My vehicle has lurched forward upon the first engine start of the day, under no throttle, on several occasions, throughout the life of my vehicle. Shortly after starting my 6.2 engine in my garage, if I shift my 8L90 transmission into Drive as soon as the engine RPMs settle to idle, the vehicle will lurch forward, screeching the tires. Luckily, I had my foot on the brake hard enough such that no
... one was injured. The harsh transmission shift into drive is dangerous, possible injuring passengers or my garage door. I've learned that if I wait 5 to 10 seconds, allowing the engine to idle (while the transmission oil becomes pressurized ??), then the shift into drive is normal, without any lurch.Read more

The airbag warning light has remained illuminated since I acquired the vehicle in January 2025. The warning light stays on continuously and does not reset. This indicates a fault in the airbag system, and I have been advised that when this light is on, the airbags may be disabled and may not deploy in a crash. The vehicle has also experienced ongoing electrical problems, including lighting failur
... es, blown fuses, and shift-interlock malfunctions, which suggests an electrical system fault affecting multiple safety-related systems. These issues create concern that the airbag control system may be impacted by the same electrical malfunction. The vehicle has not been repaired or diagnosed by a dealer for the airbag issue. The condition is still present and the warning light remains illuminated. This failure creates a serious safety risk because the airbags may not function in the event of a collision.Read more

Subject: URGENT: Global Safety Defect Report – Jaguar Land Rover (JLR) Powertrain Control Module (PCM) and Fuel Pump Driver Module (FPDM) Failure. Description of Defect: I am writing to formally report a critical safety defect affecting Jaguar Land Rover (JLR) vehicles involving the Powertrain Control Module (PCM) and the Fuel Pump Driver Module (FPDM). These modules are experiencing spont
... aneous "reset" or "reboot" events during vehicle operation, leading to a sudden and total loss of propulsion. Safety Hazard: Affected vehicles are experiencing power loss at highway speeds, often resulting in the vehicle stopping abruptly in live lanes of traffic. This defect has directly led to near-misses and reported accidents. The loss of power frequently occurs without warning, rendering the vehicle a "sitting duck" on high-speed roads, creating an extreme risk of rear-end collisions and multi-vehicle pileups. Manufacturer Inaction: Despite internal technical bulletins (e.g., NHTSA TSB MC-10218879-0001) and mounting consumer complaints, JLR has failed to issue a global safety recall or provide a permanent hardware/software resolution. While the Chinese State Administration for Market Regulation (SAMR) has reportedly summoned JLR officials regarding these stalling issues, the manufacturer has yet to implement a verified fix or acknowledge the systemic nature of the failure. Demand for Action: We request that your agency launch an immediate investigation into the frequency and severity of these stalling events to compel Jaguar Land Rover to initiate a safety recall and protect the motoring public.Read more

The Tumble Generator Valve (TGV) assembly within the intake manifold failed on my 2019 Subaru Outback at only 48,000 miles causing catastrophic engine failure. This is a critical engine component that is expected to last significantly longer than the vehicle's current low mileage. The premature failure of this part triggers engine warning lights and disables key driver-assist safety features (such
... as EyeSight), creating a distraction and potential hazard while driving. This indicates a material defect in the TGV components used in this model year, causing catastrophic failure well before reasonable service life expectations.Read more

Subject: URGENT: Investigation Request – Systemic Loss of Propulsion in Jaguar Land Rover (JLR) Vehicles (2017–2026). 1. Description of Defect I am formally reporting a critical, life-threatening safety defect involving the Powertrain Control Module (PCM) and the Fuel Pump Driver Module (FPDM) in Jaguar Land Rover (JLR) vehicles. These modules suffer from spontaneous software "resets" or
... "reboot" loops during operation. When the PCM resets, it severs communication with the fuel system, causing an immediate and total loss of propulsion (motive power). 2. Safety Hazard and Real-World Impact This defect triggers without warning at highway speeds, resulting in vehicles stopping abruptly in high-speed lanes. This creates a "sitting duck" scenario, rendering the vehicle unable to move to a shoulder and significantly increasing the risk of catastrophic rear-end collisions and multi-vehicle pileups. Reported incidents include near-misses and accidents directly attributed to this sudden stalling. 3. Timeline of Manufacturer Awareness (2017 – Present) JLR has been aware of this specific PCM/FPDM communication failure since at least 2017. Internal documentation, such as NHTSA Technical Bulletin SB-10095121-1020, identified short circuits in the PCM/FPDM line for 2017 F-PACE and XE models. Subsequent bulletins, including MC-10218879-0001, admit that software "flashing" is ineffective and that FPDM hardware replacement is often required. Despite nearly a decade of internal knowledge, JLR has failed to initiate a comprehensive global safety recall for all affected VINs, opting instead for localized "Service Actions" that do not permanently resolve the hardware instability. 4. International Context and Manufacturer Inaction The Chinese State Administration for Market Regulation (SAMR) has already summoned JLR officials regarding these stalling failures. However, JLR has not provided a verified, permanent solution. 5. Demand for Action We demand an immediate, multi-agencyRead more

1. Component and Availability: The panoramic sunroof shattered spontaneously. The failed component was replaced by Myrtle Beach Hyundai, but I have documented the damage with photos, and the vehicle is available for inspection to show the remaining glass debris inside the structure and the damaged headliner. 2. Safety Risk: The sunroof shattered with a loud, gunshot-like noise while driving at 60
... mph on a highway (I-95), causing extreme driver distraction and potential loss of vehicle control. Furthermore, a large amount of glass shards fell into the cabin and remains trapped inside the headliner. These hidden fragments continue to rattle, causing ongoing distraction and posing a risk of injury to occupants if they fall through the damaged interior fabric. 3. Confirmation by Dealer: Yes, the problem was confirmed by Myrtle Beach Hyundai. They replaced the sunroof under a "goodwill" gesture, although they initially claimed it was not a warranty issue. 4. Inspection by Others: The vehicle was inspected and repaired by the authorized Hyundai dealer (Myrtle Beach Hyundai). No police or insurance reports were filed as it was a spontaneous failure without a collision. 5. Warning Symptoms: There were no warning lamps, unusual noises, or any symptoms prior to the failure. I am writing to report a serious safety incident involving my 2022 Hyundai Ioniq 5 and to request a thorough investigation and guidance on next steps. On Jan. 3, 2026, at approximately 6:15 p.m. on a snowy day, we parked our car in the driveway of a weekend rental home near Lake Tahoe, CA. The driveway had a slight slope toward the house, and we backed the car down the driveway with the rear of th
... e car facing the house. My husband turned off the car, then all five occupants exited the vehicle, closed the doors, and began unloading items from the trunk. The car began rolling backwards toward us and the house. It only stopped after pinning my husband into a snowbank created by a snowplow. The other three adults and our [XXX] niece narrowly avoided being struck. While pinned behind the vehicle, my husband had to dig out some snow to reach into his pants pocket and retrieve the key fob. I then moved the car forward a few feet to free him. Although he was pinned in the snow, my husband was (thankfully) not seriously injured. After freeing my husband, I put the car in Park and turned it off. In this model, the parking brake engages automatically; there is no separate parking brake button or lever. A few minutes later, without anyone in or near the vehicle, the car again rolled backwards down the driveway into the same snowbank. Tracks in the snow clearly indicate that the wheels rotated and the car rolled; it was not sliding on ice with the brakes applied. In other words, the parking brake and/or Park mechanism failed to hold the vehicle in place in sub freezing, snowy conditions. The temperature at this location and time was in the mid 20s °F. After returning home from our trip, we took the car to our Hyundai service center, which has performed all maintenance since we purchased it in 2022. The service center performed all standard diagnostics but was unable to replicate the failure conditions. I am submitting this email as a formal safety defect notification regarding a front suspension knuckle cracking condition observed on my 2016 Range Rover Td6, which appears to directly correspond with Land Rover Recall D027. Attached are photographs of the passenger-side front suspension knuckle showing a distinct structural crack running vertically through the knuckle body at the upper control a

According to the recall description, cracking in this location may result in loss of clamp load and potential separation of the suspension arm, posing a significant risk to vehicle control and occupant safety. The condition observed on my vehicle appears consistent with the failure mode identified by the manufacturer. I am submitting this information to: - Document a real-world instance of the suspected defect - Provide photographic evidence supporting the concern - Support evaluation of recall scope and remedy effectiveness This submission is provided in the interest of vehicle safety and defect reporting. I respectfully request confirmation of receipt and inclusion of this information in your defect review records. Please advise if any additional details or documentation are required.Read more

Dear Sir or Madam, I am writing to formally report a safety-related defect involving the power unlatching tailgate switch on my Chevrolet 1500 truck and to request that this make and model be added to the current safety recall affecting Chevy HD vehicles. The power tailgate unlatching switch malfunctioned and would repeatedly attempt to open and close the tailgate while the truck was parked. On
... one occasion, this occurred continuously overnight. The following morning, my key fob was completely unresponsive, and the truck had zero battery life. The battery would not accept a charge, and the vehicle had to be towed to the dealership. Upon inspection, the dealership confirmed that the battery had been fully depleted and permanently damaged due to a short in the power tailgate switch. The faulty switch continuously drew power as it attempted to actuate the tailgate, ultimately destroying the battery. This failure created a serious safety and reliability concern, leaving me stranded with a completely inoperable vehicle. Given that this issue resulted from an electrical short and caused total battery failure, I believe it represents a significant safety defect. Based on the similarity to issues addressed in the current Chevy HD safety recall, I strongly believe this make and model should be included in that recall. I respectfully request that this matter be reviewed and addressed to prevent other owners from experiencing the same hazardous situation. Thank you for your time and attention to this matter. I look forward to your response. Sincerely, Charles SennRead more

I was driving when an "Electrical Fault Detected" warning came on. The car's instructions were to pull over as soon as possible, as the car's electrical system was draining the battery and the car could shut down within a few miles. We had the car towed to a repair shop where they ran tests on the car and replaced the battery. This did not fix the issue, and they came back a day later to find the
... brand new battery had drained. I then had it towed to another repair shop who replaced the alternator but that didn't solve the problem so they had it towed to the dealer. The dealer discovered that the DC converter in the car is bad, but said they have 5 cars in the shop with the exact same problem and the part is backordered without an ETA. I frequently drive late at night or early in the morning between a small town and a city for work. It's 100 miles each way, most of which is either in mountains with poor cell service or an interstate with a speed limit of 80 MpH. During this time of year, the temperatures in this area are often well below freezing. Had this issue happened on one of these drives, I could have very well been stranded with only a few minutes of warning with a high risk of hypothermia or even death. Had it happened on the interstate, the car would have lost complete power, including the steering, endangering not only my life, but my family and anyone else driving nearby. Land Rover has acknowledged in their notifications to dealer service departments that this is a known issue, but they have not issued a recall. I strongly believe this is a HUGE safety risk and that Land Rover should be held accountable for correcting this issue in all affected cars. They are endangering the lives of their customers and anyone who is on the road near Land Rover vehicles. NOTE: The doc provided in this submission is for an alternator change out that was done because of this electrical issue. The DC converter, which is where the true issue is, will be thousands as well.Read more

While using the vehicle’s BlueCruise driver assistance system, the driver monitoring system generates frequent false driver-attention alerts despite normal posture and active attention to the roadway. The system often fails to recognize attentive driving unless the driver deliberately exaggerates head position or facial expression. Routine and unavoidable behaviors—including briefly touching t
... he face, wiping the nose, or checking side-view mirrors for traffic—frequently trigger alerts, even though these actions are part of safe driving. These false positives create alert fatigue and increased cognitive load, forcing the driver to divert attention from driving to manage the monitoring system itself. The intrusiveness of the alerts becomes distracting and undermines the intended safety benefit of the system. Due to the frequency of false alerts, the driver is often compelled to disengage BlueCruise entirely. I am concerned the current driver-monitoring calibration may inadvertently increase distraction for attentive drivers and warrants review. The front passenger airbag warning light came on with a dash warning flashing light. I isolated the fault to the passenger seat weight sensor, which prevents the airbag from deploying when a passenger of less than 90 pounds is sensed in the front seat. I called my local Infiniti dealer in Tacoma, WA and they were able to supply a part number but it has been discontinued for the 2011-2013 Infiniti M37 models. Engine consumes large amount of oil requiring oil refill every week, without turning on check engine and/or engine oil light. Raising the chances to cause accidents due to engine failure while driving, and requiring mechanical intervention to rebuild the engine or a total engine swap. There are over 40 other complaints about this same issue, and it needs to be addressed.

Vehicle brake pedal is not fully depressing to normal state after use. This is causing the break lights to remain on until you pull the pedal itself back to resting state. If you do not do this the cruise control will not work. The ESC error light and Service Trailer Brake System error continue to come on.

While driving approximately 75 mph on an interstate highway, the engine suddenly shut off without warning. There were no warning lights or messages prior to the failure. Power was lost while the vehicle was still moving at highway speed. When the engine shut off, I lost propulsion and had to coast across lanes to reach the shoulder while surrounded by fast-moving traffic. Steering and braking wer
... e reduced and required extra effort. This created an immediate and serious safety risk to myself and other drivers. After safely stopping, I was able to restart the vehicle. It drove again, but the cause of the shutdown was unknown. I brought the vehicle to a Volkswagen dealership for inspection. They stated they could not replicate the issue and no fault codes were found, so no repair was performed. The vehicle has not been repaired and remains in the same condition as when the failure occurred. The component that failed is unknown, but it appears to involve the engine, fuel, electrical, or computer control system that caused the engine to shut off while the vehicle was in motion. This event put my life and the lives of others at risk due to sudden loss of power at highway speed. I do not feel the vehicle is safe to drive because the failure occurred without warning and without explanation, and it could happen again.Read more
