I was operating the motor vehicle at roughly 60 mph on a two-lane divided highway around 830AM. I was driving toward the sunrise to the east and passed under a highway overpass. As I cleared the overpass from the shade back into the sun, the vehicle flashed a collision warning on the windshield and immediately applied aggressive braking for no reason. No other cars were in the area around me at

It is always something, and this isn't the only time it has happened. It has simply annoyed me enough to cause me to submit a report.Read more

For several months the 2006 Rav4 with only 118k miles had been experiencing intermittent failure of power steering while driving at speeds between 5-20mph. These failures resulted in the steering wheel to become extremely difficult if not impossible to turn while the car is driving. In each incident, this resulted in stopping the car and putting on emergency lights until we can safely turn off the
... car and hopefully the issue goes away several minutes later. This issue has proven to be very dangerous when in traffic with no place to pull over and with the steering tightening up, it is not even possible to pull over in most cases. Four different mechanical shops tried to replicate the issue and none were able to determine the root cause nor replicate it consistently. This issue has been happening more frequently now and the car cannot be taken out safely. As [XXX] drivers, this is not safe by any means and Toyota should consider this a critical safety recall and cover towing costs to move this. There are no signs of instrument lights or any leaks coming from the car. This seems to be entirely a malfunction of steering components. With regard to incidents resulting from this, my mother is the primary driver, she is [XXX] old and injured her wrist trying to move the steering wheel to a safe location. Her wrist is still in pain several days later. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

In Feb of 2023, my van had no known issues and out of nowhere it started making a terrible noise, every light in the dash came on, my power steering stopped working and then my engine shut off. I had it towed to a mechanic shop, they said it needed an engine. I had it towed to our local KIA dealership. They said it did not need an engine, it only needed a timing chain. That was $8000. We decided t
... o get it done. 4 weeks later exactly from when we picked it up, I was on the interstate and the exact thing happened. I had it towed back to our local KIA. They stated it now needed an engine and it would be a used engine because they do not manufacture the engine anymore. It had over 60K miles on it and it would be $13K not including shipping or labor. We decided not to do this. After further investigation, this should have been under warranty the entire time and we never should have spent any money on it. This has been a terribly unreliable “family car”. I will NEVER buy a KIA again. My family is still suffering from paying $8K+ to have it “fixed”.Read more

I was at a traffic light today and my rear passenger window just completely spider webbed, while my daughter was in the back seat. I had to pull over and clean as much glass from the window to prevent falling on her and causing any additional damage. There were no other vehicles or pedestrians around. The only thing I can think of is that it was temperature related from being in the sun for a few
... hours and then having the AC on for about 5 minutes. I have researched on different volvo forums and threads and see other owners having similar issues. [XXX] [XXX] Thermal stress and expansion: When exposed to high temperatures, the glass expands, and if this expansion is uneven (perhaps due to varying thicknesses or materials), it creates thermal stress, potentially weakening the glass. When I reported the incident to the dealership they denied any other complaints or recalls instantly put the blame on an external impact without even inspecting the vehicle. There has been no official inspection by any party. Blower motor failure, available for inspection upon request. The failure occurred while driving in muggy conditions, resulting in rapid fogging of the windshield and front side windows. With no functioning airflow, the vehicle’s defrost/defog system was inoperable, and visibility through the windshield became significantly impaired. This required me to reduce speed and open windows in inclement
... weather to maintain a minimal view of the road, creating additional distraction and exposure to hazardous conditions. The blower motor produces no airflow at any fan speed setting. Failure was confirmed during inspection. No warning lights, messages, or other indicators appeared before, during, or after the failure. This is not an isolated incident. A brief search reveals numerous reports from Kia K5 owners describing the same sudden blower motor failure, indicating a likely systemic defect in the HVAC system design. Kia has not issued a recall, technical service bulletin, or other remedy despite repeated reports from vehicle owners. Kia’s continued inaction in the face of widespread, well-documented blower motor failures represents a disregard for both driver safety and basic manufacturing responsibility. Numerous Kia K5 owners have already been victimized by this defect, and doubtlessly many more will be as long as these vehicles remain on the road unrepaired. The loss of climate control directly compromises a driver’s ability to maintain a clear windshield, safely see the road, and operate the vehicle in adverse weather. These are not “comfort” issues, they are safety-critical failures. By ignoring the mounting evidence and leaving affected vehicles on the road without a remedy, Kia is willfully allowing a known hazard to persist, exposing thousands of drivers and passengers to unnecessary and avoidable risk.Read more

While operating my 2025 Lexus RZ 300e at normal road speeds, the vehicle experienced a sudden and dangerous propulsion loss when the traction battery reached 0% state of charge. The only advance notice was a single soft audible alert. There were no escalating warnings (visual or audible) and no meaningful low-battery reserve to allow the driver to safely exit traffic or reach a charging location.
... Within seconds of the single alert, the vehicle entered reduced-power “limp mode,” creating an immediate traffic hazard. This abrupt reduction in speed occurred in active traffic among large trucks, putting my safety and the safety of other road users at serious risk. The lack of progressive alerts and absence of a usable reserve buffer prevented any safe corrective action and increased the risk of a collision or injury. Lexus marketing materials, sales claims, and the owner’s manual indicate that the RZ’s battery management system provides intelligent, progressive warnings and adequate reserve energy to prevent drivers from being stranded. My vehicle did not perform in accordance with these stated safety features. This represents a potential defect in the warning system and energy reserve calibration. The vehicle is currently in the custody of the authorized dealership. I requested that the service department attempt to recreate the incident to determine whether the system functions as designed. They refused, citing an open claim with Lexus corporate. Lexus corporate closed my initial claim without investigation six days after it was filed and has not inspected the vehicle. This failure mode—no escalating alerts, no sufficient reserve, and sudden loss of propulsion—is a clear safety concern that should be investigated for possible noncompliance with FMVSS requirements for driver warnings and vehicle control.Read more

The main head unit for the infotainment system, specifically the software. Upon failing a software update in August 2024, the vehicle started switching itself from English to Italian, from Standard to Metric units, the high beams would no longer function, the auto locking doors after 15mph would no longer work, the emergency pedestrian sensing braking was turned off, the entire infotainment screen
... randomly shuts off and comes back on as well. Without readily available high beams my ability to see further distances at night was diminished. 2)The uninitiated change in language and units of measure made it more difficult to verify vehicle speed and other important settings with intervention to return the system to English. 3) Largest concern, auto locks not turning on, without notifying the driver, left my young children at risk of opening the back doors and falling out of the vehicle at high speeds. The dealership has tried to fix this twice, they have updated the software to current version and reset the system. They also reset the customer programmable features, which corrected the issue with the high beams, but the other issues with software remain, including the auto locks at speed, language change, units change and screen shutting off and on. I have also contacted Jeep customer service, they do not have a remedy in place and verified the software is up to date. After reviewing Jeep Wagoneer forums, this is effecting many owners of the Wagoneer after that fall 2024 update, and all the dealerships are scratching their heads. But without autolocks at speed, this is a big issue. No warning lights came on to let me know, and the bug seems to be in all software versions for the Wagoneer from Fall 2024 to current.Read more

A failure of the subframe while driving could cause a complete loss of steering control and the collapse of the front suspension, leading to a serious or fatal crash. The problem was independently reproduced and confirmed by both an independent service center and an authorized Audi dealership. Both establishments have deemed the vehicle unsafe to drive due to this specific issue. The manufacturer (Audi of America) has been made aware of the issue through their dealership network. They have acknowledged the defect by offering a "goodwill" repair to replace the subframe, confirming their awareness of this failure mode. However, the manufacturer has made this safety-critical repair contingent on the owner first paying for several thousand dollars of unrelated maintenance items, creating a financial barrier that prevents the safety fix from being implemented. There were no warning lamps, messages, or other symptoms prior to the failure's discovery. This is a silent failure that provides no warning to the driver. Jeep has refused to address or recall the problem despite it clearly impacting driving safety and vehicle usability. This malfunction significantly impairs my ability to operate the vehicle safely. I respectfully request that NHTSA investigate this issue as a potential safety defect. I have taken my car to the dealership and was advised that the whole radio unit needs to be completely replace. I have contacted Jeep and inquired about it, they told me they are aware of the issue but at this time, unless the car is under warranty, they do not offer any financial assistance. And it is a rather expensive issue to fix considering there is no fault of the car owners that the items installed in their vehicles are faulty.Read more
