My 2024 Mazda CX-90 continues to have a malfunctioning fuel gauge despite multiple dealer visits and the recall repair (Recall 25V568). The dealer confirmed the issue was fixed, but it is not. The digital fuel gauge does not update in real time. It remains stuck for long distances and only partially recalibrates after restarting the car. For example, on November 5, 2025, I drove more than 60 mile
... s, but the gauge dropped by only about 10 miles. After turning the ignition off and on, the gauge adjusted slightly but still did not reflect the true fuel level. Photos from that day are attached. I have serious safety concerns. The recall notice states: “If the vehicle incorrectly indicates an inaccurate fuel level, there is a chance of unexpectedly running out of fuel while driving. This would result in an engine stall and vehicle being unable to restart, increasing the risk of a crash.” This issue persists even after the recall service was completed, suggesting that the current fix may not fully resolve the defect. This has been an ongoing issue. I visited the dealership again on Nov 5th, 2025.Read more

The vehicle was bought June 2023, starting September 2023 we have had continual electrical problems. The blind spot warning system constantly activates and takes over. The automatic braking system randomly activates for no reason. The car will go into full braking and stop. This will happen at all speeds to include highway speeds. Now I am getting check engine lights and the code changes every tim
... e the light activates. When we take it to the dealership for service all we get is "we can't replicate the issue" and nothing is fixed. I ran a Car Fax and found that instead of coding the problem that it was brought in for, Jeep has coded it as maintenance inspection or electrical system checked. This is even when modules, radios or electrical systems have been replaced. I have a feeling that Jeep is coding this way to avoid recalls or to hide problems. I would think this constitutes fraud. My Jeep has serious electrical issues that Jeep will not address. I am stuck driving it as it is my only vehicle.Read more

My GMC dealer falsely reported that recall work was completed on my engine but it was proven by an independent service center that it was not done. Items that have been proven to have not happened were 1. Replace engine oil with higher viscosity oil, 2. Replace oil filter to match new oil, 3. New oil fill cap labeled with oil type, 4. New insert for owner's manual. The way I initially learned the
... work was not completed was by warnings on my dash about low oil pressure and low oil level in my engine this past weekend. The independent service center provided confirmation that the invoice from my GMC dealer was falsified and that the work they stated was completed was not done. As a result of the failure to complete the work, my engine ran on insufficient oil and, based on the dirtiness of the oil in my engine, it compromised its condition.Read more

On August 26, 2025, I brought my 2018 Kia Optima Plug-In Hybrid to an authorized dealer for recall SC218 (engine inspection and KSDS software update). On November 4, 2025, NHTSA’s official VIN lookup confirmed the recall remains “Incomplete.” On October 21, 2025, Kia America emailed me under Case #XXX stating that SC218 was “confirmed completed” and that the dealer performed a software update to the ECU, referencing RO #XXX. However, NHTSA’s database still lists recall SC218 as incomplete. This contradiction between Kia America’s internal record, the dealer’s invoice, and the federal NHTSA database raises a serious safety concern that the recall inspection or software update was never actually performed or was never reported correctly. I have requested written proof and diagnostic data (Battery SOH report) multiple times but have not received them. If accurate, this means I was led to believe my vehicle’s engine-safety recall had been repaired when federal records still list it as open. I am requesting NHTSA to review this reporting discrepancy, ensure Kia America and the dealer produce verifiable proof of completion, and confirm that the federal record is corrected. I currently have a 2026 Tesla Model Y and was unfortunately involved in a collision due to what appears to be a Full Self-Driving (FSD) software failure. I had engaged FSD mode while driving my child to his doctor’s appointment. The navigation directed me through narrow residential streets. While traveling on a narrwow residential street, I encountered a garbage truck stopped in the middle of th
... e road, blocking traffic. My vehicle came to a stop behind the truck (with FSD engaged and my full attention on the road). The FSD system then began to “lurk” forward, seemingly assessing whether it could pass the truck through a very tight space on the left side. At first, I was uncertain whether there was enough clearance, but I decided to trust the FSD’s judgment, as it relies on multiple exterior cameras for precise distance evaluation—far more accurate than what I could estimate from inside the vehicle. However, as the vehicle attempted to proceed, I immediately heard and felt a long scrape along the right side of my car. It became clear that the FSD system had miscalculated the available space, and the gap was too narrow for my vehicle to pass safely. As a result, the right side of my car sustained significant damage, including both wheel caps/rims, both doors, the side panel, and the rear bumper. The impact occurred against the metal fender of the garbage truck. I am deeply concerned that this incident—caused by a software error—may occur again and to others, resulting in more serious outcomes. I am attaching photos of the vehicle, the accident scene, and the repair estimate.Read more

Vehicle: 2013 Ford F-150 SuperCrew Cab Component: Rear Defroster / Rear Window Assembly Summary of Issue: Rear window glass shattered while driving when rear defroster was in use. Detail of Incident: On November 2, 2025, at approximately 5:00 AM, I was driving my 2013 Ford F-150 SuperCrew Cab at approximately 65 mph on the highway near Mogadore, Ohio. The vehicle had been operating for around 4
... 5 minutes without issue. The outside temperature was about 43°F. I had the rear window defroster and side mirror defrosters activated. Without any impact, warning lights, prior cracks, or outside interference, the rear glass pane directly behind the driver’s seat shattered suddenly and completely while I was driving. There were no vehicles near me, and I inspected the glass immediately afterward and found no evidence of anything striking the vehicle. The failure occurred instantaneously and with a loud sound, which created a risk of driver distraction and could have resulted in loss of control or injury from breaking glass. After researching, I found that this issue is widely reported by other owners of Ford F-150 models online, particularly involving the rear defroster causing excessive localized heating and thermal stress, which leads to spontaneous glass failure. Many owners report similar conditions: cold ambient temperature + rear defroster activated. The glass was replaced at my own expense, approximately $800. Receipts and photographs are available upon request. The issue was not covered by recall, warranty, or insurance. The problem has not been addressed by Ford despite multiple consumer reports. Safety Concern: This defect presents a serious safety hazard because the sudden shattering is loud, distracting, and may send glass fragments into the cabin. It could easily lead to loss of vehicle control or injury to occupants. The failure occurred under normal and intended use of the rear defroster system. Request: I am submitting this report to request that NHTSRead more

... ad nothing to say. I have had bicyclists get too close to me on the road and my vehicle engaged the emergency stop. After repairs were done on the vehicle, they had a third party come in to do a calibration of all the sensors. He could not perform the calibration due to #PIT6274: Error Message Referring to Vision Camera system with DTC B1C48 - (Nov 4, 2024). I just dropped my vehicle off at the dealer and mentioned this to them and they are claiming there is nothing they can do about it. I started looking around online and have seen several other people complain about this and I knew it affected my Adaptive Cruise Control as I have gotten this error a couple times before and I just thought my window was dirty. What I did not know is this also affected my emergency stop as it does not tell me that in the warning. If this is a known issue, this is a serious safety concern and to have the dealer tell me there is nothing they can do is not acceptable. If the camera was functioning properly that morning, there is a good chance that I would not have gotten into an accident.Read more

Incident date: Nov 1, 2025
Was driving at about 55 mph and vehicle popped up with warning and said shift into neutral to restart. I tried to and all power turned off. This was at around 8 p.m. so no headlights or anything. Luckily got it stopped without hitting anything. Had vehicle towed and motor was locked up. Spun bearing they said. Same issue apparently as other 6.2 liter engines have been having.
Incident date: Nov 1, 2025
On the highway, the vehicle would not accelerate and was losing speed even though the gas pedal was fully depressed. Initially, the vehicle was in ECO drive mode, traveling approximately 65 mph and the steering wheel paddle to toggle the regenerative breaking between levels 1-3 prior to getting onto the highway. The vehicle speed dropped from ~75mph into the low 50mph range on the highway and was
... able to maintain speed then slowly regain speed when then phenomenon would happen again. The engine light was not initially on but did come on eventually. The engine light flashed at first but then stayed solid. Eventually a safe space was found on the side of the highway to pull over and restart the car. After restarting, the vehicle was able to regain highway speed (using caution, slow acceleration to merge back onto the highway), however when attempting to accelerate faster to see if the problem persisted, it did and the car again would not accelerate even when fully depressing the gas pedal. It felt similar to when you push the gas pedal while in park where the engine revs with high rpm's but doesn't move. The road trip was completed using cruise control, which seemed to have less issues (although not zero issues). After exiting the highway, at lower speeds (<30mph) the vehicle was shaking like it was going over rumble strips until it could be turned off and restarted. Component: power train. Yes, it is available for inspection. Safety issue: While traveling on the highway at times, the vehicle lost speed, was unable to maintain speed and/or accelerate. This could have easily caused a crash at highway speeds (60mph+). inspected by others? No. Will be taken to dealer on 11/3/25 Warning lights: The engine light came on after the initial incident. Kia Connect app, the following diagnostic codes were received; P030000, P030100, P030300.Read more

Breaks lock down when driving. Had multiple things replaced and it still does it. Said they don’t know what could be still causing the problem.

Vehicle acts like it’s going to die. No maintenance lights well maintained. Rattle noise and acts like it’s misfiring. Went to turn at traffic light and almost got into a wreck due to no horse power. This has been happening since less than 40,000 miles but car dealership keeps saying “nothing is wrong” because there isn’t a warning light on! It’s getting worse!
