Vehicle Info: Make: Tesla Model: Model 3 Year: 2021 Incident Date: April 7, 2025 Mileage: 49,000 miles Component: Electrical System / Software / Powertrain While driving on a public road, my Tesla Model 3 experienced a total system lag and shut down unexpectedly. The screen and vehicle controls froze, and the vehicle became unresponsive, leaving me stranded in the middle of the road. Tesla
... service later stated that the car computer had “slow FPS” (frames per second), which they claim caused the issue. They cleared memory, updated firmware, and said no further diagnosis was needed. However, no root cause was provided, and I am concerned that this issue could reoccur. This kind of unexpected shutdown while driving is a serious safety concern. I was lucky to avoid a collision, but if this had happened on a freeway or near traffic, the outcome could have been worse. I am filing this report in hopes that NHTSA will investigate whether this is an isolated issue or part of a broader safety defect related to Tesla's onboard software or power management systems.Read more

Formal Complaint Summary (VIN: [XXX] ) I am submitting this formal complaint concerning a 2023 Subaru Ascent Onyx Edition Limited (VIN: [XXX] ), which continues to experience brake-related safety defects despite recall repairs and repeated service attempts during the warranty period. Subaru of America issued Service Bulletin 06-92-24 in July 2024, which extended warranty coverage and acknowledge
... d systemic issues with front brake components—specifically brake pulsation, squealing, pedal softness, and vibration. Subaru developed new components and repair protocols to address these issues, including new rotors, tie bars, pad kits, and a revised installation process. In September 2024, I brought the vehicle to CMA’s Subaru of Winchester after experiencing symptoms covered under the recall. Subaru replaced all required recall components. However, the defect returned in March 2025, while driving from Indianapolis to Winchester, now worse than before—resulting in violent steering wheel vibration, loud grinding noises, and unsafe braking behavior, particularly during downhill braking. Instead of replacing the parts again, Subaru resurfaced the upgraded rotors—despite the bulletin’s warnings against doing so. This resurfacing served as a temporary patch, not a true fix, and failed to resolve the underlying safety issue. I have made 21 lease payments totaling $13,707.96 and paid a $6,000 down payment, totaling $19,707.96 in out-of-pocket costs. Subaru has offered only a $2,500 goodwill payment, which I rejected. Relief Requested: I am requesting a full reimbursement with buy back under Magnuson-Moss Warranty Act. Investigation to the reoccurring break issues despite being serviced for a recall. Shortly after purchase (8/2/23) I began noticing a persistent engine rattle between regular oil changes. This escalated to stalling issues, including on the highway and at intersections and around sharp curves. I brought the car in for service multiple times, and although a part was replaced, the symptoms soon returned. On one occasion, the vehicle stalled while being driven by your service adviso
... r, who confirmed it was “critically low” on oil—despite no dashboard warning, no visible leaks, and adherence to routine oil changes every 3,000 miles. Since that point, I followed all diagnostic procedures requested of me, including multiple oil consumption tests that required me to repeatedly take time off work. These tests produced inconsistent results: at times oil levels were acceptable, but shortly after, the same symptoms would return. At approximately 80,000 miles, a case was finally opened for suspected oil consumption, and I was told my concerns were valid and would be honored—even beyond the 100,000-mile warranty mark due to early documentation. Now, at 90,000 miles, I have been informed that the latest oil consumption is “within acceptable range,” and that no further action will be taken. Meanwhile, I am left with a car that unpredictably stalls from unindicated low oil levels—a serious and unsafe condition that has gone unresolved for the majority of my ownership. The lack of dashboard alerts means this issue places myself and others at risk each time I drive. To be clear, this is not just an inconvenience; it is a matter of safety and reliability. The engine has likely suffered long-term damage from prolonged oil starvation. I should not have to manually check the dipstick before every trip in a vehicle that is less than four years old and still under warranty. Moreover, I was never made aware of this issue at the time of sale (9/6/21).Read more

After taking my 2017 Honda Ridgeline to First Honda in Simi Valley, CA for its 100,000-mile maintenance, including a brake service and an oil change, I experienced a major engine failure just two days later. I let my son drive the vehicle from Simi Valley to the East Bay area for college. The next day, the vehicle broke down and had to be towed to Hayward Honda. At Hayward Honda, they informed m
... e that the oil was extremely dark and looked like it had not been changed, despite being billed for it at First Honda. Furthermore, they diagnosed that the engine failure was due to a connecting rod bearing failure — the same issue currently under investigation by the NHTSA. This breakdown happened without warning right after scheduled maintenance and may indicate both negligence by the servicing dealer and a defect in the engine. I believe this incident should be included in the NHTSA investigation related to engine failures in 2017 Honda Ridgeline models.Read more

This is BOTH a safety issue and an emissions issue, but the replacement part cannot be found anywhere to purchase anymore so I am having to drive this vehicle with this ongoing safety issue. Due to the nature of the EGR Valve blowing the fuse all the time and that it affects the safety of the vehicle a nationwide recall needs to be done on these vehicles to force GM to start making the part again. The engine in my 2017 Hyundai Santa Fe (VIN [XXX] ) began knocking, losing power, and showing signs of imminent failure. These symptoms directly match those identified in Recall #168 (NHTSA Campaign 17V578000), which involves engine bearing wear due to crankshaft irregularities in Hyundai 3.3L GDI engines. However, my VIN is not currently included in the recall batch. Hyundai has denied engine re
... placement under warranty or goodwill, citing a gap in oil change records between 5,000 and 45,000 miles — mileage which occurred before I purchased the vehicle used at ~60,000 miles. I have since performed multiple oil changes, though not always on a tight schedule. The vehicle is now at ~89,000 miles and located at Murdock Hyundai of Logan, UT, where technicians confirmed the engine is failing. This defect poses a serious safety concern due to the risk of sudden power loss while driving. The issue has been reproduced and confirmed by Hyundai service technicians, but they declined coverage and repair despite the failure matching a known federal recall issue. There were no warning lamps initially; the knocking and loss of power began subtly, then worsened rapidly. The vehicle is available for inspection, and all failure symptoms are consistent with others affected by Recall #168. I am requesting that NHTSA investigate this VIN for potential inclusion in the recall and take action to expand the recall scope, as this appears to be the same dangerous engine defect already recognized under federal recall. There are many incidents to report. 1. No Braking consistency and sometimes a jerk forward on braking, many incidents about 10. Also loss of regen braking at times. 2. ... of swiping another car. Only slight damage to my vehicle. 3. Driver door handle water leakage causing door to not open with NFC chip and is constantly very warm on the handle as if there is electrical current always on. 4. I had to buy a new KEY FOB as old one has failed. 5. Rear passenger door reading light will come on for no reason. Some adjustment needed. 6. Broken air vents. Poor quality plastic apparently. Pushed circular park button simultaneously with emergency brake tab wh
... ich we believe ultimately stopped the car. No usual parking system alerts, beeps, lights, etc., turned on during the collision or near pre-collision. Finally, when car reversed to go home, the car made an unusual jerking movement without the usual reverse 'beeping' noise for the hybrid. Car then behaved normal for the 10 minute tense drive home. We are afraid to drive the car for fear of fatal brake failure to ourselves or others. Vehicle is intact and currently at local Kia dealer for past several days, though purportedly they 'found nothing' and are offering to return the car. Auto claim still pending and once car returned to us, we will have auto insurance rep evaluate the car.Read more
