While traveling on vacation April 15, my 2025 F150 truck suffered a total loss of forward momentum on the highway and in city of Los Angeles —a severe safety failure. The repairing Ford dealership, diagnosed a factory-born "OPEN" in the main wiring harness (#14A005) behind the dash (DTCs P2128/P2138). To rectify this assembly-line defect, the dealership had to completely remove the dashboard (I
... nstrument Panel). As an experienced truck owner, I know that an invasive repair of this magnitude on a brand-new vehicle significantly increases the risk of long-term electrical issues and permanent dashboard rattles. My confidence in this specific vehicle has been severely compromised.Read more

To whom it may concern. Our 2017 XC90 unexpectedly, spantaneously, and catastrophically caught fire under the hood and was a total loss 5 minutes after parking it on April 19, 2026. There were no leaks or other signs of mechanical issues. The vehicle had all regular maintenance completed at the Byers Volvo Service Department in Columbus, OH. It had no open recalls. It was always parked overnight i
... n a garage. The Fire Inspector noted a possible vehicle manufacturing defect as the cause of the fire, however the vehicle was too heavily damaged to be definitive. We do have video of the entire incident as the Volvo was parked on Federal Government Property at the time. We are happy to share any and all information if this helps future vehicles to be safer. It should be noted that there is an active recall for the 2016 XC90 for a coolant hose leak that can cause a fire. Given that the 2017 XC90 is of the same design. Upon checking the internet and Reddit, it appears that our situation is not unique and there are other instances and reports of similar fires.Read more

On [XXX], my 2022 GMC Yukon Denali XL (VIN [XXX] ) experienced a complete loss of propulsion while I was driving on [XXX] in El Paso, Texas, with my wife and two young children in the vehicle. Mileage was approximately 50,364. While in motion, the vehicle was displaying abnormal behavior, concerned the vehicle would stall on the highway, I exited at the Geronimo exit. The transmission locked, and the vehicle became inoperable in the middle of the roadway with traffic passing on both sides. OnStar SOS was activated to communicate the unsafe location. The El Paso Police Department was requested to divert traffic. A city tow vehicle arrived after approximately fifteen minutes; my family was assisted across the roadway. Roadside assistance did not arrive for approximately three hours. This was the second loss-of-propulsion event on this vehicle in less than twelve months. The first occurred in March 2025 and resulted in engine replacement under NHTSA Safety Recall 25V274 (L87 Engine Loss of Propulsion). The [XXX] failure occurred after the recall remedy had been applied and the engine was replaced. The vehicle was towed to the dealer who replaced the throttle body, intake air duct and a service item titled "Repair open ground circuit". I am filing this report to document a second loss-of-propulsion event on a vehicle that already received the remedy under Recall 25V274, and to ensure NHTSA has visibility into the potential pattern of recurring or inadequate remedy on the L87 platform. While driving on the highway, the vehicle experienced sudden engine failure at highway speeds, putting the safety of our son and others drivers at risk. The engine completely lost power. The diagnosing dealership confirmed failure of a connecting rod and bearings and internal engine damage with metal present throughout the engine, requiring full engine replacement at an estimated cost of over $17,
... 000. The car is still at the South Pointe Honda dealership in Tulsa, OK. The diagnostic was also confirmed by a Honda certified mechanic at another independent shop. No others have inspected the vehicle. This appears consistent with reported issues involving connecting rod bearing failures in Honda V6 engines of similar model years. The failure occurred prematurely and without warning, raising concerns about reliability and potential safety risks if loss of power occurs during operation. There were no warning lamps, messages, or other symptoms of a problem prior to failure. The vehicle has been properly maintained and was not subject to abnormal use. I am submitting this complaint due to the severity of the failure, the high repair cost, and concern that similar failures may pose a safety risk to other drivers.Read more

I purchased my 2023 BMW 430i, coupe, X-Drive brand new on 04/14/2023 from BMW, Wesley Chapel Florida. On 05/14/2025, I observed my driver's side front seat belt to be extremely frayed and in an unsafe condition (see photo 05142025) and reported it to BMW Wesley Chapel immediately. The service manager advised me this was very unusual and he has never seen this condition before, but explained BMW
... Warranty will likely call it "normal wear and tear." The vehicle is under factory warranty for 5 years or 50K miles and it should be noted at the time of this incident the vehicle had 34,758 miles. BMW Wesley Chapel then offered a "one time goodwill replacement" for the seat belt as they could not figure out what caused this safety issue (seat belt only replaced). On 06/08/2025, uncertain mileage, I returned to BMW Wesley Chapel as the driver's side front seat belt began fraying for the second time since replacement (see photo 06082025). Again, they could not determine the causation for the fraying, but replaced the extender arm and the damaged seat belt. On 02/08/2026, at approximately 45,000 miles, I returned to BMW Wesley Chapel as the driver's side front seat belt began fraying for the third time (see photo 02082026). The "Service Tech Expert" as I was informed, was unable to determine the cause and the seat belt was not replaced and described it as normal wear and tear. I was advised if it got worse to return. I was also advised by my service advisor that he had another customer with the same exact problem with the same exact vehicle. Research has shown, this is becoming noted safety issue by BMW drivers (see below screenshot photos). I am currently on my third seat belt in three years and concerned BMW will not replace the belt or fix the problem. I am under factory warranty for approximately 1 more year and 4K additional miles. BMW USA has been advised and will not provide information regarding seat belt life spans or related issues.Read more

I am filing a complaint regarding an unresolved safety recall repair on my 2019 Ram 1500 (VIN: [XXX] ). The vehicle was brought to an authorized dealership for a safety recall related to the ABS system. At the time of the initial visit, there were no warning lights or system faults present. After the recall repair was performed, multiple warning lights and system errors appeared that were not pr
... esent prior to service. The vehicle has since been returned to the dealership on three separate occasions, and the issue remains unresolved. The dealership has now indicated a potential wiring harness issue but claims this is not related to the recall. They have requested approximately $1,200 for further diagnostic work, which we declined. The manufacturer, Stellantis, has stated that the recall for this VIN is closed and has declined to intervene, characterizing the situation as a dispute between the customer and dealership. As a result of these unresolved issues: * The vehicle cannot pass inspection in Texas * The vehicle cannot be registered or legally operated * Warning lights related to safety systems remain active We believe this represents either an incomplete recall repair or a condition introduced during the recall service process. We are requesting that this matter be reviewed as a failed recall remedy, as the safety issue has not been properly resolved and the vehicle remains in a compromised condition. Supporting documentation, including service invoices from all visits, is available upon request. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

To Whom It May Concern, I am submitting a formal safety complaint regarding my Kia Sorento and a recurring brake light failure that poses a serious risk to my safety and the safety of my children. After every single oil change, without exception, my brake lights stop functioning properly. This has happened repeatedly and consistently. Most recently, immediately after an oil change, both brake li
... ghts went out. I replaced the bulbs, but the issue did not resolve. Instead, the lights are now stuck on continuously, which strongly suggests a failure related to the brake light switch or associated electrical components—not normal wear and tear. This pattern is extremely concerning. The fact that the failure occurs every time the vehicle is serviced indicates either a defect in the design or a component that is highly susceptible to failure during routine maintenance. Brake lights are a fundamental safety feature. When they fail, other drivers are not alerted when I am slowing down or stopping, putting me, my children, and everyone around me at risk of a serious rear-end collision. I understand that brake light switch failures have been a known issue in Kia vehicles in the past, including recalls for similar defects. Given the repeated nature of this problem and the safety implications, I do not believe I should be responsible for the cost of repairs for what appears to be a known or recurring defect. I am requesting that this issue be formally investigated and that Kia be required to repair or replace the faulty brake light switch and any related components at no cost to me. Additionally, I ask that this matter be reviewed to determine whether further action, including a broader recall or service campaign, is necessary to protect other drivers. This is an ongoing and dangerous issue that has now occurred multiple times. I should not have to worry that a routine oil change will leave my vehicle unsafe to drive.
