My 2025 Honda Odyssey has been completely shutting off after the stop-start shuts the engine off. When the car actually “stalls” is when I let off the brake and it’s time to start going. Then I hear a ding, the dash lights come on, and I have to frantically hit the ignition button. This is happening at stop lights where there is the risk for other cars to safely go when the light turns green

For a few weeks prior to 12-21-2025, I've experienced minor Jeep "death wobble" (suspension issue) at highway speeds when encountering an irregularity (bump) in the road surface. On 12-21-2025, I experienced moderate Jeep "death wobble" at around 60 mph (several times). On a desert 2 lane highway, I was seriously in danger of running off the road to the right (crashing into rock faces bigger tha
... n the car) or running into oncoming traffic to the left. I was about an hour away from home and was able to reproduce the problem. I found that the issue was minimized if I kept it under 58 mph. Unfortunately, this motivated many drivers to dangerously pass me. The next day, I took it to the local off-road mechanic and they determined it was a worn tie-rod and recommended replacing the tie rod and installing/upgrading the steering stabilizer. From what I have read during my research is that those are band aid approaches to a more serious engineering/design issue with the Jeep Wrangler. I've further learned that Fiat-Chrysler (or the current owner) has continued to address the issue that has been problematic for decades. There are no recalls for my vehicle and the relevant TSBs I could find apply to older models (latest I saw was 2020 JL). However, it's clear the issue has not been resolved on my more current model (2021 Jeep Wrangler Rubicon JL). The vehicle is available for your inspection.Read more

While driving, the vehicle’s infotainment system suddenly froze and then rebooted on its own. During the reboot, the display was unavailable for several minutes, causing loss of access to navigation, vehicle status information, and warning messages while the vehicle was in motion. This occurred without warning and distracted the driver while driving, creating a potential safety risk.

I was driving this vehicle (2020 Honda CRV hybrid touring) and the brakes became very hard to stop. It was like standard braking without power assist and required great effort to stop the vehicle. Many warnings appeared on the dash including: 1. Brake system problem 2. Electric parking brake problem 3. Brake hold system problem 4. Hill start assist problem 5. Collision mitigation system problem 6.
... Vehicle stability assist (VSA) problem 7. Road departure mitigation problem 8. Adaptive cruise control problem The emergency brake light is on and won't go away. Sometimes when you start it, the warnings will clear after pressing the "!" button. Other times they won't clear making it not driveable because of the hard braking. I took it to First Class Halt and they found some code they had never seen. They cleared the code and 2 days later it started up with the same thing. I took it to Hendrick Honda and they also just cleared a code. A few days later it started all over making the car undriveable at least in a safe manner with full control over the brakes. It seems that neither an independent shop nor a Honda dealership can correct this dangerous problem.Read more

As I was traveling from Houma, La on a major highway to an out of state meeting. I started to hear a slight tapping sound and pulled over to inspect the sound. Upon inspection it seemed as though it was coming from the lower engine, or exhaust near the engine area. I located the nearest Honda dealership which was Community Honda of Lafayette with intentions of dealership inspection. It was approxi
... mately a 20 minute drive from the current location. As I was approaching the exit to the dealership the warning system lit up an check engine light. The Manager at the dealership took the vehicle to the back and ran a scan and responded that it sounds like an exhaust leak. I got back on the road for my destination. Approximately an hour later while I was on [XXX] all of the warning lights lit up at once, the engine jerked and shut off. I pulled over to inspect the vehicle again. I restarted the vehicle to work myself to a safe exit. The vehicle would not drive over 20mph. Once at a safe location I made concessions to have the vehicle carried back to my local Honda dealership, Barker Honda in Houma, La Upon inspection by the Honda technicians it was found to have irreparable damage to the engine internals. Technician states this could occur to low oil and/ or lack of maintenance; however, the technician was perplexed by the failure because all maintenance and records in their system showed that the vehicle was properly maintained by their staff. This is the third time my vehicle has been recalled for battery issues. Twice it has been noted that I should not drive, charge, park my vehicle in my garage, or park it near any building or person because of vehicle fire risk. In both 2023 and 2024 they performed a software update and yet this issue still persist. In 2024, since they didn't have a fix for the problem they said we should not drive
... our vehicle and I was without a vehicle for 2 months. Now in 2025 they are recalling for the same fire issue for the third time. Again, I am advised not to drive, not to charge, not to park in my garage, and to keep the vehicle away from people and buildings. I have again been without a vehicle for almost a month. I spoke to the dealer this morning and again they have a software update as a fix. I asked to have my battery replaced since the two previous software updates did not fix the issue. They said they were only authorized to do the software update and Mopar would need to authorize a battery replacement. Spoke to Mopar and they say they will do the software update and only replace the battery if needed. I do not find this acceptable. I do not feel that my car is safe to drive especially with my elderly parents, children, or pets inside and yet Mopar will not authorize the replacement of my battery. I feel that after two software updates that did not fix the issue and continued vehicle fires that a battery update should be mandated, not another software update. I now have a care I don't feel safe to drive. Florida Attorney General's Office Lemon Law Hotline suggested I report this issue here.Read more

The Front Passenger Seat Occupancy Sensor Mat (CIS) in my 2018 BMW X3 failed at 56,000 miles. This failure is confirmed by the specific BMW fault code 930A1C (Seat Occupancy Detector Malfunction) stored in the Advanced Crash Safety Module (ACSM). The component is currently installed in the vehicle and available for inspection. This defect poses a severe safety risk: the system fails to consistentl
... y detect a seated front passenger. As a result, the passenger airbag may be disabled and fail to deploy in a collision, significantly increasing the risk of injury or death. The dashboard displays a "Passenger Restraint System" malfunction warning, accompanied by an airbag light and chime. The issue is intermittent and load-dependent. It triggers specifically when an adult passenger occupies the seat for extended periods (e.g., highway driving), indicating a physical fatigue failure of the sensor mat grid under compression. I diagnosed the fault using BMW factory-level diagnostics. However, both the local BMW dealership and BMW North America have refused to cover the diagnosis or repair under the existing warranty extension (SIB 72 01 25) for this exact component, stating my specific VIN is excluded despite the vehicle exhibiting the known safety defect.Read more

Dear National Highway Traffic Safety Administration (NHTSA),I am submitting this report to express a serious safety concern with my 2015 Buick Verano, which I believe is unsafe for shorter drivers due to poor visibility and design limitations.I received this car as a gift from my father, but the moment I drove it off the lot, I realized I could not safely operate it. I am 4 feet 11 inches tall, an
... d this vehicle seems designed for much taller drivers. The windshield is too close, and there are several significant blind spots that make it extremely difficult to see vehicles, pedestrians, and obstacles around me.To try to fix this issue, we installed four extra mirrors and I used a booster seat, but these adjustments did not resolve the safety problems. I have experienced numerous close calls, including one incident where I nearly struck a pedestrian I could not see until he moved out of the way. Driving this car has become extremely stressful and frightening due to these visibility issues.I have spoken with several friends who are also under 5 feet 3 inches tall, and they reported the same difficulties when driving a Buick Verano. It appears that this model’s visibility design creates unsafe driving conditions for shorter individuals.I request that the NHTSA investigate this design concern to determine whether the 2015 Buick Verano meets safety standards for drivers of different heights, and whether design changes or warnings are necessary to protect shorter drivers.Thank you for your attention to this important safety issue.thank you.Read more

They said they have sent me emails with the pictures which I have yet to recieve. I went to the dealer and asked them to jack up my van so I can see the damage personally and they don’t have a lift available at the time, so I told them I would be back in town at a certain time from work and to call me so I could see it and they have yet to show me the scuffed IDCM module or the dented muffler. We haven’t run over anything to cause this kind of damage to a module like this because all the hybrid batteries are under the van. If the Chrysler Pacifica Hybrid is so cheaply made for a $62,000 vehicle that something can “scuff” a module housing and shut a vehicle down, this is potentially very dangerous if someone were driving on the highway if it were to shutdown and someone (including myself or any of my family) were driving and lose all propulsion and steering (because it’s electric steering) or worse braking! It could be catastrophic with people dying. This needs to be investigated to see how fragile this component is and if they need to “toughen” it up or re-locate it. I am reporting a concern regarding the driver’s seat design in my Toyota RAV4. I do not experience similar pain when driving other vehicles for comparable durations, including a Toyota Highlander, Toyota Camry, Subaru Impreza, Ford Focus, Rivian truck, or a pickup truck. This strongly suggests the issue is related to the RAV4’s seat ergonomics rather than a preexisting condition. Because of this issue, I have begun avoiding driving my own vehicle when possible. I lease a 2022 Jeep Grand Cherokee 4xe that has developed serious and unresolved safety issues. In addition to the loss of power, the vehicle regularly displays warnings related to critical systems, including electric mode being unavailable, fuel and oil refresh in progress, emergency/SOS system unavailable, hybrid power loss, and an airbag system warning. These warnings are documented and have appeared across multiple service visits. The vehicle has been taken to two different authorized Jeep dealerships and has spent nearly a month in service between them. Despite extended diagnostic efforts, neither dealership has been able to identify or repair the underlying issue. The problem remains unresolved. In addition, two active recalls continue to show as incomplete in the system and have not been successfully addressed. As of now, the vehicle is still at the dealership and is not available for use. Given the unresolved loss of power at highway speeds and the incomplete recalls, I do not consider the vehicle safe to operate. I have contacted both Jeep/Stellantis and Chrysler Capital in writing to request assistance and resolution. Despite providing documentation and allowing ample time for review, I have not received a response or acknowledgment. A Terrifying Ordeal and a Broken Promise: M.A’s Story A Known Danger Ignored In August 2025, M.A, a [XXX] woman living with Multiple Sclerosis, received a letter from Ford Motor Company. It was a recall notice for her 2022 Ford Escape, warning that a defect in the turbo system could cause engine fires. Despite this known danger, the letter contained a chilling instruction: there was "no recom
... mendation to stop operating the vehicle." Trusting the manufacturer, M.A continued to drive her car—her essential link to independence. A Nightmare on the Road Two months later, in October 2025, that trust nearly cost M.A her life. While driving in Windsor, Ontario, she saw smoke, then flames erupt from under the hood. In a horrifying instant, the engine compartment burst into fire. Paralyzed by panic—a "brain freeze" moment where she couldn't even unlock her door—she was trapped. Only with the help of bystanders was she pulled to safety, where she watched helplessly as her car and everything she owned—clothes, jewelry, two laptops, an iPad, and three suitcases—were consumed by flames. Physical and Emotional Devastation The fire was extinguished, and the car was towed away, but the damage to M.A’s life was just beginning. The traumatic event triggered a severe relapse of her Multiple Sclerosis, worsening her physical disability. The profound stress and sadness have been overwhelming. She is now without a vehicle, struggling daily with lost mobility and independence. Abandoned by the Company at Fault When M.A contacted Ford for help, seeking accountability for the defect they had already acknowledged, she was met with indifference. Customer service representatives, both by phone and on Facebook, directed her only to her insurance company, washing their hands of responsibility for the catastrophic failure their recall had warned about INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
