Title: Possible Defect – Uneven Airbag Deployment Description: On July 20, 2025, around 5:30 PM, i was driving a 2021 Volvo XC90 on a main road when another vehicle suddenly entered from the right side without stopping at a stop sign. I was unable to avoid a collision and my vehicle T-boned the other car (front impact on my vehicle). During the crash, I believe the front airbag system did not
... deploy properly or deployed unevenly. I felt like the left side of the airbag (from the steering wheel) was full of air but not the right side (that side was flat when it deployed), and I believe I hit my head on the steering wheel. I experienced severe injuries to my head, face, and right eye, including commotio retinae, lens displacement, corneal abrasion, corneal edema, phacodonesis, iridodonesis, and concussion. I am concerned that the Supplemental Restraint System (SRS) malfunctioned, contributing to the severity of my injuries. No modifications have been made to the airbag system. I request that NHTSA investigate whether there is a defect in the airbag deployment of the 2021 Volvo XC90, particularly for asymmetric or partial deployment. The automatic emergency braking (AEB) system also did not activate despite the imminent collision. There were no warning lamps, messages, or unusual symptoms prior to the crash. The issue has not yet been reproduced or confirmed by a dealer or service center, but The police was at the scene. My insurance company has inspected the vehicle. I have also reported this to the customer care team at Volvo.Read more

This report is on the paint defect issue on the 10th generation Honda Civic that can potentially impact the vehicle's structural integrity from the windshield damage. ... f ownership. The size of the paint chip has now extended to the rear body of the vehicle. A section of bare steel body around the rear window is exposed to the elements. Based on the inspection and evaluation by the authorized Honda paint and body shop that was recommended by the dealership, I have been informed that the cause is commonly due to improper paint priming during the manufacturing process. According to the paint and body shop owner, such paint peeling is a widely known issue with certain color paint for many auto manufacturers that use the same paint primer. I was warned that paint peeling will likely worsen with the water being captured underneath the paint. I am reporting this incident as a safety concern because the rust on the bare metal chassis of the vehicle can cause corrosion under the windshield adhesive over time and lead to the window breaking. The windshield is critical to the vehicle's structural rigidity as it contributes to both the roof support and the airbag deployment. The Honda dealership service manager submitted goodwill on my vehicle past warranty for a paint defect from the manufacturing process, but American Honda Motor Company has denied the claim as they determined it as cosmetic damage only. There are multiple reports online on paint defects from other Honda Civic owners from various places. There was a lawsuit settlement on older generation Honda Civic vehicles, but it looks like the known issue is still present on the newer generation. Based on the location of the paint defect, it can become a safety concern.Read more

Complaint Message – 2016 Kia Optima Engine Failure Without Oil Light Warning I am the owner of a 2016 Kia Optima (VIN: [XXX] ). My vehicle suffered complete engine failure due to oil starvation — without any prior oil warning light or indication. The engine died while driving, and it was only afterward that I discovered the engine oil was extremely low. The oil pressure light never illuminated
... at any point to warn me. I am aware that Kia previously recalled earlier model years (2011–2014) for similar engine issues under Recall SC147 and others, related to the Theta II engine and oil pressure-related failures. Many owners reported the exact same symptom — catastrophic engine failure without oil light warnings — that I am now experiencing in my 2016 Optima. Although my model year is not included in the existing recall, the failure pattern appears identical. I believe this presents a significant safety issue that should be investigated. A critical warning system (oil pressure light) failed to function properly, which directly led to engine damage. I am requesting that this issue be reviewed and investigated by NHTSA and Kia for potential expansion of the recall or warranty coverage to include the 2016 Optima. I have already contacted Kia and am pursuing further review through the dealership and customer support. Please note that other 2016 Optima owners have reported similar complaints online and in forums, and this issue may affect a broader number of vehicles than currently acknowledged. Although this 2020 Honda CR-V vehicle is not covered by the “sticky steering” recall we are experiencing the same symptoms reported by owners of newer model cars included in the recall. The car has
... 60k miles on it. We have just had new tires and breaks installed, a front end alignment and all linkages checked. The problem unfortunately persists. Clearly there is a problem with the steering on this vehicle. We haven’t yet experienced an accident but the car pulls to the left at high speed and takes considerable force to get it corrected. The vehicle could easily veer into traffic in the other or oncoming lane on its own. The ability to recover in time to avoid an accident is questionable. I am unsure if other owners have reported similar problems with this model and year but I believe this is something the agency should be looking into and discussing with the manufacturer.
