There is a known TSB out for 2019 toyota rav4 and some select other years. The issue is a slow speed transmission issue. It is due to a programming issue. At slow speeds, if you try to go from the brake pedal to the gas pedal too quickly, there is a serious lag before power is delivered to the wheels. Which is very dangerous when making specific turns. An example is what happened to my wife and my
... self the other day. We were in a median turning lane, cars were coming, as we were coming to almost a stop, my wife went to apply the gas. There was a serious lag before power was delivered and it almost resulted in a T bone collision. My wife went to apply gas, there was no power for several seconds, which caused her to panic and hit the brakes again. In that process, the power was finally delivered to the wheels and caused the car to pull about halfway into the oncoming traffic before it stopped from her applying the brakes. She then hard to quickly reverse to avoid the collision. The TSB calls for a flash of the TCM and a relearn procedure to be done. Which I am having done today due to the severity of the incident we had. This could have 100% caused a serious or fatal accident. The two dealerships I spoke with severely down played it as an issue. This deserves a investigation and a recall to be sent out. I was never advised of the TSB or anything.Read more

The shade that covers the moonroof has fallen off the tracks (or something) and it's sagging down from the headliner. There wasn't a snapping or breaking sound, I just went to open it and it sagged instead of sliding back under the headliner like it should. I was driving the Jeep on Route 35 in NJ when all of a sudden, the car lost power, jerked forward thrusting be towards the windshield. The dealership updated the vehicle and said it was safe to drive but I am not convinced based on the experience of others, which I have read in forums, who have had the same thing happen multiple times even after having the dealership update installed. With the car stopped & the transmission in the Drive or Reverse position with foot off the brake pedal: 1. Pushing the Power button once to turn the car off results in the transmission automatically shifting from Drive or Reverse to Neutral without automatically setting the Parking Brake. 2. If the car's front-end is on a slope pointed up-slo
... pe the car will freely roll backwards. If the car's front-end is pointed down slope the car will freely roll forward. The above sequence is very dangerous when on a steep slope. This problem was first experienced in a Home Depot parking lot that was uneven. Parked with the nose up-slope in a small depression. The Power button was pushed while the transmission was still in the Drive position & the car immediately started rolling backwards. Fortunately, nobody was behind the car. I quickly pushed the brake pedal to stop the rolling. The proper way to shut off the car, of course, is to put the transmission in the Park position first, which automatically engages the Parking brake, confirmed by a light on the manual Parking Brake switch. I also own a 2007 Prius, bout new. That car has a paddle trans lever, Parking Brake & Power switches. The parking brake is all mechanical with a parking brake pedal. Whereas the 2025 RAV4 does NOT have a parking brake pedal -- it has a switch. Pushing the Prius' Power Switch (off) automatically locks the transmission regardless of the trans mode (D, R, N) such that the front wheels lock& the car will not move, regardless of the parking brake setting. Thus, the Prius is intrinsically safe with locked front wheels unable to turn on a slope regardless of the parking brake setting. I discussed this with Toyota & my dealer. The dealer reported another customer had also complained about this problem. Toyota quickly closed my complaint #251006003193 with no explanation stated. See the attached PDF doc.Read more

I own a 2022 Ford F-150 Lightning Lariat (VIN [XXX] ). Around June 6, 2025, I brought my truck to Apple Ford Shakopee for a scheduled appointment because I noticed moisture collecting inside the rear light bar. The service representative told me Ford considers that moisture normal, likely from a car wash, and that it would dry out on its own. At that time the truck had fewer than 36,000 miles and
... was still under the bumper-to-bumper warranty. The moisture never went away, and the light bar is now failing — my backup lights are dimming and flickering. I’ve since learned there was a recall (23S30) on 2022–2023 F-150 Lightning Lariat and Platinum trucks for micro-cracks in the rear light-bar lens that allow moisture in. My VIN currently shows no open recalls, but this failure appears identical to what the recall described. Additionally, owner forums are full of reports from other Lightning owners experiencing the same issue — including those who already had the recall repair completed, whose replacement light bars are now showing moisture again. This suggests the defect may persist even after recall repairs. I’m reporting this because the issue compromises the rear lighting and visibility of the vehicle. I hope Ford and NHTSA will review whether the recall scope or remedy should be expanded to include affected vehicles like mine and ensure owners aren’t left with expensive out-of-pocket repairs for a known manufacturing defect. Since purchase, I have experienced nine tire-related failures, including four separate blowouts at highway speed, leaving the vehicle disabled on major roads multiple times. The most recent occurred on [Insert Date], when the car suffered another blowout and is now disabled at my residence. This pattern of failure has occurred despite tire replacements and service visits by Tesla. The issue began
... on the day of delivery, when a tire failed within 70 miles, and service staff admitted the car had not been safety inspected prior to delivery. Tesla has replaced nearly all tires, reimbursed a third-party shop once, and confirmed on a recorded call that the level of tire failure is abnormal. Still, no root cause has been identified. The failures have occurred under normal driving conditions and pose a serious safety hazard to me, my family (including children), and others on the road. My family now refuses to ride in the car. This issue is ongoing, unresolved, and I believe there is a systemic defect involving the vehicle’s suspension, alignment, or structural components that is contributing to abnormal tire wear and catastrophic blowouts. I am requesting that NHTSA investigate this vehicle and Tesla’s CPO inspection process, and that appropriate safety action be taken before someone is seriously injured or killed.Read more

The inverter radiator is punctured during typical road use by road debris. This results in a total loss of circulating coolant in the inverter/electric motor cooling system. This renders the car undrivable, with a concern for electric motor overheating, given that the hydrid mode and electric power can not be turned off. This is available for inspection. It is unclear whether the electric motor

Reproducibility: There are many examples online of this occurring during normal road use of Kia electric and hybrid vehicles. There is a warning 'Check drive motor cooling system' in this model. Refilling coolant turned the light off but with coolant being actively sprayed out of the pin hole in the radiator at all times (the pump does not turn off when the engine is cold) resulting in rapid coolant loss.Read more

Took delivery of a Tesla Model Y 2026 Juniper (not stated VIN #) in September 2025. This vehicle includes the mandated illumintated Emergency Release Button in the Front Trunk ("Frunk"). Then took delivery of a Tesla Model Y 2026 Juniper (VIN [XXX] ) in October 2025. This vehicle does NOT include any illuminated Emergency Release Button in the Front Trunk ("Frunk"). I visited the Tesla Servic
... e Center the day after I took delivery of the second vehicle, and spoke to the Service Technician, and then the General Manager for that location. Both were unaware that Tesla removed the illuminated Emergency Release Button in the Tesla Model Y 2026 Juniper. We opened several other Tesla Model Y 2026 Juniper Frunks, and none of the new ones in the lot had the button. However, we found one Demo Car that was from an older batch that had the button. I made a service appointment for this issue (and other issues I was having with the vehicle) and left. The General Manager then did some research and when I called back a couple days later he said that in Tesla's system, he was allowed to re-order a replacement Frunk panel WITH the illuminated Emergency Release Button for my first Model Y 2026 (delivered in Sep. 2025). But for my second Model Y 2026 (delivered a few days earlier in Oct. 2025), he could only order the Frunk panel WITHOUT the illuminated Emergency Release Button. In the meantime, I removed the Frunk panels from both of my vehicles to see if there were any differences in the wiring. And sure enough, in my first Model Y (Sep. 2025) there were wires and a connector coming off the power harness to the powered Frunk latch, that connected to the illuminated Emergency Release Button. In my second Model Y (Oct. 2025) these wires & connector is missing (since there isn't any illuminated Emergency Release Button to connect to). This tells me that this removal was an intentional design change by Tesla, which may violate 49 CFR § 571.401 and FMVSS 401. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACTRead more

Dear Sir or Madam, My tracking# is 11692602. Kia America apologized to me on 10/14/2025 and said if the government regulatory agency reaches out to them (Kia America), they will be sure to comply with your request. Thank you very much for your efficient handling. I'm writing to you today because the vehicle suddenly sounded an alarm while I was driving today, it seemed like there was a problem
