I am reaching out regarding the cam carrier leak on my 2019 Subaru Outback. After researching this issue, I discovered that cam carrier leaks are extremely common among Subaru vehicles equipped with the boxer engine, especially Outbacks from this generation. Many owners have reported the same exact issue, often at relatively low mileage, with repair costs commonly ranging between $3000 - 5000 Wha
... t has been most discouraging is learning that Subaru dealerships and technicians openly acknowledge how common this problem is, yet Subaru has not issued a recall or meaningful warranty extension to help loyal customers facing these expensive repairs. This does not appear to be caused by neglect or misuse, but rather by a known design issue involving the sealant used on the cam carriers.Read more

Year/Make/Model: 2013 Toyota Prius • Component: Brake Booster Assembly with Master Cylinder (Part #47070-47060) Description of the Defect: After approximately 30–40 minutes of driving, my dashboard illuminates with the Brake, ABS, and VSC warning lights simultaneously. The brake pedal becomes inconsistent and requires significantly more effort to stop the vehicle. This is a known internal flui
... d leak within the brake booster assembly that causes the accumulator pump to run continuously and ultimately reduces braking performance — a direct safety hazard. Why This Requires Federal Attention: This is not an isolated incident. Hundreds of 2013 Prius owners have reported identical symptoms, with associated diagnostic trouble codes C1391, C1252, C1256, and C1253. Toyota acknowledged this defect by issuing Technical Service Bulletin TSB-0089-20 and a Customer Support Program (ZJB) providing limited extended warranty coverage. However, that program was not well publicized . I was never informed as a vehicle owner . And it has now expired, leaving thousands of owners with a dangerous defect and repair bills costing thousands . A safety defect of this magnitude — one that can result in sudden, significant loss of braking ability — should not have an expiration date on its remedy. Toyota has not issued a mandatory recall, and owners continue to unknowingly drive vehicles with compromised brake systems. Requested Action: I respectfully urge NHTSA to: 1.Open a formal investigation into this defect across all affected 2013–2015 Toyota Prius model years 2.Require Toyota to issue a mandatory safety recall 3.Compel Toyota to cover repair costs for all affected owners regardless of mileage or age of vehicleRead more

The Rear Axle Drivetrain Assembly failed. Specifically, a critical structural fastener (bolt) associated with NHTSA Recall #25V090 (MBUSA #2025020010) completely detached from the rear axle. The vehicle is currently in my possession and the failed component/repair is photographed. The detachment of this structural bolt caused a complete loss of propulsion while the vehicle was in a commercial driv
... eway. Had this occurred minutes earlier during my 500-mile round trip on the I-95 highway, it would have resulted in a catastrophic drivetrain separation or axle lock-up at highway speeds (65+ mph). This posed a fatal risk to myself, my passengers, and other motorists. The failure was confirmed and documented by an independent service center (Crown Auto, [XXX] ) and subsequently confirmed by an authorized Mercedes-Benz dealer (Mercedes-Benz of White Plains). White Plains issued a strict "Do Not Drive" order and dispatched emergency roadside assistance to transport the vehicle. The vehicle has been inspected by Mercedes-Benz of White Plains (authorized manufacturer representative) and Crown Auto. I have photographic evidence of the failed component prior to the re-repair. Mercedes-Benz of El Cajon (the originating dealer) has also been provided with photographic evidence but has refused to acknowledge the failure. No warning lamps or messages appeared prior to the failure. The first symptom was a complete loss of propulsion and a "spinning" sensation from the rear axle upon arriving in Queens. This lack of warning is particularly dangerous as the vehicle gave no indication of the impending structural failure during a 500-mile highway journey. The Service Director at the originating dealership, Ernie Serrano, initially denied the existence of the safety recall in writing and subsequently suggested that I misrepresent the nature of the failure to avoid a 'failed recall' record. This suggests a systemic attempt to hide safety-critical repair failures from the manufacturer a INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFRead more

While flat towing my 2022 Ford F‑350 behind a motorhome using Ford’s Neutral Tow Mode procedure, the vehicle suffered a catastrophic transmission failure. The Ford dealer documented this clearly on the service invoice: “HAPPENED WHILE THE VEHICLE WAS FLAT TOWED.” After the tow, the truck had severe slipping, delayed engagement, high RPM with no movement, and a wrench light. The dealer conf
... irmed fault codes P0741, P1744, and P27B4. The dealer found catastrophic internal damage: “fluid is burnt with metal debris… filter is packed with metal debris… torque converter has came apart and filled transmission with metal debris… it is catastrophic failure.” These components are available for inspection. The dealer also documented a low battery condition: “batteries. low state of charge.” Ford’s Neutral Tow Mode relies on electronic control modules that require stable voltage, but Ford provides no warning that low voltage can cause the transfer case to exit Neutral Tow Mode. This creates a safety risk because the transmission can be rotated internally without lubrication while being towed, which can lead to sudden drivetrain lockup or loss of control. The problem was confirmed by the Ford dealer, and the vehicle was fully inspected by them. The only warning symptom occurred after towing: wrench light, slipping, and no movement. There were no warnings during the tow itself. This failure mode is not disclosed by Ford and cannot be detected or prevented by the owner.Read more

I am filing a formal complaint regarding my brand‑new 2025 Toyota Sienna, purchased in February 2025 for nearly $70,000. From early ownership, the vehicle had issues including multimedia malfunctions and unsafe lane‑change behavior on rough roads. In October 2025, Toyota issued a recall for faulty second‑row
... seats due to improper welding, creating serious safety concerns. This recall has effectively defeated the purpose of purchasing the vehicle. The Sienna was bought as a seven‑seater, but due to the recall it functions as a four‑seater, making it impractical and unsafe for my family. Daily use now causes: Increased back pain from bending to seat and secure children Children kneeling to access the third row, causing pain and injury risk Risk of dropping a child, slipping in rain/snow/ice, or being struck by passing vehicles when exiting backward Significant difficulty for an elderly parent with back pain Ongoing fear that the recalled seats could fail in an accident and injure occupants I contacted Legends Toyota (Kansas City 66109) and Toyota headquarters. The dealership offered unsuitable replacements, including a smaller vehicle or one recently repaired for other issues. Toyota corporate has provided no meaningful remedy, only loaner vehicles that are not equivalent to what I purchased. I believe this represents serious consumer harm: Toyota collected a large sum for a vehicle that cannot be safely or reasonably used and has failed to resolve a known safety defect while continuing to sell newer models without guarantees. I respectfully request an investigation and a fair remedy, such as a refund or equivalent replacement. Thank you for your attention to this matter.Read more

Vehicle, a 2018 Ford Edge SEL, recently experienced misfiring and overheating with the "check engine" light illuminated. Took the vehicle to an automotive repair shop and, after performing diagnostics, they advised the vehicle's engine coolant is leaking into the cylinders, which was the subject of a prior Technical Service Bulletin #19-2208 from Ford, and is a KNOWN DESIGN DEFECT with the 2.0L E
... coBoost engine produced by Ford for this and other model vehicles. After researching this issue I determined it was the subject of a prior recall, technical service bulletin, and class action lawsuit which lead Ford to offer free repairs of the engine block and extended warranty coverage to affected vehicle owners. WE WERE NEVER NOTIFIED OF THE RECALL, TSB, OR CLASS ACTION and, therefore, were not aware of the problem and unable to take advantage of any corrective program. Now, Ford Customer Service and the Ford dealership are both refusing to effect the necessary repairs without cost to me. The vehicle's ENGINE has an ACKNOWLEDGED DESIGN FLAW that can cause misfires, hydro-locking, overheating, and total engine failure with a fire risk. The vehicle is available for inspection upon request. Safety risks to the driver, occupants and others is obvious. The problem has been confirmed by a Firestone automotive repair shop. No other inspections have occurred due to a fear of driving the vehicle. The "check engine" light did activate when the problem first occurred, approximately 5 days ago (04/29/2026).Read more
