, demonstrating it is intermittent and unpredictable. The combination of sudden loss of engine power, stalling, and shaking in traffic creates a foreseeable risk of collision, particularly at intersections and in situations where predictable vehicle response is critical. Videos and diagnostic evidence document these recurring failures.Read more

The engine check light is on and the car is using more oil than it should. I had the oil changed and had to add quarts before 2000 miles. I called Kia and was told my VIN is not part of the recall. I got an inspection and Cylinder 2 is misfiring and has low pressure so the entire engine needs a replacement.

My Lexus 2019 RX450H is making a squishing squeaking brake actuator noise when pressing the brake pedal. I located well publicized TSB’s documenting the awareness by Toyota/Lexus of the problem. They did in fact, confirm the brake actuator noise. I reject that concept because WHO could know ahead of time if a part experiences a (1st time) total failure without throwing codes and turning on dashboard lights? As I read the symptoms of severe problems experienced in a higher level of failure by brake actuators, I would not want to have to experience a crash accident in which the actuator would operate correctly in a hard stop condition or have ABS failure and brake lockup occur. Personal harm and property damage should not have to be an indicator if I can have the manufacturer cover a repair. Please advise What I should do to increase the response in the direction of getting this known problem repaired. Issues with 1.5L turbo engine and fuel injector causing safety features to be disabled and flash on dashboard repeatedly, also heard of many issues with head gasket failure in this specific engine which could lead to driving in limp mode on a highway(20miles or less). These safety concerns could potentially be LIFE THREATENING if these issues occur on a highway

I received a recall on my 2022 ford f150 vehicle stating that the wheel end hub bolt in the rear axle may become loose or break. Continued use of the vehicle can cause damages to other vehicles and/or cause crashes. I tried to have this repaired when I received a recall notice back in November 2025 but they were unable to help. I dropped my vehicle off on 1/6/26 for recall repair and they are tell

At this time I do not feel safe removing my vehicle from the lot or driving it. I spoke with Earl at Ford Recall Assistance Center on 1/23/26 (ref call number: [XXX]) . I can not go without a vehicle, this is absolutely unacceptable and I am submitting this complaint and will be moving forward with legal counsel.
