Subject: LS2 Rebellion Helmet Recall 26E001 – Manufacturer Not Honoring Recall / Providing Conflicting Information I received a safety alert from Amazon stating that my LS2 Rebellion motorcycle helmet is included in NHTSA Recall 26E001 and that I should stop using it immediately. The official NHTSA recall filing lists affected production dates from December 15, 2016 through July 30, 2025 for si
... zes XL and 2XL. My helmet was purchased on October 10, 2019, which falls within that range. I contacted LS2 multiple times for the required remedy, but they have been unresponsive/slow to respond and are now claiming the recall only applies to helmets manufactured between June 1, 2024 and July 31, 2025, which contradicts the dates in the NHTSA filing. They have not provided any confirmation that NHTSA approved a revised recall population. They also have not offered a replacement, refund, or clear safety guidance, leaving me without a usable helmet. I am concerned that LS2 is minimizing or misrepresenting the scope of the recall and delaying required remedies. I am requesting NHTSA’s assistance to ensure the manufacturer complies with the official recall and provides a remedy consistent with federal requirements. I have been keeping the emails and txt messages for reference and can make these available upon request.Read more

I received the recall Notice 23S64/NHTSA23V905 from the dealership where I purchased my Ford Ecosport in August 2025. I called the dealership to schedule the repair on August 28, 2025. And only could offer a loaner car if the part did need to be replaced. Leaving me without a vehicle for at least two weeks. I work and needed transportation for my commute. The issue was at no fault of my own. He also stated that it was not a big deal and the worst that could happen if the part was defective was that the car would just stall. I called back in September if 2025 and was told they still did not have the part available to them. The notice said there was no order to stop driving the car. However, about the time I received the 2nd notice to have the recall service scheduled immediately. My car began showing signs of engine problems. It has been at the dealership since Jan 20th. I was told they have no loaner vehicle. I am without a car due to this repair. I called moments ago and was told the damage to the car was extensive due to the recall part the engine will have to be replaced and they are waiting on parts. I again asked for a loaner vehicle. I was told again, they did not have any to offer me. But said that it’s be a few weeks before my car was repaired! Again, this is by no fault of my own. The car was purchased, paid for entirely. And it is my only means of transportation to ensure a way to work. I am filing this complaint regarding a serious and ongoing safety issue with my GMC Yukon Denali that began with a complete engine failure while driving on the highway with my entire family in the vehicle. While driving at highway speed (about 60–70 mph), the vehicle suddenly lost power and shifted into neutral on its own. While still moving, I tried multiple times to shift back into Drive so I
... could safely get off the road, but it would not engage. I was only able to coast partially onto the shoulder before the vehicle came to a complete stop, still partly exposed to traffic. The engine shut off and would not restart at all. It would not go into Drive or Neutral, and the vehicle was completely dead. And it had to be towed. The dealer later told me the engine had failed and that a new engine was installed. About a week later, I picked the vehicle up. Less than 20 minutes after leaving the dealership and getting back on the highway, smoke started coming from the vehicle and warnings appeared, messages that the engine was overheating and that I needed to stop the engine immediately. The vehicle was not drivable again and had to be towed back. I again had to arrange transportation home. A few days later, I picked the vehicle up again. Since then, I have had serious four-wheel-drive and drivetrain issues. The vehicle became stuck in 4 Low and I could not get it out for over a day. When it finally came out, the drive mode buttons stopped working correctly. Selecting 2 High sometimes puts it into 4 High, selecting Auto puts it into 2 High, and Auto mode no longer appears. Even when it shows 4 High, the steering is restricted cannot steer 100% to the right or left and the vehicle binds, pops, and jumps when turning. I purchased my 2020 Yukon Denali on July 8, 2024 from Classic Buick GMC in Painesville, Ohio. The check engine light started going on and off a few months after purchasing the vehicle. When I called a mechanic he stated that I should wait until the light is on and then bring in the vehicle. In early May 2025, I took my vehicle to Mike’s Automotive in Middlefield, Ohio after the check engin
... e light began coming on during extended drives (approximately five hours or more). The shop ran a diagnostic scan and advised that the oil pump needed to be replaced. Seeking another opinion, on May 14, 2025, I brought my vehicle to Classic Buick GMC Service, the dealership where I purchased my GMC Yukon. After diagnosing the vehicle, they advised that the check engine light was related to an Engine Oil Pressure Control Solenoid stuck in the off position (P06DD). This fix would cost $3,584. This fix would require the oil pump to be replaced as indicated by the previous repair shop. On May 29, 2025, I took the vehicle to Avenue Auto Clinic in Painesville, OH, where the oil pressure valve and sensors were replaced. The mechanic also advised that I change the oil every 3,000 miles, use OEM oil filters, and thicker oil. He also advised me to check the oil every 1,000 oil and add as needed. I do have to add oil approximately every 1,500 miles and I change the oil every 3,000 miles. I am concerned that my vehicle has the same engine-related defect as the Yukon engines currently under recall. FORMAL COMPLAINT Safety Recall 94B (NHTSA Campaign No. 24V-838) I hereby submit this complaint regarding the failure to properly complete Safety Recall 94B (NHTSA Campaign No. 24V-838) involving the HCU/ABS module on my 2018 Dodge Durango. On December 23, 2025, I contacted an authorized Dodge dealer located at 5404 S. Illinois Route 31, Crystal Lake, IL 60012, to remedy the HCU/ABS defect under
... the recall. I was informed that the recall repair would be performed free of charge and an appointment was scheduled for December 29, 2025. At the scheduled visit, the vehicle was returned to me in less than 30 minutes without any recall repair being performed. Instead, I was told that paid diagnostics were required and that the estimated repair cost could range from $3,000 to $5,000. I was also informed that the recall repair could not be completed because the HCU/ABS module was not in stock. The dealer later stated that the module would be ordered, but the delivery timeline was repeatedly postponed. On January 27, 2026, the vehicle was returned to me with the same HCU/ABS-related malfunctions. The dealer stated that Recall 24V-838 / FCA 94B had been closed and refused to replace the HCU/ABS module under the recall. Although FCA systems reflect the recall as “completed,” the safety defect has not been remedied. The vehicle continues to display ABS/ESC warnings, and diagnostic trouble codes related to the braking and electronic stability control systems remain present, confirming that the defect described in the recall persists. Pursuant to 49 U.S.C. § 30118(b) and 49 CFR § 573.6(c)(10), when a safety defect persists or reoccurs after recall repairs, the manufacturer is required to implement additional corrective measures, including repeat repair or replacement of the affected component or system. Component Failure: This vehicle has suffered a recurring "Loss of Motive Power" (LMP) while in motion. On [XXX], for the third time, the internal combustion engine shut off completely while the vehicle was being driven in traffic. The vehicle is available for inspection upon request. Safety Risk: This incident occurred on a public road in New York during a severe winter storm with over a foot of
... snow on the ground. There were two children in the vehicle. The sudden loss of propulsion in heavy traffic and hazardous weather conditions created an immediate risk of a rear-end collision and left my family stranded without power or heat in freezing temperatures. Confirmation & Inspection: This is a documented, repeat failure. The manufacturer has confirmed the defect previously and even reimbursed me for a prior repair attempt. However, the issue remains unresolved. The vehicle was inspected by an authorized Jeep dealer during previous failures, but the "remedy" provided was ineffective. I am currently awaiting a new inspection for this third occurrence. Symptoms & Warnings: Prior to the total engine shutdown, the Malfunction Indicator Lamp (MIL) illuminated on the dashboard. In previous instances, the manufacturer cited software errors or internal contamination as the cause. I suspect this is related to existing NHTSA Recall 25V-766 (Engine Sand Contamination) or Recall 25V-710 (Hybrid Control Software Failure). Additional Context: Despite the life-threatening nature of this repeat defect, the local dealership refused to provide a loaner vehicle, and the manufacturer (Jeep Wave) is refusing to fully cover the cost of a rental vehicle. I am currently paying out-of-pocket interest on delayed reimbursements from the manufacturer. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I brought it to the Hyundai dealership for evaluation, and they instructed me to continue driving the vehicle and return after 1,000 miles of driving so they could document the consumption. I returned after only 500 miles b
... ecause of the rapid loss of oil and my concern for driving the vehicle in that condition. While following the dealership’s required oil‑consumption test procedure, the engine performance deteriorated. The vehicle eventually began stalling and had to be towed to the dealership. Because I was required to keep driving the vehicle as part of Hyundai’s diagnostic process, I accumulated an additional 4,000 miles, which pushed the odometer just over 100,000 miles. After completing the test, the dealership submitted a claim for an engine replacement, but Hyundai Corporate denied the claim solely because the mileage exceeded 100,000—despite the fact that the over‑100,000 mileage resulted directly from Hyundai’s own required testing procedure.Read more

My Lexus 2019 RX450H is making a squishing squeaking brake actuator noise when pressing the brake pedal. I located well publicized TSB’s documenting the awareness by Toyota/Lexus of the problem. (TSB: MC-10245447-9999) Brake Squawk/Knock Noise. I checked with my local Lexus dealership to see if my vehicle had been repaired. It had Not!. Temecula Valley Lexus, 42081 Dealer Drive, Temecula, Ca
... . 92591. (951)365-6834. I scheduled an appointment for repair on 1/26/26. They rejected making the repair because my vehicle DID NOT have any codes in the computer OR any lights on the dash. They did in fact, confirm the brake actuator noise. I reject that concept because WHO could know ahead of time if a part experiences a (1st time) total failure without throwing codes and turning on dashboard lights? As I read the symptoms of severe problems experienced in a higher level of failure by brake actuators, I would not want to have to experience a crash accident in which the actuator would operate correctly in a hard stop condition or have ABS failure and brake lockup occur. Personal harm and property damage should not have to be an indicator if I can have the manufacturer cover a repair. Please advise What I should do to increase the response in the direction of getting this known problem repaired. System involved: The vehicle experienced a sudden electronic/network failure consistent with loss of ECU/BCM/CAN bus communication. The instrument cluster, throttle control, and HVAC system became inoperative simultaneously. • Safety risk: While driving, the vehicle abruptly lost throttle response and critical driver information with no warning, followed by an uncommanded engine shutdown after c
... oming to a stop. This created a serious risk of collision due to loss of propulsion, inability to react to traffic conditions, and lack of situational awareness. • Confirmation of issue: The vehicle was evaluated by an dodge service center, which identified an ECU software mismatch and performed a system update. The failure has not been intentionally reproduced due to safety concerns, but the incident is documented. • Inspections conducted: The vehicle has been inspected by an dodge service center. It has not yet been inspected by the manufacturer, police, or insurance representatives, as no accident occurred. • Warnings or prior symptoms: No warning lights, messages, or abnormal behavior were present prior to the failure. The malfunction occurred suddenly and without advance notice.Read more

While driving normally, the vehicle developed engine performance issues and was diagnosed with a catastrophic camshaft failure at approximately 85,000 miles. The vehicle has received regular oil changes with full documentation and has not been abused or neglected. A licensed mechanic confirmed that the camshaft was severely worn and damaged, consistent with AFM/DFM lifter failure, which is a know
... n issue on GM V8 engines. This failure caused significant internal engine damage requiring major repair. The failed camshaft has been retained as physical evidence. A camshaft should not fail prematurely under proper maintenance. This appears to be a manufacturing or design defect, not normal wear and tear, and presents a potential safety concern due to sudden loss of engine power.Read more

In early December (2025), I experienced Hybrid System Failure, with the vehicle showing this fault in the dashboard and the check engine light coming on. I took it to the dealership, as the hybrid system is still within warranty, and they diagnosed a failed hybrid battery module. They said I could continue to drive the care in combustion-only mode and would order the module. Having heard nothin
... g, I checked in early January and was told the module would not be available until May or June of 2026. I escalated to Volvo corporate. They opened a case and keep promising to have a manager reach out to update me, but they have failed to follow-through for almost a month. I am now faced with the prospect of operating an impaired vehicle that can't pass an emissions test (and which is under warranty) for more than half a year.Read more

The car stopped being able to charge on AC level 1 and level 2, only able to charge on DC Fast. Took it to the dealership and they diagnosed an ICCU failure, rendering it undriveable. The part is under warranty but is backordered. I have already been waiting for 14 days, and they have no update for when they'll get the part in.
