I have the Foundation Cyberbeast, the model which has an OEM light bar on windshield. While driving on 2-22-2026 at approximately 3:30 pm on the 5 freeway near Santa Ana, the appx 4 ft long light bar flew off and ricocheted violently on vehicle, while still attached by the power cord. This damaged many parts of my vehicle, specifically the glass roof, front windshield, and various body components
... which were shattered or scratched. I cut my hands. Your database says there are 0 open recalls and that is a lie by Tesla, because they sent me a notice months ago saying they are awaiting mechanical attachment methods. So they know they need to change the attachment method from epoxy to mechanical, but are falsely reporting to the government that they already did so.Read more

NHTSA Complaint Summary Vehicle: 2025 Hyundai Santa Fe Issue: Airbag non‑deployment in a 40‑mph side‑impact collision On January 20, 2026, my 2025 Hyundai Santa Fe was struck by another motorist directly along the driver side awaiting to make a left turn. The other motorist crossed the centerline at an estimated impact speed of approximately 40 mph, according to the police report. The coll
... ision caused significant damage along the front and entire driver side of the vehicle. Despite the severity and direct nature of the impact, neither the side curtain airbags nor the front airbags deployed. The impact was so much that the other motorist ended up behind my husband in the left turning lane once he stopped. My husband, who was driving, suffered a head abrasion and a concussion from the force of the impact. Since the crash, he has continued to experience persistent headaches and intermittent dizziness when standing, which began immediately after the collision and have not resolved. The lack of airbag deployment in a high‑energy side‑impact crash raises concerns about a potential malfunction in the airbag sensors or side‑impact detection system. This incident presents a significant safety risk, and I am reporting it so NHTSA can evaluate because as I've researched there have been other incidences of similar non‑deployment issues are with 2025 Hyundai Santa Fe vehicles. I can provide photos, the police report, medical documentation, and repair estimates upon request.Read more

Own a 2023 Hyundai Santa Fe Hybrid and have experienced the following issues over the past year and a half. Fast forward another 5 or 6 months and it happens again. Take it to the dealer and they keep it a week to go over it but still nothing. Within a week of getting it back it happens again and this time I am videoing it happening to me. Back to the dealer and provide the video because again the car did not log the event nor did it update my app. They hold it again for a week and try different things. They still cannot figure it out and I get it back.... it happens again.... This time the dealership was able to come up with an answer and that the oil level was too high. I got the car back and it seemed to work. Probably now 6 months go by and then this week I drive over to Jacksonville (5 hr drive) and visit. Again the car scares us on the highway and I have to pull over. Havea friend meet us with tools and we jack up the car and let out enough engine oil to insure we can read between the two marks on the dipstick. We drive off but the engine keeps hestitating and we almost make it back before it completly shuts down. We tow it to the Dealer in Jacksonville. They call the next morning like they can't find anything wrong! Vehicle Information: 2021 Honda Pilot Elite Component: Forward Collision Avoidance: Camera System Lane Departure: Lane Keeping Assist Advanced Driver Assistance Systems (ADAS) Failure Description: The windshield-mounted forward-facing camera (Lane Keeping Assist System / Honda Sensing camera) failed without warning on my 2021 Honda Pilot Elite. This camera is mounted behind the windshield and c
... ontrols multiple advanced safety systems, including Lane Keeping Assist, Lane Departure Warning, Forward Collision Warning, and Collision Mitigation Braking. There were no warning lamps, dashboard messages, or other symptoms prior to the failure. The malfunction appeared randomly approximately one month ago. The issue was diagnosed and confirmed by an authorized Honda dealer service department. I was informed that the windshield-mounted camera system requires replacement and was quoted approximately $1,800 for the repair. Because this repair is extremely expensive and currently unaffordable, I have lost multiple critical safety features. The vehicle no longer has functioning Lane Keeping Assist, Lane Departure Warning, Forward Collision Warning, or related Honda Sensing technologies. The sudden and complete loss of these safety systems significantly reduces vehicle safety and increases the risk of unintended lane departure or reduced collision avoidance capability for myself, my passengers, and others on the road. I believe a critical safety system should not fail without warning and should not be this costly to repair, especially when its failure disables multiple core safety features. This is a known issue across multiple threads and owners and Honda should take ownership and recall this issue. The vehicle and failed component are available for inspection upon request.Read more

1. System Failed: Power Train (Transmission). Component Failed: The 6R80 6-speed automatic transmission, specifically the internal lead frame and the direct drum. Inspection: No, the failed unit was replaced on December 19, 2025, but itemized diagnostic reports and repair invoices from both a Ford dealer and an independent shop are available for inspection. 2. The vehicle experienced sudden, unexp
... ected downshifting and a temporary loss of power while in motion. These incidents occurred frequently and randomly, particularly while accelerating or traversing inclines, creating "unsafe driving scenarios" where the vehicle could have been rear-ended or lost traction due to abrupt gear changes. Furthermore, the vehicle eventually entered "limp mode," where it would "buck" during shifts and fail to accelerate under load, leaving the driver vulnerable in traffic. 3. Yes, by both. Independent Center: Super Tech Automotive confirmed on December 11, 2025, that the transmission line pressure dropped from 158 psi to 20 psi under load, causing the vehicle to go into "limp mode". Dealer: South West Ford Service Department verified the concerns on December 16, 2025, documenting a "torque converter shudder" and "direct drum failure" with "excessive metal debris in fluid". 4. Yes. The vehicle was inspected by a manufacturer-authorized dealer, South West Ford Service Department, on December 16, 2025 (Service Advisor Ryan Forbis; Transmission Specialist 'Dietrich'). 5. Yes. Symptoms: The symptoms first appeared in August 2025 as a "sudden, temporary loss of power or unexpected down-shifting". Warning Lamps: Initially, no warning lights or error codes were produced, which prevented early detection during multiple diagnostic attempts. Later Symptoms: On December 11, 2025, the SES (Service Engine Soon) Light finally appeared when the truck entered "limp mode". Prior to the total failure, the transmission also exhibited "harsh/jerky downshifts" and would "hang up in gear".Read more

I was going 35 MPH, normal operation speed and there was no extreme weather or road conditions. I slowed down to get into the center turn lane ended up coming to a complete stop due to on coming traffic. I was stopped for about 30 seconds to maybe a minute when I finally had enough clearance to make my turn. When I pressed the accelerator it began to accelerate but stalled out almost immediately.
... So I pressed and depressed the accelerator rapidly several times then it finally accelerated. When it did finally accelerate it accelerated much to quickly like it just took off. I’d like to say that this excessive acceleration was terrible but it saved my passenger from getting slammed by the now on coming traffic due to the lack of power. This is not the first time it has happened it does it allot when the weather gets warmer as well as when exiting the highway coming to a complete stop and then when trying to accelerate it stalls I’d say 25% of the time. I’ve taken it to multiple repair shops and they all say they can’t find the problem. I know their is a problem and I’ve read of this happening to many other owners so why is there no recall on it yet I’m surprised no one has been seriously injured or worse. In my opinion I feel like the manufacturer has complete disregard to the consumers safety. It’s unsettling.Read more

My 2019 Ford F-150 (2.7L EcoBoost) is subject to Emission Recall 24E12, which addresses a defective PCM calibration that Ford acknowledges "could, over time, reduce the effectiveness of the emissions control system and the monitoring accuracy of the catalytic converter's efficiency." COMPONENT: Catalytic converter system and powertrain control module (PCM). Vehicle is available for inspection at R
... ich Ford in Albuquerque, NM, where it has been serviced for this issue. TIMELINE: Prior to the 24E12 recall repair, my truck had intermittent P0420/P0430 codes (catalyst efficiency below threshold). The check engine light would illuminate and then clear on its own. I brought the vehicle to Rich Ford for the 24E12 PCM reprogram. Within approximately one month of the recall repair, the same catalytic converter codes returned, except now the condition is constant and no longer clears. This timeline is documented in my FordPass app and the dealer's service records. SAFETY RISK: The constant check engine light prevents the vehicle from passing Bernalillo County emissions testing, which is required for registration in Albuquerque, NM. I am unable to legally register my vehicle due to a condition caused by a manufacturer-acknowledged defect. DEALER CONFIRMATION: Rich Ford diagnosed the recurring P0420/P0430 codes. They initially attempted to charge me $300+ for extended diagnostics. They are now consulting with Ford about whether a catalytic converter replacement will be covered under the recall. PRIOR SYMPTOMS: The intermittent check engine light with P0420/P0430 codes existed before the recall repair but was not constant. The recall repair worsened the condition from intermittent to persistent. PATTERN: Ford's recall documentation states the MIL "may illuminate even though a catalytic converter repair may not be necessary," yet numerous owners report being quoted $1,900-$2,100+ for cat replacement at their own expense after the 24E12 reprogram. Ford corporate confirmed the $300Read more

My 2022 Ioniq5 left me stranded late at night in a freezing rainstorm in March of 2024 when I was 75 miles from home at it failed to charge both D/C fast charging and A/C level 2 charging, despite my trying several different chargers. The car gave me the dreaded "Check E/V System" warning when it failed to charge. It turned out to be the notorious ICCU but also the AC inverter, which had to be r
... eplaced. It took 12 days. The dealership's initial diagnosis was something draining the 12-volt battery, so I asked if they also replaced that. They said no because it had tested fine, but 9 days after I picked up the car, it did not start because it turned out the 12-volt battery was dead, and my dealership spent 2 days getting Hyundai to approve replacement of the 12-vold under warranty. Last month, 17 January 2026, my Ioniq5 again left me stranded. This time 120 miles from home in a snowstorm. It D/C charged for 10 minutes before the charging stopped and it gave me the dreaded “Check E/V System” warning. I tried two nearby Tesla Superchargers with my Hyundai issued adapter, and they both failed. I had just enough charge to make it to a dealership 110 miles from my home, where the vehicle stayed for nearly a month while they diagnosed and repaired it by replacing the AC inverter (note that had been replaced the last time) and also the AC compressor. The ICCU failures are well-known and well-documented, but the AC inverters that failed on me twice do not seem to be as common and should probably be investigated. In both cases I was stranded on Saturdays when no dealership service departments were open until the following Monday, so I had to pay for a motel overnight, which Hyundai refused to reimburse because I was not at least 150 miles from home.Read more

Dear NHTSA Investigators, I am writing to formally report what I believe is a significant safety defect in certain 2007–2011 Toyota Camry vehicles, specifically related to excessive engine oil consumption in models equipped with the 2AZ-FE 2.4L four-cylinder engine (primarily 2007–2009, with some reports extending to later years). This issue causes the engine to burn oil at an abnormally high
... rate—often 1 quart or more every few hundred to 1,200 miles—without any visible external leaks. Owners frequently report no adequate warning from the dashboard oil light or other indicators before the oil level drops critically low. If the engine runs out of oil while driving, it can lead to rapid overheating, internal component damage (such as scored bearings or cylinder walls), and sudden engine seizure or catastrophic failure. Such a failure while the vehicle is in motion poses a serious safety risk, including: potential for crashes, including rear-end collisions if the vehicle stops abruptly without warning, or veering into other lanes/obstacles. Sudden loss of engine power. Increased likelihood of loss of control etc. This problem has been widely documented through thousands of owner complaints (e.g., hundreds on sites like CarComplaints.com for the 2007 model alone), NHTSA ODI reports, and Toyota's own Technical Service Bulletins (TSBs) and Warranty Enhancement Programs (such as ZE7 for certain models). While Toyota addressed it via a warranty extension (not a full recall) involving piston/ring replacements for qualifying vehicles after an oil consumption test, many owners—particularly those outside the original coverage period or with higher mileage—have faced expensive engine repairs or replacements out of pocket. The lack of a mandatory recall means the issue persists in many vehicles still on the road. I am concerned that this defect creates an unreasonable risk to drivers, passengers, and other road users. I had an issue with this and I needed to fill up 4.5QRead more

After 8 months of driving my car brand new I started experiencing multiple intermittent malfunctions in the dash board & infotainment system. I have documentation of DTC codes current in my ECU. These codes each dealership refused to find and instead replaced my car battery, wiping the codes & replacing my infotainment system after it completely failed. After repeated repair attempts each dealersh
... ip failed to successfully repair under warranty & told me to drive the car as normal when these malfunctions continued. These electrical malfunctions were at one point affecting my alternator which I also have documentation of. The car has lost power yet the engine stays running while I am driving. My handsfree is intermittent everyday. I am now way past my warranty & extended warranty due to following dealerships advice to drive the car as normal and bring back. These ongoing malfunctions create unreasonable safety risk which I believe falls under 49 U.S.C. Chapter 301 — the National Traffic and Motor Vehicle Safety Act. A third party diagnostics shop told me my car would have to be taken apart to find the grounding issues as this is clearly a manufacturing defect. These repairs have cost me money and I refuse to keep paying these dealerships if they aren't willing to resolve the core problem. I get constant push back from each dealership. I've already engaged with Nissan consumer affairs, a Lemon Law attorney, and filed with my State AG, AG recommended while undergoing an IDR that I file a complaint here as well. Please help.Read more

We purchased a 2025 Toyota Sienna Woodland Edition in July 2025. On October 7, 2025 we were notified that vehicle is subject to a safety recall [NHSTA Recall No. 25V-668, Toyota Recall No. 25TA12 (Interim Notice 25TB12)] due to improperly welded seat rails. The recall states: "The subject vehicles contain second-row seats that are mounted on seat rails that may have been improperly welded. If
... these seats are occupied during certain high-speed collisions, the seat may lose structural integrity, increasing the risk of injury. All known owners of the subject vehicles will be notified to return their vehicles to a Toyota dealer. For all involved vehicles, the dealer will replace the second-row seat rails with rails that have proper welding, free of charge. For all involved vehicles, Toyota will notify customers by early December 2025." Following the recall notice, we returned our Toyota Sienna to the dealer due to the safety risk of putting our children (or any passenger) in the second row seats and we are currently driving a rental car. Toyota has not yet indicated a solution for this recall and has not provided a remedy as originally stated by December 2026. Given that it is now February 20, 2026 and our vehicle has been out of service for it's intended purpose for over 120 days without a remedy, we would like to officially log a complaint with the NHSTA.Read more

The EHRS coolant system failed and had a leak causing the coolant to be excessively burned off as thick white smoke. This caused me to not be able to see anything behind me and forced others to stop or slow down dramatically to avoid colliding with me. I had my engine overheat and force me to pull over on the side of an unsafe road because the exhaust system burned off my entire tank of coolant in
... less than a mile. I have taken this to a Hyundai dealership and they confirmed it was the EHRS system failure and told me it was a fairly known issue with the 2020-2023 Sonata Hybrids. I was told that even if I get it replaced the possibility of having it happen again are pretty high. This seems extremely unsafe to myself and others.Read more
