I had children in the car at the time, and this situation created an immediate and severe safety hazard for everyone in my vehicle and on the road. After this happened, I contacted (by phone) Ford directly. Without any inspection, hesitation, or further explanation, I was immediately told that the vehicle “needs a new steering rack and pinion.” The quoted repair cost exceeds $3,000, on a vehicle I am still paying $500 a month for. I have since discovered that there are thousands of other Ford Explorer owners reporting the exact same issue, with the same model years, same sudden failure, and the same required repair. This is clearly not an isolated incident. This pattern strongly indicates a widespread manufacturing or component defect that poses a significant risk of injury or death. I am deeply concerned that Ford has not issued a formal recall for a problem that can cause an immediate loss of steering control at highway speeds and in everyday driving conditions. The number of publicly documented complaints online demonstrates that this is a systemic and dangerous failure that should not be ignored. I am requesting that NHTSA fully investigate this issue and require Ford Motor Company to issue a recall for the defective steering rack/power steering system in the 2015 Ford Explorer Sport 3.5L ecoboost w/o park assist, as well as any related model years or trims using the same components. Volvo declined these requests, requiring me to pay for the test and any related repairs. I proceeded with the official oil-consumption test at my own expense, and the results confirmed abnormal oil consumption. Despite this, Volvo again declined reimbursement or goodwill assistance and did not provide any written, technical explanation based on the confirmed test results. This condition poses safety concerns, including possible engine damage, stalling, or sudden loss of power if oil drops unexpectedly. To maintain safe operation, I must frequently check and top off the oil. I also previously owned another Volvo model (XC60 2015.5) equipped with the same engine family, which experienced a piston-ring failure in late 2021. At that time, the recommended repair was piston-ring replacement—the same repair now recommended for this S60. Volvo declined coverage for that prior failure as well. Experiencing similar defects in two Volvo vehicles with the same engine family suggests a broader pattern that may impact safety. At one point the indicator lights went off for a few minutes but then came back on. We spent the night in San Bernadino, and the next day (11/26/2025), we took the car into the San Bernadino Toyota Dealership as we felt it was unsafe to keep driving. The dealer conducted a thorough full brake and hybrid system diagnostic and confirmed that the failure was inside the brake booster system. The repair estimate states: "Brake booster assembly with master cylinder and brake booster pump assembly have an internal leak." Thus, Toyota's own technician identified an internal hydraulic leak within the booster system, affecting the booster assembly, master cylinder, and booster pump assembly. The total repair estimate is $3,959.99. This is a major safety defect affecting sealed hydraulic components that are not part of routine wear-and-tear. There were 4 people in the car. Fortunately, the brake failure did not result in an accident, but it could have if we had kept driving on to our destination. We contacted the Toyota Brand Engagement Center to ask for Goodwill Assistance, but they were unable to help us. In addition to the repair costs, we have incurred rental car costs. BMW North America reviewed the case but denied responsibility and closed it, even though the defect affects a powered safety-critical seat. We still have no repair, no explanation, and no assurance that the seat is safe. I am filing this because the powered seat poses a clear risk of injury, and BMW has not resolved or acknowledged the safety defect. I request that NHTSA investigate the 2020 BMW X7 third-row seat system for potential entrapment hazards. They claimed the engine was “hydrolocked” and stated that water in the engine “voids any recall” and that outside influence is not covered. I requested that Kia Corporate dispatch a Field Service Engineer (FSE) for an independent inspection, but corporate told me that I cannot request an FSE directly and that only the dealership can request it. Since the dealership refuses, corporate will not act. Safety Concern: Stalling and shutting off while driving is a serious safety hazard that can lead to accidents. The vehicle is part of a federally mandated engine recall, but the dealership and corporate have refused to perform the proper recall inspection. The refusal to inspect the engine or acknowledge stalling incidents leaves me and other potential drivers at risk of unsafe operation. Requested Action: Investigate the dealership’s refusal to perform the recall inspection. Ensure Kia performs a full recall diagnostic, including internal inspection of piston oil rings and related engine components. At first it was unclear whether the smell was coming from outside, but when traffic slowed, smoke was seen coming from the driver-side wheel well and from under the front hood. No warning lights, alerts, temperature warnings, or malfunction messages appeared before or during the smoke.

Both white and black smoke were present. Police and fire department personnel responded to the scene and extinguished the fire. The vehicle was a nearly new plug-in hybrid with low mileage, and there were no dashboard warnings or prior symptoms of failure. The cause of the fire is unknown, but it appears related to a malfunction involving the electrical, battery, thermal-management, or propulsion systems. My rear shocks are making bumping and rattling noise at slow speeds. It also has a stiff ride in some situations.

The contact owns a 2025 Audi Q5. The contact stated while driving at undisclosed speed on three occasions, the contact noticed that there was an abnormal knocking sound coming from the dashboard. The vehicle was taken to the dealer to be diagnosed, but the cause of the failure could not be determined. The vehicle remained at the dealer unrepaired. The manufacturer was not notified of the failure.

In January 2025 I noticed light ticking/knocking at startup, idle, and while driving. On January 23, 2025 I searched online for how to check the oil level and for an L322 dipstick location (there is none). Only after finding a YouTube video with an undocumented override (double-pressing the “Cancel” button) was I able to see the oil level on January 31, 2025, and it showed zero. I added oil im
... mediately, but the knocking gradually worsened over the following months until very loud clattering began on July 4, 2025. The electronic oil level monitoring and warning system failed. When I followed the owner’s manual procedure to check the oil, the display repeatedly showed “Oil Level Not Available” and never gave any low-oil or no-oil warning, even when the actual oil level was zero. Because this system is the only way to check oil level on this vehicle (there is no dipstick), it prevented me from knowing the engine had no oil. The engine then suffered progressive internal bearing/rod damage and now makes extremely loud clattering. The vehicle and components are still in my possession and are available for inspection upon request. The system’s failure meant I unknowingly drove the vehicle with zero oil, which can cause sudden engine failure, loss of power, and possible engine seizure while driving at highway speed. I used this vehicle on Los Angeles freeways, so a sudden loss of power could have led to an accident or left me stranded in a dangerous location. The vehicle now runs with severe clattering and is unsafe to drive, so I have parked it to avoid a complete failure in traffic. The vehicle has been parked since the severe failure on July 4, 2025 because I have not yet been able to afford towing and diagnostic work. VSA Modulator on Honda has failed. This is a defective part but Honda has not recalled. This is a vital safety issue and can cause brakes to lock. Honda refuses to acknowledge defect and will not replace the part free of charge.
