My [XXX] daughter was a passenger in a vehicle involved in a crash in which the vehicle struck a tree and a fire was present. She was unable to exit the vehicle and did not receive medical attention in time. Public records indicate the vehicle had an open federal safety recall at the time, listed as incomplete. I am submitting this information for awareness and review. I am not making any concl

ICCU failure at 16k miles. The ICCU was replaced at the dealership on January, 29th of this year, so it is not available for inspection. The failure tripped a breaker at my home while it was charging possibly indicating a short circuit. The short circuit could have potentioally overheated and caught fire in my home. I took the vehicle to the dealership and they confirmed it was the ICCU and re

I am submitting a safety complaint regarding a serious incident involving my 2025 Kia EV9. The vehicle was serviced on March 1 at World Kia in Joliet, Illinois. On April 4, only four days after that service, the vehicle experienced a sudden and unexpected ICCU failure while I was driving. During my normal travel, in active traffic and at speed, the vehicle abruptly entered limp mode and displayed
... multiple system warnings. This caused an immediate and significant reduction in power, creating a hazardous situation for myself and surrounding drivers. I was forced to maneuver the vehicle out of traffic while it was rapidly losing propulsion capability. The vehicle is currently at Berwyn Kia in Berwyn, Illinois for diagnosis and repair. I am reporting this incident due to the clear safety implications of a sudden loss of power during highway driving operation. A failure of this nature poses a risk of collision, especially in high‑speed or congested traffic conditions. I am requesting that NHTSA review this issue as part of its ongoing monitoring of Kia/Hyundai/Genesis Integrated Charging Control Unit (ICCU) failures and related safety concerns. Thank you for your attention to this matter.Read more

I am submitting a complaint regarding my 2011 Kia Optima (VIN: [XXX] ), which is currently experiencing significant engine issues and is undrivable. This issue is currently being reviewed under Kia Case #XXX. This vehicle is part of the Theta II engine group that has been the subject of recalls, service campaigns, and extended warranty coverage due to known engine defects, including rod bearing f
... ailure and potential engine seizure. The vehicle has also received the Knock Sensor Detection System (KSDS) update associated with these known issues. My vehicle is currently at the dealership and has been there since March 10, 2026. It is not drivable, and I have been without transportation during this time. The dealership has diagnosed the issue as oil consumption; however, Kia has denied coverage, stating that oil consumption is not related to rod bearing failure. Kia has not confirmed whether a full internal engine inspection was performed to rule out rod bearing damage, nor have they confirmed that a Techline engineering review has been conducted. Given the known history of engine defects in this model, I am concerned that the current condition may be related to internal engine failure that has not been properly evaluated. Oil consumption can be a precursor or symptom of internal engine damage, and I do not believe the issue has been fully or adequately diagnosed. Additionally, my vehicle has an open safety recall related to potential engine compartment fire risk, which further raises concerns about the safety and reliability of this vehicle. I am requesting that this issue be reviewed as a potential safety-related defect, as the vehicle is undrivable and may be at risk for sudden engine failure or fire. I am also concerned that the manufacturer is relying solely on a dealership-level assessment without a thorough engineering evaluation of a known defect condition. Thank you for your review of this matter. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle was driven to the destination. An image of an engine was displayed on the instrument panel, and the vehicle continued to make a knocking sound. The vehicle was towed to a certified mechanic, who diagnosed that the engine was failing and needed to be replaced. The certified mechanic informed the contact about NHTSA Campaign Number: 21V259000 (Engine); and associated the vehicle with the recall; however, the VIN was not included. The vehicle was not repaired. The vehicle was then towed to the dealer, who confirmed the diagnosis; however, the dealer had not repaired the vehicle. The manufacturer was informed of the failure. The manufacturer declined to provide the engine replacement under recall or warranty. The failure mileage was approximately 79,000.Read more

While driving on a sunny day on a 35mph road, my dashboard reported “Check Electrical Vehicle System”. A red battery symbol appeared and my cruise control disengaged with a message “requirements not met”. When I got home I read my 12 volt battery monitor that reported the battery was 11.99v and only had a 60% state of charge. Just prior to this short drive from home, my battery was 12.77v
... and had a 100% state of charge. I put in my ODB reader and it provided two error codes: P1A9096 and P1A961C. The first error code is the result of an ICCU failure in its DC to DC circuit (the one that charges the 12V battery from traction battery). Using a battery charger, I was able to fully recharge the 12volt battery. My monitor says that it’s healthy with 100% state of charge. Turning the car back on resulted in a rapid drain of the battery from 100% to 93% in just five minutes while parked. The electrical system failure warning lights are still lit. I’m having my car towed for repair. If this failure had occurred more than 10 minutes from my home, I would’ve been stranded.Read more

Driving on the highway with no obstacles or vehicles in proximity, the sunroof spontaneously shattered. I am reporting a failure in the integrity of Federal Safety Recall completion and documentation at Kia of Daphne. On 2/6/26, I was issued official paperwork stating a recall part was 'Special Ordered.' On 4/3, I attempted to call for FU in part progress. Informed “recall was done and replaced 2/6, and no part was ordered.” Advisor requested I speak to manager, I declined, request they speak t
... o advisor. No call back until my VM left 4/6 afternoon requesting FU call. On 4/6/26 late afternoon, unprofessional call from another advisor who stated “we don’t see part ordered,” Advisor manager intercepted call, extremely unprofessional, stated he reviewed 4/4 and found nothing to support my statement, yet no FU call was placed to I, nor was any contact ever attempted by Kia. The dealership management proceeded to claim the recall was 'Complete' despite no record of the part arriving or being installed, and other advise “misspoke”. When I presented in person and requested clarification on this discrepancy in documentation/information/last signed document by I based on statement of part is order and will call when arrived to schedule; a male staff member became physically aggressive and volatile. I am concerned that the dealership is inaccurately reporting recall completions to NHTSA/Kia America without actually performing the required repairs or using the specified parts.I am also concerned for the integrity and safe environment of this dealership for customers and staff. I cannot attach the 11 minute video of the encounter, but am happy to provide via email to those who inquire given discretion for other parties.Read more

Engine light turns on every couple months. The Kia either stops accelerating and goes into “limp mode” or gives code for misfiring cylinders. Took to dealership several times, they did nothing but reset battery so light goes off. Unsafe when driving my family around and has turned into an unreliable vehicle.
