This is a new vehicle. It had been serviced according to the manufacturer’s recommendations at 5000 miles, around 3 weeks prior to the date of this incident. While driving on the freeway, I got an alert that the check engine light came on and there was a “malfunction in the electronic control of the engine, throttle, or automatic transmission has been detected”. Shortly after several more a
... lerts popped up: “a malfunction in the hybrid system” and “a malfunction in the Drive Start Control System” were detected. While attempting to get to a safe location to pull over, the throttle no longer functioned. I was able to coast down an off ramp. Once stopped at the light at the off ramp, the accelerator pedal no longer worked and the vehicle was stopped blocking a lane of the off ramp. After waiting and restarting the car, I was eventually able to pull to a safe location away from the off ramp before the engine failed again.Read more

When putting my vehicle in park it jumps forward about 1 foot. This happen about 70% of the time when I shift into park. Also, my vehicle will brake randomly while driving. This happened one time on the freeway where my car came to a complete stop, luckily there was no one driving close behind me. More often this occurs while backing out of my driveway. My vehicle is available for inspection at a
... ny time. These problems are concerning because if my car jumps forward after being placed in park, I could potentially hit a vehicle parked in front of me or a person walking by. The braking while driving is very scary, especially since I was on the freeway and it could've possibly resulted in a serious accident. I took my vehicle to the dealership, and they had it for several days. When I picked it up, I was told that they were unable to recreate the issue of the car jumping forward. Regarding the braking while driving I was told that "a bulletin had been issued for this problem but since there is currently no fix, we advise the safety features be turned off." The mechanic then went on to say, "I'm unable to turn these features off because it would be a liability, but I can show you how." I spoke to the head of the service department the next day because I am scared to drive my young children around in this vehicle and was told that the braking sensors are just sensitive. At this point I'm not driving the car for safety reasons. No messages come on when the vehicle jumps forward after parking. When the vehicle brakes completely while driving I've seen the message saying once "obstacle detected" but other times I've been so startled I don't look at the dash, I'm more worried about getting hit in that moment. I did notice my seat vibrate every time this has happened. This all began shortly after buying my vehicle in February 2025.Read more

My car experienced sudden unintended acceleration. I was at a stop light preparing to turn right when my car accelerated of its own accord crashing into the vehicle in front of me. My foot was firmly on the brake, but I had lost control over the vehicle. Whatever electrical defect I experienced, it was sufficient to rev the engine by itself and override the braking system enough that the vehicle
... advanced forward in an out-of-control manner. It was unintended, unexpected and beyond my ability to control. Failing to recognize the braking system, my vehicle collided into the vehicle a second time. Because my foot was on the brake and my vehicle still advanced forward, I initially thought I had been hit from behind. However, the erratic RPM behavior and the vehicle's continued acceleration while braking confirmed I was no longer in control of the vehicle. Acting promptly, I immediately took the gear shifter out of drive and put into neutral. It was then and only then that I regained control of the vehicle: it stopped accelerating, stopped revving of its own accord and began responding to the braking system. Please note: my foot was not on the gas pedal, for as soon as I took it out of drive, the RPM returned to normal (i.e. the erratic behavior indicated by the tachometer's RPM, stopped). Had my foot been on the gas pedal instead of the brake pedal, the revving would have continued to behave as before; but it did not. (I do not have an all-weather floor mat. It is neither here nor there, because in the event that I did have one, you would expect the RPM behavior to continue when put into neutral; but again, all behavior returned to normal when the vehicle was put in neutral.) Though I am sympathetic to those that panic in a moment of distress, I was of a present enough mind to know what was happening. An electrical engineer with intimate knowledge of the computer system for this vehicle would be the best to assess what went wrong from a technical perspective.Read more

On several occasions while loading items into the rear storage area, the rear hatch suddenly closed on me without warning, striking me on the head and pinning me under it for several seconds until I was able to force the hatch open. After the first couple of incidents, I thought that maybe I had inadvertently placed my foot under the rear bumper and triggered the swiping sensor causing the hatch
... to close. After being aware of the very sensitive sensor, I made sure my feet are not anywhere near the bumper, but l am still experiencing the same problem when I am standing near the rear bumper for any length of time loading items into the rear storage area. The owner’s manual describes a beeping sound emits when the rear hatch is opened and closed when using the swiping method under the rear bumper with a leg. I did not hear any warning sound and now that I am aware of this warning method, I listen for it and still do not hear the sound. I have taken the vehicle to the dealer for an inspection. While there they activated the rear hatch in my presence with the swiping motion and said that they could hear a faint beep sound which they explained is the same on all Volvo SUV’s. They demonstrated the swiping method on another SUV while I was there. Again, I did not hear the warning sound, but they were able to hear the faint sound. I was informed by the service advisor that there was no remedy to make the warning sound louder. It should be noted that the approximate date listed is the last time it occurred. It started shortly after purchasing the vehicle on August 31, 2024 and occurred on serveral occasions throughout ownership.Read more

While operating my 2025 Lexus RZ 300e at normal road speeds, the vehicle experienced a sudden and dangerous propulsion loss when the traction battery reached 0% state of charge. The only advance notice was a single soft audible alert. There were no escalating warnings (visual or audible) and no meaningful low-battery reserve to allow the driver to safely exit traffic or reach a charging location.
... Within seconds of the single alert, the vehicle entered reduced-power “limp mode,” creating an immediate traffic hazard. This abrupt reduction in speed occurred in active traffic among large trucks, putting my safety and the safety of other road users at serious risk. The lack of progressive alerts and absence of a usable reserve buffer prevented any safe corrective action and increased the risk of a collision or injury. Lexus marketing materials, sales claims, and the owner’s manual indicate that the RZ’s battery management system provides intelligent, progressive warnings and adequate reserve energy to prevent drivers from being stranded. My vehicle did not perform in accordance with these stated safety features. This represents a potential defect in the warning system and energy reserve calibration. The vehicle is currently in the custody of the authorized dealership. I requested that the service department attempt to recreate the incident to determine whether the system functions as designed. They refused, citing an open claim with Lexus corporate. Lexus corporate closed my initial claim without investigation six days after it was filed and has not inspected the vehicle. This failure mode—no escalating alerts, no sufficient reserve, and sudden loss of propulsion—is a clear safety concern that should be investigated for possible noncompliance with FMVSS requirements for driver warnings and vehicle control.Read more

Subject: Fuel System Failure Resulting in Engine Damage – 2020 RAM 2500 Pickup VIN: [XXX] Complaint Summary: I am submitting this complaint to formally report a dangerous and costly fuel system defect in my 2020 Dodge Ram 2500 Pickup (VIN: [XXX] ) that has resulted in catastrophic engine damage. Although Chrysler has issued a recall for the high-pressure fuel pump (HPFP) in many 2020 Ram truc
... ks equipped with the 6.7L Cummins engine, I was told that my vehicle was not included in the recall—even though it exhibits all the classic symptoms of HPFP failure. This is a safety hazard and a potential emissions violation. Details of the Incident: In June 2025, I brought my truck to Tracy Chrysler Dodge Jeep RAM after experiencing engine misfires, loss of power, and a knocking noise. The dealership diagnosed a P0303 cylinder misfire and noted a fuel smell in the engine oil, as documented in repair order #[XXX]. Despite extensive diagnostic work—including compression tests, leak-down, and injector swaps—the misfire remained unresolved, and a tapping noise persisted. The technician explicitly noted fuel contamination in the oil and suspected internal engine damage due to a fuel system issue. To further investigate, I submitted an oil sample to Blackstone Laboratories. The lab confirmed the engine oil was heavily diluted with diesel fuel—over 10% fuel contamination, which reduced the viscosity from 5W40 to the equivalent of 5W20. This is consistent with HPFP failure leaking fuel into the crankcase. The oil report also noted excessive steel wear and recommended immediate oil change and fuel system inspection. At no time was a check engine light present, nor was there a clear diagnostic code for HPFP failure. Chrysler continues to deny responsibility even though the evidence clearly points to a failed HPFP—a well-documented issue for this engine model and year. The cost of diagnosis and parts alone has exceeded $4,000, and the engine is likely destroy INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIARead more

Vehicle began exhibiting multiple electrical safety issues around April 2025.
