Subject: Urgent Safety Concern – Driver Seatbelt Malfunction I am writing to formally report a serious safety concern regarding the driver-side seatbelt in my vehicle. The seatbelt does not retract properly and frequently becomes caught in the door. In addition, the retractor fails to pull the seatbelt back completely, leaving slack in the belt. More concerning, there have been multiple instance
... s where the seatbelt did not properly engage or lock when braking was applied, even though it was securely fastened. I brought the vehicle to the dealership for inspection, but I was advised that this condition is “normal.” I strongly disagree with this assessment. A seatbelt that does not retract correctly or fails to lock during braking poses a significant safety risk and does not meet expected safety standards. This issue raises serious concerns about the reliability of the restraint system in the event of an accident. I am requesting a thorough inspection of the driver-side seatbelt system, including the retractor mechanism, and that any defective components be repaired or replaced immediately. Please document this concern formally and provide written confirmation of your findings and recommended corrective actions. I am very concerned for my safety and expect this issue to be handled with the urgency it deserves.Read more

The panoramic sunroof shattered while the car was parked and there were no occupants. Lexus USA refuses to acknowledge this defect and the dealership insists there was an impact that caused this damage. There was no impact. I have asked the dealer for a written report of their determination of the point of impact and to return all broken glass to me. I do not know yet if they will comply with
... either request. As the car was parked, there were no immediate safety concerns, but due to the risk of additional glass falling onto the driver and passenger seats, the car was not in a drivable condition. Had this happened while driving, the sound of the shattering and the glass falling may well have resulted in a reaction from me causing immediate danger to myself and others on the road. UNKNOWN if problem has been reproduced, but this is not an isolated incident. Vehicle component had not been inspected by manufacturer, police , insurance reps or others. There was no warning or other symptoms of this problem.Read more

My engine failed as I was driving down the highway at high speed. I was thankfully able to move over lanes and park safely on side of the rode before it came to a complete stop. It never turned back on. Lexus had to send a tow truck to pick it up and has been @ Dealership in Lubbock tx since 3/9/26

The issue started with the engine stalling while I’m driving, and one day the car failed to start. I towed it to the dealership who said there is nothing wrong with the fuel pump and advised me to replace the battery. I replaced the battery and drove it for a couple more months, while the engine continued to stall from time to time and eventually failed to start again yesterday. It’s very appa
... rent that the issue is with the fuel pump, and there is a recall for the fuel pump for the 2015, 2016, 2019, and 2020 models of the Lexus GX460, and my car is the 2017 model. I believe the recall should have been issued for all the models from 2015 to 2020.Read more

My 2023 Lexus RX350 with 67,000 miles displayed a check engine light along with a Drive-Start Control Malfunction warning. The dealership confirmed the wastegate actuator has failed and requires replacement. The part is currently on backorder with over 800

Toyota has issued Technical Service Bulletin T-SB-0084-23 covering this exact fault code on the same 2.4L turbocharged engine used in the 2023 Lexus RX350. The bulletin states the repair is covered under the Federal Emission Warranty for 96 months or 80,000 miles. However, the Lexus dealership has declined warranty coverage citing the absence of a Lexus-specific bulletin, despite the identical engine and fault code. The failure puts safety at risk as the Drive-Start Control Malfunction warning indicates loss of traction and stability control systems. The problem has been confirmed by the Lexus dealership. Warning lamps appeared on the dashboard prior to the visit. The vehicle is available for inspection upon request.Read more

The passenger rear tire (235/60R19 - Michelin Primacy A/S) randomly blew out while driving at high speeds on the highway. No trauma via a pothole or other object was noted. My vehicle has the V6 twin turbo engine that’s been recalled on other years and models and was diagnosed with an internal engine failure. The mechanic at the dealership couldn’t figure out the problem so an expert TAS Lexus specialist was called in and couldn’t figure out the problem either. They’ve had the car for 3 months and since they don’t know how to fix it they are telling me to
... just replace the engine with a used engine at a cost of $25,000. The vehicle is just out of warranty and Toyota/Lexus is refusing to cover any of the costs. The vehicle was serviced regularly so this shouldn’t have happened. This is a common problem with this model of engine. I believe it’s a design flaw as this model engine is assembled at different plants/locations and over long periods of time that would indicate a design flaw and not due to Toyota’s official stance of debris left in engine block during assembly. Toyota should recall and replace all these engines at no charge to the owner of the vehicle.Read more

The driver-side front door window developed a crack suddenly while the vehicle was in normal use. There was no known impact, and no object was seen or heard striking the glass. The crack originated near the edge of the window close to the side mirror and spread across the glass. There were no warning messages, alerts, or prior symptoms before the failure. The failure appeared spontaneous. This c
... ondition poses a safety concern because the integrity of the side window is compromised. The crack continued to spread, which could reduce occupant protection in the event of a side impact and may lead to sudden shattering. It also has the potential to affect driver visibility. The issue was not inspected by the dealership prior to repair. The dealership advised that they could not perform the repair in-house and directed the repair to a third-party glass facility, where the window was replaced. No definitive impact point was identified prior to replacement. Based on the fracture pattern and lack of impact evidence, the failure appears consistent with a stress-related crack rather than road debris damage. Additionally, there are multiple reports from other owners describing similar cracks occurring in the same location on this vehicle model, suggesting a possible defect trend. The original failed component is no longer available for inspection, as it was replaced during repair.Read more

The Lexus TX 500h is equipped with a 6-speed sequential-shift automatic transmission, which is paired with the Lexus DIRECT4 all-wheel drive system. This setup utilizes a 2.4-liter turbocharged hybrid powertrain to deliver 366 horsepower and 406 lb-ft of torque. During many occasions, the transmission on the 2026 Lexus TX500h would shudder during acceleration or deacceleration. I noticed it hap
... pening around 25-40mph mostly and it has happened more than once. I have demonstrated the issue to the dealer on Wed, May 20th around 10:15am Pacific but they didn't know how to address the issue. I did a ride along with the service tech from Lexus of Pleasanton and it happened again during acceleration. The dealer noted that there were no error code when they scanned the system and no observed leaks. I have a video clip but was unable to upload it. The issue with transmission shift thump when this issue happen feels like the transmission locked up. It would shake the vehicle and lose power while accelerating before it starts to gain power back in 1-2 seconds. This causes a safety issue since that pause in acceleration thus this can be dangerous while merging onto a freeway or areas with lots of traffic. I noticed the same issue on some occasions, it would shake during deceleration as well. This issue is intermittent. Maybe Lexus/Toyota can address this with the transmission shifting update. IMO, It's a safety issue that needs to be looked and fixed by Lexus/Toyota. Dealer asked me to come back only. They reset my information screen to address a sliding driving issue where when the power is turned off on the vehicle the seats will go back and immediately go forward.Read more

Around 11 am on Sat., May 2, 2026, I was sitting in a very long line in Starbuck's drive thru when my 2025 Lexus RX hybrid suddenly and uncontrollably accelerated--like an uncontrollable bucking horse. Brakes didn't work. I couldn't stop my out of control car, and the multiple computer sensors also didn't stop the car. It was like the throttle was engaged because of a computer glitch. In an effort
... to avoid hitting the car in front of me, I immediately tried to swerve, but still hit the car, then jumped the exceptionally high curb and ended up in the grass below and almost into the busy road ahead of me. It was terrifying. The car in front of me hit the car in front of her. The exact same issue happened to me on June 18, 2025 when my car unintentionally accelerated--also in a very tight space and going at 2 mph. There was no accident at this time because my car's computer stopped me from plowing into the building immediately in front of me after unintentionally accelerating. The next day at the recommendation of the person who sold me the car, I took my new Lexus to the service department. They gave me a loaner and told me to contact Toyota's/Lexus' Brand Engagement department in order to start the review, which I did. The review was conducted and ended around July 9, 2025 . On Mon., May 4, 2026 I took my car back to the Lexus dealer where I purchased, but was not given a loaner, which I requested, and was told I would need to wait several hours to have this inspected. I then took my car to Boyce Body Werks. A State Farm agent examined my car and initially approved almost $24K worth of damages. Boyce Body Werks is saying when they disassembled my car that there was more damage underneath my car. I also notified Toyota/Lexus about the same issue that happened one year prior. They also refused to give me a loaner, and I am now forced to pay my rental fees. Boyce Body Werks--a Lexus certified dealer has multiple pictures of the damage. My car is still under warranty.Read more

Description of Defect: When the vehicle is shifted into reverse, the rearview camera display freezes for several seconds before going black. This malfunction creates a direct and serious safety hazard — during the period the display is frozen, the image shown does not reflect actual real-time conditions behind the vehicle. Any pedestrian, child, or object that moves into the path of the vehicle
... after the display freezes will not be visible to the driver. This significantly increases the risk of a backover collision. Dealership Inspection Outcome: I brought the vehicle to an authorized Ford dealership for diagnosis. Upon inspection, the technician was unable to detect the diagnostic trouble code associated with Recall 25S89, and as a result, the vehicle was deemed to have passed inspection. However, the malfunction continues to occur. The failure to trigger the recall code does not resolve the underlying issue — the camera display still freezes and blacks out during reverse operation, leaving the driver without a functioning rearview safety system. Safety Concern: A rearview camera is a federally mandated safety feature. A display that intermittently freezes and then goes dark is functionally equivalent to having no camera at all during those critical moments. This is not a minor inconvenience — it is a safety system failure that puts pedestrians and other individuals behind the vehicle at risk every time the vehicle is put in reverse. Requested Action: I am requesting that NHTSA investigate whether Recall 25S89 adequately captures all failure modes associated with this defect, and whether vehicles that present with this symptom but do not trigger the associated diagnostic code are being improperly cleared at dealerships. A remedy should be identified and applied to all affected vehicles regardless of whether the recall code is detected during inspection.Read more

My Cadillac has experienced a history of repeated mechanical/electrical issues and has spent extended periods of time out of service at the dealership for repairs. Due to the complete loss of power and unsafe operating condition, the vehicle had to be towed. I will get it to the dealership when they open again for business on Monday. Given the repeated repair history and this most recent dangerous failure while driving, I no longer believe the vehicle is safe or reliable to operate. I am requesting investigation of this safety-related defect and have initiated a formal buyback review request with General Motors/Cadillac.Read more

I am submitting this complaint regarding a serious brake-related safety issue involving my Audi vehicle after recent brake service performed by an authorized Audi Meadowland dealership in [XXX] . In December 2025, I brought my vehicle to an authorized Audi dealership for replacement of the brake pads. The repair was completed by the dealership, and I trusted that the vehicle was safe to drive aft
... erward. However, only approximately six months later, and after driving only around 1,000 miles since the repair, the vehicle experienced a dangerous brake-related failure involving the brake pad assembly becoming loose/detached while the vehicle was in operation. This incident occurred while my entire family was inside the vehicle, creating a serious safety hazard and placing all occupants at risk. A brake system component should not fail after such a short period of time and limited mileage following dealership service. In addition to the safety concern itself, I am also extremely dissatisfied with the dealership’s handling of the matter. The dealership did not proactively contact me regarding the repair status, and multiple attempts to contact them by phone went unanswered. I believe this issue may involve improper installation, defective parts, insufficient inspection procedures, or other serious safety-related service failures. I respectfully request that NHTSA review and investigate this matter to determine whether similar incidents may affect other vehicles serviced under similar conditions. Thank you in advance for your review on this matter. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I bought my vehicle around May 2024. Initially, everything seemed fine. However, I noticed that my car was starting to slow down and would enter limp mode, preventing me from exceeding a certain mileage. Subsequently, all the lights on my dashboard began flashing, prompting me to pull over and call a mechanic shop. I had all the necessary maintenance and repairs performed as per the plan. After st
... arting the vehicle, all the lights turned off, and it drove smoothly. A few weeks later, all the lights came back on again, and nothing showed on the OB2 Reader, which I thought was odd because something shouldve popped up if all the lights on my dashboard was going all crazy. The mechanic said the same thing and maybe thought it was the battery, sensors going bad, or even the wiring. But he checked everything and said he didnt find any issues. So I went home and the lights on the dashboard stayed on for 30 seconds and then turned off after I started the car. The lights turning on all at once on the dashboard became more frequent and this time, the check engine light illuminated, indicating a misfiring issue. I took the vehicle to a mechanic shop for a thorough inspection, and they made a few minor adjustments. However, two months later, the same problem recurred. All the lights came on again, and the vehicle misfired once more, accompanied by the check engine light. Upon further investigation, it was discovered that the issue was a head gasket leak. Additionally, the seat belt alert for the passenger and rear seats, as well as the child car seat, failed to function properly.Read more
