I am submitting this complaint to report a serious and recurring safety issue with my Honda Odyssey involving the camera system. Multiple warning messages and fault codes appeared on the dashboard simultaneously, including: -Collision Mitigation System Problem -Adaptive Cruise Control Problem -Lane Keep Assist Problem -Auto High Beam Problem These systems are all disabled. This failure occurred wi
... thout warning and immediately compromised several critical driver-assistance and safety features. We have serviced at Honda dealerships and they said the only fix is a new $1500 camera (but the camera still works). Despite this, no recall has been issued. We have owned Honda vehicles for decades and never had these type of issues. A recall needs to be announced and owners allowed a fix at no charge. The transmission warning lights are also coming on intermittently and we paid to have Honda diagnose. The transmission is fine they said. Please help.Read more

I am reporting a catastrophic suspension failure resulting in complete wheel separation on my 2023 Tesla Model X Plaid shortly after Tesla service. While driving, the front left wheel separated from the hub, rendering the vehicle not drivable and creating an immediate risk of loss of control, collision, and serious injury or death. Tesla towed the vehicle back to its service center and issued wr
... itten service documentation stating (verbatim): “The front left wheel had separated from the hub. The issue was caused by missing hardware on the upper control arm, leading to further damage.” Tesla did not attribute the failure to impact, road debris, wear, or misuse. The missing hardware was at a critical suspension attachment point. As a result, Tesla replaced multiple major safety-critical components under Goodwill / Internal Service, including the front left active damper/air spring, upper control arm bracket, halfshaft, wheel speed sensor, and multiple structural fasteners. A wheel-off event caused by missing suspension hardware represents a severe safety defect with high risk of catastrophic injury or fatality. This incident raises serious concerns about suspension assembly, post-service quality control, and whether other vehicles may be operating with missing or unsecured hardware. I respectfully request NHTSA review this as a suspension/wheel retention safety defect and evaluate whether additional investigation or corrective action is warranted.Read more

At the time of engine failure, my husband was on the offramp between 140N and 24N at approximately 8:30pm. This ramp is under construction and narrow, but state troopers stopped and were able to push the car to the side of the road. As it was nighttime, my husband and /or other motor vehicle drivers/passengers could've been seriously injured. The car did not display any warning lights or signals.
... The car was then towed home and then to Empire Hyundai in Fall River, where it was assessed. It was determined that there were 5 outstanding recalls on the vehicle. Hyundai fixed all, with the exception of Recall #966, which was issued and not repaired PRIOR to my purchasing the vehicle. Hyundai Corporate was contacted by Aaron at Empire Hyundai. There is an active recall for the 6.2 L engines, which is what I have and it’s an issue with burning oil and catastrophic engine failure due to rods and bearings. And I just got this vehicle in November and I have already went through 15 quarts of oil in two months so I’m waiting for my engine to blow up going down the road to cause a wreck like has happened to multiple vehicles already and I
... also have a rod knock now due to this issue so it could be any time now that my engine fails and I end up getting hurt or my family I took it to the Chevy dealership and I have called GM to do an oil consumption test and to put in a claim so that way if it does fail, then it has paperwork to follow, but it is a major safety concern but something needs to be done considering there’s already 600,000 recalls on this issue already there needs to be more for the rest of the people that are having this issue like myself and I’m spending $50,000 on this truck for the engines to fail and me end up dying before something‘s done. and it’s also very distracting as you’re driving down the road and your vehicle tells you that you’re low on all so you have to try to find a way to get off the highway to have someone bring you all so you can continue to drive and it’s not just a cordial. It is up to 5 quarts of all at a time.Read more

While using the vehicle’s BlueCruise driver assistance system, the driver monitoring system generates frequent false driver-attention alerts despite normal posture and active attention to the roadway. The system often fails to recognize attentive driving unless the driver deliberately exaggerates head position or facial expression. Routine and unavoidable behaviors—including briefly touching t
... he face, wiping the nose, or checking side-view mirrors for traffic—frequently trigger alerts, even though these actions are part of safe driving. These false positives create alert fatigue and increased cognitive load, forcing the driver to divert attention from driving to manage the monitoring system itself. The intrusiveness of the alerts becomes distracting and undermines the intended safety benefit of the system. Due to the frequency of false alerts, the driver is often compelled to disengage BlueCruise entirely. I am concerned the current driver-monitoring calibration may inadvertently increase distraction for attentive drivers and warrants review. This issue has been reported to the manufacturer and logged under Ford Product case number [XXX]. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

This vehicle has experienced multiple hybrid system failures while driving, including a complete loss of power “Hybrid System Warning – Turn Engine Off” on the interstate in June 2022 through November 2025. These events have placed me and my loved ones in dangerous driving situations. Despite a warranty‑covered engine replacement at 82,015 miles (January 2023), the same hybrid system issu

The dealership claims it was denied — creating a serious inconsistency in communication and accountability. In addition, I have identified multiple NHTSA complaints in the database from other owners of 2016 Sonata Hybrids (VINs beginning with KMHE) describing similar highway shutdowns and hybrid system failures, which raise concern about a broader, unaddressed safety defect not currently covered by existing TSB or recall actions. I am respectfully requesting: A regional inspection and review of my vehicle prior to any further work. A review of Hyundai’s handling of this warranty claim and the discrepancy between dealer and corporate communication. Assessment of whether the hybrid system failures in KMHE‑prefix Sonata Hybrids warrant further safety action.Read more

Starting at around 1300 miles, we noticed jerking upon acceleration. We took it into the dealership at 1500 due to a check engine light. They replaced the coil in cylinder 1. Then again at 3200 miles, we took it back in for the jerking sensation. They could not replicate the problem. At 4380 miles, we took it back to the dealer for the same issue. They drove it 580 miles and found the lifter valve
... s in bank 1 to need replacement. The repair was performed and then on Saturday 1/17/2026, the car started giving check engine lights every time we accelerated. It would jerk non stop. The car hesitates during acceleration and feels like someone is pressing on the brake as we are accelerating, causing a jerking motion. Coming from a stop, the car feels like it has a hard time accelerating, almost like it is moving against a hard force to try to come up to speed. The car is at the dealer again today for this issue. It is unsafe to drive due to the hesitation during acceleration.Read more

After the Mercedes dealer replaced the corroded rear sub frame under the 20 year extended warranty, my brake line failed so I took it back & found out that the brake lines were corroded. I paid nothing for the subframe since it was under the warranty but I ended up paying $4114 to replace the corroded brake lines. The dealer should have checked for corrosion while the subframe was out of the car,
... especially since this is a known issue. The parts were only $380 and $3800+ was labor. My brakes completely failed while driving on Christmas Eve. I took the car to the dealership on 12/26/25 and they told me that the car was unsafe to drive & I had to leave it. This should be covered by Mercedes under the rest subframe warranty since from what I’ve read, is a related issue. This was extremely dangerous. I was scheduled to travel back to NY, almost 300 miles from VA, where I got the car fixed. The dealership did not do their due diligence when the rear subframe was replaced so they had to drop the subframe to fix the brake lines, resulting in the huge labor charge. I got the upgraded nickel lines to avoid corrosion in the future but this is definitely a safety hazard that could have put me and my family in complete danger, had I not gone out on Christmas Eve to grab something from the store. I am looking for reimbursement from the dealership and/or MB for the extra hours of labor that they charged me. This brake issue is very concerning and was extremely scary. My brake went all the way to the floor, all the lights came on and this happened without any type of prior warning. Something needs to be done to make sure that the brake line repair is also done at the same time that the rear subframe is done. Also, after the brake line was repaired, I had an issue with the brakes. I did not have this issue prior to the brake line replacement & now they are asking me to pay $1300 for front brakes & rotors. I had no issues with my brakes at all, nor did any sensors go off.Read more

I experienced an ongoing grinding/rattling noise from the wheel/brake area. The vehicle was serviced multiple times by a franchised dealer, which diagnosed the issue as brake-related and performed brake repairs. The noise was not resolved, and the vehicle was returned in the same condition. Shortly after service, an independent mechanic discovered a cracked/broken brake pad installed on the vehic
... le. Due to safety concerns, the manufacturer authorized inspection at a separate franchised dealership under different ownership. That inspection confirmed aftermarket brake pads, rotors, and related components of unknown origin installed on both the front and rear, missing OEM hardware, improper lubrication, and wheel bearing issues contributing to the noise. The inspecting dealer documented that the braking system must be returned to OEM specifications before safe operation and further diagnosis could continue. Despite this, the manufacturer has declined to require OEM remediation, and the vehicle continues to operate with unauthorized aftermarket brake components. The grinding noise has continued and has since shifted toward the rear. The condition is reproducible, documented by video, and confirmed by independent inspection. I believe the vehicle was returned to service in an unsafe condition involving a critical braking system.Read more

Vehicle is current at 101,102 Miles, however, the issue started to occur at 75,350 miles. It is the common issue crack in the exhaust pipe just before the flex exhaust tube due to poor welding of the bracket causing the exhaust system to rattle and leak exhaust into the cabin. The same issue has been addressed under Special Coverage N222385510 for 2021 vehicles, and another one was issued for 2019

GM Bulletin PIP5681C describes the issue in detail and acknowledges that the same process was used on their 2018-2022 vehicles, but they are only covering the repair of the 2019s and 2021 vehicles. Exhaust fumes do reach inside the cabin of the vehicle especially on calm mornings that have no breeze. This is a potential carbon monoxide hazard. I was driving on the road and my car just turned off. The car turned off mid drive. When I tried to turn the car on again, the engine revved and within a few seconds the car turned off again. The dash showed an error message "Error: Drive system. Please service vehicle." When the tow truck came, the tow truck driver said he has seen this issue before with VW Atlas cars. He tried to turn the car on
... again, and the same thing happened. When this happened, I felt extremely unsafe because the car just stopped in the middle of the street and I was unable to turn it back on or get it to move from the middle of the street. Thankfully it was not a busy street and there were no cars behind me. There were no warning messages prior to the failure.
