These Hankook iON Evo tires started vibrating wildly at the first service that I took the car to for a rotation. We took it back to the dealership (Ourisman Kia in Fairfax VA) three times where they kept the car for several days to diagnose. After opening the tires up, they state that the inner sound insulating foam on all four tires has separated. The tires were at 7/32 and now 5/32 and the car
... only has 18,200 miles on it. The tires have a 50k treadwear warranty, but this isn't covered as part of the warranty. The inner foam separation on a Kia EV9 is a known issue and widely discussed on the internet. For an $80k vehicle with OEM tires that cost over $1500 to replace, it's ridiculous that they aren't covered by the dealership or by Hankook.Read more

My 2021 Kia Soul is exhibiting a severe safety defect documented in Recall SC336: frequent, unpredictable stalling in traffic ("Loss of Motive Power") and audible engine knocking. These symptoms create a high risk of a rear-end collision or being stranded in a dangerous traffic situation. The vehicle is currently at Kia of Streetsboro. Despite being provided with evidence of these active mechanica
... l failures, the dealership is attempting to "remedy" the safety recall based solely on a "Pass" result from the Piston Ring Noise Sensing System (PNSS) software diagnostic. As the TSB SC336 notes that the root cause is physical damage to cylinder walls caused by defective piston oil rings, a software diagnostic "Pass" is not a physical repair and does not address the existing mechanical stalling. I have not received any status updates or communication from the dealership service department since Friday. The dealership is refusing to perform a borescope inspection or open a Kia Techline case to investigate the stalling, effectively denying a proper safety remedy. I am reporting this as a failure of the manufacturer and dealership to provide a safe and effective remedy for a known life-safety defect. The car remains unsafe to drive.Read more

Office of Defects Investigation National Highway Traffic Safety Administration (NHTSA) 1200 New Jersey Avenue SE Washington, DC 20590 Subject: Safety Complaint – Premature Engine Failure Risk Due to Excessive Oil Consumption and Confirmed Cylinder Scoring – 2020 Kia Sorento Dear Sir or Madam, I am submitting a formal safety complaint regarding a 2020 Kia Sorento that is experiencing severe
... and abnormal engine oil consumption along with confirmed cylinder wall scoring. These conditions indicate progressive internal engine damage and present a significant risk of premature engine failure. The vehicle has been regularly maintained in accordance with manufacturer service recommendations. Despite proper maintenance, the engine began consuming excessive amounts of oil between normal service intervals. During inspection by the servicing dealership, visible cylinder wall scoring was identified within the engine. Cylinder scoring is a clear indication of internal engine wear and deterioration and can lead to rapid progression toward engine failure. Despite the confirmed presence of internal engine damage, the manufacturer has refused to authorize an engine replacement under the extended warranty program unless the engine experiences complete mechanical failure. The dealership has indicated that the vehicle must continue to be operated and monitored until catastrophic breakdown occurs before replacement will be approved. Requiring the continued operation of a vehicle with confirmed cylinder wall damage and abnormal oil consumption raises serious safety concerns. Progressive internal engine damage can lead to sudden engine seizure, stalling, or loss of power while driving, which creates a potential hazard for the driver, passengers, and surrounding traffic. Given the confirmed cylinder scoring and ongoing excessive oil consumption, I respectfully request that the National Highway Traffic Safety Administration investigate.Read more

... ve of a centrifugal lubricant leak from the driveshaft/CV assembly. The proximity of this flammable grease to the hot exhaust system during high-torque operation (mountain climbing) presents a critical fire hazard. Despite visual confirmation of the slinging grease, the dealership issued a blank service invoice, omitting the defect from the official record and returning the vehicle in an unsafe condition. This represents a documented safety defect being suppressed by the manufacturer, posing an unreasonable risk of vehicle fire or catastrophic driveline failure.Read more

I am filing this complaint to report a serious safety failure involving the service braking system and the failure of Forward Collision‑Avoidance Assist (FCA) and Automatic Emergency Braking (AEB) on my 2021 Kia Soul. During an imminent collision event, the vehicle failed to brake as expected, and FCA/AEB did not activate, despite conditions that should have triggered emergency braking. This cre

The vehicle is currently subject to an active recall and has now been inoperable at the dealership for approximately one month under the lemon law process. The failure involved federally regulated safety systems, not supplemental or convenience features. I reported this incident to Kia America Customer Care, but the manufacturer has declined to conduct a manufacturer‑level safety investigation unless I personally pay for removal and shipment of the ACU/Event Data Recorder (EDR). Kia America has also declined to provide any transportation assistance while the vehicle remains unsafe and inoperable and has referred the matter back to the dealership and my insurance instead. Kia America has stated in writing that they rely solely on dealerships for diagnosis and repair and that they will not investigate further unless I incur additional costs. This refusal to independently investigate a braking system failure raises serious safety concerns, particularly given the potential risk to other drivers if this defect is not addressed. Due to the prolonged loss of use of the vehicle, I am experiencing significant financial hardship and risk to my employment, as I can no longer afford a rental vehicle. Rear‑view camera intermittently fails to provide a usable image when shifting into reverse. Symptoms include delayed activation, black screen, frozen frame, or extremely choppy video. This results in intermittent loss of rear visibility and does not comply with FMVSS 111. The defect has persisted for several years despite multiple Polestar software updates and multiple NHTSA recall campaigns int
... ended to correct the issue. The failure is reproducible and appears strongly load‑dependent. It occurs most often immediately after vehicle startup or when the infotainment system is busy with background activity such as app initialization, updates, or UI redraws. Under these conditions, shifting into reverse frequently produces no usable camera image. If I wait several seconds for the system to stabilize before shifting into reverse, the camera usually works, which indicates a resource‑contention or performance‑limitation issue within the Intel Atom–based infotainment head unit used in earlier Polestar 2 vehicles. The recall remedies have not resolved the defect, and the failure continues to occur under predictable, real‑world conditions. Because rear‑view camera activation is time‑critical for detecting pedestrians, children, pets, and obstacles, this ongoing defect presents an unreasonable safety risk. The repeated failure of software‑only remedies suggests the underlying cause may be hardware‑related. Newer Polestar and Volvo vehicles equipped with the Snapdragon‑based infotainment unit do not exhibit this behavior, which further supports the possibility of a hardware limitation in the original head unit. I request that NHTSA re‑evaluate the effectiveness of the current recall remedies and determine whether a hardware‑based fix is required to bring the vehicle into full compliance with FMVSS 111. This defect remains active despite multiple attempts to correct it and continues to compromise rear visibility and overall safety.Read more

The issue appears to involve the electric oil pump and related components. My 2021 Kia Sorento had a recall for the electric oil pump that was marked “completed” in January 2023; however, the dealership only performed a software update and did not replace any mechanical components because no fault code was present at that time. Beginning around October 2025, the vehicle started experiencing dr
... ivability issues including hesitation and bogging down when accelerating from a stop. A dealership representative later stated the electric oil pump and housing likely need replacement and that the transmission may also be affected. The vehicle is available for inspection upon request. This issue creates a safety risk because the vehicle frequently hesitates or bogs down when attempting to accelerate, especially when pulling into highway traffic. When pressing the accelerator, the vehicle sometimes struggles to engage and move forward, which increases the risk of a collision. I brought the vehicle to Kia of RBC and spoke with Service Advisor Justin Hawkins. At the time of inspection, the check engine light had turned off and I was told they could not diagnose or repair the issue unless the warning light was active. I was instructed to continue driving the vehicle until the light returns. During a later conversation, dealership staff indicated the electric oil pump and housing likely need replacement and that the transmission may also require replacement, but they could not confirm this without the warning light being present.Read more

NHTSA COMPLAINT NARRATIVE — SAFERCAR.GOV Vehicle: 2024 Tesla Model Y Component/System: Full Self-Driving (Supervised) Software, Version 14.2.2.5 On March 14, 2026, at approximately 5:22 PM, a 2024 Tesla Model Y was traveling on U.S. Route 3 in Franconia, New Hampshire, at approximately 45 mph within the posted speed limit. The vehicle was operating with Full Self-Driving (Supervised) software v
... ersion 14.2.2.5 actively engaged. Three occupants were present in the vehicle, including two minor passengers. All occupants were properly restrained with seat belts. Component/System That Failed: The Full Self-Driving (Supervised) system, version 14.2.2.5, failed to safely detect and respond to a snow-covered road surface. The system lost control of the vehicle upon encountering a snow patch on the roadway, causing the vehicle to strike a tree. The FSD system is the primary failed component. The vehicle has been towed and is available for inspection upon request. How Safety Was Put at Risk: The FSD system was in active control of the vehicle's steering, braking, and acceleration at the time of the failure. The system provided no auditory alert, no visual warning, and no driver takeover request prior to the loss of control event. The failure occurred without any warning whatsoever, leaving insufficient time for the driver to intervene and prevent the collision, despite the driver actively supervising the system in full compliance with Tesla's own supervision requirements. Three occupants were placed in immediate risk of serious injury or death. The vehicle struck a tree and was totaled. Emergency services responded and evaluated all occupants on scene. An official police report was filed. Prior Warning Lamps, Messages, or Symptoms: None. The FSD system issued zero warnings — no auditory alerts, no visual prompts, no haptic feedback, and no takeover requests — at any point prior to or during the loss of control event. 1. The issue involved the engine and emissions system, specifically a cylinder 3 misfire, accompanied by illumination of the check engine light and white smoke emitted from the exhaust. The vehicle was evaluated by an authorized Jeep dealership, which confirmed the cylinder misfire through diagnostic testing. At that time, per the diagnosis, coil 3 and all spark plugs were exchanged. This did not
... solve the issue. When picking up the car, check engine light was still illuminated and white smoke emitted from the exhaust. The affected components are in possession of the dealership, unsure if available upon request. 2. The engine misfire and associated white exhaust smoke created a risk of engine performance degradation, including potential loss of power, observed hesitation and stalling, which could impact the ability to safely operate the vehicle in traffic. Additionally, white smoke from the exhaust can impair visibility for surrounding drivers and may indicate an underlying engine condition that could worsen without warning, increasing the risk to the driver and others sharing the roadway. The chemical smell could also nauseate the person operating their vehicle, impeding their awareness and concentration. 3. This issue was proposed by an authorized Jeep dealership, then confirmed by two other mechanics. Dealer diagnostic records document the cylinder 3 misfire and investigation related to the check engine light and exhaust condition. 4. No inspection from manufacturer, police, or insurance representative. 5. Yes, the check engine light illuminated and the vehicle exhibited white smoke from the exhaust which prompted the service visit. Auto-start was not functioning as normal, with an error displayed on screen. Engine would sometimes stall upon start up during cold temperature days. Description of Defect: I am reporting a chronic and dangerous structural failure of the factory-equipped alloy wheels on my 2023 Mercedes-Benz CLS 450 4MATIC. Since purchase, this vehicle has suffered 8 documented incidents of wheel failure, consisting of 5 rim cracks and 3 complete wheel replacements. The failures are occurring under normal, paved driving conditions in the Dallas-Fort Worth area.
... This persistent cracking indicates a severe structural design defect where the wheel’s load rating is insufficient for the vehicle’s weight and low-profile tire configuration. Safety Risk: Each failure results in sudden and unpredictable loss of tire pressure. This creates a high risk of catastrophic tire deflation (blowouts) at highway speeds, which can lead to loss of vehicle control. Despite 7 prior repair/replacement attempts by both authorized Mercedes-Benz dealerships (Park Place Dallas) and independent specialists, the defect persists. The vehicle is currently at the dealership for its 8th failure. I believe this wheel configuration is fundamentally unfit for purpose and poses a recurring threat to the safety of the driver, passengers, and other motorists.Read more

Description of Defect: My 2020 Kia Telluride experienced catastrophic engine failure and excessive oil consumption, which is a known issue for this model year (to the point that I had to sell it just 1 year after I bought it). The vehicle consumes oil at an alarming rate without any dashboard warning lights, or low oil pressure indicators, leading to sudden loss of engine power and potential stall
... ing while in motion. This failure occurred at 93,200 miles, well before the expected service life of the powertrain. Safety Risk: Sudden engine stalling or power loss while driving at highway speeds creates an immediate risk of rear-end collisions and leaves the driver without power steering or braking assistance. The lack of any 'Check Engine' or 'Low Oil' light prior to the mechanical crisis meant there was zero opportunity to safely maneuver off the road before the vehicle became disabled. Furthermore, excessive oil consumption in this engine type has been linked to potential fire risks under the hood. Failure/Incident Details: Despite following all manufacturer-recommended maintenance, the vehicle suffered a major [Engine/Transmission] failure. Kia America has acknowledged the failure but refuses to provide transparency or assistance, despite Technical Service Bulletins (TSB SA428 and TSB 089) existing for these exact powertrain issues. This represents a significant safety defect that puts my family and other drivers at risk.Read more

I was pulling out of my driveway when I noticed that my mailbox was blown open from a recent storm. I stopped and put it in reverse (well tried to) but the vehicle would not shift. Quickly I continued to drive to make a u-turn instead and at that point my vehicle would not drive normally. The rpms would not go up despite pressing the gas and the engine was reving very high.gm dealer said $700 to p
... ull the transmission to determine the issue, but after my research I’m certain it’s the valve failure in the transmission just as it’s done the same to others. Why is this transmission not recalled, but the disel engines on the same model are? Seems as though it is a similar issue. I’m afraid to see what they will charge me as my vehicle has 76,000 miles and I’m told is outside warranty coverage.Read more

The most recent incident the emergency brake locked on and the dash flashed system malfunction , maintenance needed, , emergency braking system shut down and several indicators were shut down . This continued for 90 minutes while I was on the phone with Nissan and was waiting for the tow truck when arriving at Nissan 72 miles away , they were not able to find any codes and the car was fine no issu
... es . Luckily I have video to back my story . I have pictures and videos of other times it has done this , but no one will look into why it keeps doing this and it’s done it several time over 2 1/2 years . They changed the stop /start button and it helped but now it’s starting to flash the emergency braking system is off again which is a build up to a major episode again. My rogue has less than 40, 000 miles on it and has been one issue like this after another with them slapping band aides on it and not finding the source of the problem. They even claim it cleared the bearing failure test but the test was preformed while it was on route to Nissan on a tow truck .Read more

My wife was driving here Highlander at freeway speeds when the vehicle started slowing and would not respond to the throttle. No warnings lights. It would drive but not accelerate to any reasonable speed. She pulled off the freeway and found a parking lot to test, restart car, etc. Again, no warning lights. Once in park, the vehicle would not shift into gear but only revved. The vehicle could n
... ot be driven and towed to a Toyota dealer and after a number of tests, it was diagnosed as a blown transmission. It was replaced at great expense. I do not know what the dealer did with the transmission but it was not tested. The safety issue is the sudden and expected loss of power/drive train on a freeway. The vehicle has been well maintained. At 100K, plugs and transmission fluid were replaced. Although the vehicle had around 130K miles, the transmission should not have failed in my opinion.Read more

Vehicles of previous model years, this model year, and future model years have all experienced this issue. Issue was reported on thru a youTube channel in order to let the public know and understand the issue and how to fix/repair the issue. It was then discovered thru view time and the linking of the part to repair that this was a rempent issue of concern for possible recall. Issue of hesitati
... on upon acceleration was noticed for 18 months. Vehicle was taken into dealership for diagnosis - main Nissan dealer and second repair facility diagnosed the need for a new transmission. Transmission was replaced thru the second repair facility for $7500.00. Issue of hesitation upon acceleration was not repaired thru the replacement of the transmission. Owner of the vehicle researched the issue and determined it was caused by holes and degredation of the "Air-Intake-Hose" assembly. This part is a $35.00 part availalbe on Amazon and easily replaced in about 10 minutes time by the owner. The need for recall is that Nissan does not throw an error code in order to diagnose this problem. The only error code thrown is for a NEW TRANSMISSION.Read more

I had just left my house and pulled onto a 5 lane road when the ICCU died. The car lost power while underway. I am filing a formal safety complaint regarding my 2020 Hyundai Palisade due to severe and ongoing engine oil consumption that presents a safety risk, combined with an active federal airbag safety recall. For approximately one year, my vehicle has been consuming excessive engine oil. Approximately 1,500–2,000 miles after an oil change, the vehicle becomes dangerously low on oil. On January 19, 2
... 026, at 171,650 miles, the dealership initiated Hyundai’s official oil consumption test. At intake, the engine oil level was so low that nothing was visible on the dipstick. The dealership confirmed no external oil leaks were present and began the manufacturer’s oil consumption protocol. After driving approximately 1,000 miles, I returned on February 2, 2026, at 172,657 miles. The inspection showed the engine was 2.6 quarts low. The dealership stated combustion chamber cleaning was needed; however, Hyundai denied the repair based solely on mileage and the vehicle being out of warranty. This level of oil consumption presents a serious safety concern. Driving with critically low oil can cause sudden engine failure, stalling, or loss of power while traveling at highway speeds. This could create a crash hazard without warning. Additionally, my vehicle is subject to NHTSA Recall #26V034000 (Recall #292 – Ejection Mitigation), involving potential failure of the side curtain airbags for third-row passengers to deploy properly in certain crash scenarios. The remedy is not yet available. The combination of excessive oil consumption and an unresolved federal safety recall raises significant concerns regarding the overall safety and reliability of this vehicle. I am requesting that NHTSA investigate the engine oil consumption issue in the 2020 Hyundai Palisade, as excessive oil burning may indicate a broader manufacturing defect affecting engine integrity and driver safety.
