My passenger seat headrest popped open without any incident or warning and the clip is broken. Air bag deployed from head rest. There was a warranty in place that expired in 2024 but mine just happened while I was driving. It scared me and caused me to swerve. It’s a known issue. Why no recall for safety? Dealership wants to charge 1100 to fix a defect.

There were no warning signs, no sensors activated, in fact, the Chevy App showed (and still shows) the vehicle as "everything looks good" "no diagnostic issues!" We had the vehicle towed back to our hometown/dealership (Missoula, Chevrolet in Missoula, MT), where it has now been determined, the engine completely seized up/failed, the original "fix" was not adequate, and we are now waiting for a new engine to be put into the vehicle! After reading about other vehicle owners experiences, I am quite skeptical of the new engine as there have been multiple experiences where the new engines had the same issue. This recall is impacting the safety of drivers, most concerning was our [XXX] left stranded in a rural area with very spotty cell service, and the vehicle would not even shift to neutral to get it to a safer area, this needs to be addresses as soon as possible to prevent others from being put into this situation. I own a 2021 Volvo XC40 Recharge (pure electric). At 49,030 miles, while slowly driving in a parking lot, the vehicle suddenly began shuddering and jerking as if driving over rumble strips. A warning message appeared stating “Propulsion System Failure” and indicated the vehicle would reduce speed. The vehicle lost propulsion and came to a stop. After stopping, the vehicle would not re-enter D

The instrument cluster and infotainment systems still powered on, but the vehicle repeatedly displayed messages such as “Vehicle system check. Wait” and would not enter a ready-to-drive state. The vehicle had to be towed to the dealership with a dolly. It took 4 hours to move it because the first tow truck could not safely move it with the wheels stuck in place. This failure occurred without prior mechanical warning or other preceding symptoms. Dealer diagnosis identified an internal failure of the front axle inverter high-voltage assembly (IHFA). Service records document a 400A fuse failure, welded battery disconnect unit (BDU) contactor relays (R1 and R2), and overheating/damage to high-voltage battery disconnect components. Repair required replacement of the IHFA, BDU contactors, and high-voltage fuse. The dealership stated this was not a normal wear-and-tear issue. I have since found reports from other Volvo Recharge owners describing similar propulsion failures involving inverter and high-voltage battery disconnect components. My safety concern is the sudden loss of propulsion while the vehicle was in motion, with no warning and complete vehicle immobilization. Had this occurred at highway speed or in traffic, it could have created a serious collision hazard. The dealership service person agreed that my only chance to get the vehicle to a safe location was to coast. After that, it'd be stuck. Almost all functions connected to the steering column quit working. I do not have working turn signals, high beams or wipers, unless the wipers are on high. None of the functions on the steering wheel work. It seems like a safety issue to not have functioning turn signals or properly finctioning wipers and high beams. No lamps or warnings.

... agnostic scans appeared normal. No safety feature or diagnostic system reported this failure! I was told to take the vehicle because they had nothing to fix without a diagnostic report. May 2026- I drove the vehicle for a few weeks before this occurred again, with the same warning and complete deceleration. The vehicle was brought to a 2nd dealership, nearest to this incident. They have had the vehicle for 29 days with no safety or diagnostic issues to report and no ability to duplicate the problem. During this occurrence, my daughter was able to video the dashboard and deceleration from the passenger seat. I have provided this video to KIA. The national KIA team is refusing a buy-out option for this vehicle after my family and I have repeatedly been put in danger on the road. It also puts drivers around us in danger as we frantically attempt to proceed to safety while quickly decelerating. It is also extremely alarming that the safety systems and diagnostics do not show that these problems are occurring. While the local dealership has been understanding and professional to date; however, the National brand and its refusal to put safety first, especially on a brand-new vehicle, is unacceptable. ... ed the entire engine under this recall in June 2025. The original engine serial was [XXX]; the replacement serial is [XXX]. On XXX, at about 93,000 miles — roughly 27,000 miles after the recall repair — the REPLACEMENT engine GM installed began making the same knocking noise. A dealer (Turner Automotive, Montrose, CO) has confirmed the engine is failing and has ordered a second replacement engine. The recall remedy GM performed did not fix the defect; the engine GM installed to cure the recall is now exhibiting the same failure mode (knocking / loss of propulsion). When the engine failed on XXX, GM Roadside Assistance refused to cover towing despite the engine's replacement coverage, and I had to pay roughly $484 out of pocket to tow the vehicle about 74 miles to the nearest dealer. This is a safety issue: the defect causes loss of motive power, and in my first failure the vehicle shut down while I was driving. I am reporting this because the recall remedy itself appears defective, consistent with NHTSA Recall Query RQ26001 regarding the adequacy of the remedy for campaign 25V-274. While my vehicle was in neutral and moving through an automatic car wash on a track, the Automatic Emergency Braking (AEB) system malfunctioned. My vehicle shifted itself into park and shut down. This sudden engagement caused the vehicle behind me to collide with the rear of my vehicle. As a result, the driver of the following vehicle has filed a collision claim against my insurance policy. My vehicle has been taken to the dealership 6 times for this AEB-related issue, along with other ongoing concerns, on the following dates: 09/11/25, 10/07/25, 11/12/25, 01/19/26, 03/09/26, and as of 6/05/26 is back at the dealership. The vehicle will unexpectedly shift out of gear at low speeds, requiring me to stop and place it back into gear before I can continue driving. The vehicle is currently at the dealership as of 06/05/26. Despite several repair attempts, the problems have not been resolved. I am extremely concerned that the AEB system will continue to malfunction causing a more serious collision and injury. The dealership was able to reproduce the problem in September; however, they have not been able to reproduce it again since that time. The vehicle came to a complete stop at approximately 11:58 a.m. I attempted to restart the vehicle once, but it did not restart. No additional restart attempts have been made. There were no warning lights displayed prior to the engine shutting down, and there were no visible fluid leaks underneath the vehicle after it came to a stop. The vehicle received a routine oil change on June 16, 2026, approximately three days before this incident. I have retained the service receipt and all related documentation. Despite this known safety hazard, the dealership returned the vehicle to me unrepaired on June 22, 2026, and deemed it safe to drive. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Fort Mill Hyundai (805 Gold Hill Road, Fort Mill, SC 29708) has confirmed they can physically feel the stalling issue but stated they do not have sufficient data to submit to Hyundai Corporate for transmission replacement authorization. An independent mechanic also previously confirmed the engine issue that the dealership initially dismissed. Fort Mill Hyundai, an authorized Hyundai dealership, has inspected the vehicle multiple times over several years. As it was squeezing his shoulders I was pushing back on the sliding door with all my strength, and after a few seconds it opened back up. If I had not been standing behind my son, I believe the door would have continued shutting more, enough to sustain serious injuries if not more. The vehicle passed three consecutive NY State safety inspections: January 2024, February 2025, and March 6, 2026 at 73,553 miles — only 2,546 miles before complete failure. Full maintenance history documented. No owner neglect. No warning lamps appeared before complete failure. Problem has not yet been confirmed by dealer as vehicle has not been towed for diagnosis. A rear-end collision was avoided only after I shifted the transmission into Park, which brought the vehicle to a stop. All driver assistance systems were off, including adaptive cruise, lane keeping, highway driving assist, and steering assist. This occurred during normal low-speed braking, not aggressive or high-speed driving. I have had similar but less severe ABS behavior before when braking over potholes, bridge joints, or rough pavement, with excessive ABS activation and reduced braking performance. Dashcam footage is available showing the pothole/road irregularity, vehicle speed, steering/hand vibration, my avoidance attempt, and the near-collision. Despite 6+ documented repair attempts over 9 service visits from September 2025 through May 2026, the defect remains unresolved. Repairs attempted include: two 4-wheel alignments, road-force balancing, steering angle sensor calibration, front camera calibration, new front view camera installation, and a complete steering gear box (R-MDPS) replacement. The vehicle was out of service for 129 documented days. CRITICAL ADMISSION: Dealer Repair Order #XXX(11/5/2025) explicitly states that Hyundai's Technical Service Bulletin procedures for this steering defect only apply at speeds of 40 MPH and below, and that "above that speed there are too many variables." The defect occurs at highway speeds above 40 MPH. This is a written admission that no manufacturer-approved repair procedure exists for the defect as experienced. Hyundai Techline Case #XXX was opened in January 2026. After Techline involvement and new camera installation, the technician's own repair order states "concern still present" after road testing. Prior to the recall remedy becoming available, the vehicle began experiencing erratic transmission shifting and illuminated the check engine light, wrench symbol, and codes P0720 and P0722 were present. The technician determined that the erratic shifting condition was related to DTCs P0720 and P0722 and recommended replacement of the transmission lead frame/conductor plate. The technician also noted the need for additional diagnostics involving the Output Speed Sensor (OSS) circuit within the transmission. The vehicle remains available for inspection at the dealership. This condition created a safety concern because the transmission was shifting erratically and the vehicle could not be relied upon for normal operation. Unpredictable transmission behavior creates a risk during acceleration, merging, highway travel, and other driving situations where consistent vehicle response is required. At the time, I was preparing for a cross-country military PCS move and did not believe the vehicle was safe or reliable to drive long distance. The problem was confirmed by an authorized Ford dealership who documented the condition, diagnostic codes, and repair recommendation. The findings were provided to Ford Motor Company through Corporate Case Number CXH-08096574-L9M6T4. Ford representatives advised that the Automotive Safety Office was notified regarding the technician findings and the open recall. My concern is that the vehicle developed transmission-related symptoms involving P0720, P0722, erratic shifting, and a recommended lead frame/conductor plate replacement while subject to an active safety recall affecting related transmission systems.
